ABSTRACT

Objective To forecast the number of healthcare professionals at China′s primary medical institutions from 2016 to 2020 ,so as to provide the healthcare authorities with references for optimizing healthcare human resource allocation .Methods The Grey model was used to predict numbers of healthcare professionals in China from 2016 to 2020 on the base of health technicians data from 2010 to 2015 .Results The forecast results show that the number of healthcare professionals may be increased to about 600000 people a yearfrom 2015 to 2020 .The number of healthcare professionals at China′s primary medical institutions is expected to grow about 204000 people a year ,and general practitioner is increased to about 22000 people a year .The annual growth rates of medical(assistant) practitioners ,medical practitioners ,registered nurses ,pharmaceutical personnel and examiners in China are expected to be 3.20% ,2.81% ,6.88% ,1.53% and 2.16% respectively. The annual average growth rates of postgraduates ,undergraduates and junior college graduates are expected to be 16.95% ,12.08% and 5.46% respectively .Conclusions in the future ,the requirements for healthcare professionals at primary institutions will still be higher than demand in China ,with a gap to be filled and their professional makeup and academic title make to be optimized. Therefore greater efforts are required for their development to enhance their competence and professionalism ,with reasonable staffing of the human resources .

ABSTRACT

Objective: To search for the weighting the doctor’s service value and method to give the payment to doctors. Methods:From the hierarchy of medical service items, medical service items are classified, the weight of medical service item are calculated by analytic hierarchy process (AHP) , the Harvard University doctor workload measurement model is established. Results: According to the weight of medical service items, to calculate the workload of doctors integrated with practical work quantity. Conclusion:By applying the theoretical results into practices, the estimation system of doctor workload was smoothly operated in the organization.

ABSTRACT

An indexing system is constructed,based on the implementation of prevention and control technology for chronic non-communicable diseases in community health service institutions,using both literature review and Delphi method,and using the Analytic Hierarchy Process (AHP) to calculate the weight of the indexing system.The system comprises three level-1 indexes,8 level-2 indexes and 33 level-3 indexes.The research proposed a transition from emphasis of therapeutic means to prevention,and emphasis on prevention and control technologies to the high-risk population in the prevention and control technology of chronic non-communicable diseases,along with development of techniques and measures to encourage behavioral changes of the population with chronic diseases.

ABSTRACT

Objective: To analyze the efficiency of chronic disease prevention and control of primary health service institutions in Shandong, and provide policy references. Methods:Using CRS and VRS models from DEA to analyze the representative input and output indicators in primary health Service institutions. Results: The overall efficiency of chronic disease prevention and control in primary health service institutions is low, and the actual output was insufficient compared with the target output. Conclusion: Strengthening chronic disease control measures in high-risk group and establishing management mechanism leaded by the goal of chronic prevention and control in order to achieve optimal allocation of resources.

Through the prospective of salary system for medical personnel, it analyzed the current situation and problems of the low salary of medical staff, the unreasonable distribution of salary elements, the imbalanced structure of salary and so on. It also put forward the following proposals as to determine the reasonable salary with dynamic regulation for the medical staff, to ascend the position value and to establish a guaranteed and flexible salary structure and so on.

ABSTRACT

Excessive and fast rise of medical expenses in China recent years has worsened the financial burden of both the government and individuals alike,making it a hotspot issue countrywide.To alleviate the difficulties of "accessibility to medical services" and "poverty or repoverty resulting from sickness" faced by the Chinese people at large,we built a multi-factor analysis model to identify key factors contributing to such a fast rise of medical expenses based on an analysis of the medical expenses in Shandong Province.In addition,we discussed the causes for GDP and headcount of medical staff in relation to the rise of medical expenses,and the strategies to curb expenses.

With the further expansion of the connotations of the quality of medical services and the establishment of the bio psycho social medical model, it has become necessary to redefine quality evaluation of medical services. The authors give their interpretation of the quality of medical services and its evaluation from several perspectives, discuss the goals of quality evaluation of medical services, and expound in a rational manner the entire process and content of quality evaluation. On the basis of this, they make a comment on previous quality evaluation units and methods and put forward the tentative idea of conducting overall quality evaluation of whole process medical services.

ABSTRACT

The paper explicitly defines and classifies the medical quality evaluation indexes from the aspects of uses, sources and nature and analyzes problems and shortcomings in the current medical quality evaluation index system from such perspectives as the coverage, specificity, sensitivity, reliability, and scientificalness of the index system and the collection and application of the indexes. On the basis of this, the paper expounds the future development trends of the index system and the principles and methods employed in determining the indexes. It points out that it is necessary ① to further strengthen the integration between the medical quality evaluation indexes and the evaluation units, ② to reinforce the quality evaluation of the links, ③to evaluate medical quality in a comprehensive way from different angles, ④ to further develop mathematical evaluation models related to the statistical indexes, ⑤ to enhance the applicability of the quality evaluation index system in institutions of different grades and scales.

ABSTRACT

Objective To evaluate the efficiency of the township health centers with the appropriate use of data envelope analysis (DEA) so as to provide methodological basis for such evaluation. Methods The composite indexes of the township health centers medical and preventive service output were designed by means of service equivalent and expert consultation and the township health centers relative efficiency scores were calculated by means of DEA. Results There were 9 units where DEA was effective, accounting for 5 4% of all the units, and the township health centers DEA scores took a downward turn year by year. Conclusion ① By developing the composite indexes of the township health centers service output, the present study has put more preventive and medical service output information in the DEA output index system. ② The overall efficiency of the township health centers is pretty low and is moreover taking a downward turn year by year.

ABSTRACT

The paper analyzes the potential drawbacks of the current medical liability insurance from the perspectives of hospitals, insurance companies and patients. It puts forward suggestions for the sustainable development of medical liability insurance, viz. involvement of insurance brokers in coordinately solving the issue of application for medical liability insurance, involvement of loss assessors in solving medical disputes, adoption of compulsory medical liability insurance in due time, and further expansion of the scope and categories of insurance.

ABSTRACT

Objective To explore effective excitation and restriction mechanisms for public hospital managers under market economy conditions and establish innovation mechanisms for hospital management posts. Methods On-the-spot surveys, literature reviews and discussions were conducted to collect data, which were then analyzed using such statistical methods as description and principal component analysis. Results Investigations into existing excitation and restriction methods and purposes for public hospital managers led to the discovery of some problems in the current mechanisms and suggestions were put forward for establishing on the basis of reform in the property right system new distribution mechanisms, competition mechanisms for appointing hospital managers, and innovation mechanisms for adopting human capital management with regard to hospital management posts. It was also suggested that restriction of hospital managers' powers by rules and health policies be reinforced. Conclusion It is proposed that excitation and restriction mechanisms that fit in with market economy conditions be established.
