ABSTRACT

Purpose@#Locally advanced rectal cancer (LARC) is managed by chemoradiotherapy (CRT), followed by surgery. Herein we reported patients with metastases during or after CRT. @*Methods@#Data of patients with LARC who received CRT from 2008 to 2017 were reviewed. Patients with metastases after CRT were included. Those with metastatic tumors at the initial diagnosis were excluded. @*Results@#Fourteen patients (1.3%) of 1,092 who received CRT presented with metastases. Magnetic resonance circumferential resection margin (mrCRM) and mesorectal lymph nodes (LNs) were positive in 12 patients (85.7%). Meanwhile, magnetic resonance extramural vascular invasion (mrEMVI) was positive in 10 patients (71.4%). Magnetic resonance tumor regression grade (mrTRG) 4 and mrTRG5 was detected in 5 and 1 patient respectively. Ten patients (71.4%) underwent combined surgery and 3 (21.4%) received palliative chemotherapy. @*Conclusion@#Patients with metastases after CRT showed a higher rate of positive mrCRM, mrEMVI, mesorectal LNs, and poor tumor response. Further studies with a large number of patients are necessary for better survival outcomes in LARC.

ABSTRACT

Background: many reasons can lead to an aphakia without sufficient capsular support for a posterior chamber intraocular lens, such as intraoperative complications during phacoemulsification, intracapsular cataract extraction, ocular trauma and lens dislocation caused by various reasons


Aim of the Work: was to compare retropupillary fixation of an iris-claw IOL [artisan aphakia lens] with transscleral suturing fixation of aposterior chamber IOL for aphakic eyes without sufficient capsular support as regards safety, visual recovery and complications of the procedure


Material and Methods: this a prospective interventional case series study included a total of 45 eyes of 42 patients of the age group 13-60 years, selected from those attending the Ophthalmology Department at Al-Azhar University Hospitals according to the inclusion and exclusion criteria


Results: in group A, the duration ranged from 15 to 45 min with a mean of 30.30 +/- 6.06min, in group B, the duration ranged from 35 to 85 min with a mean 60.60 +/- 12.41min. The P-Value was< 0. 001. The IOP showed at day 1 postoperative was higher in group B 20.06 +/- 4 than group A A16.40 +/- 3.9 p value was 0.012, however IOP was nearly at the same level at the end of the follow up period 15.23 +/- 3.63 in group A and 15.23 +/- 3.63 in group B with p value 0.713


Conclusion: the results of our study indicated that IC-IOL and SF-PCIOL implantations are both satisfactory in correcting aphakia without sufficient capsular support to hold an IOL in the posterior capsule

ABSTRACT

Objectives: Gastrointestinal symptoms are a common feature in children with pervasive developmental disorders, drawing attention to a potential association with celiac disease or gluten sensitivity. However, studies to date regarding the immune response to gluten in Pervasive Developmental disorders and its association with celiac disease have been inconsistent


Subjects and Methods: This cross sectional case control study included 45 patients aged 3 to 12 years [with or without gastrointestinal symptoms] diagnosed with Pervasive Developmental Disorders according to DSM- IV TR, Childhood Autism Rating Scale [CARS] and Gallium test for autistic characters. EEG was done to diagnose epilepsy. They had been regularly attending out patient clinic of center for care of children with special needs, institute of postgraduate childhood studies; Ain shams University, Egypt for at least one year. Forty five apparently healthy children of matched age and sex were recruited as a control group. Serum levels of IgG, IgA, IgM class antibodies to gliadin were measured by using EL1SA methods


Results: A total of forty five autistic children with confirmed diagnosis aged between 3 to 12 years were studied. They were 36 males and 9 females with male to female ratio 3.5:1. The mean age of introduction to cereals was 6 months [range 4- 8 months]


The main gastrointestinal symptoms as abdominal distension was present in 20 patients [44.4%], constipation in 16 patients [35.6%], chronic diarrhea in 8 patients [17.8%], vomiting in 9 patients [20%], anorexia in 19 patients [42.2%], iron- deficiency anemia that does not respond to iron therapy in 24 patients [53.3%], feeding difficulties in 10 patients [22,2%]. None of the autistics examined were positive for IgA and IgG antibodies tested, and 60% patients showed high serum levels of IgM antibodies to gliadin


Conclusion: The increased anti- gliadin antibody response and its association with GI symptoms points to a potential mechanism involving immunologic and/or intestinal permeability abnormalities in affected children. Immunological detection of, IgA, IgM and IgG antibodies dass to gliadin are useful tool in the diagnosis and follow-up of the disease

ABSTRACT

Objective: To determine the prevalence of Cysticercus tenuicollis (C. tenuicollis) metacestodes in five oryx species kept in Al Ain Zoo animal collection. Methods: This study was based on a retrospective analysis of post-mortem records covering a four year period (July 2010 to July 2014). Results: A total of 213 individual animals were recorded dead during the four year period (July 2010 to July 2014). Out of this, 12 (5.6%) were recorded with C. tenuicollis. More females (8) than males (4) were recorded to have C. tenuicollis, although this was not statistically significant (P = 0.373. 7). Conclusions: This study shows that, Arabian oryx, beisa oryx, fringe-eared oryx, gemsbok and scimitar-horned oryx are susceptible to C. tenuicollis. Based on the epidemiology and the life cycle of this parasite, it is possible that these captive animals ingested the parasite through contaminated feed which could have happened in the pasture land or stray dogs and wild canidae (e.g. fox) visited the zoo contaminating the oryx feed. Stray dogs and wild canidae should be prevented from visiting pasture land and a captive animal facility.

ABSTRACT

The World Health Organization [WHO] reclassified atypical meningiomas in 2000, creating a more clear and broadly accepted definition. We evaluated the pathological and clinical management for atypical meningioma patients according to the new WHO grading system at Cairo University Hospital. A total of 30 atypical meningiomas occurring in 30 patients between 2005 and 2007 were prospectively reviewed to determine diagnosis, postoperative treatment trends and early outcomes. In the 3-year period, 371 meningiomas were operated on at Cairo University Hospital; of these, 320 [86.5%] were benign, 30 [8%] atypical and 21 [5.6%] malignant. The histological parameters permit an accurate identification of atypical and malignant meningiomas with high growth potential and recurrence rates that far exceed that of benign meningiomas. We hope that the use of these relatively simple histological criteria will help to further the understanding of the biology of this important group of tumours

ABSTRACT

In this study, we report the clinical and radiological outcomes of patients with sacral masses. Ten patients presented to Neurosurgery Department in Kasr EI-Einy Hospital from January 2006 -December 2008 with sacral masses. Surgical resection using posterior approach with or without spinopelvic fixation by using Modified Galveston Technique if the sacroiliac joints are involved more than 50% by the tumour. The pathological diagnosis was giant cell tumour in 2 cases, aneurysmal bone cyst in 3 cases, chordoma 3 cases and neurofibroma in 2 cases. Total resection was done in 9 cases [90%]. Low back and leg pain recovered well in 8 cases while remained for about 5 months in the 2 cases with giant cell tumours in which there were instability and fixation was used but fusion did not happen except after 6 months. Sphincteric disturbance recovered after 3 months in 4 cases [40%], 3 of them were aneurysmal bone cyst and one case of neurofibroma. The surgical management of sacral tumours requires partial or total sacrectomy and spinopelvic reconstruction. These lesions present a great surgical challenge because most spine surgeons are unfamiliar with the techniques required for these procedures
