ABSTRACT

Objetivos: Evaluar la posibilidad de realizar cirugía conservadora y su posterior evolución. Analizar el impacto de la incorporación de taxanos al tratamiento neoadyuvante. Reclasificar a los tumores tratados de acuerdo a su perfil inmunohistoquímico analizando la respuesta de los diferentes subtipos al tratamiento neoadyuvante. Material y método: Se analizaron en forma retrospectiva las historias clínicas de 274 pacientes con carcinoma de mama localmente avanzado (CMLA) no inflamatorio, desde enero de 1989 hasta diciembre de 2004 inclusive. Se analizó el tratamiento quirúrgico implementando (mastectomía radical modificada o cirugía conservadora) y su evolución en cuanto a recidivas locales (RL) y metástasis a distancia. Se analizaron las respuestas clínicas y patológicas subdividiéndolas en completas, parciales y enfermedad estable luego del tratamiento neoadyuvante con antraciclinas más ciclofosfamida o antraciclinas más taxanos. Se reclasificaron los tumores en base a su perfil inmunohistoquímico de acuerdo a la expresión de RE, RP y HER2, subdividiéndolos en: luminal, HER2 y triple negativo (TN), analizando la respuesta clínica y patológica, RL y a distancia, SLE y SG. Resultados: Se realizó en 238 pacientes (87%) mastectomía radical modificada y en 36 pacientes (13%) cirugía conservadora (CC). Las recidivas locales fueron del 8,3% en CC y 15% en mastectomías. Las metástasis a distancia fueron del 30% y 39%, respectivamente. De las 274 pacientes, 167 pacientes fueron tratadas con ciclofosfamida más antraciclinas (61%), mientras que 107 pacientes (39%) fueron tratadas con antraciclinas más taxanos. La respuesta patológica global fue del 85%. La respuesta patológica completa (pRC) fue de 29 pacientes (10,5%). Las respuestas parciales fueron 206 pacientes (75,0%) y 40 pacientes (14,5%) permanecieron con enfermedad estable.

ABSTRACT

Objetivos: valorar la respuesta a la quimioterapia neoadyuvante en el cáncer de mama localmente avanzado mediante [18F]-FDG-PET/CT. Evaluar la detección de metástasis a distancia en estadios localmente avanzados mediante [18F]-FDG-PET/CT y su correlación con estudios convencionales. Material y métodos: se realizó [18F]-FDG-PET/CT en 26 pacientes antes de efectuar la biopsia de mama y antes del inicio de los tratamientos. En 23 casos se efectuó un segundo PET/CT luego del primer o segundo ciclo de quimioterapia neoadyuvante. Todas las pacientes recibieron el mismo esquema de quimioterapia con antraciclinas más paclitaxel, realizando tres cursos preoperatorios. Veintiún pacientes fueron sometidas a cirugía. Sobre éstas se compararon los hallazgos del PET/CT respecto de la patología. Se valoró el porcentaje de disminución de la SUV en el PET posquimioterapia versus el previo y se correlacionó con los hallazgos patológicos. Se valoró la detección de metástasis mediante este método y su correlación con estudios convencionales de detección de metástasis. Resultados: en 12 casos se objetivó disminución del SUV superior al 50% y en 5/12 esa disminución fue superior al 80%. En estos 5 casos se correlacionó con respuesta patológica completa (1 caso) o enfermedad residual mínima (4 casos) en la pieza operatoria. En los 7 casos restantes se halló respuesta patológica completa en 1 caso y enfermedad macroscópica en 6 casos. En 8 pacientes se observó una disminución del SUV inferior al 50%. Todas ellas mostraron enfermedad residual macroscópica en las piezas operatorias. En 3/23 pacientes se evidenció progresión de la enfermedad mediante PET/CT, ya sea a nivel local/regional o por progresión de metástasis a distancia. De ellas, 1 paciente fue operada y 2 no operadas. La operada mostró enfermedad residual macroscópica. El valor del SUV inicial se correlacionó con la respuesta.

ABSTRACT

Diabetic mellitus is the most common metabolic disorder. Intake of glucose or any disaccharides containing glucose is prohibited in these patients. In this study changes in blood glucose of diabetic patients were determined after eating jelly jams with sorbitol as the sweetener and white bread as a reference. A total of 30 diabetes type II patients were studied and the effects of the jams on their blood glucose levels were evaluated. To determine the glycemic index of these jams, we used white bread as the reference and blood glucose levels were measured after eating each of them at different time intervals of 0, 15, 30, 45, 60, 90, 120 minutes. Increased values for white bread compared to fructose jam at 120, 90, 60, and 45 minutes were as follows: 36/6% [P < 0.001], 50/6% [P < 0.001], 37/4% [P < 0.02], and 21/7% [P < 0.02], respectively. Blood glucose response evaluation in patients showed that glycemic index of white bread and sorbitol jam was 100 and 27/9 respectively. According to the results of our study, the glycemic index of this kind of jams is low and considering the positive clinical effects of the foods with low glycemic index, using this kind of jams in the diet of diabetic patients is recommended

ABSTRACT

Knee arthroscopy is an approved technique for the diagnosis and treatment of intra-articular lesions. Moderate to severe pain is experienced after surgery; thus, relieving pain post arthroscopy, will help patients in performing their daily activities as soon as possible. Many studies have been performed for reducing pain after arthroscopy. The aim of this study is to compare the efficacy of intra-articular injection of morphine with marcaine in patients for pain relief after arthroscopy. 30 patients were considered for arthroscopic surgery, due to the tearing of the menisci. In this simple non-probability trial, patients were divided in two groups. The first group received 7cc intra-articular marcaine at 0.5% and the second group received 10mg of intra-articular morphine after the arthroscopy. The response was measured by VAS in hours 6, 12, 18, 24 postoperatively and by flexion, extension and walking. The results showed that there was no significant statistical difference between the two groups, except in hour 6 after surgery, indicating marcaine is more effective than morphine. There were no side effects experienced within the two groups. Age, gender, height and weight also had no effect in reducing the pain in patients. Intra-articular Injection of marcaine is more effective than morphine six hours after surgery; however, there are no differences between them after that time frame. More research is needed in order to reduce pain after arthroscopy

ABSTRACT

Associations studies were attempted between the type of feeding, duration, and time of starting of solid foods in infancy and the incidence of insulin-dependent diabetes mellitus (IDDM). The study subjects comprised 52 IDDM patients and 52 control subjects matched for sex, age, social status, country, geographical location and selected from pediatric departments of different hospitals in Tehran. Diabetic children (21 boys, 31 girls) were of the ages of 1.5 to 14 years. Information about the pattern of their feeding at the first two years of life were collected through questionnaires administered to the mothers. The questionnaire was designed to evaluate the duration of complete or partial breast-feeding and the age at which dietary products containing cow's milk were introduced into the diet. A large proportion of the diabetic children rather than the control children had been breast-fed, and the risk of IDDM among children who had not been breast-fed was below unity. No significant difference in the duration of breast-feeding was observed between diabetic and control group. Our data do not support the existence of a protective effect of breast-feeding on the risk of IDDM, nor do the data indicate that early exposure to cow's milk and dairy products has any influence on the development of IDDM.
