ABSTRACT

Introduction: mass contamination by the new coronavirus caused enough damage for the pandemic to extend indefinitely Objective: the objective of this study was to analyze the emotional factors acquired by students from a dentistry course during the COVID-19 pandemic period and the influences related to the resumption of face-to-face activities. Material and method: it is an observational, cross-sectional and analytical research conducted at a public university in the state of Pará, Brazil. A research questionnaire, prepared by the authors and validated by the ethics committee of the Federal University of Pará, was applied to the academic community for a period of three months (November 2020 to January 2021). The analysis was performed using the Excel program, Kappa statistics and the Chi-squared test, whose results were presented as frequency and percentage. Result: a total of 188 dental students participated in the research. The findings proved to be in accordance with the hypothesis generated by the researchers, about 19.2% had emotional crises and 3.7% increase in the lockdown period. As for the expectations for care services, there was an increase in the oppressive state of psychic crises (anxiety, stress, anger, fear), considered influential in the cognition of behavioral traits during post-pandemic care. Conclusion: the study identified that the emotional changes acquired in the pandemic outbreak had a negative impact on student performance; however, depressive symptoms were expressed in graduating students.


Introdução: a contaminação em massa pelo novo coronavírus promoveu danos suficientes para que a pandemia se estendesse por tempo indeterminado. Objetivo: analisar os fatores emocionais adquiridos por estudantes do curso de odontologia no período pandêmico COVID-19 e as influências relacionadas ao retorno das atividades presenciais. Material e método: pesquisa observacional, transversal e analítica, realizada em uma universidade pública do estado do Pará. Foi aplicado na comunidade acadêmica um questionário de pesquisa, próprio, validado pelo comitê de ética da Universidade Federal do Pará, por um período de 3 meses (novembro de 2020 a janeiro de 2021). Para a análise foi pelo programa Excel, estatísticas Kappa e o teste Qui-quadrado exibidos por frequência e porcentagem. Resultado: participaram da pesquisa 188 alunos de odontologia. Os resultados obtidos na pesquisa foram de acordo com a hipótese gerada pelos pesquisadores, cerca de 19,2% e 3,7%, apresentaram crises emocionais elevando-as no período de confinamento. Quanto as expectativas para o atendimento, houve aumento de estado opressivo de crises psíquicas (ansiedade, estresse, raiva, medo), consideradas influentes na cognição dos traços comportamentais durante o atendimento pós pandemia. Conclusão: o estudo identificou que as alterações emocionais adquiridas no surto pandêmico desencadearam impacto negativo no rendimento estudantil, todavia, a sintomatologia depressiva esteve expressa em acadêmicos concluintes.

ABSTRACT

While the deploy planning, we come across morphological changes in the intervention area and the bone volume, which is extremely important for the treatment outcome. The objective of this study is to describe the technique "Split-Crest" through a clinical case in which success was obtained. Demonstrate indications and advantages of the technique in order to gain a significant increase in bone density to achieve an excellent aesthetic and functional result. Female patient, leucoderma, 46 years old, attended the Dental Clinic of Specialization in Implantodontics of ESAMAZ, with report of early teeth loss and poorly adapted prosthesis in the upper anterior region of the pre jaw. During the clinical evaluation was observed Edentulism in the anterior arch of the jaw. Total tomographic filming was done on the jaws, the initial diagnosis was a severe resorption present in the premaxilla requiring regenerative process to achieve bone gain. During surgery the patient had immediate installation of previous implants in the region of the elements 12 and 22 through the technique of "Slplit-Crest", which consisted of two vertical cuts on the vestibular cortical portion and longitudinal osteotomy followed by the the shift vestibular cortical-spongy plate. Eight weeks later, the patient had clinically significant bone gain, proving the viability and success of the technique. A minimally invasive surgical procedure with well executed manipulation of tissue grafts and accelerates the clinical outcome, the working hours by the end of the treatment is smaller, has lower morbidity and lower operating costs.


Durante o planejamento do caso, foram encontradas alterações morfológicas na área de intervenção e do volume ósseo, que se torna extremamente importante para o resultado do tratamento. O objetivo deste estudo é descrever a técnica "Split-Crest" através de um caso clínico em que o sucesso foi alcançado. Assim, demonstrar indicações e vantagens da técnica, a fim de obter um aumento significativo na densidade óssea para obter um excelente resultado estético e funcional. Paciente do sexo feminino, leucoderma, 46 anos, procurou a Clínica de Especialização em Implantodontia da ESAMAZ, com relato de perda precoce de dentes e prótese mal adaptada na região anterior superior da pré-maxila. Durante a avaliação clínica foi observado o edentulismo no arco anterior da mandíbula. Solicitou-se uma Tomografia Computadorizada dos maxilares, e o diagnóstico inicial foi uma reabsorção grave presente na pré-maxila que requer processo regenerativo para obter ganhos ósseos. Durante a cirurgia, o paciente apresentou instalação imediata de implantes anteriores na região dos elementos 12 e 22 por meio da técnica de "Slplit-Crest", que consistiu em dois cortes verticais na porção cortical vestibular e osteotomia longitudinal seguido pela cortical vestibular - placa de repouso. Oito semanas depois, o paciente apresentou ganho ósseo clinicamente significativo, provando a viabilidade e o sucesso da técnica. Um procedimento cirúrgico minimamente invasivo com manipulação bem executada de enxertos de tecido e acelera o resultado clínico, tempo clínico menor, menor morbidade e menores custos operacionais.

ABSTRACT

Every day have been increased the number of patients in the dental office looking for oral reabilitações, many of them being a challenge to the dentist surgeon. Currently, implants are installed on guided bone regeneration, the use of membranes, titanium threads and bone substitutes are key parts for correcting bone defects and to support the implant. Thereunto, the aim of this study was the description of a case addressing the methods and materials used for guided bone regeneration with the use of L-PRF for vertical bone gain. The patient showed the absence of the elements 22, 23 and 24, and a bony defect arc in this region. The installation of the implants, followed by guided bone regeneration support with a titanium mesh was indicated. The use of guided bone regeneration technique involving the mesh titanium implants, bone and lyophilized bovine L-PRF membrane showed a viable technique, the succeeding vertical bone gain until the implant platform.
