ABSTRACT

Background and Objectives: Various studies have proven the effect of closure technique on the repair outcome. But there are few studies to investigate the functional and cosmetic outcomes related to different closure techniques. As regards to method of wound and ruptures closure has effects on healing process and choosing the best technique by physician has the priority, the aim of this study is to compare three different suture techniques


Materials and Methods: In this study, sixty patients with anterior cruciate ligament rupture were studied. After subcutaneous tissue closure, skin incision was sutured by three different methods includings: Simple technique, vertical mattress or semisubcuticular suture techniques. To evaluate the cosmetic outcomes visual analogue scale was used. During the study wounds were observed for inflammation, infection or discharges


Results: In semisubcuticular method there was kr167 with p<0.001 between second and third observer, k=0.35 [p-A.001] between first and second observer and kA.22 [p4].001] between first and third observer. In simple technique group the result of second and third observer there was k=0.61 [p=0.01] and kappa results of first and second observer and first and third observer of 0.19 and 0.17 [p-A.002] respectively. The maximum calculated kappa for vertical mattress technique was k=0.54 [p<0.001] between second and third observer. All sutures were free of infection and dehiscence


Conclusion: It seems that semisubcuticular technique has better cosmetic results to other two techniques. So, using this closure technique may increase the patients' satisfaction from operation

ABSTRACT

Anterior Cruciate Ligament [ACL] tear is one of the most common sport injuries of the knee and arthroscopic reconstruction by bone drilling and graft placement is the accepted treatment. The aim of this study was to compare the results of two different techniques of ACL reconstruction namely transportal [anatomic] and transtibial [non-anatomic] femoral tunnel drilling. In a comparative clinical study, 60 ACL-deficient patients were operated using transportal ACL reconstruction and compared with another 60 patients treated with transtibial technique. They were compared in terms of performance and ligament stability. Our results showed that comparing the two groups, there were no differences in terms of Lysholm score [transtibial: 96.2 versus transportal: 97.2], post-operative Tegner score [transtibial: 6.3 versus transportal: 6.6] and knee range of motion but IKDC score was significantly better in the transportal group [p<0.001]. Anatomic transportal ACL reconstruction technique yields superior results in terms of knee ligament stability comparing to non-anatomic transtibial femoral tunnel drilling

ABSTRACT

Nowadays, it is well known that the attention-deficit/hyperactivity disorder [ADHD] is not confined to children and adolescents. Recent data showed that a considerable portion of the general adult population may be affected by ADHD. On the other hand, the impact of ADHD on driving performance, a major area of adult life, has gained enthusiasm. More recent studies revealed an association between adult ADHD and undesirable driving problems. This study was performed to determine the association between presence of adult ADHD and traffic injuries. In this case-control study, in a 13 month period, 140 subjects [70 drivers/riders injured in traffic accidents, and 70 age- and sex-matched non traumatic controls] were selected to participate in the study and were placed in two groups. Subjects with psychiatric comorbidities were excluded. The Conners' Adult ADHD Rating Scale [CAARS] self-report [screening version] was used for screening adult ADHD in both groups. Finally, the occurrence of this condition was compared between the case and control groups. Sixty- nine males and one female were recruited in each group with a mean age of 29.31 +/- 9.32 [18-61] years in the case and 29.03 +/- 9.07 [range: 18-60] years in the control groups [P > 0.05]. Previous history of traffic accident was significantly higher in the case group [15.7% vs. 4.3%, P=0.024]. The mean driving time in a day was also significantly higher in the case group. In the case group, the scores of CAARS [the ADHD index, ADHD symptoms total, inattentive subscale and hyperactive/impulsive subscale] were positive [higher than 70] in 4.3, 10, 7.1 and 10 percent of patients respectively. Among the controls, the corresponding proportions were 4.3, 14.3, 8.6 and 8.6 percent respectively. The two groups were comparable for the mentioned rates. The results of this study revealed no significant association between ADHD and traffic injuries in Tabriz- Iran
