ABSTRACT

Objective To observe the therapeutic effect of acupoint injection with nucleotide plus conventional western medicine treatment for moderate and severe chronic obstructive pulmonary disease (COPD).Methods Sixty patients confirmed as moderate and severe COPD were evenly randomized into treatment group and control group.The treatment group was given conventional western medicine treatment and injection with nucleotide on bilateral Dingchuan points,and the control group was given conventional western medicine treatment alone.The treatment lasted for one year.The frequency of COPD acute onset and hospitalization times within one year were recorded.Meanwhile,the severity of dyspnea was evaluated with Medical Research Council (MRC) dyspnea scale scoring.Results (1)The frequency of COPD acute onset and hospitalization times within one year in the treatment group were lowered,and the differences were significant compared with those in the control group (P < 0.05 or P < 0.01).(2) After treatment,MRC dyspnea scale scores in the treatment group were improved (P < 0.05 compared with those before treatment),but the improvement of the scores was insignificant in the control group(P > 0.05).The intergroup comparison showed that the effect on improving MRC dyspnea scale scores in the treatment group was superior to that in the control group (P < 0.05),indicating that the severity of dyspnea was much improved in the treatment group as compared with the control group.Conclusion Acupoint injection with nucleotide is effective on enhancing the therapeutic effect of conventional western medicine treatment for moderate and severe COPD.

ABSTRACT

Objective To investigate the application value of MRI in the diagnosing and assessing the severity of acute pancreatitis (AP).Methods 78 cases suspected with AP in our hospital underwent MRI.According to the golden standard of combination with the observation of the patients during hospitalization and the surgical pathological results,the sensitivity,accuracy and specificity of MRI in diagnosing AP were calculated.MRI Balthazar scoring system was used to evaluate the severity grade of AP.The results were compared with Ranson evaluation criterion usually used in clinic to evaluate the severity of AP.Results A total of 64 patients were final diagnosed by means of clinic and pathology.The sensitivity,accuracy and specificity of the MRI were 95.31%(61/64),93.59 % (73/78) and 85.71%(12/14),respectively.With the clinical and pathological diagnosis,the patient with mild AP were 45 cases,and the severe AP were 19 cases.The accuracy of MRI Balthazar severity grading was 95.31% (61/64),which was significantly higher than that of Ranson evaluation accuracy 82.81 %(53/64),with P =0.023.Conclusion MRI is an effective method for clinical diagnosis of AP,and it is superior to Ranson evaluation criterion for assessment of the severity,of which has a significant role in guiding clinical diagnosis and treatment.

ABSTRACT

Objective To analyze the MRI features of hydrosalpinx and to investigate its clinical value.Methods MRI and ultrasound manifestations in 40 patients with hydrosalpinx in 53 fallopian tubes confirmed by operative and pathological findings were analyzed retrospectively,and these findings were also compared with the results of aparoscopy and pathology.Results Among 53 fallopian tubes with hydrosalpinx,bilateral tubes in 13 patients and 27 unilateral tubes in other patients were found.The tubes were botuliform in 32, retort-shaped in 1 6,pouch-shaped in 5.Incomplete separation of the lumen were found in 1 9.In 9 patients with acute salpingitis,1 5 tubes were found with empyema and expansion.In other 31 patients with chronic salpingitis,38 tubes were with hydrops and expansion,14 of whom were with hematocele.The sensibilities of MRI and ultrasound diagnosis for hydrosalpinx were 94.3%(50/53)and 88.7%(47/53)with no statistical differences(P >0.05),however the specificitis were 90.6%(48/53)and 77.6%(41/53)with obvious statistical differences(P <0.05).Conclusion The locating and qualitative diagnosis of MRI for hydrosalpinx is superior to ultrasound.MRI can discriminate the quality of cyst fluid and thus can definitely diagnose the hydrosalpinx caused by acute or chronic salpingitis.

ABSTRACT

A sensitive and robust liquid chromatography-tandem mass spectrometry [LC-MS/MS] method was developed and validated for the determination of corosolic acid [CRA] in rat plasma. The plasma samples were prepared by a simple protein precipitation treatment. Chromatographic separation was performed on an Agela MG-C[18] column [50 mm x 4.6 mm; i.d., 5 microm] at 30[degree]C. The mobile phase consisting of methanol-10 mM ammonium acetate-formic acid [80:20:0.1, v/v/v; pH=3.8] was delivered with a flow rate of 0.6 ml/min and a running time of 2.5 min. Quantification was performed with positive electrospray ionization [ESI] in the multiple-reaction monitoring [MRM] mode at m/z 490 - m/z 205 for CRA, and m/z 471 - m/z 203 for TEOA [internal standard, IS], respectively. Linearity was evaluated over the concentration range of 1.00-1000 ng/ml with a lower limit of quantification [LLOQ] of 1.00 ng/ml. The validated method was successfully applied to the pharmacokinetic study of CRA after the drug was orally administered in Sprague-Dawley rats.

ABSTRACT

<p><b>OBJECTIVE</b>To develop a LC-MS/MS method for determination of astragalin in rat plasma and study its pharmacokinetics in rats.</p><p><b>METHODS</b>The analytical column was packed with ZORBAX SB-C18, and the mobile phase was methanol-water containing 10 mmol/L ammonium acetate-formic acid (80:20:0.15, V/V/V). Quercetin was used as the internal standard (IS). Multiple reaction monitoring (MRM) mode was employed, and the transition of m/z was m/z 449.1→m/z 287.1 for astragalin, and m/z 301.1→m/z 151.1 for IS.</p><p><b>RESULTS</b>A good liner relationship was obtained within the range of 1.00-1000 ng/ml (r(2)=0.9929), and the lower limit of quantification of astragalin was 1.00 ng/ml in rat plasma. The extraction recoveries were all above 93%. After oral administration of astragalin, the maximum plasma concentration of astragalin was 231.1∓67.3 ng/ml and the time to reach this value was 0.5∓0.1 h, with a half-life of 3.9∓1.3 h and an AUC of 782.6∓152.8 ng·h/ml.</p><p><b>CONCLUSION</b>The method is highly sensitive, selective and rapid for determination of the concentration of astragalin in rat plasma to facilitates the study of its pharmacokinetics.</p>

ABSTRACT

Objective To analyze and identify the chemical composition in water extracts from Hugu capsule and provide evidence for its pharmacological study and quality control. Methods A HPLC-DAD method was used. The separation was performed on Kromasil C18 column with acetonitrile and 1%glacial acetic acid as mobile phase by gradient elution at a flow rate of 0.8 mL/min. The chemical composition in water extracts from Hugu capsule were identified by comparison with the related chromatographic fingerprint. Results Thirteen characteristic peaks were found in the HPLC chromatographic fingerprint, and four peaks were identified. Conclusion The HPLC-DAD fingerprint expressed the general character of the chemical composition in water extracts from Hugu capsule. It can be used for qualitative analysis of water extracts from Hugu capsule, and improve the quality of Hugu capsule.

ABSTRACT

Objective To evaluate the effects ofbeta-endorphin (β-EP) and substance P (SP)with sedative and analgesic drugs on mechanical ventilated patients. Methods Twenty-eight mechanical ventilated patients were randomly divided into two groups: midazolam group (M group, 14 cases) and midazolam combined with fentanyl group (M + F group, 14 cases). Eight healthy persons were as control group (C group). The sedative target was VAS≤3 scores and Ramsay 2-4 scores. The levels of serum β -EP and SP were tested before sedation and 12, 24 h after sedation in mechanical ventilated patients and at 8 Am in C group. The sedation levels were evaluated and the hemodynamie and respiratory parameters were recorded before sedation and 1, 12, 24 h after sedation in mechanical ventilated patients. The oxygenation index was measured before sedation and 1,12, 24 h after sedation. Results The levels of serum β -EP and SP in M and M+F group were significantly higher than those in C group(P< 0.05). After sedation, the level of SP in M+F group [(101.42 ± 12.46) ng/L]was significantly lower than that in M group [(132.72 ± 23.82) ng/L] (P < 0.05). Compared with before sedation, there were significant differences in heart rate, VAS and Ramsay scores between M group and M+F group (P< 0.05). Compared with M group, pressure airway and respiratory rate at 12, 24 h and total after sedation were lower in M+F group (P <0.05). The amount of serum SP in mechanical ventilated patients. Fentanyl improves the ventilator synehron and reduces the dose of midazolam.

OBJECTIVE:To prepare solid dispersion for increasing solubility of quercetin METHODS:Using polyvinyl pyrrolidone(PVP) as the carrier,solid dispersion of quercetin was prepared by solvent -volatilization method The solubility of the solid dispersion was detected IR spectrometer and UV spectrometer were used to investigate the physical and chemical properties of the solid dispersion RESULTS:The solubility of solid dispersion was significantly increased compared with quercetin and the physical mixture Quercetin existed as ultrafine crystalline or molecular form in solid dispersion,so there was no chemical reaction between quercetin and PVP CONCLUSION:PVP could be used as the carrier of solid dispersion to increase the solubility of quercetin in water

OBJECTIVE:To optimizate base material composition and technology for5F scar emulsion.METHODS:According to the factors that affect the stability of emulsion,orthogonal design method L 9 (3 4 )was used,glyceryl monostearate,triethanolamine,sodium lauryl sulfate and emulsifying temperature were selected as variable factors.RESULTS:The optimum base material composition and technology were glyceryl monostearate6%,sodium lauryl sulfate0%,triethanolamine0.5%,emulsifying temperature80℃.CONCLUSION:The5F scar emulsion prepared by this composition and technology accords with the stipulation of Chinese Pharmacopoeia(2000).

ABSTRACT

Objective To investigate pathogens and drug resistance of lower respiratory tract infection(LRTI)in Intensive Care Unit(ICU).Methods Retrospective study of the clinical data,the distribution and the drug-sensitivity of pathogens of 220 cases with LRTI in ICU.Results Totally 280 strains of pathogens were identified by bacterial culturing.The ratio of G-bacteria to total pathogens isolated was 63.5%,of the G+ bacteria was 25.1%,and of the fungi was 11.4%.The main kinds of the G-bacteria were Klebsiella pneumoniae(17.1%),Pseudomonas aeruginosa(13.2%),Acinetobacter baumannii(12.5%),and Stenotrophomonas maltophilia(10.4%).Staphylococcus aureus(SA)(91.4%)was the most prominent in G+ bacteria,and MRSA was 98.4% in SA.The result of drug sensitive test in vitro showed the multiple drug fast rate of Pseudomonas aeruginosa was comparatively high,Stenotrophomonas maltophilia to Levofloxacin was low,Klebsiella pneumoniae and Acinetobacter baumannii were highly sensitive to carbapenems.The susceptibility rate of MRSA to vancomycin was 100%.Conclusion G-bacteria are the majority of the pathogens,isolated from patients with LRTI in ICU.Klebsiella pneumoniae,Pseudomonas aeruginosa,Acinetobacter baumannii,and Stenotrophomonas maltophilia are the chief G-pathogens.Except Stenotrophomonas maltophilia,imipenem and merpenem are relatively active against the G-bacilli.The proportion of MRSA and fungal infection is increasing.It is suggested that there be urgent need for surveillance of bacterial resistance and rational use of antimicrobial agents during clinical therapy.

OBJECTIVE:To establish a method for determination of the content of quercetin in quercetin-PVP solid dis?persions.METHODS:The detection wavelength for dertermination of quercetin content was set at374nm.RESULTS:The caliberation curve was linear in the range of5.0~25.0?g/ml(r=0.9998).The average recovery was99.12%,RSD=0.70%(n=6).CONCLUSION:This method is stable,quick,accurate and simple,and is suitable for quality control of quercetin in quercetin-PVP solid dispersions.
