ABSTRACT

Background: Frozen Sections (FS) are used to assess margins, for staging, and primary diagnosis. FS guide intraoperative treatment decisions in oncological gastro-intestinal tract surgeries and further management of the patients. Aim: To analyze the distribution, sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of frozen sections in gastrointestinal pathology in our institution during the period of 3 years (2016–2018). Material and Methods: This study was an audit to determine the accuracy of FS reports by comparing them with the paraffin section (PS) reports. The FS diagnoses and their PS diagnoses were noted in 1704 gastrointestinal surgeries during the period from 2016 to 2018. Discrepancies were noted and slides of discrepant cases were reviewed to determine the cause. Sensitivity, specificity, positive predictive value, negative predictive value, and accuracy were calculated using the standard formulae. Results: Out of 1704 cases, correct diagnosis on frozen section was made in 1649 cases (96.77%), 20 (1.17%) were deferred cases, and 35 (2.05%) were discrepant cases. The commonest discrepancies were seen in the primary diagnosis of the gall bladder and gastrectomy margins. The commonest causes for discrepancies were interpretation errors and technical errors. Sensitivity was 91.71%, specificity was 99.69%, positive predictive value was 98.84%, negative predictive value was 97.68%, and accuracy was 97.92%. Conclusion: FS diagnosis is a reliable guide to surgeons for intraoperative management. Studying deep cuts and careful sampling at frozen sections will help reduce discrepancies.

ABSTRACT

Primary hepatic epithelioid hemangioendothelioma (HEHE) is a rare tumor with an incidence of <0.1 per 100,000. The clinical course is variable with variable outcomes. Due to its rarity, treatment protocols, prognostic and predictive factors are not well established underscoring the need for such a study. Pathologists’ awareness of this entity, a meticulous morphologic examination coupled with immunohistochemistry can aid in accurate diagnosis.

ABSTRACT

Introduction: Placenta is related to mother and foetus viaindirect interaction with maternal blood that spurts out ofuteroplacental vessels. By study of placental bed newinformation has come to light, especially for pre – eclampsiaand intra uterine fetal growth retardation.Aim: To study changes in basement membrane in pregnancyinduced hypertensive and normotensive placentae.Materials and Methods: The present study was conducted inthe Department of Anatomy, MMMCH, Kumarhatti, Solan tostudy the histological changes in placenta in pregnancyinduced hypertensive parturients as compared to normotensivesubjects. It was based on 100 cases, 25 from normotensivesubjects (control group) and 75 from clinically proven cases ofpregnancy induced hypertension (study group). An attempt was made to see any changes in histologicalfeatures of placentae of pregnancy induced hypertensive casesand compare it with normotensive placentae. Results and Conclusion: The microscopic study showedsignificant thickening of basement membrane in PIH placentae. As compared to normotensive placentae (control) and are dueto occlusion or narrowing of utero placental vasculaturesuggesting thereby compromised foeto – placental circulation.

ABSTRACT

BACKGROUND: The aim of this study was to look at the outcome of patients with metastatic pancreatic cancer treated at a tertiary cancer center in India. PATIENTS AND METHODS: A total of 101 patients with locally advanced and metastatic pancreatic cancer diagnosed between May 2012 and July 2013 were identified from a prospectively maintained database at the tertiary cancer center. Overall survival (OS) was computed using the Kaplan–Meir product limit method and compared across groups using the log‑rank statistics. Cox proportional hazards model, adjusted for a number of patient and tumor characteristics, was then used to determine factors prognostic for OS. RESULTS: Median age at diagnosis was 55 years (range: 21–81 years). 57.4% (n = 58) of patients were male, 22% (n = 22) had performance status (PS) of <2 at diagnosis and 89% received first‑line chemotherapy, while the rest received best supportive care. For the whole cohort, 6 month and 1‑year OS was 57% (95% confidence interval [CI]: 46–66%) and 47% (95% CI: 35–57%), respectively. In a multivariable model, PS <2 and oligometastatic disease were associated with a significantly decreased risk of death. CONCLUSION: Results from our analysis indicate that the prognostic outcome among Indian patients with metastatic pancreatic cancer is poor with survival outcomes similar to those reported in North America and Europe.

ABSTRACT

The study involves the gross and microscopic examination of coronary arteries of 200 cases of all age groups brought to mortuary of Rajindra Hospital Patiala for post mortem from Malwa Region of Punjab. The study was carried out to find out prevalence, pattern, frequency and severity of atherosclerotic changes in coronary arteries. The study was conducted in the department of Forensic Medicine and Department of Pathology Government Medical College Patiala.

ABSTRACT

Liver is the site of many diseases, many of which become symptomatic while some are diagnosed only on autopsy. Involvement of liver is secondary to cardiac, metabolic and social problems like alcoholism. In this autopsy study of one hundred cases of liver specimens, fatty change, venous congestion, cirrhosis of liver, malignancy, hepatitis and chronic abscess are reported; fatty change being the predominant finding.

ABSTRACT

A fifty year male (K.S.) presented with protrusion (Proptosis) of left eye which was gradually increasing in size for the last one year. The swelling was accompanied by pain for the last fifteen days. Preoperative X-ray & computed tomography showed erosion of frontal sinus & roof of maxillary sinus. Clinical diagnosis of a malignant tumour of eye was made & enucleation of the left eye, along with the mass was done. The specimen was received in the pathology department on 16.9.98. Grossly, encapsulated mass attached with the intact eye ball was received. The mass measured 5x3x2.5 cms. Microscopic examination revealed a benign spindle cell neoplasm and a diagnosis of fibrous histiocytoma (dermatofibroma) was made.

ABSTRACT

Antibody isotypic levels (IgM, IgE and IgG subclasses) to infective larvae (L3) of Wuchereria bancrofti were measured in 104 normal individuals from a filaria-endemic region in Orissa. The titres of antibodies were considerably higher in adults (n = 25, 25·1 ± 3·8 year) than in children (n = 52, 7·1 ± 2·1 year). Young children (n = 14) less than four years were seronegative to all isotypes other than IgM, the sero-conversion to which was achieved in the children (n=15) by the age of 7·5±1·2 years. The prevalence of other isotypes increased with age and reached a maximum in early adulthood (18·6 ± 1·6 years), which persisted in older adults (> 30 years). However, the increase in IgG3 prevalence with age was less marked. IgG2 was detected only after 10 years of age. Compared to the high prevalence (100%) of IgM, IgE, IgG1, and IgG2, in adults. IgG3 and IgG4 prevalences were low, 35% and 28% respectively. IgA level to L3 antigen was found to be extremely low even in adults. These data indicate that the prevalence of L3 antibodies was different for different isotypes and the acquisition of antibody response essentially followed an age dependent pattern.
