ABSTRACT

Recent work in decision neuroscience suggests that visual saliency can interact with reward-based choice, and the lateral intraparietal cortex (LIP) is implicated in this process. In this study, we recorded from LIP neurons while monkeys performed a two alternative choice task in which the reward and luminance associated with each offer were varied independently. We discovered that the animal's choice was dictated by the reward amount while the luminance had a marginal effect. In the LIP, neuronal activity corresponded well with the animal's choice pattern, in that a majority of reward-modulated neurons encoded the reward amount in the neuron's preferred hemifield with a positive slope. In contrast, compared to their responses to low luminance, an approximately equal proportion of luminance-sensitive neurons responded to high luminance with increased or decreased activity, leading to a much weaker population-level response. Meanwhile, in the non-preferred hemifield, the strength of encoding for reward amount and luminance was positively correlated, suggesting the integration of these two factors in the LIP. Moreover, neurons encoding reward and luminance were homogeneously distributed along the anterior-posterior axis of the LIP. Overall, our study provides further evidence supporting the neural instantiation of a priority map in the LIP in reward-based decisions.

ABSTRACT

Objective To study the therapeutic effect of ankylosing spondylitis treated with sacroiliac joint injection under C-arm X-ray guidance. Methods Four hundred and eighty-three patients of ankylosing spondylitis were divided randomly into two groups according to the treatment method. Four hundred and twenty-one cases were in group A:sacroiliac joint injection under C-arm X-ray, the injection medicine were diprospan or triamcinolone acetonide and methotrexate, and with oral sulfasalazine (SASP) as well. Sixty-two cases were in group B: oral medicion SASP and meloxicam alone without injection. Self-evaluation, finger-to-floor distance, changes in Schober's sign, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) before and after treatment of 3 d, 1 week,2 weeks, 3 weeks, 4 weeks and 3 months were observed. Results Two indicators including self-evaluation and finger-to-floor distance at initial 3 d were significantly lower than those before treatment in group A: (6.18 ±0.55) scores vs. (8.89 ±0.53) scores, (30.10 ±3.94) cm vs. (50.20 ±3.93) cm, P<0.05. Schober's sign and occiput-to-wall distance at 1 week after treatment were significantly lower than those before treatment in group A:(3.44± 0.32) cm vs.(3.19±0.47) cm, (9.49±0.68) cm vs. (10.72±2.36) cm, P<0.05. Two indicators including self-evaluation and finger-to-floor distance at 1 week after treatment were significantly lower than those before treatment in group B:(7.07 ± 0.72) scores vs. (9.08 ±0.46) scores, (48.14 ±5.62) cm vs. (50.84 ±4.33) cm, P<0.05. Schober's sign at 4 weeks after treatment was significantly lower than that than before treatment in group B:(3.49± 0.56) cm vs. (3.22±0.58) cm, P<0.05. Occiput-to-wall distance at 2 weeks after treatment was significantly lower than that before treatment in group B:(9.08±1.54) cm vs. (10.11±1.58) cm, P<0.05. Conclusion The method of C-arm X-ray guided sacroiliac joint injection for treatment of ankylosing spondylitis is effective and feasible.

ABSTRACT

ObjectiveTo investigate the incidence and gene mutation spectrum of childhood thalassemia in Sanya city. MethodsThere were 938 children in our hospital screened by routine blood test, C-reactive protein test and hemoglobinelectro-phoresis. Based on the screening results in children with thalassemia and its subtypes, the alpha thalassaemia children were diag-nosed by gap - polymerase chain reaction, while the beta thalassaemia ones were diagnosed by polymerase chain reaction-reverse dot blot, and all the results were retrospectively analyzed.ResultsScreening positive rate of thalassemia anemia was 13.65% (128/938), genetic diagnosis positive rate was 11.41% (107/938). In 107 cases diagnosed with thalassemia by genetic test, 59 were alpha thalassaemia, and 46 were beta thalassaemia, and 2 cases were alpha thalassaemia combined with beta thalassaemia. In 59 cases of alpha thalassaemia, 31 had genotype of - SEA/alpha alpha , 13 cases with genotype alpha 4.2 / alpha alpha, and 6 were of HbH disease. In the 46 cases of beta thalassaemia children, six gene locus mutations were found : CD41-42 (CTTT) in 21 cases, TATAbox - 28 (A - G) in 13 cases, CDs14/15 (G) in 5 cases, IVS - Int 654 (C - > T) in 4 cases, CD17 (A - > T) in 2 cases, and 27-28 (C) in 1 case.ConclusionHigh prevalence of thalassemia in children from Sanya city warrants genetic counseling, premarital check-up and prenatal diagnosis.

ABSTRACT

In this study, a closed-loop controller for chest compression which adjusts chest compression depth according to the coronary perfusion pressure (CPP) was proposed. An effective and personalized chest compression method for automatic mechanical compression devices was provided, and the traditional and uniform chest compression standard neglecting individual difference was improved. This study rebuilds Charles F. Babbs human circulation model with CPP simulation module and proposes a closed-loop controller based on a fuzzy control algorithm. The performance of the fuzzy controller was evaluated and compared to that of a traditional PID controller in computer simulation studies. The simulation results demonstrated that the fuzzy closed-loop controller produced shorter regulation time, fewer oscillations and smaller overshoot than those of the traditional PID controller and outperforms the traditional PID controller in CPP regulation and maintenance.

ABSTRACT

Objective:In the clinical mechanical ventilation practice, the medical workers often set the ventilator parameters only depend on their own experience. In order to change that,we design a Closed-Loop Automatic Mechanical Ventilation Controller.Methods: Design two closed-loop control subsystems to meet the requirements of closed-loop control system. PETCO2 control subsystem used to keep the concentration of PETCO2, and another subsystem design to regulate the respiratory elasticity called PEEP control subsystem. PETCO2 control subsystem can work on its own or work with PEEP control subsystem.Results: Using this closed-loop controller, the partial CO2 pressure and respiratory elasticity of patient was regulated in real time.Conclusion:By using this closed-loop automatic mechanical ventilation controller, the respiratory elasticity was largely reduced with the adequate gas exchange. Therefore, the ventilator using this closed-loop automatic mechanical ventilation controller can improve the ventilation quality and also reduce the risk of lung injury.

ABSTRACT

A fast method composed of the quick, easy, cheap, effective, rugged and safe ( QuEChERS) and temperature-assisted ionic liquid dispersive liquid-liquid microextraction ( TA-IL-DLLME) sample preparation coupled with high performance liquid chromatography ( HPLC ) for the analysis of 5 dyes residues in navel orange was developed. The QuEChERS sample preparation involved the quick extraction with acetonitrile in the presence of anhydrous MgSO4 and NaCl and the purification with primary secondary amine ( PSA ) sorbent. The TA-IL-DLLME sample preparation was processed using 1 mL of the extract obtained by QuEChERS as dispersive solvent and 60 μL of 1-octyl-3-methylimidazolium hexafluorophosphate as extractive solvent under 55 ℃ of water-bath temperature and 12 min of water-bath time. The ultimate solution was detected by HPLC-UV and the contaminated sample was further confirmed by UPLC-MS/MS under multiple reactions monitoring (MRM) mode. The recoveries of five dyes were in the range from 70. 3% to 93. 6% at two spike levels of 0. 01 and 0. 05 mg/kg, the relative standard deviations (RSDs) were between 3. 5% and 9. 2% and the limits of quantification (LOQs) were between 1. 1 and 2. 8 μg/kg.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of carvacrol pretreatment on myocardial ischemia-reperfusion (I/R) injury and its underlying mechanisms.</p><p><b>METHODS</b>Wild-type male C57 BL/6 mice were randomized into 5 groups (n=13), namely the sham-operated group, vehicle (DMSO in saline)+ I/R group, carvacrol (20 mg/kg) + I/R group, carvacrol (40 mg/kg) + I/R group, and carvacrol (60 mg/kg) + I/R group. The mouse models of myocardial I/R injury were established by a 45-min occlusion of the left anterior descending coronary artery (LAD) followed by reperfusion for 2 h. Carvacrol or vehicle was administered intravenously 15 min before LAD occlusion. After reperfusion, the mice were examined for myocardial oxidative stress level and apoptosis rate.</p><p><b>RESULTS</b>Compared with the vehicle group, the 3 carvacrol-pretreated groups showed significantly reduced myocardial infarct size, oxidative stress level and cardiac myocyte apoptosis rate (P<0.01).</p><p><b>CONCLUSION</b>Carvacrol can protect against myocardial I/R injury by inhibiting myocardial oxidative stress and apoptosis in mice.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect of C-reactive protein (CRP) on the expressions of Notch pathway components in human peripheral blood endothelial progenitor cells (EPC) in vitro.</p><p><b>METHODS</b>Mononuclear cells isolated by density gradient centrifugation of human peripheral blood mixed with 6% hydroxyethyl starch (Hes) were plated on fibronectin-coated 6-well culture dishes. After 7 days, the adherent cells were cultured in the presence of 10 and 20 mg/L CRP for 48 h, and the proliferation, migration, and adhesion abilities of the cells were observed. The mRNA expressions of Notch-1 and its ligand Jagged-1 in the EPCs were measured by RT-PCR, and their protein expressions by Western blotting.</p><p><b>RESULTS</b>CRP at 10 and 20 mg/L caused a significant reduction in the number of viable EPCs (61∓3 and 54∓3, respectively) as compared with PBS (71∓4, P<0.05). CRP also resulted in a significant suppression of the proliferation, migration and adhesion capacities of the EPCs. The mRNA and protein expressions of Jagged-1 and Notch-1 in the EPCs significantly increased following CRP exposure in comparison with PBS treatment.</p><p><b>CONCLUSION</b>CRP can suppress the proliferation, migration and adhesion capacities of the EPCs probably by affecting the expressions of the Notch-1 pathway components.</p>

ABSTRACT

Objective To evaluate the influence of nursing intervention on anxiety of surgical patients with cranio-orbital tumors. Methods From November 2006 to October 2009 in our hospital, 48 patients with cranio-orbital tumors surgery were randomly divided into the observation group (25 patients)and the control group (23 patients), the control group received routine care, the observation group strengthened nursing intervention of anxiety on the basis of the control group. Results The total effective rate of the observation group was 92.0%, slightly higher than 87.0% of the control group, but there was no significant difference between two groups. the anxiety score in patients of the observation group was significantly lower than the control group, there was significant difference between two groups. Conclusions Nursing intervention can alleviate anxiety of surgical patients with cranio-orbital tumors, which should be given clinical concern.

ABSTRACT

This paper summarizes nursing care of a patient with wound infection following systemic lupus erythematosus (SLE) accompanied by herpes zoster. Aimed at the patient's characteristics of long-term disease course of SLE,weak immune system caused by long-term use of glucocorticoid and immunosuppressant,big wound area with severe infection,and hard healing,moist therapy was implemented under the theoretical guidance of moist healing environment. The Purilon Gel and Biatain-Ag Foam dressing were applied in the wound combined with systemic use of antibiotics and treatment of SLE. At the same time,psychological nursing and health education were provided for the patient. These measures speeded up the wound healing.

ABSTRACT

Objective To explore the significance and correlation between MCP-1 and cardiovascular complication of diabetes. Methods 65 patients with diabetes and 64 patents with IGT and 60 healthy persons as control group are chosen from a population of 1231residents at Jiangnan community in Guangzhou city. Ultrasonic inspection of carotid artery was applied to the intimae media thickness (IMT),and the levels of-MCP-1 were detected by Elisa. Result There was significant difference in the levels of MCP-1 among the normal control group, IGT group and DM group (P=0.000).The levels of MCP-1 in IGT complicated with AS patients were significantly higher than that in IGT alone patients. The levels of MCP-1 in DM complicated with AS patients were significantly higher than that in DM alone patients. The levels of MCP-1 were positively correlated with IMT in these patients. A forward LR Logistic regression analysis showed that IMT was a dependent variable, gender, MCP-1 and age are independently correlated with IMT. Conclusion MCP-1 is correlated with diabetes and its cardiovascular complication and it may be served as the target of therapy.

Objective To evaluate the clinical value of transcranial Doppler ultrasonography(TCD) in detecting cerebral hemooynamic changes at the early stages of cerebrovascular disease in the diabetics.Methods TCD was used to measure the peak wave in the spectra,the pulsatility index(PI) and the mean blood flow velocity(Vm) of each cerebral artery in 142 normotensive type 2 diabetic patients without clinical symptoms of cerebrovascular disease and 62 age and gender matched healthy controls.Results The incidence of cerebral hemodynamic changes detected with TCD was 87.3% in diabetic patients,whose PI for each cerebral artery was significantly higher than those in the controls.Significant increases in the Vm of the extracranial nternal carotid artery,the middle cerebral artery and the basilar artery were detected in the diabetic patients when compared with the controls,suggesting lowered cerebrovascular compliance and smaller vascular diameter in the patients,which leaded to indicate that the pathological changes had spread to the intemal carotid artery and the vertebral artery system.Conclusion TCD can be helpful in detecting cerebral hemodynamic changes at the early stages of cerebrovascular disease in the diabetics.

ABSTRACT

Objective To investigate the clinical efficacy of transabdominal preperitoneal laparoscopic hernia repair (TAPP). Methods Three trocars were introduced into the abdomen at the umbilicus and both sides 10 cm from the umbilicus, respectively. The parietal peritoneum was opened at 5 cm above the internal ring and near the external border of the umbilicus. The hernial sac was dissected and a patch overlying the defect was stapled with the pectineal ligament and the conjoined tendon. Results The laparoscopic operation was completed successfully in all the 50 cases. The operation time was 20~70 min (39.8?10.5 min). Postoperatively, there were 1 case of groin pain and 2 cases of urine retention. The duration of postoperative hospital stay was 1~4 days (mean, 2 days). Follow-up observations in the 50 cases for 1~11 months (mean, 7 months) found no recurrence. Conclusions Transabdominal preperitoneal laparoscopic hernia repair is a safe and effective tension-free hernioplasty.

ABSTRACT

Objective To investigate the clinical value of wire-guided tubal recanalization under hysteroscope combined with laparoscope for the treatment of occluded oviduct infertility.Methods A total of 63 cases of infertility were diagnosed by hysterosalpingography(HSG) as having bilateral oviduct obstruction,of which at least one side of oviduct was proximally occluded.After laparoscopic exploration and hysteroscopic hydrotubation were performed for clarifying the diagnosis and managing related pelvic lesions,wire-guided tubal recanalization was conducted under hysteroscope combined with laparoscope for the treatment of proximal oviduct obstruction.Results Out of the 63 cases,a total of 118 oviducts were involved(eight oviducts had been previously resected).Of the 118 oviducts,59 were proximally occluded,45 were distally occluded or blinded,and 14 were proximally and distally occluded.After the surgery 99 oviducts were unobstructed.Intrauterine pregnancy was obtained within 1 year in 23 cases(36.6%).There were 25 cases of unpregnancy within 1 year,which were re-examined with HSG,presenting a re-obstruction in 7 cases.Conclusions Wire-guided tubal recanalization under hysteroscope combined with laparoscope is an ideal option for the treatment of occluded oviduct infertility.

ABSTRACT

Objective: To establish a curve of dosage gradient-liver signal intensity of superparamagnetic iron oxide (SPIO) in normal rats and to find an appropriate dose for enhancement. Methods: Seventy-two SD rats, 4 rats a group at random, underwent MR enhancement with 0, 2, 5, 8, 10, 12, 15, 20, 30, 40, 50, 60, 70, 80, 100, 140, 210, 280 μmol/kg SPIO respectively after plane examination. The signal-to-noise ratio of liver was measured and a curve of dosage gradient-liver signal intensity was made. Results: (1)With the increase of SPIO dose, the signal intensity of both T1 and T2 weighted images of liver declined. (2)T2 weighted images was more sensitive than T1 weighted images in small dose, the ED50 of T1 weighted was 8 μmol/kg, and the ED50 of T2 weighted was 5 μmol/kg. (3)When the dose was greater than 15 μmol/kg, the signal intensity of T1 weighted images declined more rapidly than T2 weighted, the effect of enhanced T1 weighted images resembled T2 weighted ones, and its images was with more fine resolution. (4)At the dose of 40 μmol/kg, the signal intensity of T1 weighted images approached the background noise, and at the dose of 15 μmol/kg, the signal intensity of T2 weighted images approached the background noise. Conclusion: Good effect of T1 and T2 enhanced MR imaging can be acquired at the dose of 20-10 μmol/kg SPIO respectively, the best contrast-to-noise ratio is found on T2 weighted enhanced MR image.

AIM To study the effect of rutoside (Ru) on blood rheology in murine acute pancreatitis (AP). METHODS Blood rheology, enzymatic levels and pathology of the pancreas were evaluated in all Wistar rats. The pancreatic sections were scored and compared. RESULTS In AP group, the whole blood viscosity, the plasma viscosity, the eryrocyte sedimentation, the hematocrit, the agglutinatin index of red blood cell, and the rigidity index were significantly elevated. The pancreatitis injury was described as evident pancreatic acinar necrosis and infiltration of inflammatory cells. Ru 120, 60, 30 mg?kg -1 (sc) ameliorated the abovementioned adverse pathophysiological changes seen in the control group. CONCLUSION During the course of AP, rutoside ameliorated the blood rheology abnormality and showed a protective effect on pancreatic tissue.

ABSTRACT

Thrombolysis is still an important treatment modality in patients with ST-segment elevation myocardial infarction (STEMI) in a hospital without percutaneous coronary intervention (PCI) facility. However, the optimal treatment after thrombolysis is still unclear. Conservative treatment after thrombolysis was recommended in the past. "Guidelines for percutaneous coronary intervention (2009)" also suggested that the condition of STEMI patients should be evaluated to determine whether the patient should be transferred to a hospital with PCI facility. But this suggestion lacksed evidence-based support. In recent years, several clinical trials suggested that immediate transfer to the nearest interventional centre for PCI was plausible regardless of success or failure in thrombolysis in local hospital, and this might reduce the incidence of cardiovascular events in patients with STEMI.

Objective To investigate the effect of partial sleep deprivation (PSD) on cardiac electric activity and function of vascular endothelium. Methods Eighteen Sprague-Dawley rats were randomly divided into two groups (9 each):PSD group and control group. PSD lasted 10 days,20 hours each day. Body weight and surface electrocardiogram (S-ECG) were examined and recorded 1d before PSD,and 1d,4d,7d and 10d after PSD. Serum concentrations of glucose,total cholesterol (TC),triglyceride (TG),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C),creatin kinase (CK),creatin kinase-MB (CK-MB),high-sensitivity C-reactive protein (hs-CRP),nitric oxide (NO),von Willebrand factor (vWf),endothelin-1 (ET-1) and E-selectin were determined the next day after 10d of PSD.
