ABSTRACT

Objective:Build a research index performance model with discipline growth evaluation, to evaluate the scientific research performance level of various disciplines in hospital more objectively, fairly and dynamically.Methods:Take the research projects, papers, patents and achievements as the main evaluation indicators and establish the hierarchical and classified scoring standards, to form the weight index evaluation model for the total score, per capita score and growth of the discipline research.Results:Using the growth research index to evaluate the scientific research performance of departments and individual researcher between January 2018 and December 2020. Excellent departments were selected for the top 10% of the scientific research index, those whose scientific research scores increased by more than 20% compared with the previous year were selected as the progressive departments, and the top 1% of individual scientific research scores were selected as the advanced researchers, which were commended and encouraged. For the departments ranked in the bottom 5% or whose scientific research index significantly declined compared with previous year, early warning, guidance and supervision were implemented. Since the implementation of the evaluation system, the research performance of disciplines has been significantly improved, and many achievements were made.Conclusions:This evaluation mode can stimulate the enthusiasm of the disciplines and scientific researchers for entrepreneurship and innovation to set up the standards and promote the continuous improvement of the research capacity of the whole hospital.

ABSTRACT

Objective To explore changes of the myeloid derived suppressor cell (MDSC), regulatory T cell (Treg), traditional T cell, and their mechanisms in lung tumor mice. Methods Twenty C57BL/6 mice were randomly divided into the experimental and the normal control groups. The experimental group was injected with Lewis lung cancer cells (LLC, 100μL 1 × 106) subcutaneously to prepare the lung tumor model mice, the normal control group was given the same amount of saline (NC). Spleen cells were obtained from LLC and NC groups. Flow cytometry was used to detect the ratio and number changes of MDSC, Treg, CD4+and CD8+T cells in the lung tumor of mice. CD4+and CD8+T cell apoptosis were detected by Annexin-Ⅴstaining, and their proliferation were detected by 5-bromine deoxidization uracil nucleoside (BrdU) incorporation. Results Compared with normal control mice, the ratio and number of MDSC in spleen increased significantly in LLC group (P<0.01), in addition, the ratio of CD4+Foxp3+Treg in CD4+T cells and their number in spleen increased significantly in LLC group. However, the ratio and number of CD4+and CD8+T cells in spleen decreased significantly in LLC group (P<0.05). The proliferation of CD4+and CD8+T cells decreased significantly in LLC group compared with that of NC group (P<0.05), while the apoptosis of CD8+T cells increased significantly (P<0.05). Conclusion MDSC and Treg cells increase in lung tumor model mice, which inhibit proliferation of CD4+and CD8+T cells and promote apoptosis of CD8+T cells.

ABSTRACT

BACKGROUND:Lots of in vitro experiments have explored the toxicity of upconversion nanoparticles, but its toxicity in vivo is little reported. OBJECTIVE:To investigate the toxicity of upconversion nanoparticles in mouse organs. METHODS:After tracheotomy, 36 Balb/c mice were randomly divided into three groups, fol owed by instil ed with 28 mg/kg upconversion nanoparticle (experimental group), the same volume of normal saline (control group), and nothing (sham operation group), respectively. The functional changes of the lung, liver and kidney were detected at 1 day, 1 and 2 weeks postoperatively, and meanwhile, the morphological changes of the lung, liver, kidney, and heart were observed. RESULTS AND CONCLUSION:At 1 day postoperatively, the pH values in the experimental group were lower than those in the control and sham operation groups (P<0.05), while the glutamic-pyruvic transaminase level was higher than that in the control and sham operation groups (P<0.05). The oxygen partial pressure in the sham operation group was higher than that in the other two groups at 1 day postoperatively (P<0.05). The oxygen partial pressure and glutamic-pyruvic transaminase level did not significantly differ among groups at 1 and 2 weeks postoperatively. The carbon dioxide differential pressure and kidney function showed no significant differences among groups at different time points after surgery. At postoperative 1 day, in the experimental group, hyperplasia and inflammation were most obvious, distorted alveolar cavity and congestion of blood vessels were visible. In the control group, obvious hyperplasia and inflammation were found, the alveolar cavity was crimped and the gap between alveoli was broadened. The sham operation group had normal alveoli with no inflammations. Lung lesions in the experimental and control groups became mild with time at postoperative 1 and 2 weeks. One day postoperatively, hepatocyte swel ing and vacuolar degeneration were severer in the experimental group. Moderate hepatocyte swel ing and vacuolar degeneration occurred in the control group. The sham operation group showed mild hepatocyte swel ing and vacuolar degeneration. The morphology of the liver in each group returned to normal at 2 weeks postoperatively. Fortunately, the heart and kidney structure showed no overt changes in each group. These findings suggest that upconversion nanoparticles cause transient damage to the mouse lung and liver.

ABSTRACT

Objective To explore the effect of p21Waf1/Cip1 methylation changes on the process of cellular senescence .Methods Bisulfite sequencing was used to analyze the methylation changes of p21Waf1/Cip1 in the process of cellular senescence;p21Waf1/Cip1 ex‐pression was detected by RT‐PCR and Western‐blot ;Middle‐aged 2BS cells was treated by 5‐aza‐CdR and cellular senescence was detected by MTT and SA‐β‐Gal staining .Results Bisulfite sequencing analysis of p21Waf1/Cip1 promoter showed that CpGs were methylated by 1 .25% in the young 2BS cells ,by 27 .27% in the middle‐aged 2BS cells ,while only by 0 .64% in the senescent cells . The expression of p21Waf1/Cip1 was low in the young(28 PD) 2BS cells ,it increased first(35 PD) but decreased later in the middle‐aged(42 PD) cells .In the senescent 2BS cells ,p21Waf1/Cip1 expression was further increased .5‐aza‐CdR treatment resulted in de‐creased growth rate but increasedβ‐Gal staining of middle‐aged 2BS cells .Conclusion The process of cellular senescence is regula‐ted by the status of p21Waf1/Cip1 methylation ,and p21Waf1/Cip1 demethylation accelerates cellular senescence .

ABSTRACT

Objective To investigate the effect evaluation of spironolactone in the treatment of hypertensive heart disease by the real time 3-dimensional echocardiography .Methods Totally 104 patients with hypertensive heart disease were in the study from Jan 2011 to Jan 2012 of our department and divided into objective group and control group .Conventional treatments were put in all the patients and the spironolactone was used in objective group patients only .The results of ventricular morphology and function indexes were compared be -tween the two groups, and then investigate the relationship between cystatin C (CycC) and left ventricular remodeling index (LVRI) at the same time.Results There were significant differences in the comparison of ventricular morphology and function indexes .The left ventric-ular end-diastolic volume(LVEDV), left ventricular end systolic volume (LVESV) and left ventricular ejection fraction (LVEF) of ob-jective group were more advantageous than flowse of control group for prognosis .The same results were observed in the comparison of Cys C and LVRI.Moreover, Cyc C and LVRI had certain correlation in the study .Conclusion The spironolactone can play a positive role in the treatment for the patients with hypertensive heart disease .In the meantime, the RT-3DE is a good evaluation method for the develop-ment of ventricular morphology and function .

ABSTRACT

Background and purpose:To perform whole mount technique in the diagnosis of the prostate cancer could provide orientation to the specimen. Whole mount technique has great value in pathologic diagnosis and morphological research. However, limited by the specimen-making technique, shortage of equipment and heavy workload, this technique has not been generally accepted in China. The aim of this study was to evaluate the signiifcance of whole mount technique in the diagnosis of the prostate cancer by comparing the clinical and pathological variables between whole mount patients and conventional ones after radical prostatectomy (RP).Methods:A total number of 229 patients’ whole mount RP specimens were recruited in the study from Dec. 2012 to Feb. 2014. The control group included 393 patients’ specimens which underwent conventional sampling from Jan. 2010 to Jun. 2012. We compared the clinical and pathological variables between the groups, including age, preoperative PSA level, methods of diagnosis, preliminary diagnostic Gleason score, clinical T stage, postoperative Gleason score, pathological T stage, positive surgical margin, extraprostatic extension, seminal vesicle invasion and pelvic lymph node metastasis.Results:Two groups shared similar preoperative parameters. Also there was no signiifcant difference between the whole mount and the conventional sampling groups in postoperative Gleason score, pathological T stage, extraprostatic extension and pelvic lymph node metastasis. However, positive surgical margin and seminal vesicle invasion rates were much higher in the whole mount group than the control one and both of the differences reached statistical signiifcance (26.2%vs 17.6%, 23.1%vs 17.0%;P=0.010, 0.025)Conclusion:After compared the clinical and pathological variables, we could conclude that whole mount technique has prevalence in the diagnosis of the positive surgical margin and seminal vesicle invasion compared with the conventional sampling technique. Thus, whole mount technique should be strongly recommended in the diagnosis of prostate cancer.

ABSTRACT

Objective To investigate the effect of family nursing intervention on infant brain injury rehabili-tation therapy. Methods 102 cases of brain injury infants were randomly divided into observation group(32 cases),intervention group(38 cases) and control group (32 eases), respectively. The patients in observation group received routine treatment, while those in intervention group received family nursing intervention based on routine treatment.The children in control group whose parents refused the comprehensive rehabilitation therapy. All subjects were as-sessed by Gesell test before and after rehabilitation treatment. Results Compared with the control group patients, all indicators in the observation group and the intervention group were significantly improved. There were also signifi-cant differences between observation group and the intervention group( P < 0.01 ). Conclusion Family nursing in-tervention can significantly improve the effect on infant brain injury rehabilitation therapy.

ABSTRACT

Objective To explore the effects of low frequency electric stimulation at bilateral mastoid processes on motor function and cerebral blood flow of children with cerebral palsy (CP). Methods Ninety children with CP were randomly divided into two groups: an electric stimulation group (group 1) and an conventional rehabilitation control group (group 2), thirty healthy children served as the normal control group. The children of group 1 were treated with FES in addition to the routine rehabilitation treatment. The children of group 2 were given of the routine rehabilitation treatment only. All the patients were treated successively with the above protocol for 3 months. The clinical effect and motor development were evaluated with the gross motor function measure (GMFM), and the blood flow velocities of anterior cerebral artery (ACA), middle cerebral artery (MCA), posterior cerebral artery (PCA) were measured by transcranial doppler (TCD) ultrasound before and after treatment. Results In group 1, significant or some improvement were achieved in 19 and 24 CP children, respectively, with an effective rate of 95.6% . In group 2, significant or some improvement were achieved in 10 and 27 CP children, respectively, with an effective rate of 82.2%. ABSTRACT

<p><b>OBJECTIVE</b>To study the morphologic and immunohistochemical features of gastrointestinal stromal tumors (GISTs) and to explore the reference parameters for malignancy.</p><p><b>METHODS</b>Seventy six (76) cases of primary GISTs were distinguished from a group of gastrointestinal mesenchymal tumors by use of a panel of antibodies such as CD117, CD34 by immunohistochemical EnVision method, their biologic behaviors were analyzed by including their follow-up data.</p><p><b>RESULTS</b>All patients were adults, age range 32 to 81 years (mean 54 year), male 39 cases and female 37 cases; the tumors were situated in stomach (36 cases), in small intestine (23 cases), colon (2 cases) and rectum (15 cases). The most common symptoms were abdomen mass, vague pain and GI bleeding. Forty eight (48) cases were mainly located within the muscularis propria, 25 cases outside the serosa, and 3 cases below the mucosa. Grossly, they were of soft consistency often with hemorrhage, cystification or necrosis. Microscopically, the tumors were composed of spindle cells (46 cases) or epithelioid cells (9 cases) and of both cells (21 cases), arranged in interlacing fasicles, diffusing sheets, pallisading, whirling, alveolar and giant pseudo-rosette shapes. Tumor cells often had abundant cytoplasm with light to moderate eosinophilic or slight basophilic in staining, the nuclei generally showed spindle, blunted ends, round or signet in shape with nucleoli. Immunohistochemically, CD117 and CD34 showed diffuse strong expression, the positive rates were 98.7% and 68.4% respectively, alpha-SMA, MSA, S-100, PGP9.5 showed focal expression, the positive rates were 25.0%, 19.7%, 23.7% and 17.1% respectively, vimentin were all positive and desmin, GFAP, NF were all negative. Nine cases were benign, 19 cases borderline and 48 cases malignant. Follow-up of 20 cases with benign and borderline tumors found patients alive without tumor. In the malignant group of 34 cases, 10 cases were alive without tumor, 10 cases developed recurrence or metastasis, and 14 cases died of tumor. Coagulative necrosis, mitotic activity over 10/50HPF, high cellularity and obvious pleomorphism were all in the malignant group. In this group, tumor necrosis, adhesion in operation, tumor, over 5 cm in diameter, mitotic activity over 5/50HPF had significant differences among three groups and the 3 years survival rate had a significant difference in tumors with or without coagulative necrosis and also in tumors with or without mitotic activity over 5/50HPF.</p><p><b>CONCLUSIONS</b>GISTs predominantly occurred in middle aged or old patients, the tumors had varied cell types and different arrangements, the immunohistochemical characters were positive for CD117 and CD34, negative for desmin, occasional positive for alpha-SMA, MSA, S-100 and PGP9.5, which were helpful to differentiate GIST from leiomyomas and Schwannomas. Coagulative necrosis, mitotic activity over 10/50HPF, high cellularity with obvious pleomorphism were also helpful parameters for diagnosis of malignancy aside from metastasis and invasion. Adhesion, over 5 cm in diameter and mitotic activity over 5/50HPF but less than 10/50HPF might be the potential malignant parameters.</p>

ABSTRACT

Objective To investigate the inhibitory effect of maspin gene on metastasis and invasion of gastric cancer and its mechanism.Methods Maspin gene was ligated to the expression vector PCR2.1 with T4 DNA ligase after the PCR2.1 was digested by HindⅢ/XbaⅠ.The recombinant vector maspin/PCR2.1 was transfected into human gastric cancer cell lines MKN28 and SGC7901,then the mRNA and protein expression changes of maspin gene,uPA and uPAR were detected respectively by PT-PCR and Western blotting.Results After identified by digestion and sequencing,the reconstructed plasmid was confirmed to contain the correct and full nucleotide sequence of maspin gene.The expressions of maspin gene were up-regulated in MKN28 and SGC7901 cell lines,while the expressions of uPA and uPAR were down-regulated.Conclusion The expression vector maspin/PCR2.1 is constructed successfully and can be expressed in eukaryotic cells.Expressions of uPA and uPAR can be inhibited by maspin gene.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinicopathologic features of CD30-positive sinusoidal large B-cell lymphoma (CD30 + SLBCL) and its relative correlation with Epstein-Barr virus (EBV).</p><p><b>METHODS</b>Two cases of CD30 + SLBCL, a 65-year-old men and a 85-year-old women were morphologically and immunophenotypically analyzed. EBV status was also evaluated through not only the polymerase chain reaction (PCR) amplification to the EBV Bam HIW DNA sequence, but also an immunohistochemical detection of the latent membrane protein 1 (LMP1).</p><p><b>RESULTS</b>The patients presented with similarly superficial lymphadenopathy. One of them died of the tumor within 10 months. Microscopically, both of the neoplasms were characterized by a cohesive sinus growth pattern and the monomorphic cytology of the tumor cells. Immunohistochemically, They were both positive for CD45, CD30, and CD20 or CD79alpha, whereas neither expressed EMA, ALK1, nor any histiocytic/T-lineage markers. No evidence of EBV-infection could be found either.</p><p><b>CONCLUSIONS</b>CD30 + SLBCL is a morphologically and immunophenotypically distinctive variant of diffuse large B-cell lymphoma, which should be distinguished from T/null cell type anaplastic large cell lymphoma and some other nodal lesions with a predominantly sinusoidal infiltrative pattern. CD30 + SLBCL may not be correlation with EBV.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinicopathological, immunohistochemical, and molecular biologic characteristics of esophageal stromal tumors and smooth muscle tumors.</p><p><b>METHODS</b>Twenty four cases of esophageal mesenchymal tumors were reclassified by a panel of antibodies such as CD117, CD34 etc. The sequence of 11 exon of c-kit gene were detected in some cases.</p><p><b>RESULTS</b>There were 3 cases of esophageal stromal tumors, 20 leiomyomas, and 1 leiomyosarcoma. The 3 esophageal stromal tumors occurred in 3 men aged 71, 56 and 60 years respectively. The tumors originated from muscularis propria with the size of 4 cm, 8 cm and 14 cm in diameter. Microscopically, the tumor cells were spindle and epithelioid shaped with slightly basophilic appearance, arranged in intersecting fascicles, diffusing and palisading patterns. Immunohistochemically, the tumors were positive for CD117 and CD34. The mutation of 11 exon of c-kit gene was detected in one case. In comparison, esophageal leiomyomas occurred in a younger population. The age ranged from 30 to 60 years (mean age 41.6 years), 12 male cases, 8 female cases. 15 cases of esophageal leiomyomas were intramural tumors with a diameter of 0.8 - 10.5 cm (mean 4.5 cm) originating from muscularis propria and 5 cases which were intraluminal polyps with a diameter of 0.2 - 1.0 cm originating from muscularis mucosae. Leiomyomas were strongly eosinophilic in appearance, diffuse positivity for alpha-SMA, MSA, and desmin, and no c-kit gene mutation. One male case of leiomyosarcoma had a diameter of 5 cm and originated from muscularis mucosae and displayed a sausage-shaped polyp.</p><p><b>CONCLUSIONS</b>Leiomyoma is still the most common mesenchymal tumor of the esophagus, the stromal tumor can be similar to gastrointestinal stromal tumors. Typical esophageal leiomyosarcoma is very rare and has different clinicopathologic and molecular biologic features.</p>

Objective To explore relationship between Liver and Spleen of Tradicational Chinse Medicine in T cell activation. Methods The expression of IFN-r and IL-4 of T cells in asthenic spleen qi model and hepatic depression model was detected by RT-PCR. Results IFN-r level of T cells in asthenic spleen qi model significantly decreased, in hepatic depression model significantly increased, in asthenic spleen qi model treated with decoction of four mild drugs rose to normal range, and in hepatic depression model treated with powder of bupleuri dispersing stagnated hepatic qi fell to normal range. The IL-4 level of T cells in asthenic spleen qi model significantly increased, in hepatic depression model significantly decreased, in asthenic spleen qi model treated with decoction of four mild drugs fell to normal range, and in hepatic depression model treated with powder of bupleuri dispersing stagnated hepatic qi rose to normal range. Conclusion Liver and spleen are related to the regulation of THO cells functions.

Objective To construct a recombinant eukaryotic vector expressing maspin cDNA,and to explore the effect of maspin overexprssion on the proliferation of human gastric carcinoma cell line SGC7901.Methods The fragment of maspin gene was amplified by polymerase chain reaction(PCR).An eukaryotic vector expressing maspin(maspin/PCR2.1)was constructed with PCR2.1,and transfected into SGC7901 cells.The expression of maspin at mRNA and protein level was detected by RT-PCR and Western blotting.The proliferation of SGC7901 cells was observed by MTT.Cell cycle was analyzed by flow cytometry.Results Recombinant plasmid maspin/PCR2.1 was constructed and transfected into SGC7901 successfully.The mRNA and protein levels of maspin were significantly higher in the maspin/PCR2.1 group(33.6?1.2,23.4?1.6)than that in the blank PCR2.1(15.0?1.5,12.3?1.5)and the untreated group(13.7?2.0,12.0?1.3)(P
