ABSTRACT

<p><b>OBJECTIVE</b>To compare acoustically evoked short latency negative responses (ASNR) elicited from normal and profound hearing loss guinea pig ears and to confirm their vestibular nuclei origination.</p><p><b>METHODS</b>Forty healthy guinea pigs were employed in the experiment, which were randomly divided into the control group (8 subjects, 16 ears), the masking group (16 subjects, 32 ears) and the deafened group (16 subjects, 32 ears). Air conductive white noise was chosen for masking. Masking dilemma was avoidable by an appropriate 15 dB gap between stimulus and masking sound. Both the masking group and the deafened group were further divided into ASNR group and non-ASNR group based on the presence of ASNR. Electrolytic lesion was conducted to the vestibular nuclei, followed by ABR/ASNR recording. The lesioned brainstem slices were microscopically verified.</p><p><b>RESULTS</b>In the masking group, ASNR were present in 24 ears (75.0%, 24/32) and 12 ears (46.2%, 12/26) in deafened group, showing statistically higher presence rate for masking group (χ(2)=5.07, P=0.024). There were no significant differences for the ASNR threshold and latency between the masking ASNR group and the deafened ASNR group. For the two ASNR groups, electrolytic destruction to the vestibular nuclei subsequently eliminated the ASNR. Brainstem slice proved the accurate sites of electrolytic lesion.</p><p><b>CONCLUSION</b>Moderate white noise masking prevents hearing system potential overlap without affecting vestibular system, therefore, ASNR is successfully elicited in normal guinea pigs. Both ASNRs from normal and deafened guinea pigs are of similar natures and origination from, the vestibular nuclei.</p>

ABSTRACT

OBJECTIVE@#To evaluate the inhibitive effect of matrine modification X on the growth of human nasopharyngeal carcinoma CNE2 cell xenografts in nude mice.@*METHOD@#Tumor model was established by subcutaneous inoculation of nasopharyngeal carcinoma cell CNE2 into nude mice, which was used to evaluate the antitumor effect of matrine modification X in vivo. The expression levels of Bax, Bcl-2, Caspase3 were detected by real-time PCR and western blot.@*RESULT@#The growth of xenografts in nude mice was significantly suppressed after application of matrine modification X in a dose-dependent manner. The inhibition rates were 32.55% and 44.89% when treated at medium and high dose respectively. Real-time fluorescence quantitative-PCR and Western Blot results showed that the expression of Bax and Caspase3 increased, while the expression of Bcl-2 decreased in a dose-dependent manner. The change of high dose group was obvious, and the difference was statistically significant (P < 0.05).@*CONCLUSION@#Matrine modification X could significantly inhibit the growth of human nasopharyngeal carcinoma CNE2 cell xenografts in nude mice, probably by inducing the apoptosis of nasopharyngeal carcinoma cells, and the possible mechanism is related to regulating the expression level of Bax/Bcl-2 and Casepase3.

OBJECTIVE@#To evaluate the inhibited effect of matrine on human nasopharyngeal carcinoma CNE2 cells and the expression of apoptosis-related gene Caspase-3 mRNA, Caspase-3, Caspase-8, Caspase-9 protein. And to explore the inhibiting effect of matrine on the apoptosis of human nasopharyngeal carcinoma CNE2 cells.@*METHOD@#In vitro experiments, MTT assay was used to detect the effect of matrine on CNE2 cell proliferation with different concentration. The expression levels of Caspase-3 were detected by real-time PCR. Western Blot was used to gauge the levels of Caspase-3, Caspase-8 and Caspase-9 protein.@*RESULT@#MTT results showed significant inhibitory action of matrine on CNE2 cells proliferation in does-dependent manner, which could up-regulate the expression of Caspase-3 mRNA and Caspase-3, Caspase-8, Caspase-9 protein in a dose-dependent manner.@*CONCLUSION@#Matrine could inhibit CNE2 cells proliferation in a does-dependent,that was closely related to the up-regulation of Caspase-3 mRNA and Caspase-3, Caspase-8 and Caspase-9 protein expression, and to the cascade reaction of Caspase.

ABSTRACT

<p><b>OBJECTIVES</b>Sonotubometry is wildly used in developed countries, offers a easy, versatile and objective way to assess the function of Eustachian tube. The objectives of this study are to develop a low-cost digitalized device.</p><p><b>METHODS</b>The digitalized sonotubometry device was made of a personal computer with sound input and output accessories. The PC sound card, connected to a signal amplifier, exported probe band noise, while feedback sound was collected by a microphone. All 36 volunteers (72 ears) were involved in the sonotubometry test with JK-04A and CHN-08, in the conditions of dry-swallowing and wet-swallowing. The data were statistically analyzed using SPSS 13.0.</p><p><b>RESULTS</b>The basic electroacoustic performance of the CHN-08 was acceptable. CHN-08 (7 kHz) detection and JK-04A consistency of kappa index was 0.472; CHN-08 (8 kHz) and JK-04A consistency of kappa index was 0.487; CHN-08 (7 kHz) and CHN-08 (8 kHz) consistency of kappa index was 0.688.Under the JK-04A, CHN-08 (7 kHz) and CHN-08 (8 kHz) three setups, Eustachian tube opening frequency was significantly lower for wet-swallowing than dry-swallowing, P < 0.05.</p><p><b>CONCLUSIONS</b>A low-cost digitalized sonotubometry is successfully developed. Its electroacoustic performance proved acceptable by authority assessments. The detection rate of CHN-08 is in accordance with that of JK-04A moderately. The study revealed that Eustachian tube is opened more easily by dry-swallowing than by wet-swallowing.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the learning curve of cochlear implantation and its guiding significance for clinical surgery.</p><p><b>METHODS</b>A retrospective analysis of the clinical data of two otologists in early cochlear implant surgeries, including 98 cases of Dr. A and 54 cases of Dr. B.Statistics of the two doctors incidence of complications and operating time. Operation time as index were drawing a, b two groups of cases of scatter plot, the oscillating sine curve model Y = sin(X)-P/2 and logarithmic curve model Y = b1ln(X)+b0 curve fit were analyzed. Then, extract the early 90 cases of surgery by Dr. A which was divided into a, b, c three groups with 30 cases a group. The operating time and complications were further compared and analyzed(SPSS 16.0).</p><p><b>RESULTS</b>From the operation sequence,A and B physicians, early operation cases cost more operating time, and more complications.Learning curve before and after about 30 cases appeared inflection point, showing a rapid decline in period (learning phase) and slow decline period (consolidation phase) in two stages.Group contrast to Dr. A's early 90 consecutive cases, the operating time of Phase b and Phase c decreased significantly than Phase a (P(a-b) < 0.01, P(a-c) < 0.01), while there was no significant difference between group Phase b and Phase c (P(b-c) = 0.68), the complication rate of Phase b and Phase c decreased significantly than Phase a (P(a-b) < 0.01, P(a-c) < 0.01), while there was no significant difference between group Phase b and Phase c (P(b-c) = 0.15).</p><p><b>CONCLUSIONS</b>Our department of cochlear implantation followed the learning curve rule. The minimum number of cases that should accumulate in the learning phase needs about 30 cases.</p>

ABSTRACT

<p><b>OBJECTIVE</b>This study established a model of acoustically evoked short latency negative response (ASNR) in guinea pigs. Stereotaxic coordinate guided electrolytic lesion was applied to animal brainstem nuclei, the vestibular nucleus and the cochlear nucleus, to define the neural origin of ASNR.</p><p><b>METHODS</b>Twenty four guinea pigs with normal hearing were randomly divided into the control group (8 subjects, 16 ears) and the deafened group (16 subjects, 32 ears). Each animal experienced the auditory brainstem response (ABR) test. According to the presence of ASNR, the deafened group was further divided into ASNR group and non-ASNR group. Electrolytic lesion was conducted to the vestibular nucleus and cochlear nucleus respectively, followed by ABR test. The lesion structures were verified by brainstem slice and microscope.</p><p><b>RESULTS</b>In deafened group, the ASNR was elicited in 10 ears (31.3%). The ASNR was eliminated due to the electrolytic destruction to the vestibular nucleus, but it remained unchanged after the same procedure to the cochlear nucleus.</p><p><b>CONCLUSION</b>It is clear that the ASNR is originated from the vestibular nucleus, but not the cochlear nucleus.</p>

ABSTRACT

Objective To establish examination methods of electromyography (EMG) for tensor veli palatini (TVP) ,to obtain EMG values and EMG graphic patterns of TVP in normal healthy population .Methods A total of 20 healthy adults were selected for the study .Under the guidance of nasopharyngoscope ,especially designed elec-trode needles were inserted into different sites of TVP .All the 20 healthy adults were asked to perform series of swallowing and recorded the EMG values and EMG waveforms .Results At a scanning speed of 5 ms/div ,and with the TVP point as tie point ,the results of 20 healthy adults were obtained for 38 sides of TVP EMG .With the TVP contraction duration (0 .863 ± 0 .255 s) ,the peak voltage was produced by the contraction of 445 .100 ± 246 .808μV .The wave forms of EMG were considered as the interference .TVPs were observed bilaterally in synchroniza-tion with the contraction during swallowing .Action potential durations were 9 .142 ± 2 .178 ms ,and the amplitude of the action potential was 254 .260 ± 191 .544 μV .The action potential graphic configuration was multi -phasic with 2~3 waveforms .Conclusion This experimental study showed that when the TVP point was as tie point and the scanning speed was set at 5 ms/div record ,the results of action potentials were obtained more stable and clearer and the TVP normal values were also obtained for the normal population .

ABSTRACT

Objective To explore the significance of the Self -assessments of Voice Handicap Index (VHI) and The objective detection of Dysphonia Severity Index (DSI) in polyp of vocal cord perioperatively .Methods The Self -assessments of VHI had been completed in 28 patients with polyp of vocal cord before the operation and one week after the operation ,including function(F) ,physiological(P) ,emotion(E) ,and the sum denoted by T .In the meantime ,The DSI had been completed in these patients by acoustic analysis of DIVAS 2 .5 .After one month ,19 pa-tients had been re -examined .Results The DSI value was increasing ,but there was no significant difference be-tween pre-operation and one week after the operation(P>0 .05) ,indicating that the function of vocal cord was not recovered .In VHI scale ,the parameters were significantly different between the time preoperatively and one week postoperatively in F ,E and T(P0 .05) ,indicating that the function of vocal cord was not completely recovered as same as the result of DSI .After one month ,there were significant differences in DSI and VHI ,which showed a full recovery of vocal cord fanction .Conclusion The Self -assessments of VHI and The objective detection of DSI in polyp of vocal cord can evaluate simultaneouly the function of laryngeal .Additionally ,there was a good correlation between the DSI and the VHI score .

OBJECTIVE@#To analyze characteristics and curative effect of sudden sensorineural hearing loss (SSHL) inpatients with different audiometric curve and provide the reference for the diagnosis and treatment of SSHL.@*METHOD@#Retrospective analyze of 1033 cases of SSHL inpatients, compare the gender, the ears, the ages, with or without tinnitus and vertigo, hearing loss degree and the efficacy of distribution characteristics in different audiometric curves.@*RESULT@#Lnpatients with SSHL are mostly flat (27.24%), peak valley at least (5.94%); Gender distribution: male to female ratio was 1.3:1, audiometric curve for the rise type more than the decline type and flat type and completely deafness type (P < 0.01) in female. The ears distribution: the flat type more than increase type ( P < 0.01) in double eras SSHL. The age distribution: the flat type more than 60 years of age was significantly higher than that rising type (P < 0.01). Completely deafness type compared with the other types, the decline type compared with the rise type, the incidence of vertigo was significantly different (P < 0.01). Peak valley type with tinnitus was higher than the flat type. Degree of hearing loss distribution: completely deaf type in the very severe hearing loss was significantly higher than other types (< 0.01), flat type in severe hearing loss was significantly higher than other types (P < 0.01). The total effective rate was 51.01%. Rise type, peak valley type were more efficient than other types (P < 0.01); completely deafness type was less efficient than other types (P < 0.01).@*CONCLUSION@#The characteristics and curative effect are different in SSHL inpatients with different audiometric curves. Flat type was the most, peak valley type was the least. The curative effect of rise type and peak valley type are the best, completely deafness type is the worst. The degree of hearing loss and whether associated with vertigo is closely related in efficacy.

OBJECTIVE@#To evaluate the position and shape of inserted electrode arrays by plain X-ray during cochlear implantation.@*METHOD@#Cochlear view of implanted electrode arrays by plain X-ray were observed during operation in 54 patients received cochlear implant.@*RESULT@#The image in cochlear view could provide the information about position and shape of inserted electrode arrays. The spiral-shape electrode arrays without distortion and folding were found in 52 cases. The complete insertion of intracochlear electrode in these patients were confirmed by surgery. Of 54 cases, the other 2 cases showed improper position and C-shape of intracochlear electrodes, one was partial insertion and the other was bending inside the cochlea. The bending electrode array was reinserted immediately during operation.@*CONCLUSION@#Routine plain X-ray after inserted electrode during operation was a best way of knowing the shape and depth of the electrode array, especial for the difficult cochlear implantation, and also could avoided anesthetization again.

ABSTRACT

OBJECTIVE@#To assess the effect of treatment on acute low-tone sensorineural hearing loss and the related factors on the prognosis.@*METHOD@#A total of 398 inpatients with sudden deafness were retrospectively analyzed. Among them, 41 cases were diagnosed as acute low-tone hearing loss and then subjected to combined treatment with glucocorticoids. The relativity between patients' gender, disease duration and age of onset and curative effect were analyzed.@*RESULT@#The acute low-tone sensorineural hearing loss accounted for 10.30% of sudden deafness. Among which, female cases were significantly more than men, accounting for 70.73% of the total. At the first diagnosis, the average hearing threshold of three low tones was (48.43 +/- 11.67) dB against (18.86 +/- 9.40) dB of three high tones. After treatment, those two values were (27.07 +/- 11. 52) dB and (17.60 +/- 9.15) dB, respectively. There was statistical difference between three low tones and three high tones before treatment (t = 15.42, P 0.05). No obvious difference of the cure rate and effectiveness was found between male and female patients (P > 0.05). The incidence of acute low-tone sensorineural hearing loss was high in the age range of 21-40 years, accounting for 68.29% of the total. The cure rates were statistically different between patients less than 40 years old and 41-60 years old (chi2 = 6.662, P 0.05). There was no difference of cure rate and effectiveness between those with duration less than or equal to 7 days and those with more than 7 days (P > 0.05).@*CONCLUSION@#The combined therapy with glucocorticoids shows good effects on treating acute low-tone sensorineural hearing loss. Neither gender nor duration affect the prognosis. Age of onset is related to the prognosis.

ABSTRACT

Allergic rhinitis (AR), with an increasing uptrend of the prevalence in many developed and developing countries, is a global health problem that affects people of all ages and ethnic groups. However, data on the prevalence of self-reported AR in western China are rare. This study investigated the epidemiological features of self-reported AR in western China. In the cross-sectional, population-based study, a validated questionnaire survey on self-reported AR was carried out in 4 major cities in western China by multistage, stratified and cluster sampling, from January to December 2008. The total prevalence rate was 34.3%, with 32.3% (Chongqing), 34.3% (Chengdu), 37.9% (Urumqi), 30.3% (Nanning), respectively. The prevalence presented to increase with age before 30 years old while decrease with age after 30 years old, and the highest prevalence was in 19-30 years group in Chongqing, Chengdu and Nanning which significantly showed "persistent and moderate-severe" type (P<0.0001); In Urumqi, there wasn't a significant increasing or decreasing trend of prevalence rate with age but with an "intermittent and mild"predominance (P<0.0001). There were no distinct sexual differences in prevalence rates in the 4 cities. The morbidity was positively related to monthly average temperature and sunshine (r=0.76645, P=0.0036; r=0.67303, P=0.0165), but negatively associated with relative humidity (r=-0.64391, P=0.0238) in Urumqi. Interestingly, the monthly morbidity was negatively associate with average temperature, sunshine and precipitation in Nanning (r=-0.81997, P=0.0011; r=-0.60787, P=0.0360; r=-0.59443, P=0.0415). Self-reported AR is becoming common in western China with a rapid development in recent years, affecting about three persons out of ten. The climatic factors may have an indirect impact on the prevalence rate through the effects on the local allergens.

ABSTRACT

Objective:To evaluate the expression of vascular endothelial growth factor C(VEGF-C),Epstein-Barr virus(EBV) latent membrane protein 1(LMP1),cyclooxygenase-2 (COX-2) in nasopharyngeal carcinoma(NPC).Method:LMP1, COX-2 and VEGF-C were detects by immunohistochemical staining for 57 case NPC tissue.Result:The positives rates of LMP1,COX-2 and VEGF-C detected by immunohistochemical staining were 49.1%(28/57), 75.4%(43/57)and 59.6%(34/57), respectively. The expression of LMP1, COX-2 and VEGF-C were correlated to each1 other in NPC(P<0.05).Conclusion:LMP1 and COX-2 may induce expression of VEGF-C directly or LMP1 induce expression of VEGF-C by induce COX-2 expression, may contribute to lymph metastasis and develop NPC.

OBJECTIVE@#To investigate the diagnosis and treatment of the simple congenital malformation of ossicular chain.@*METHOD@#Curative effect analysis for 4 cases of treatment of simple congenital malformation of ossicular chain were performed in our department from June 2005 to August 2009, retrospectively. 4 patients underwent middle ear exploration and ossicular chain reconstruction were followed up for 1-4 years, and air conduction threshold of speech frequency(0.5, 1.0, 2.0 kHz) were detected, 15 dB increase of which were as the standard of successful operation.@*RESULT@#Three cases was abnormal incus, 2 cases were absence of instruction on the stapes footplates, and stapes footplates fixed, one case was absence of the incus. Hearing level had significantly improve after hearing reconstruction in 3 patients. The averaged postoperative air conduction threshold of speech frequency had 47 dB increase, bone conduction threshold have no change, while one case of hearing level without change. Tinnitus, vertigo, facial paralysis and other complications occurred were found in all the patients.@*CONCLUSION@#The congenital conductive hearing loss without external ear malformations should be suspected of congenital malformation of ossicular chain, which could be diagnosed by tympanic exploration. Ossicular chain reconstruction can improve the hearing effectively.

OBJECTIVE@#To investigate the reason for the adhesion of vocal cord after CO2 Laser laryngeal surgery.@*METHOD@#One hundred and nineteen patients who were untaken CO2 Laser laryngeal microsurgery.@*RESULT@#Among 119 cases, 9 cases of bilateral vocal cord paralysis, 42 cases of vocal cord polyps and 3 cases of precancerous laryngeal lesions did not show vocal cord adhesion after CO2 Laser laryngeal microsurgery. Contrarily, one of 5 cases with laryngeal stenosis, 5 of 41 cases with laryngeal papilloma and 6 of 19 cases with laryngeal carcinoma shew the adhesion of vocal cord after CO2 Laser laryngeal microsurgery.@*CONCLUSION@#Through selecting the appropriate patient and surgery spot, processing correctly during and after surgery, using the suitable dosage of laser and reducing the heat radiation, the adhesion of vocal cord can be avoided after CO2 Laser laryngeal surgery.

ABSTRACT

OBJECTIVE@#To evaluate the expression of vascular endothelial growth factor C (VEGF-C), Epstein-Barr virus (EBV) latent membrane protein 1 (LMP1), cyclooxygenase-2 (COX-2) in nasopharyngeal carcinoma (NPC).@*METHOD@#LMP1, COX-2 and VEGF-C were detects by immunohistochemical staining for 57 case NPC tissue.@*RESULT@#The positives rates of LMP1, COX-2 and VEGF-C detected by immunohistochemical staining were 49.1% (28/57), 75.4% (43/57) and 59.6% (34/57), respectively. The expression of LMP1, COX-2 and VEGF-C were correlated to each other in NPC (P < 0.05).@*CONCLUSION@#LMP1 and COX-2 may induce expression of VEGF-C directly or LMP1 induce expression of VEGF-C by induce COX-2 expression, may contribute to lymph metastasis and develop NPC.

ABSTRACT

Objective:To investigate the reason for the adhesion of vocal cord after CO_2 Laser laryngeal surgery.Method:One hundred and nineteen patients who were untaken CO_2 Laser laryngeal microsurgery.Result: Among 119 cases,9 cases of bilateral vocal cord paralysis, 42 cases of vocal cord polyps and 3 cases of precancerous laryngeal lesions did not show vocal cord adhesion after CO_2 Laser laryngeal microsurgery.Contrarily, one of 5 cases with laryngeal stenosis, 5 of 41 cases with laryngeal papilloma and 6 of 19 cases with laryngeal carcinoma shew the adhesion of vocal cord after CO_2 Laser laryngeal microsurgery.Conclusion:Through selecting the appropriate patient and surgery spot, processing correctly during and after surgery, using the suitable dosage of laser and reducing the heat radiation, the adhesion of vocal cord can be avoided after CO_2 Laser laryngeal surgery.

ABSTRACT

OBJECTIVE@#The analyze the relationship between secretory otitis media (SOM) and injury of tensor veli palatini (TVP) muscle injury after radiotherapy, then to explore the pathogenesis of SOM in patients with nasopharyngeal carcinoma (NPC) after radiotherapy.@*METHOD@#The cross section area (CSA) of TVP and medial pterygoid (MP) muscle were measured in MRI of 32 patients with NPC before and after radiotherapy, meanwhile the incidence of SOM were surveyed after radiotherapy, then to analyze the relationship between the change of TVP and the incidence of SOM after radiotherapy.@*RESULT@#Of 48 ears without SOM before radiotherapy, 27 ears developed post-irradiation SOM, including 24 ears with TVP atrophy and 3 ears without TVP atrophy, and 21 ears had no post-irradiation SOM, including 8 ears with TVP atrophy and 13 ears without TVP atrophy. chi2 test showed significant difference (P < 0.01). It indicated that post-irradiation SOM have correlation with TVP atrophy. The more possibility of TVP atrophy occurred after long time radiotherapy.@*CONCLUSION@#The atrophy of TVP in patients with NPC usually occurs 6 months after radiotherapy, and this may result in the post-irradiation SOM. The pathogenesis of post-irradiation SOM need further study functionally.

Objective:To explore the feasibility and validity of tubo-tymanoaerodynamic graphy(TTAG)determinating the Eustachian tube function of health adult group.Mthod:The ventilation function of Eustachian tube was measured by the TTAG method in health adlut group(132 ears),the results and the graphs were also analyzed.Reslut:The positive rate in health adult group using Valsalva method was 93.93%(124/132).The positive ears were divided into typeⅠand typeⅡ, the mean value of nasopharynx press of type Ⅰand Ⅱ has no significant differences(P>0.05),but the mean value of external auditory canal press had significant differences(P<0.01).Conclusion:TTAG meathod has the clinical value in determinating the states of Eustachian tube function of health group.
