ABSTRACT

Objective:To assess the effectiveness and characteristics of intratumoral radioactive seed implantation in pancreatic cancer pain management.Methods:Clinical data of 160 patients with pancreatic cancer receiving radioactive seed implantation were retrospectively analyzed. Both pre- and postoperative pain intensities were evaluated using the visual analog scale (VAS) .Results:About 71.88% (115) of 160 patients experienced abdominal or low back pain. Postoperative pain in 104 patients was relieved at various degrees after radioactive seed implantation with an analgesic efficacy of 90.43% (the efficacy for abdominal and low back pain relief was 86.52% and 96.34%, respectively). The between-group difference was statistically significant. Pain relief was observed 1-7 days postoperatively, and the maximal degree of pain relief was achieved 2-14 days after treatment initiation.Conclusion:Intratumoral implantation of radioactive seeds was microinvasive, quick-acting, and effective in pancreatic cancer pain management.

ABSTRACT

Ultrasound-guided radioactive 125I particle implantation for the treatment of advanced gallbladder cancer is susceptible to factors such as ribs, respiratory activity, and biliary reflex, which brings great inconvenience to the operation. We reported one case of gallbladder cancer patients with unclear ultrasound imaging under general anesthesia mechanical ventilation and successful transplantation after sustained inflation with general anesthesia in order to providing basis of clinical diagnosis and treatment.

ABSTRACT

Objective:To investigate the expression, prognostic value and potential mechanism for the role of SNHG4 in gastric cancer.Methods:The expression of SNHG4 in gastric cancer was analyzed by UALCAN database. The relationship between SNHG4 and prognosis of gastric cancer was analyzed by Kaplan-Meier Plotter. SNHG4-miRNA-mRNA regulatory network was constructed by StarBase, Targetscan, microT-CDS and Cytoscape. The target genes were analysis GO and KEGG pathway enrichment by DAVID database.Results:The expression of SNHG4 in gastric cancer was significantly higher than that in normal tissues ( P=8.882E-16) . The overall survival time of patients with high SNHG4 expression was lower than that of patients with low expression ( P=8.900E-05) . Through the construction of RNA regulatory network, we found that hsa-let-7a-5p ( P=1.02E-03) , hsa-miR-152-3p ( P=4.51E-06) , hsa-miR-204-5p ( P=6.68E-04) and hsa-miR-363-3p ( P=8.06E-03) could be used as the binding sites of SNHG4 in gastric cancer, and these four miRNAs further regulated 250 downstream target genes. Through GO and KEGG enrichment analysis of the target genes, we found that these target genes played roles in the biological process of protein phosphorylation regulation, transcription negative regulation, RNA polymerase II promoter transcription, and participated in the occurrence and development of gastric cancer by blocking or activating Wnt and other signal pathways. Conclusions:SNHG4 can be used as a potential tumor marker for gastric cancer to judge the prognosis of gastric cancer. By constructing a SNHG4-miRNA-mRNA regulatory network, the pathogenesis of gastric cancer can be studied at the molecular level. This provides a clear direction for experimental and clinical research.

ABSTRACT

Objective:To identify the significance of interstitial implantation of radioactive seeds for stage IV pancreatic cancer.Methods:Clinical and follow-up data of 37 patients of stage Ⅳ pancreatic cancer with ultrasound-guided percutaneous radioactive seeds implantation as the primary treatment were retrospectively analyzed.Results:The postoperative survival time of 37 cases in the whole group was (7.49±4.08) months, ranging from 3 to17 months. Of the 30 patients who had upper abdominal and/or back pain before operation, 27 patients got different degrees of pain relief after operation; 20 cases were found with ascites before operation, but ascites in 18 cases decreased or disappeared after operation.Conclusion:The treatment of interstitial implantation of 125I radioactive seeds can effectively upgrade the life quality of stage IV pancreatic cancer patients.

The apphcation of radioactive seeds implantation in treatment of malignant solid tumor of the abdomen has been 18 years in China.The teatment is applied by more and more hospitals,however,the operation procedure is not the same in each hospital,which affected the therapeutic effect of radioactive seeds implantation,and means more safety risks.This paper summarizes the operation procedures of radioactive seeds implantation in treatment of malignant solid tumor of the abdomen,and minimizes the loopholes in clinical operation,in order to improve the therapeutic effect and reduce the safety risks.

ABSTRACT

Objective:To discuss the clinical effects of implantation of radioative 125I seeds by the way of percutaneous puncture and laparotomy under the guidance of ultrasound in the treatment of locally advanced pancreatic cancer, and to provide the basis for choosing surgical methods in treating advanced pancreatic cancer.Methods:The clinical materials of 73 patients with advanced pancreatic cancer were collected, including 42 patients who underwent implantation of radioactive 125I seeds by percutaneous puncture(group A) and 31 patients who underwent impantation of radioactive 125I seeds by laparotomy(group B).The pain relief, local control of tumor, postoperative survival time and complications of the patients were compared between two groups. Results:The rates of pain relief of the patients in group A and group B were 91.89% and 86.40%,and there was no significant difference(P=0.815).The local control rates of the patients in group A and group B were 71.43% and 77.42% ,and there was no significant difference(P=0.564).The medium survival time of the patients in group A and group B were 11 months and 12 months;the one-year survival rates were 36.9% and 35.8%, and there was no significant difference(P=0.664).Seven patients in group A got fever;in group B, six patients got fever, two got calf muscle venous thrombosis, one got gastric retention, one got bilioentric anastomosis, one got abdominal distension and one got intestinal obstruction in the early stage after operation.The incidence rates of complications of the patients in two groups were 16.67% and 38.71%, and there was significant difference(P=0.034).Conclusion:Percutaneous implantation of radioactive 125I particles guided by ultrasound causes less complications in the treatment of locally advanced pancreatic cancer.Moreover, the percutaneous way reaches the same effect as the intraoperative way does on the pain relief, local control of tumor and survival time prolonged.

ABSTRACT

Objective To assess the efficacy and safety of ultrasound-guided percutaneous puncturing gastric wall implantation of 125 I seeds in advanced pancreatic carcinoma . Methods Sixty-five cases ( 65 tumors ) of advanced pancreatic carcinoma implanted 125 I seeds by ultrasound-guided percutaneous puncturing gastric wall were retrospectively analyzed . The change of length diameter of tumor and pain relief were evaluated . Results The total 65 cases were performed successfully according to the preoperative plans . In the following 3 months after the operation ,the results showed that complete remission (CR) was achieved in 19 cases ,partial remission( PR) was 33 ,stable disease( SD) was 10 and the progressive disease ( PD) was 3 . The overall response rate was 80% . About the relief of pain ,complete relief patients were 36 , partial response patients were 11 and the invalid patients were 5 . The pain relief rate was 90 .38% . There were no bleeding ,pancreatic fistula ,biliary fistula ,gastric perforation which need to do the surgery therapy . Two cases of pancreatic pseudocyst occurred when reviewed in the third month after the operation . Conclusions It is effective and safe to adopt ultrasound-guided percutaneous puncturing gastric wall implantation of 125 I seeds in advanced pancreatic carcinoma .

ABSTRACT

objective To clarify the significance and puncture path of 125I radioactive seeds implantation in treatment of postoperative local recurrence and pelvic metastasis in patients with rectal cancer.Methods Clinical data of 8 rectal cancer patients with postoperative local recurrence and pelvic metastasis treated by ultrasound guided 125I radioactive seed implantation were retrospectively analyzed and the therapeutic effects were observed.Results After the treatment,the symptoms of 5 cases suffering from hematochezia and 2 cases with vaginal bleeding disappeared within 21 to 30 days.7 patients with preoperative pain relieved after treatment.There was no tumor progression from postoperative imaging examination.Conclusion 125I radioactive seed implantation can effectively improve the life quality of rectal cancer patients with postoperative local recurrence and pelvic metastasis.

ABSTRACT

Objective To make clear the clinical safety of ultrasound guided percutaneous 125I radioactive seed interstitial implantation for treatment of pancreatic cancer.Methods 92 cases of pancreatic cancer treated with ultrasound guided percutaneous 125I radioactive seed interstitial implantation were retrospectively ana lyzed.Results All the 92 cases were performed successfully according to the preoperative plan.25 cases suffered puncture-hemorrhage and blood loss about 2 to 20 ml,and all got hemostasis successfully.2 cases had pancreatic duct injury during puncture,but no pancreatic fistula was found after conservative treatment.No case had bile duct injury.No digestive tract perforation occurred in the 55 cases whose puncture path was through the gastrointestinal tract.No serious complications occurred during the surgery.Conclusions Ultrasound guided percutaneous 125I radioactive seed interstitial implantation for treatment of pancreatic cancer is safe and reliable.

ABSTRACT

Objective Subjective to identify the significance of interstitial implantation of 125Ⅰ seeds in stage Ⅳ pancreatic cancer patients.Methods Retrospectively analysis the clinical datum of 18 cases of stage Ⅳ pancreatic cancer patients treated with the 125Ⅰ seeds implantation, and observe the pain relief rate and the average survival period, then compare with the bulk of domestic statistical datum.Results The rate of easement of pain was 70.6% (12/17) , and the mean time begin to relieve was 2.9 days .The life span after the operation was be-tween 2.5 and 11.5 months , and 5.2 months on average,which is longer conspicuously than the statistics of the past.Conclusions The methods of interstitial implantation of 125Ⅰ seeds can upgrade the quality of the life and living efficiently of stage Ⅳ pancreatic cancer patients.

ABSTRACT

ObjectiveTo investigate the clinical characteristics of nodular goiter in Changchun area. Methods An retrospective analysis was made on 4 453 cases of nodular goiter proved by pathology in a period of 39 years. Results Surgery performed for nodular goiter accounted for 44.48% of all thyroid operations, with a rate of male to female of 1∶4.95. The male patients were significantly older than female. The correct diagnosis rate of nodular goiter was 71.95%, thyroid adenoma and thyroid cyst were common misdiagnosis. The right lobe was more subject to thyroid goiter than the left one, and canceration developed in 3.66% of all nodular goiter. Conclusions Changchun is an area of iodine deficiency. There are close relations between pregnancy, lactation, menses and the development of nodular goiter. Nodular thyroid goiter is subject to canceration.goit

ABSTRACT

Objective To explore the mutation of RET gene exon 11 in sporadic medullary thyroid carcinoma and to clarify the relationship between RET mutation and sporadic medullary thyroid carcinomas. Methods Genomic DNA was extracted from 17 sporadic medullary thyroid carcinoma tissues. Exon 11 of RET gene was amplified by polymerase chain reaction, analyzed by automated DNA sequencer after purification and results were compared with normal sequence. Results RET mutation was detected in 1 of 17 cases and the mutation revealed the wrong base at the position 15165(G→A) and the missense codon at 691 (ser 691 →lys). Conclusion The low rate of RET exon 11 mutation in sporadic medullary thyroid carcinomas indicate that the key gene mutations may locate on other exons of RET gene or other genes.

Objective To study the constituent ratio of thyroid nodules in inland Changchun area and compare it with the data in the coastal Shanghai area Methods A retrospective analysis was made on 9216 cases of thyroid nodules proved by pathology in a period of 39 years in Changchun and on 7018 cases in a period of 37 years in Shanghai, to compare the difference of the constituent ratio of thyroid cyst, nodular goiter, thyroid adenoma and thyroid carcinoma in these two areas Results The constituent ratio of thyroid cyst in Changchun and Shanghai area was 1 83% and 8 68%, that of nodular goiter was 48 31% and 18 62%, that of thyroid adenoma was 39 30% and 49 94%, and that of thyroid carcinoma was 10 55% and 22 76%, respectively There was a significant difference in the constituent ratio of thyroid nodules between the two areas Conclusion The difference is significant in the constituent ratio of thyroid nodules between China inland and coastal areas

ABSTRACT

DNA was extracted from thyroid nodular tissue in 16 patients with toxic multinodular goitor (TMG). After PCR, the product of PCR was sequenced, and 3 cases with point mutation (A1964T) and 2 cases with insertion mutation (a G was inserted after nucleotide 1928) in TSH receptor gene were found. It is postulated that these mutations may play an important role in the pathogenesis of TMG.
