Background: The influence of patient factors such as age; sex; weight; body mass index (BMI) and spatial orientation on operative difficulty of impacted mandibular third molar (M3) surgery is a subject of controversy in the literature. Aim: To assess the risk indicators of operative difficulty of mandibular third molar surgery at our institution. Subjects and Methods: A descriptive cross.sectional study involving patients that presented for wisdom tooth extraction between January 2010 and December 2011. The correlation between patientsf factors such as age; sex; weight; height; BMI; radiographic spatial relationship of the impacted tooth and operation time was determined with Spearmanfs rank correlation coefficient. Statistically significant variables were selected for multiple regression analysis to determine which factors contribute most to operative difficulty of M3. P value was set at 0.05. Statistical analysis used SPSS 17.0. Results: Only patientsf age and radiographic spatial relationship showed a statistically significant correlation with operation time (P = 0.038 and 0.008; respectively). Linear regression analysis of patientsf age and angulation of M3 showed that both contribute 44.8 risk of increased operation time (regression coefficient = 0.448); with M3 angulation contributing more significantly to increase in operation time (P = 0.001) than increasing age of the patient (P = 0.005). Conclusions: Findings from this study have shown that increasing age of the patient and the angulation of M3 impaction increases the risk of operative difficulty of the impacted M3 significantly

Background : Patients' complaints of pain; swelling and limitation in mouth opening following lower third molar surgery is an important factor affecting their daily life. The aim of this study was to review the different methods of minimizing postoperative morbidity following lower third molar surgery. Methods : Relevant literatures from Scirus; Pubmed and Medline computerized search on management of postoperative morbidity following third molar surgery were evaluated and highlighted. Information was also obtained from texts and journals in the medical libraries of Federal School of Dental Technology and the University of Nigeria both in Enugu. Results : studies reviewed have not pointed to the effectiveness of the routine use of any systemic or local agent for minimizing postoperative morbidity after third molar surgery without other undesirable effects. Oral surgeons across the world have devised several methods; which include the use of drugs; different surgical techniques; laser therapy and the use of drains. Conclusion: Postoperative morbidity after the surgical removal of impacted lower third molar still remains an important factor in patients' recovery and comfort. The awareness of different methods of reducing morbidity after lower third molar surgery would help both the surgeon and the patients in the management of impacted lower third molar

Objective: The effects of gender on pain; swelling and trismus was evaluated in eighty patients of both gender referred for extraction of impacted lower third molar at the Oral and Maxillofacial Surgery Clinic of the University of Nigeria Teaching Hospital; Enugu. Patients and Methods: Eighty patients of both gender; randomly divided into two equal groups; participated in this study. Group 1 (n= 40) were the male patients while group 2 (n= 40) were the female patients. Their ages ranged between 18 and 40 years (mean 26 +6.4). The mean preoperative mouth openings between the two groups were similar. All the patients were treated under the same surgical and postoperative protocols. The lower third molars were extracted using the buccal approach method. The length of surgical intervention was recorded as the time between the first incision and the placement of the last suture. Pain; swelling and trismus were evaluated at 24 hours; 72 hours and 5 days postoperatively. Results: The result of the study indicates that the female patients had a statistically significant shorter operating time and experienced less swelling than the male patients (p 0.05). Pain was significantly more in the female patients than in the males (p 0.05). There was no significant difference (p 0.05) between the two groups as regards trismus. Conclusion: The inflammatory response following third molar surgery increases within 24 - 72 hours after surgery with the male patients exhibiting more inflammatory swelling than the females. However; women appear to be more sensitive to pain than men

Background: Pain has been identified as the most common cause of postoperative morbidity after the surgical removal of impacted lower third molars which could be severe enough to affect the patients' daily life. Several studies have been done on impacted mandibular third molars in developed countries; where several millions of dollars are spent annually on the management of impacted third molars; yet morbidity following lower third molar surgery still remains a great concern to many clinicians. Objective: The aim of this review is to evaluate and highlight the different methods available for the control of pain after lower third molar surgery. This will enable oral surgeons carrying out lower third molar surgical procedures to be properly guided and well informed on postoperative pain management.Conclusion: The studies reviewed did not confirm the effectiveness of the routine use of any systemic or local agent for the control of pain after third molar surgery without other undesirable effects. Consequently several non-therapeutic methods have been devised by oral surgeons to control the postoperative pain to a tolerable level

Une evaluation de la prevalence de perte precoce de la deuxieme molaire de lait en tant facteur etiologique des dystopies et encombrements dentaires dans le secteur antero-lateral a ete faite par l'auteur. Le resultat n'a pas ete trouve significatif; hormis la tendance a l'inclusion de la deuxieme premolaire plus elevee dans le cas d'extraction que dans le cas de non extraction de cette derniere. Le facteur etiopathologique est le plus determinant dans l'installation des dysharmonies alveolaires et contenu dentaire
