ABSTRACT

Objective:To understand the current living and health status of leprosy survivors in Fengxian District, Shanghai, and to provide scientific evidence for improving their quality of life. Methods:In January 2018-June 2020, professionally trained CDC staff performed a household investigation on 41 patients with leprosy (case group) and 82 non-leprosy persons (control group) every year. Living and health status, and routine laboratory examinations such as blood pressure, blood glucose, and blood lipids were monitored. Results:Majority of the patients with leprosy had low educational level (68.29% being primary school) and were mostly farmers (51.22%).The patients had abnormal residual Ⅱ level 5 (12.20%) and mostly were tb-like (TT) (53.66%). Moreover, some patients were not incapacitated (46.34%), had no family financial difficulties (29.27%), did not acquire the national subsistence allowance policy (58.54%), and asked for "regular physical examination" (68.29%).Compared to the 82 control persons, the patients with leprosy had significantly different body mass index (BMI) and blood pressure (P<0.05). Conclusion:Living and health status of leprosy survivors in Fengxian District remains to be further improved. The China Disabled Persons’ Federation (CDF), Red Cross, civil affairs, and health administration need to further strengthen the cooperation.

ABSTRACT

Objective To review the clinical experience with on pimp mitral valve repair (MVP) and off-pump coronary artery bypass grafting (OPCABG) for coronary atherosclerotic heart disease (CAD). Methods From April 2017 to March 2018, a total of 32 patients received MVP combined with OPCABG in Beijing Anzhen Hospital. The echocardiographic results before and after the operation were compared and SPSS 20.0 statistical software was used to analyze the clinical data of these patients. Results There were 3.69 grafts per case. Artificial ring forming was performed in 26 cases, quadrangular resection in 4, chordal replacement in 2. No death of these patients occurred in the hospital. Thirty patients were followed up from 6 months to 30 mouths with improved heart function. Heart function was class in 26 and class Ⅱ in 4. The postoperative cardiac ultrasonography for reexaminations indicated that 8 with slight mitral regurgitation, 20 with mild regurgitation and 2 with moderate regurgitation. Patients' cardiac function and size of heart (LVESD, LVEDD and LVEF) were remarkably improved. Conclusions OPCABG and MVP should be performed concomitantly when CAD and mitral insufficiency disease present simultaneously. OPCABG operation at the same time line for the treatment of MVP effect is better. The patients' quality of life could be significantly improved.

ABSTRACT

OBJECTIVE This study aimed to optimize polysaccharides extraction from Urena lobata L.and investigate its antioxidant activity.METHODS The mathematical model was established by re-sponse surface method (RSM) based on the results of single factor experiments, using polysaccha-rides extraction rate as response value,and using the ratio of water to material,cellulase concentra-tion,extraction temperature and time as experimental factors,which was used to screen optimum poly-saccharide extraction conditions from Urena lobata L.. Antioxidant activity of polysaccharides was stud-ied by DPPH and ·OH free radical elimination method. RESULTS The optimum conditions obtained by RSM were as follows:the cellulase level was 10.8 g·L-1,extraction time duration was 72 min,the ra-tio of water to feedstock was 7 mL·g-1,extraction temperature was 43℃,the pH value was 5.0.Under the optimal conditions, there was a difference of less than 5% between predicted extraction rate 13.37% and experimental extraction rate 13.32%. The polysaccharide yield was most significantly af-fected by cellulase concentration,followed by extraction time,water to material ratio and extraction tem-perature.IC50of DPPH and·OH were 1.082 g·L-1and 3.202 mg·L-1,respectively.Antioxidant activity of sample polysaccharides was weaker than those of vitamin C. CONCLUSION The polysaccharide extraction process from Urena lobata L. by cellulase enzymolysis approach was obtained, which was convenient and feasible,and extracted polysaccharides had good free radical scavenging activity.

ABSTRACT

BACKGROUND: Skeletal muscle defect is a major concern of plastic and orthopedic surgeons due to poor regenerative capability of skeletal myocytes and limited clinical treatment. Recently, cell therapy and skeletal muscle engineering based on adipose-derived stem cells (ADSCs) open a new era for skeletal muscle injury repair, which attributes to merits of ADSCs including sufficiency, easy harvest, high yield, strong proliferation and paracrine capability, as well as differentiation potential towards skeletal myoblasts under specific induction. OBJECTIVE: To summarize the process of ADSCs' differentiation towards skeletal myoblasts, to analyze factors that may affect myogenic differentiation of ADSCs and their underlying mechanism, and to discuss the oncological safety of ADSC therapy and limits for clinical application. METHODS: Search in PubMed using the following formula ((myogenic[Title]) OR myogenesis[Title]) AND adipose stem cell[MeSH Terms]) and in SinoMed using ((("adipose tissue"[Title]) AND "Stem cell"[Title]) AND "myo-"[Title]) was done on June 10th, 2017. References related to ADSCs' myogenic differentiation or skeletal muscle repair were included, whereas repeated references were excluded. RESULTS AND CONCLUSION: Forty-seven references in PubMed were selected. Within them, 26 were published in recent 5 years. Forty-seven references in SinoMed met the criteria; however 34 of them were about ADSCs' differentiation towards myocardial cells and 6 were about ADSCs' differentiation towards smooth muscle cells. Eventually, 50 references were included. ADSCs' differentiation towards skeletal myoblasts under specific induction is a complicated process involving the interactions of varieties of genes, proteins and signal pathways.Besides the traditional in vitro chemical induction,conditioned medium or co-culture with myocytes can also facilitate ADSCs' differentiation towards skeletal myoblasts. Chemicals, biomechanics, myogenic regulatory factors, cytokines, growth factors, microRNAs and lncRNA have effects on ADSCs myogenic differentiation. ADSCs can promote the repair of morphology and function of skeletal muscle through direct differentiation or indirect paracrine of cytokines, regulating inflammatory response, suppressing apoptosis, accelerating angiogenesis, and recruiting endogenous stem cells. The future study concerning skeletal muscle repair is expected to address the construction of tissue-engineered muscle by combining various cytokines and growth factors with three-dimensional scaffolds to promote ADSCs' proliferation and differentiation.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the expression of CD4+, CD8+ T cells and cell apoptosis in oral lichen planus (OLP) and investigate the role and the relationship of immunological reaction and cell apoptosis in the pathogenesis of OLP2.</p><p><b>METHODS</b>Immunohistochemical technique was used to study the expression of CD4+, CD8+ T cells in 27 OLP cases. TUNEL was used for detecting the cell apoptotic index (AI) in 17 OLP2 cases.</p><p><b>RESULTS</b>The expression of CD4+, CD8+ T cells were obviously elevated in lamina propria of OLP group compared with control group (P<0.05). There was a strong significance when compared the ration of CD4/CD8 in both group. AI was remarkably increased in epithelia cells and significantly decreased in lymphocytes in lamina propria in OLP cases compared with its expression in the control group respectively.</p><p><b>CONCLUSION</b>The increased amount of CD4+, CD8+ T cells in lamina propria of OLP and the change ration of CD4/CD8 suggest that immune response is involved in the pathogenesis of OLP. The abnormal cell apoptosis plays an important role in the pathogenesis of OLP.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To examine the expression of TGF-beta1, Smad7 and cell apoptosis in oral lichen planus (OLP) and to evaluate the possible pathogenesis of oral lichen planus.</p><p><b>METHODS</b>Immunohistochemical technique was used to study the expression of TGF-beta1 and Smad7 in the epithelia cells of 17 OLP cases and 7 normal oral mucosa (NOM). TUNEL was used for detecting the cell apoptosis in 17 OLP cases and 7 NOM.</p><p><b>RESULTS</b>TGF-beta1 was moderately positive in the epithelia cells of OLP. All the epithelia cells in OLP showed strong cytoplasmic staining. The expression of TGF-beta1 and Smad7 were significantly increased in OLP compared with that in NOM (P < 0.05). Cell apoptotic index (AI) was remarkably increased in epithelia cells in OLP cases, and the cell apoptosis was localized in basal and suprabasal epithelial layers. There was a positive correlation between TGF-beta1 expression and cell apoptosis in the epithelia of OLP (r = 0.69, P <0.05).</p><p><b>CONCLUSIONS</b>High expression of TGF-beta1 and Smad7 in the epithelia of OLP suggests that TGF-beta1-Smad7 signal pathway was disturbed in oral lichen planus. The imbalance of TGF-beta1-Smad7 pathway may contribute to the mechanisms of cell apoptosis of epithelial cells in OLP.</p>

ABSTRACT

Objective To investigate the relationship betwee n tricuspid insufficiency (TI) and the function of the right ventricle, degree o f pulmonary vascular pathological change in patients with rheumatic heart diseas e so as to provide the indication of operation for treating TI. Methods  41 cases of rheumatic heart disease with TI accompanying with bicuspid pathological changes were reviewed. Correlation analysis was done between the s everity of TI and the right ventricular size(RVS), pulmonary artery pressure(PAP ), pulmonary artery resistance(PAR) etc. Results It was found t hat the size of the right ventricle, PAP, PAR were positively correlated to th e degree of TI. Tricuspid annuloplasty should be carried out in patients with RV D>40 mm, PAR >48 kPa*s/L, PAP>8 kPa. Conclusion TI resulted ma inly from insufficient function of the right ventricle and marked patholog ical changes of the pulmonary blood vessels. Doppler echocardiography evaluation for the function of right ventricle and pathological condition of pulmonary blo od vessels might be of significant in deciding whether tricuspid annuloplasty sh ould be performed simutaneously in patients of bicuspid valve replacement.
