ABSTRACT

to study the hemodynamic changes in uterine artery blood flow using transvaginal color Doppler ultrasonography after endometrial thermoablation therapy in cases of DUB. Prospective randomized study. Al Zahraa University Hospital-Al Azhar University.-Al Haram Hospital for Research and Treatment. Some private hospitals-Egypt. Patients and Fifty premenopausal patients with DUB without obvious structural or premalignant lesions. All patients were in good health and they were submitted to pelvic ultrasonography, pap smear diagnostic hysteroscopy and endometrial biopsy within 6 months of our management. Those patients were randomly divided into 2 groups: first group included 22 patients who received thermal balloon endometrial ablation therapy. Second group: included 25 patients who had medical treatment "a three-months period of medroxy progesterone acetate "10 mg/day". The uterine thermal balloon ablation therapy [Thermachoice] was carried out on cycle days 3-8. Color Doppler measurements were also carried on cycle days 3-8. Flow waveforms were obtained from the main branch of the uterine arteries at the level of the inner cervical os on both sides. Doppler flow parameters were used for subsequent statistical analysis. The measurements took place before the initiation of treatment, on the first post operative day "thermal ablation group only" and 3, 6 months and one year after initiation of the study. Endometrial thermoablation was effective in the treatment of DUB. 20/22 women [90.9%] had amenorrhea, only 2 patients "9.1%" underwent hysterectomy during the follow up period because of problems related to pelvic heaviness recurrent uterine spotting or offensive discharge. The pulsatility index "PI" was significantly higher after endometrial ablation than pretreatment level. The PI was gradually increased throughout the period of follow up [2.8 +/- 0.9 and 2.9 +/- 0.8] at 6 and 12 months post-treatment VS [2.1 +/- 0.5] pretreatment. The hysterectomy rate was significantly higher in the medical therapy group than in thermoablation group "35.7% VS 9.1"-No statistically significant change in uterine artery PI took place after medical therapy. Thermal balloon endometrial ablation therapy induces a gradual rise in uterine artery blood flow impedance which was statistically significant at 6 and 12 months. This rise in impedance may be due to fibrosis in the uterine cavity induced by thermal balloon therapy. The uterine cavity fibrosis is a good marker for therapy success. Thermoablation technique was accepted by the patients with a high efficacy rate "90.9%"

ABSTRACT

The purpose of this study was to investigate the levels of certain peritonea fluid [PF] markers in 80 females [60 females with idiopathic infertility and 20 healthy fertile females as controls] as macrophage colony stimulating factor [MCSF] which is a marker of host defenses. Also, the inflammatory cytokines which are markers of inflammatory process and cell damage and estimation of the TNF-alpha which is a marker of gamete cytotoxicity. The PF was aspirated during laparoscopic examination for infertility problems. The functional integrity of the husbands' sperms by hypo-osmotic swelling [HOS] test of both groups was examined

ABSTRACT

800 pregnant women were included in this study. All cases were subjected to the following investigations: CBC, ABO system, RH factor, fasting and post prandial 2-hour blood glucose, BUN, serum creatinine, SGOT, SGPT, VDRL, urine, stool and ultrasound examination. Maternal weight, fetal birth weight ratio and placental ratio were calculated. 300 of them were examined for toxoplasmosis. Out of this group 75 women were considered as control. Indirect hemagglutination technique was used as screening test for detection of toxoplasmosis. It was found that there was a significant positive correlation between maternal weight and fetal birth weight ratio, while it was insignificant between maternal weight and placental ratio. So, it was important to diagnose this parasitic infestation routinely before and during pregnancy to avoid their complication, especially in the endemic areas, where warm climate, poor sanitation, bad habits and ignorance exist

ABSTRACT

This study aimed to assess the importanceofheterochromatic polymorphism of human chromosomes, 1, 9, 16 and Y on couples with habitual abortion. The study was performed on C-banded metaphases obtained from peripheral lymphocytes cultures of 15 couples with habitual abortion and 15 control couples, all Egyptians. The results showed a significant relationship between C-heterochromatin lengths and occurrence of habitual abortion.Pericentric inversionof chromosome [9] was detected in two cases [one male and one female]

ABSTRACT

Thirty couples of patients with habitual abortion and fifteen normal couples as controls were investigated. Conventional giemsa G-banding techniqueswereused.Numericalandstructuralchromosomal abnormalities were detected in nine individuals giving a frequency of 30%. No abnormalities were detected in the control group. Chromosomal aberrations included sex chromosome mosaicism [46, XX/45,XO], autosomal mosaicisms; trisomy F20 [two cases], trisomy G. structural abnormalities [two cases]; one case with deletion of the long arm of chromosome number 1 and the other one with ring chromosome number 2. The last abnormality was detected for the first time among habitual aborters.Balanced translocations were detected in three cases. Among couples investigated females were more likely to be the carrier of chromosomal abnormalities than males
