ABSTRACT

Aim: is to assess the relation between c-Myc oncogene expression and angiogenic factors namely vascular endothelial growth factor [VEGF], funs-like tyrosine kinase 1[Flt-1] in patients with diffuse large B cell lymphoma [DLBCL] and their impact on the patient outcome


Methods: Forty Five DLBCL patients beside 10 normal controls were included. c-Myc oncoprotein was assessed by immunohistochemistry and sVEGF, and sFlt-1 were assessed by immunosorbent assay


Results: c-Myc over-expression was detected in 66.6% of DLBCL. The DLBCL patient group with positive c-Myc overexpression showed significantly higher sVEGF and significantly decreased sFlt-1 as compared to group with negative c-Myc over-expression [P=0.000, 0.009 respectively]. sVEGF was positively correlated to sLDH and .v./32 microglobulin [r =0.6, p;0.000, r =0.69, P= 0.000] respectively. The non-survived DLBCL group showed significantly higher expression of c-Myc, high concentration of sVEGF and lower concentration in sFlt-1 as compared to the living group [P=0.000 for all]


Conclusion: These findings confirm the in vitro based suggestion that c-Myc over-expression orchestrate the angiogenic mritch necessary for tumor progression. c-Myc over-expression, elevated sVEGF, and normal sFlt-1 at diagnosis are poor prognostic markers· in DLBCL patients

ABSTRACT

Aim :The aim of this study was to evaluate Total Thyroidectomy asa an option for clinically benign Thyroid Disease as multimodular goiter,Grave's disease and thyroiditis.Since the re operation for the recurrentgoiter after surgery less than total Thyroidectomy.Or incidental cancermay be associated with increased complication rate many times thanthat due to total Thyroidectomy from the start


Patients and Methods: This prospective study included 45 patientswho underwent total thyroidectomy for benign thyroid disease. It included 32 women and 13 men with mean age 42.3 years. Indications of sur-gery were simple multimodular goiter 28 [ 62.2 % ] ,Toxic nodular goiter 8[17.8 %] Graves disease 6 [ 13.3 %] Hashimoto's thyroiditis 3 [6.7]


Results :Transient recurrent laryngeal nerve injury occurred in 1 patient [2.2 %].Permenant new injury in 1 patient [ 2.2 %].Transient hypocal-cemia in 2 [4.4%] Permenant hypocalcemia in 1 [2.2 %] hematoma indicat-ing reoperation in 1 [ 2.2 ] incidental thyroid cancer in 4 [8.9]


Conclusion :For patients with bilateral involvement of thyroid glandby clinically Benign disease , Total Thyroidectomy is a preferred optionbecause it decreased the future redo for recurrent diseases or incidentalcarcinoma

ABSTRACT

Aim: was to compare the technique and the postoperative continence state of Swenson's operation with its Wang's modification


Methods: 25 cases underwent a resectional surgery over 2.5 years. 13 underwent Wang's modification while Swenson's operation was performed for 12 cases. Mean age at operation was 16 months [2 months to 5 years]


Results: Wang's operation had superior results than Swenson operation regarding postoperative soiling [P= 0.027] that occurred in 6 cases after Swenson operation and in only one case after Wang's operation while anastomotic leak occurred in one case of each group. Further, Wang's operation group had postoperative resting and squeeze pressures of 49.82 +/- .31 mmHg and 90.83.80 mmHg respectively; both are significantly higher than those recorded in Swenson operation group: 40.82.30, 78.0 +/- 7.21 respectively [P= 0.0001]


Conclusion: Wang's modification of Swenson's operation represents a proper management of the internal anal sphincter during resectional surgery for HD through preservation of the anterior rectal wall and splitting its posterior wall with formation of heart shaped anastomosis
