ABSTRACT

Today, global focus of research is to explore the solution of energy crisis and environmental pollution. Like other agricultural countries, bulk quantities of watermelon peels (WMP) are disposed-off in environment as waste in Pakistan and appropriate management of this waste is the need of hour to save environment from pollution. The work emphasizes the role of ethanologenic yeasts to utilize significant sugars present in WMP for low-cost bioethanol fermentation. Dilute hydrochloric acid hydrolysis of WMP was carried out on optimized conditions employing RSM (response surface methodology) following central composite design (CCD). This experimental design is based on optimization of ethanologenesis involving some key independent parameters such as WMP hydrolysate and synthetic media ratio (X1), incubation temperature (X2) and incubation temperature (X3) for maximal ethanol yield exploiting standard (Saccharomyces cerevisiae K7) as well as experimental (Metchnikowia cibodasensisY34) yeasts. The results revealed that maximal ethanol yields obtained from S. cerevisiae K7 was 0.36±0.02 g/g of reducing sugars whereas M. cibodasensisY34, yielded 0.40±0.01 g ethanol/g of reducing sugars. The yeast isolate M. cibodasensisY34 appeared as promising ethanologen and embodies prospective potential for fermentative valorization of WMP-to-bioethanol.

@#Rail maintenance routines are necessary to enable the all rail operations to achieve its aim in maintaining a safe and efficient operation. The maintenance tasks expose the workers to vibration and noise, as they handle specialized machineries and heavy self-propelled vehicles. Exposure of noise above the permissible exposure limit and over the daily allowable duration may cause noise-induce hearing loss (NIHL). Investigation on the type of task that has high noise level exposure on the maintenance workers was conducted to justify the needs to further detailed of this study. This paper will describe the task analysis on maintenance workers and to identify possible tasks with exposure to high level of noise. It scopes down to grinding crew of the maintenance department of light rail transit. Methods used were real-time sound measurement using a sound level meter, observation and interviews. Then, data were analysed to understand the situation of exposure of noise during rail maintenance. An ergonomic risk assessment was also conducted by adoption of the latest guideline on Ergonomic Risk Assessment (ERA) at Workplace Guidelines recommended by Department of Occupational Safety and Health (DOSH), Malaysia. A hierarchical task analysis (HTA) was generated on the task of the maintenance worker, focusing on rail grinding crew. The newly generated HTA had allowed better understanding about the nature of work and the task conducted by a rail grinder during the work shifts. Tasks involving high noise level was identified. Data recorded shows that the noise level for the blowing activity was relatively high and exceeded the permissible exposure limit of 90dBA. The exposure level was currently controlled by practicing the usage of hearing protection equipment (HPE) and by controlling exposure time in accordance to recommendations of the Factory and Machinery Act (FMA). Thus, it had confirmed that maintenance workers were exposed to high noise levels when performing their daily task. Further studies are needed to investigate the relationship between the duration of exposure and noise-induce hearing loss (NIHL) with consideration of the lifestyles of the maintenance workers.

@#Many of our chronic kidney disease (CKD) patients venture into complementary and alternative medicine (CAM) to seek a cure for the disease beyond conventional management. The objective of this study was to determine the prevalence and pattern of CAM usage; and its association with the progression of CKD in our population. This was a cross-sectional survey recruited CKD patients from Universiti Kebangsaan Malaysia Medical Centre (UKMMC). Data was collected via interview using questionnaires which explore the demographics, type and pattern of CAM use. Patients’ comorbidities and serum creatinine were extracted from the medical notes to assess the association between progression of CKD and usage of CAM. Chi-square, Independent Samples t-test, Mann-Whitney U test and logistic regression were used to evaluate for an association. Three hundred and seventy two eligible patients were recruited. The prevalence of CAM use was 29% in the past year. Younger age, higher education and higher income level were associated with significantly more usage of CAM. Natural products were the commonest type of CAM used (86.1%). The belief in the effectiveness of CAM was the main reason for its uptake and about 57.8% revealed their practice to their physician. We found that the use of CAM in Malaysian CKD patients did not influence the progression of the disease.

@#Bacteraemia is a common and one of the serious complications in haematopoietic stem cell transplantation (HSCT). To date, there are no published data on antibiotic resistance and clinical outcome among HSCT recipients in Malaysia. The aims of the present study was to analyse the prevalence, antibiotic resistance and clinical outcome of bacteraemia in HSCT recipients within 100 days following transplantation. We retrospectively analysed the prevalence, antibiotic resistance pattern and mortality rate of early bacteraemia among HSCT recipients in a single centre over a 5-year period (2013-2017). Thirty patients of 85 HSCT recipients developed bacteraemia with 40 positive cultures resulting in prevalence of 47% (40/85). Gram negative bacteria (GNB) accounted for 60.5% of total isolates. Enterobacteriaceae and Coagulase negative Staphylococcus (CoNS) were the commonest pathogens isolated. GNB showed a high resistance rate to ciprofloxacin. Only 30% of recipients responded to first line empirical antibiotics for febrile neutropenia (FN). The mortality rate was 13.3% (4/30), of which 50% was attributed to multi-drug resistance (MDR) Acinetobacter and 25% to extended spectrum beta-lactamase (ESBL) Enterobacteriaceae. Bacteraemia is a frequent and life-threatening early complication among HSCT recipients with MDR GNB being the commonest cause of mortality. The high rate of resistance to ciprofloxacin and failure of the first line empirical antibiotics to treat FN calls for a thorough evaluation of the current antibiotic prophylaxis and empirical treatment protocols. These findings have important clinical implications regarding the use and selection of both prophylactic and empiric antibiotic regimens to treat FN.

The present study aimed to determine the factors associated with CKD stage 3b among type 2 diabetics attending primary care follow-up, specifically the role of angiotensin blockade dosage. This was a pilot unmatched case-control study conducted in a teaching primary care centre. Clinical data of 25 cases of diabetic patients with CKD stage 3b (GFR 30-45ml/min/1.73m2) in 2012 were selected for this study, as well as 103 controls who were diabetic patients with GFR more than 45ml/min/1.73m2 in 2012. Systematic random sampling was employed. Data was obtained from patients’ diabetic records, computerised clinical medical information system and medical case notes. Univariate analysis was done using Chi-square, t-test, Fisher’s exact test and Mann-Whitney U-test. Multiple logistic regression was used to determine the associated factors for development of CKD stage 3b. Cases and controls were different in terms of age, duration of diabetes, use and dosage of angiotensin blockade medications, systolic blood pressure and baseline GFR. Multiple logistic regression revealed that systolic blood pressure (Adjusted OR= 1.08, 95% CI= 1.02-1.14, p=0.013) and baseline GFR (Adjusted OR= 0.90, 95% CI= 0.85-0.95, p<0.001) was significantly associated with the development of CKD stage 3b among diabetics. Maximizing the dose of angiotensin blockade had a protective effect (Adjusted OR= 0.14, 95% CI=0.85-0.95, p=0.025). The results of the present study supports the need for good control of systolic blood pressure among diabetic patients to reduce the risk of chronic kidney disease progression. Dose of angiotensin blockade medications should be optimised in these patients.

The purpose of this study was to determine the prevalence of defaulters of immunization, and their associated risk factors among children age 12 to 24 months. Materials and Methods: A cross-sectional study was conducted in all government's maternal child health clinics in District of Kota Kinabalu, Sabah. Data was collected using a standardised questionnaire from July to November 2006. Results: The prevalence rate for defaulting immunization was 16.8% from the 315 respondents. Bivariable analysis showed various significant factors associated with defaulters such as mother’s employment status, family mobility, transportation and cost. Nonetheless, multivariable analysis showed only mother’s age, mother employment status and family size were the significant predictors for defaulting immunization. Immunization that had the highest rate of defaulters was DPT–OPV booster dose (56.6%), followed by MMR immunization (43.4 %) and DPT-Hib/OPV and Hep B third dose (37.7%).Conclusion: Employed mothers with bigger family size should be more closely monitored and advised to reduce the chance of defaulting on the immunization. Health promotion activities also should focus to these groups of mothers.

Introduction: Body weight of children is affected by many factors including food habits which are influenced by their parents. Studies in the West have shown that parents tend to control child feeding in response to their child’s weight status. The aim of this study was to assess Malaysian parental concerns about child weight and the control they exert on child feeding. Methods: This crosssectional study was conducted on parents and their children aged 9 to 12 years from a primary school in Kuala Lumpur. The weight status of the children was classified according to the body mass index-for-age growth chart. Parental concerns about child weight and control in child feeding was assessed using the adapted Malay version of Child Feeding Questionnaire. Results: A total of 204 parents participated in this study. The study found that being a female served as a protective factor against becoming overweight (OR:0.28, CI:0.13-0.62). Parents with overweight children were significantly older (OR:1.08, CI:1.01-1.15), concerned about their child’s weight (OR:2.77, CI:1.49-5.12) and controlled their child’s feeding by restricting food intake(OR:2.70, CI:1.30-5.60). They were less likely to pressure their children to eat (OR:0.32, CI:0.19-0.56). Parents from the low income group were more likely to have underweight children (OR: 4.15, CI:1.28-13.47). Conclusion: There was significant difference in level of parental concern across differing child weight status. Parents with overweight children were likely to be more concerned about their child’s weight, tending to control their feeding. In contrast, parents with underweight children did not exert control on their feeding.

ABSTRACT

Few studies have explored the hormonal triggers for masturbation in infants and young children. Thus, we aimed to study the sex hormones and clinical profiles of masturbating infants and young children. This case-control study involved infants and young children who masturbate and were referred to three pediatric neurology clinics between September 2004 and 2006 [n=13], and a similar control group. All children underwent basic laboratory investigations prior to referral. Other tests included electroencephalography [n=8] and brain neuroimaging [n=9]. We measured dehydroepiandrosterone sulfate, 17-hydroxyprogesterone, free testosterone, estradiol, dehydroepiandrosterone, sex hormone-binding globulin [SHBG], and androstenedione in all participants. The median age at the first incident was 19.5 months [range, 4-36 months]; the median masturbation frequency, 4 times/day; and the median duration of each event, 3.9 min. The subjects masturbated in both prone [n=10] and supine positions [n=3]; two subjects used the knee-chest position. All subjects showed facial flushing; 6, friction between the thighs; 5, sweating; 9, sleeping after the event; and 12, disturbance on interruption. EEG was abnormal in one of eight subjects tested, and neuroimages were normal in all of nine subjects examined. The case and control groups had comparable levels of all sex hormones, except estradiol, which showed significantly lower levels in the case group [P=.02]. Masturbation in children seems to be associated with reduced estradiol levels, but not with other sex hormones. Further studies are needed to confirm our findings

ABSTRACT

A cross sectional study was done to determine the prevalence and distribution of human papillomavirus (HPV) genotypes in pre-invasive (cervical intraepithelial neoplasia, grade 3 or CIN 3) and invasive cervical cancer (ICC), in Universiti Kebangsaan Malaysia Medical Centre (UKMMC). A total of 80 paraffin-embedded tumour tissue blocks (20 CIN 3, 60 invasive cancers) between 1999 to 2007 were retrieved from the archives of the Department of Pathology. Patient’s medical records were obtained from the Medical Records Office. Among invasive cancers (n=60), squamous cell carcinoma (SCC) account for 75% and adenocarcinoma 25%. The mean age of cases studied was 52.0 ± 12.2 years and Chinese was the predominant ethnicity (66.3%). Twelve HPV genotypes were identified, namely, HPV 16, 33, 18, 39, 52, 45, 58, 59, 31, 35, 6 and 11. The prevalence of HPV was 92.5% with types 16 being the most common (73.8%), followed by types 33 (30%) and 18 (22.5%). A total of 31 cases (38.8%) showed single HPV genotype, while 43 (53.8%) had multiple HPV (two genotypes or more) genotypes. In ICC, HPV 16, followed by types 33, 18, 52 and 39 were the top five common HPV genotypes detected. High prevalence of HPV and multiple HPV infections were major findings among patients with pre-invasive and invasive cervical cancer.

ABSTRACT

After a gap of approximately 20 years, a new generation of antiepileptic drugs [AEDs] has recently been developed. More than 8 drugs have been licensed in at least one country during the 1990s. While lamotrigine, gabapentin, vigabatrin and oxcarbazepine are widely used in some countries, felbamate, topiramate, tiagabine, levetiracetam, and zonisamide are still used on a narrow scale. A feeling of optimism occurs after the development of these drugs, although only a small number of epileptic patients become free of seizures after the addition of these new AEDs to their regimen. Generally, the safety profile of the new AEDs is only slightly better than that of established drugs and their efficacy is strongly associated with the use of high doses. This article reviews new AEDs by studying their clinical pharmacological effects, mechanisms of action as antiepileptic agents, side effects, drug-drug interactions and the appropriate regimen of their use

ABSTRACT

Guanosine 3',5 monophosphate [cGMP] can be used as a marker of the epileptogenicity of proconvulsant drugs. As valproic acid [VPA], at certain concentrations, acts as a proconvulsant agent in hippocampal pyramidal neurons when tested in the veratridine model, this investigation was conducted to study the effect of proconvulsant concentrations of VPA on the basal level of cGMP in hippocampal tissue. Experiments were performed using standard radioimmuonassay techniques in hippocampal tissues from rats. This study was carried out at the Department of Pharmacological and Pharmaceutical Sciences, University of Houston, Houston, Texas, United States of America between 1996-1997. We found that veratridine [0.3, 1 and 2 micro M, n=3] increased the level of cGMP in hippocampal tissue in a concentration dependent manner. However, VPA at proepileptic concentrations [0.1, 2, 5 micro M, n=3], did not significantly affect the basal level of cGMP when added alone or with veratridine [0.3 micro M]. Guanosine 3',5' monophosphate is not a marker of the proepileptic activity of VPA in brain tissues

ABSTRACT

To assess the value of proteinuria selectivity index [PSI] in predicting the response of children with nephrotic syndrome to corticosteroid therapy and its correlation with renal biopsy findings. Setting: Paediatric Department, Our Lady's Hospital for Sick Children, Dublin, Ireland. Design: Retrospective analysis of the records of 39 children admitted to the above hospital with nephrotic syndrome. PSI was performed for all children prior to therapy. All received corticosteroid therapy according to the protocol of International Study of Kidney disease in Children for initial attacks and relapses. Patients were divided into 3 groups according to their response to steroid; steroid responsive infrequent relapsers [n.15], steroid responsive frequent relapsers [10] and steroid resistant [8]. PSI of < 0.01 was statistically significant in differentiation between the steroid responsive and resistant nephrotic syndrome. All children in the latter group had non-minimal change lesions on biopsy. Protein selectivity index should continue to be one of the valuable initial tests in childhood nephrotic syndrome due to its useful additional predictive value on the response to steroid in those patients

Familial cases of congenital dyserythropoietic anemia are extremely rare. Described are a brother and sister with congenital dyserythropoietic anemia type I. Both had a history of neonatal jaundice, moderate anemia and hepatosplenomegaly. They were blood transfusion independent

ABSTRACT

Three patients with rheumatic chorea were treated with sodium valproate for their involuntary movements. A few days after the start of the treatment, chorea disappeared completely. One patient had a relapse after discontinuation of the drug, but responded quickly to the same drug after reusage. It appears that sodium valproate is a promising drug in the treatment of rheumatic chorea
