ABSTRACT

ABSTRACT Objective To assess the temporal trends of hospitalizations for pulmonary embolism (PE) in Brazil, its regions, and states between 2008 and 2019. Methods An ecological and time series study was conducted. Data were obtained from the Hospital Information System (SIH) of the Brazilian Ministry of Health. The inflection point regression model was applied for temporal trend analyses. Trends were classified as increasing, decreasing, or stationary according to the slope of the regression line. The Annual Percent Charge (APC) and the Average Annual Percent Change (AAPC) were calculated considering a confidence interval of 95% and p-value <0.05. Furthermore, spatial distribution maps of epidemiological indicators related to PE in Brazil were elaborated. Results There was an increasing trend in the hospitalization rate for PE in Brazil, ranging from 2.57 in 2008 to 4.44/100,000 in 2019 (AAPC=5.6%; p<0.001). Total and average hospitalizations costs also showed increasing trend in the country (AAPC=9.2% and 3.0%, respectively). Still, there was a decrease in the in-hospital mortality rate (from 21.21% to 17.11%; AAPC=-1.9%; p<0.001). Similar trends were observed in most regions. The average hospitalization time in Brazil showed a stationary trend. The hospitalization rate has also increased in 18 states (66.67%). Seven states showed a decrease in the mortality rate (25.93%), except for Roraima, which showed an increasing trend. Conclusion Hospitalizations for PE represent a serious public health problem in Brazil and the temporal patterns observed herein demonstrate an increasing trend in all regions and states of the country.


RESUMO Objetivo Avaliar as tendências temporais das hospitalizações por Embolia Pulmonar (EP) no Brasil, assim como suas regiões e estados no período entre 2008 e 2019. Métodos Foi realizado um estudo ecológico e de série temporal. Os dados foram obtidos do Sistema de Informação Hospitalar (SIH) do Ministério da Saúde (MS) do Brasil. O modelo de regressão de pontos de inflexão foi aplicado para análises de tendências temporais. As tendências foram classificadas como crescentes, decrescentes ou estacionárias de acordo com a inclinação da linha de regressão. O percentual de variação anual (APC) e Percentual de Variação Médio do Período (AAPC) foram calculados considerando Intervalo de Confiança de 95% (IC 95%) e significância de 5%. Além disso, foram elaborados mapas de distribuição espacial dos indicadores epidemiológicos relacionados à EP no Brasil. Resultados Houve uma tendência crescente na taxa de hospitalização de EP no Brasil, variando de 2,57 em 2008 a 4,44/100.000 em 2019 (AAPC=5,6%; p<0,001). Os custos totais e médios de hospitalização também mostraram uma tendência crescente no país (AAPC=9,2% e 3,0%, respectivamente). Ainda assim, houve uma diminuição na taxa de mortalidade hospitalar (de 21,21% para 17,11%; AAPC=-1,9%; p<0,001). Tendências similares foram observadas na maioria das regiões. O tempo médio de hospitalização no Brasil mostrou uma tendência estacionária. A taxa de hospitalização também aumentou em 18 estados (66,67%). Sete estados mostraram uma diminuição na taxa de mortalidade (25,93%), exceto Roraima, que mostrou uma tendência crescente. Conclusão As hospitalizações de EP representam um grave problema de saúde pública no Brasil, e os padrões temporais aqui observados demonstraram uma tendência crescente em todas as regiões e estados do país.

Resumo Contexto As varizes dos membros inferiores representam uma das doenças mais prevalentes na população mundial e resultam em grande impacto na qualidade de vida dos pacientes devido às limitações nas atividades diárias e no desempenho funcional. Vários autores têm ressaltado a importância da avaliação da qualidade de vida na abordagem de pacientes portadores de doença venosa crônica. Objetivos Avaliar a qualidade de vida antes e após o tratamento cirúrgico de pacientes portadores de varizes dos membros inferiores. Métodos Através de estudo de antes e depois, foram avaliados 92 portadores de varizes dos membros inferiores submetidos a tratamento cirúrgico. Os pacientes foram divididos em grupos de acordo com a faixa etária e a classificação clínica CEAP. A qualidade de vida foi avaliada através do questionário VEINES QOL/SYM, aplicado no pré-operatório e 60 dias após a operação. Resultados Do total de 92 indivíduos analisados, 82,6% (76) eram mulheres. Com relação à idade, a média foi de 45,7±12,11. A classe CEAP 2 foi a mais encontrada, em 57,6% dos pacientes. Foi encontrada diferença significativa entre os escores antes e após a cirurgia para os pacientes na faixa etária entre 30 e 40 anos. Não houve diferença entre os escores nos momentos pré e pós-operatório entre os grupos CEAP. Conclusões Não foi encontrada diferença na qualidade de vida antes e após a cirurgia na maioria dos pacientes do estudo.


Abstract Background Lower limb varicose veins are one of the most prevalent diseases in the global population. The disease is chronic and has a great impact on patients' quality of life, limiting daily activities and functional performance. Several authors have emphasized the importance of including quality of life assessment in management of patients with chronic venous disease. Objectives To evaluate quality of life before and after surgical treatment of patients with varicose veins. Methods A before and after study design was employed. Ninety-two people with varicose veins of the lower limbs were treated surgically. Patients were divided into subsets according to age and CEAP clinical classification. Quality of life was assessed using the VEINES QOL/SYM questionnaire, administered during the preoperative period and 60 days after the operation. Results The sample comprised 92 subjects, 82.6% (76) of whom were women and mean age was 45.7±12.11 years. CEAP class 2 was the most frequent clinical classification, in 57.6% of patients. There was a significant difference in scores before and after surgery among patients aged from 30 to 40 years. There was no difference between preoperative and postoperative scores between different CEAP groups. Conclusions No difference in quality of life was observed after surgery in most of the patients in the present study.

ABSTRACT Background: Cyclophosphamide plus thalidomide as induction for multiple myeloma patients eligible for autologous stem cell transplantation may be a limiting factor for cell mobilization. The minimum acceptable mobilized peripheral blood stem cell count to prevent deleterious effects during transplantation is 2.0 × 106 CD34+ cells/kg. Combining other treatments to granulocyte-colony stimulating factor, such as cyclophosphamide, could overcome the mobilization limitation. The objective of this study was to assess the number of CD34+ cells mobilized using granulocyte-colony stimulating factor with and without cyclophosphamide after induction with cyclophosphamide, thalidomide and dexamethasone. Methods: A retrospective study was performed of a cohort of multiple myeloma patients submitted to autologous stem cell transplantations at two Brazilian centers between May 2009 and July 2013. The oral cyclophosphamide and thalidomide induction doses used were 1500 mg/month and 100-200 mg/day, respectively. Mobilization doses were 10-15 mcg/kg granulocyte-colony stimulating factor with 2-4 g/m2 cyclophosphamide, or 15-20 mcg/kg granulocyte-colony stimulating factor alone for 5 days. Collection of >2.0 × 106 CD34+ cells/kg was considered sufficient. Results: Eighty-eight patients were analyzed; only 18 received cyclophosphamide. The median age was 58 years old (range: 51-62) for the granulocyte-colony stimulating factor group and 56.5 years old (range: 54-60) for granulocyte-colony stimulating factor plus cyclophosphamide group. Fifty-two patients were male. Eighty cases (90.9%) were Durie-Salmon Staging System III-A/B and 38 (44.7%) and 20 cases (23.5%) were International Staging System 2 and 3, respectively. The group that received cyclophosphamide collected a higher median number of progenitor cells [3.8 (range: 3.1-4.4) vs. 3.2 (range: 2.3-3.8)] (p-value = 0.008). No correlation was observed between better responses or number of induction cycles and the number of cells collected. Conclusion: The number of cells mobilized with granulocyte-colony stimulating factor plus cyclophosphamide was higher. However, in both groups, the median number of CD34+ cells was sufficient to perform a single autologous stem cell transplantation; no deleterious effects were reported during harvesting.

A partir do romance de Clarice Lispector, A hora da estrela, o autor tenta estabelecer um paralelo e um diálogo fictício com algumas ideias psicanalíticas, notadamente as ideias de Fábio Herrmann, autor da Teoria dos Campos, e as de Isaías Melsohn, tais como aparecem em seu livro Psicanálise em nova chave. O objetivo é mostrar que o artista, muitas vezes com outras palavras, expressa aquelas ideias na construção de sua obra literária, expressão simbólica de sentimentos e emoções.


Making use of Clarice Lispector novel A hora da estrela, the author tries to establish a parallel and a fictitious dialogue with some psychoanalytical ideas, notedly Fábio Herrmann's ones, author of Theory of the Fields and the ones of Isaías Melsohn the way they appear on his book Psicanálise em nova chave. The aim is to show that the artist, most of the times with another words, expresses those ideas during construction of his literary work, which is simbolic expression of feelings and emotions.

ABSTRACT

Objective To validate the minimal criteria of the histopathologic diagnosis of oral chronic graft-versus-host disease, based on the histopathologic classification of the National Institutes of Health and correlate them with clinical features. Methods Forty-one specimens containing both oral mucosa and salivary glands were analyzed in slides stained with hematoxylin-eosin. The histological specimens were blindly examined by two trained pathologists using criteria recommended for the histopathologic diagnosis of chronic graft-versus-host disease proposed by the National Institutes of Health Consensus. The clinical classification of chronic graft-versus-host disease was correlated with analysis of slides. Results: Our data showed that the epithelium was involved in 39/41 specimens, presenting acanthosis (29/70.7%), exocytosis of lymphocytes (29/70.7%), thickening of basal lamina (29/70.7%), and apoptosis (15/36.6%). Connective tissue presented interstitial inflammatory infiltrate (38/92.7%). Minor salivary glands showed periductal fibrosis (38/92.7%), mixed periductal inflammatory infiltrate (32/78%), ductal ectasia (30/73.2%), lymphocytes around and into acinar units (30/73.2%), and interstitial fibrosis (29/70.7%). The most common clinical manifestations were lichenoid aspect (40/97.6%), complaints of sensitivity to oral feeding (38/92.7%), and dry mouth sensation (36/87.8%). Conclusion This study validated the National Institutes of Health Consensus of minimal histologic criteria for diagnosis of oral chronic graft-versus-host disease and has not found an association between the severity of clinical manifestation and the histopathological stage. .


Objetivo Validar os critérios mínimos de diagnóstico histopatológico da doença do enxerto contra hospedeiro crônica oral, com base em critérios de classificação do National Institutes of Health, e correlacioná-los com as características clínicas. Métodos Quarenta e um espécimes contendo mucosa oral e glândulas salivares foram analisados em lâminas coradas por hematoxilina-eosina. Os espécimes histológicos foram avaliados de forma cega, por dois patologistas calibrados, utilizando os critérios recomendados para diagnóstico histopatológico de doença do enxerto contra hospedeiro crônica propostos pelo Consenso do National Institutes of Health. A classificação clínica da doença do enxerto contra hospedeiro crônica foi correlacionada após a análise das lâminas. Resultados Nossos resultados mostraram que o epitélio estava comprometido em 39/41 espécimes, apresentando acantose (29/70,7%), exocitose de linfócitos (29/70,7%), espessamento da lâmina basal (29/70,7%) e apoptose (15/36,6%). O tecido conjuntivo apresentou infiltrado inflamatório intersticial em 38 (92,7%) casos. Nas glândulas salivares menores, observaram-se fibrose periductal (38/92,7%), infiltrado inflamatório periductal misto (32/78%), ectasia ductal (30/73,2%), linfócitos em torno e migrando para dentro dos ácinos (30/73,2%), e fibrose intersticial (29/70,7%). As manifestações clínicas mais comuns foram mucosa de aspecto liquenoide (40/97,6%), queixa de sensibilidade bucal ao se alimentar (38/92,7%), e sensação de boca seca (36/87,8%). Background: Induction therapy followed by high-dose chemotherapy and autologous transplantation is the standard treatment for suitable patients with multiple myeloma. Objective: The aim of this study was to assess whether induction therapy with thalidomidecontaining regimens was associated with improved results compared to vincristine, doxorubicin, and dexamethasone, and whether cyclophosphamide, thalidomide, and dexamethasone were associated with better results than thalidomide and dexamethasone. Methods: The records of 152 patients who underwent autologous transplantation at this institution from August of 2004 to January of 2012 were reviewed, selecting those with at least partial response to a maximum of eight cycles of induction therapy and sufficient follow-up information for analysis. Results: This study included 89 patients; 44 were female, with a mean age of 55 years (there was a significant trend for increasing age over the years of the study).The median number of induction therapy cycles was four, again with a trend of increase over the years.At least a very good partial response to induction therapy was achieved more often in the cyclophosphamide, thalidomide, and dexamethasone group (61.1%) and in the thalidomide and dexamethasone group (59.2%) than in the vincristine, doxorubicin, and dexamethasone group (16.2%). The overall median progression-free survival was 34 months, with no statistically significant difference between the three groups. The overall median survival was not reached, and there was no significant difference between the three groups; the estimated five-year overall survival was 55%. Conclusion: Although the quality of responses appeared to be better with thalidomidecontaining regimens, these improvements did not translate into improved long-term outcomes. BACKGROUND: Atherosclerosis is the main cause of peripheral artery occlusive disease (PAOD) of the lower limbs. Patients with PAOD often also have obstructive atherosclerosis in other arterial sites, mainly the coronary arteries. This means that patients who undergo infrainguinal bypass to treat critical ischemia have a higher risk of AMI. There are, however, few reports in the literature that have assessed this risk properly. OBJECTIVE: The aim of this study was to determine the incidence of acute myocardial infarction in patients who underwent infrainguinal bypass to treat critical ischemia of the lower limbs caused by PAOD. MATERIAL AND METHODS: A total of 64 patients who underwent 82 infrainguinal bypass operations, from February 2011 to July 2012 were studied. All patients had electrocardiograms and troponin I blood assays during the postoperative period (within 72 hours). RESULTS: There were abnormal ECG findings and elevated blood troponin I levels suggestive of AMI in five (6%) of the 82 operations performed. All five had conventional surgery. The incidence of AMI as a proportion of the 52 conventional surgery cases was 9.6%. Two patients died. CONCLUSION: There was a 6% AMI incidence among patients who underwent infrainguinal bypass due to PAOD. Considering only cases operated using conventional surgery, the incidence of AMI was 9.6%. .


CONTEXTO: A doença arterial obstrutiva periférica (DAOP) afeta os membros inferiores (MMII) e tem como principal causa a aterosclerose. Por se tratar de uma doença sistêmica, é frequente que a aterosclerose cause o envolvimento simultâneo de outros sítios arteriais, especialmente das artérias coronárias. Desta forma, os pacientes submetidos a cirurgia de reconstrução arterial periférica, devido a isquemia crítica dos MMII, apresentam maior chance de infarto agudo do miocárdico (IAM). Apesar disso, há na literatura poucos relatos que avaliem a prevalência do IAM adequadamente neste grupo de pacientes. OBJETIVO: Avaliar incidência do infarto agudo do miocárdio no pós-operatório imediato de pacientes submetidos à revascularização de MMII devido a DAOP. MATERIAL E MÉTODOS: Foram avaliados 64 pacientes portadores de DAOP submetidos a 82 cirurgias de revascularização arterial dos MMII, no período de fevereiro de 2011 a julho de 2012. Os pacientes foram submetidos no pós-operatório imediato (até 72 horas) a eletrocardiograma (ECG) e dosagem sanguínea de troponina I cardíaca. RESULTADOS: Considerando as 82 cirurgias realizadas, em cinco (6%) casos, foram encontradas alterações no ECG e elevação sanguínea da troponina I cardíaca compatíveis com infarto agudo do miocárdio. Considerando apenas as 52 cirurgias feitas por técnica convencional, a incidência de IAM foi de 9,6%. Dois pacientes evoluíram ao óbito. CONCLUSÃO: No presente estudo, a incidência do infarto agudo do miocárdio nos pacientes submetidos à revascularização arterial de membros inferiores por DAOP foi de 6%. Considerando apenas as cirurgias feitas por técnica convencional, a incidência de IAM foi de 9,6%. .

ABSTRACT

Este artigo tem como objetivo analisar as propagandas de medicamentos veiculadas em revistas femininas de circulação nacional, conforme a RDC 102/00. Foram analisadas propagandas veiculadas de janeiro de 2008 a junho de 2009 em revistas femininas nacionais. Mais da metade dos exemplares de revistas femininas analisadas contém pelo menos uma propaganda de medicamentos. Embora as propagandas analisadas não tenham apresentado as irregularidades frequentemente encontradas na publicidade geral de medicamentos, a mais evidenciada refere-se ao tamanho da letra, o que provavelmente consiste em uma forma de 'compensação' do espaço que o texto publicitário 'perdeu' para as informações obrigatórias.


This article analyzes the drug advertisements published in women's magazines of national circulation, according to the RDC 102/00. The advertisements were analyzed from January 2008 to June 2009 in national women's magazines. More than half of the analyzed samples of women's magazines contain at least one drug advertising. Although the material analyzed have not shown the deficiencies often found in the general advertising of medicines, the more evident refers to the font size, which probably consists of a form of 'compensation' of advertising space that the text 'lost' for required information.

CONTEXTO: A Doença Venosa Crônica (DVC) dos membros inferiores apresenta uma alta prevalência, estando a cirurgia para cura das varizes dos membros inferiores entre as mais frequentemente realizadas pelos cirurgiões vasculares. Apesar disso, não foi estabelecido, na cidade de Recife e zona metropolitana, o perfil epidemiológico dos pacientes que são submetidos a essa modalidade terapêutica. OBJETIVOS: O objetivo deste trabalho foi avaliar o perfil epidemiológico dos pacientes suubmetidos à cirurgia para a cura das varizes dos membros inferiores. MATERIAL E MÉTODOS: Foram avaliados 201 pacientes submetidos ao tratamento cirúrgico de varizes dos membros inferiores, no Serviço de Cirurgia Vascular no Instituto de Medicina Integral Professor Fernando Figueira (IMIP), no período de agosto de 2006 a abril de 2007. Foram avaliados os seguintes parâmetros: sexo, idade, sedentarismo, sobrepeso e obesidade, e presença de ortostatismo prolongado durante atividade laboral. RESULTADOS: Do total de pacientes avaliados, 175 (87,1%) eram do sexo feminino e 26 (12,9%) do masculino. A faixa etária mais acometida foi a de 41 a 50 anos (32,3%), o sobrepeso estava presente em 38,8% dos pacientes, e a obesidade em 7,5% dos casos. O ortostatismo prolongado, durante a atividade laboral, estava presente em 82,1% dos pacientes avaliados. O grau de escolaridade mais comum, observado em 83,2% dos pacientes, foi de até oito anos de tempo de estudo. O sedentarismo foi encontrado em 69,2% dos pacientes. CONCLUSÃO: A maioria dos pacientes avaliados no presente estudo era do sexo feminino com idade maior que 40 anos, era sedentária e não apresentava sobrepeso ou obesidade, e desenvolvia atividades laborais com ortostatismo prolongado.


BACKGROUND: The Chronic Venous Insufficiency of lower limbs has a high prevalence, and its surgical treatment is one of the most performed. Even so, it has not been reported the epidemiologic profile of the patients that has been underwent to this surgery in the city of Recife. OBJECTIVE: The aim of this report was evaluate the epidemiologic profile of the patients underwent to varicose vein surgery of the lower limbs. MATERIAL AND METHODS: They were evaluated 201 patients underwent to varicose vein surgery of the lower limbs at the Vascular Surgery Service at the Instituto de Medicina Integral Professor Fernando Figueira (IMIP) from august 2006 to april 2007. All the patients were evaluated considering the gender, age, sedentarism, overweight, obesity and the report of long-time in a standing position during work shift. RESULTS: Over all patients evaluated, 175 (87.1%) were females and 26 (12.9%) males. The majority of them (32.3%) were aged from 41 to 50 years, overweight was found in 38.8% of the patients, and obesity in 7.5% of the cases. Long time standing during work shift was reported by 82.1% of the patients, and the time at the school, found in 83.2% of the patients, was eight or less years. Sedentarism was found in 69.2% of the patients. CONCLUSION: The majority of the patients evaluated in the present study was female and more than 40 years-old, reported sedentarism, did not have overweight or obesity and reported to stay a long-time in a standing position during work shift.

OBJECTIVES: 1) To characterize the impact of multiple myeloma on the quality of life of patients treated in two public institutions in São Paulo State, Brazil, using a generic Short Form 36 Health Survey and a questionnaire specific for oncologic patients (QLQ-C30) upon diagnosis, after the clinical treatment, and at day +100 after autologous stem cell transplantation; 2) to evaluate whether autologous stem cell transplantation can improve the quality of life of our economically challenged population aside from providing a clinical benefit and disease control. METHODS: We evaluated 49 patients with multiple myeloma (a total of 70 interviews) using the two questionnaires. The scores upon diagnosis, post-treatment/pre-autologous stem cell transplantation, and at D+100 were compared using ANOVA (a comparison of the three groups), post hoc tests (two-by-two comparisons of the three groups), and paired t-tests (the same case at two different times). RESULTS: Of the included patients, 87.8 percent had a family budget under US $600 (economic class C, D, or E) per month. The generic Short Form 36 Health Survey questionnaire demonstrated that physical function, role-physical, and bodily pain indices were statistically different across all three groups, favoring the D+100 autologous stem cell transplantation group (ANOVA). The questionnaire specific for oncologic patients, the QLQ-C30 questionnaire, confirmed what had been demonstrated by the Short Form 36 Health Survey with respect to physical function and bodily pain, with improvements in role functioning, fatigue, and lack of appetite and constipation, favoring the D+100 autologous stem cell transplant group (ANOVA). The post hoc tests and paired t-tests confirmed a better outcome after autologous stem cell transplantation CONCLUSION: The questionnaire specific for cancer patients seems to be more informative than the generic Short Form 36 Health Survey questionnaire and reflects the real benefit of autologous stem cell transplantation in the quality of life of multiple myeloma patients in two public Brazilian institutions that provide assistance for economically challenged patients.

O crescimento da população idosa brasileira tem levado a uma maior tendência ao uso de medicamentos. A sua utilização inadequada pode ser induzida por vários fatores, dentre eles a propaganda, podendo trazer danos à saúde do usuário. O objetivo do estudo foi avaliar a influência da propaganda no consumo de medicamentos por um grupo de idosos atendidos em unidade básica de saúde de Aracaju, Sergipe. No período de abril a junho de 2007, um grupo de 230 idosos, com idade acima de 60 anos, de ambos os gêneros, foi entrevistado. A maioria dos entrevistados (73 por cento) possuía pelo menos uma doença crônica e 73,9 por cento consumiam pelo menos um medicamento regularmente. Do total de entrevistados, 17,8 por cento da amostra relataram utilizar medicamentos por influência da propaganda; 2,2 por cento consideraram que os medicamentos veiculados na mídia nunca fazem mal e 6,5 por cento acreditavam que eles sempre fazem bem. No estudo, correlações foram feitas e demonstraram que quem mais consome medicamentos influenciados pela propaganda também considera que eles sempre fazem bem e vice-versa (p= 0,04). Os dados revelaram que parte dos idosos sofreu influência da propaganda no consumo de medicamentos, não levando em consideração os riscos que estes poderiam causar.


The growth of the Brazilian elderly people has led to a trend to an increase in the medication use. The inadequate use of drugs can be induced by some factors, like advertisement, with the risk of damaging the user's health. The objective of the study was to evaluate the advertisement influence in medication use in a group of elderly patients in a primary health care unit in Aracaju, Sergipe, Brazil. 230 elderly aging over 60 years with both genders had been interviewed from April to June of 2007. The majority of the interviewees (73 percent) has at least a chronic health condition and 73.9 percent consumed regularly at least one medication. 17.8 percent of the sample informed to use medication motivated by publicity influence; 2.2 percent had considered that the medication never cause damages and 6.5 percent believed that always it makes well. In this study, correlations have been made and demonstrated that those who presented a higher level of consumption influenced by advertising also think that drugs used are always beneficial and vice versa (p= 0.04). The data showed that part of elderly suffered influence of advertisement to medication use, and are not conscious of risks involved.

Abordar epidemiologicamente o uso de medicamentos impõe reconhecer que tal prática não se limita a fatores farmacoterapêuticos. O armazenamento adequado e a preservação de medicamentos são fatores fundamentais para a sua eficácia. O presente estudo teve como objetivo verificar a forma de armazenamento/uso de medicamentos em uma comunidade atendida pelo Programa/Estratégia Saúde da Família no município de Cristino Castro (PI), bem como o grau de conhecimento a respeito dos medicamentos mantidos sob sua guarda. Os dados foram obtidos em visitas domiciliares a 52 famílias, a partir de observações e da aplicação de um questionário. Observaram-se baixa renda e baixa escolaridade da população, fatores agravantes na efetiva orientação terapêutica e acesso aos fármacos. Elevado percentual dos medicamentos estocados é adquirido sem prescrição. Destacamos a classe dos antimicrobianos, reforçando o perigo da automedicação. 62 por cento dos responsáveis pela guarda não possuem a orientação mínima para assegurar o uso racional, e 66 por cento não tem ciência da toxicidade. 54,10 por cento dos medicamentos estavam ao alcance de crianças e animais domésticos e 15,92 por cento não possuíam rótulo ou qualquer identificação. Algumas famílias mantêm em estoque medicações básicas, outras acumulam um "arsenal terapêutico".


To approach the epidemiological use of medicines is necessary to recognize that such practice is not strictly limited to pharmacotherapy. The appropriate storage and the preservation of medicines are fundamental factors for effectiveness. This study was aimed at verifying the storage form/use of medicines in a community assisted by the Family Health Program in the district of Cristino Castro (PI, Brazil), as well as the level of the people's knowledge regarding drugs storage at home. The data were obtained visiting 52 families, starting with observations and applying a questionnaire. It was observed a low income and education levels, a fact that contributes in the effective therapeutic orientation. High percentage of the stored medicines is acquired without prescription. It is worth of mention the presence of a big amount of antimicrobial, reinforcing the danger of the self medication. 62 percent of those who take care of drugs have no information about their rational use, and 66 percent are not conscious of the toxicity. 54.10 percent of the medicines were at children's and domestic animals reach and 15.92 percent didn't possess label or any identification. Some families maintain in stock, basic medications, another accumulate a "therapeutic arsenal".

ABSTRACT

Tendo em vista que a automedicação é um problema de saúde pública mundial, este artigo teve como objetivo avaliar o comportamento dos futuros profissionais de saúde com relação à utilização de medicamentos, particularmente, à prática da automedicação. A amostra foi composta por 223 estudantes da área de saúde de uma universidade pública no município do Recife, selecionados por partilha proporcional a cada curso. Foram feitos questionamentos sobre o uso de medicamentos, nos últimos quinze dias que antecederam à coleta de dados. 65,5 por cento dos entrevistados confirmaram ter feito uso de medicamentos nos últimos quinze dias, 42,3 por cento por meio de uma prescrição médica e 57,7 por cento através da automedicação. Os medicamentos mais utilizados foram os analgésicos (24,0 por cento) e as vitaminas (18,9 por cento) e o motivo mais apontado foi a dor (30,5 por cento). O consumo de medicamentos entre os estudantes universitários no município do Recife é elevado, mas encontra-se dentro dos parâmetros observados por outros estudos. Todavia, por se tratar de profissionais da área de saúde, esperava-se que o consumo fosse menor e mais racionalizado.


As self-medication is a worldwide problem of public health, this article had as objective to evaluate the behavior of the future health professionals with regard to the drug utilization, particularly, to the self-medication practice. 223 academic students from a public university in Recife City were interviewed, selected by proportional share in each course. A questionnaire on the drug utilization was applied, 15 days before the interview. From the 223 students, 65.5 percent reported drug use in the last 15 days. 42.3 percent used drugs through the physician's prescription and 57.7 percent through the self-medication. The most used drugs were the analgesics (24.0 percent) and vitamins (18.9 percent) and the main reason was pain (30.5 percent). The drug utilization among academic students in Recife City is high but it is inside the parameter according to others analysis. However we have expected a low and more rationalized utilization.

ABSTRACT

O transplante de células-tronco hematopoéticas (TCTH) é um procedimento de fundamental importância na estratégia terapêutica das gamopatias monoclonais. No mieloma múltiplo, em particular, o TCTH autólogo está indicado como estratégia de primeira linha para pacientes até 70 anos de idade. Nesta capítulo serão discutidas as indicações, estratégias e recomendações envolvendo o TCTH em gamopatias monoclonais, amiloidose e POEMS, frutos da Reunião de Consenso da Sociedade Brasileira de Transplante de Medula Óssea.


Hematopoietic stem cell transplantation (HSCT) is an important strategy in the treatment of monoclonal gammopathies. For multiple myeloma, in particular, autologous HSCT is indicated as first line therapy for under 70-year-old patients. In this chapter we will discuss indications, strategies and recommendations involving HSCT for monoclonal gammopathies from the Consensus Meeting of the Brazilian Society of Bone Marrow Transplantation.

ABSTRACT

Portadores de leucemia linfoide crônica (LLC) apresentam curso clínico indolente e prolongado que devem ser diferenciados daqueles que têm doença de evolução agressiva e fatal. Pacientes mais jovens e com critérios de alto risco podem se beneficiar com tratamento mais agressivo como o transplante de células-tronco hemopoéticas (TCTH). O transplante autólogo apresenta casos com remissão citogenética e molecular, baixa taxa de mortalidade, mas não demonstram platô nas curvas de sobrevivência e alta taxa de recaídas. Os transplantes alogênicos com regime mieloablativos têm altos índices de toxicidade e mortalidade, mas evidenciam o efeito enxerto versus leucemia, que aumenta a possibilidade de cura destes indivíduos. Assim, a opção dos transplantes alogênicos está dirigida para os transplantes com regime de condicionamento não mieloablativo, que pode ser aplicado inclusive a pacientes mais idosos ou portadores de comorbidades, e manter o potencial efeito GVL. A identificação dos pacientes que podem ser beneficiados por esses procedimentos, caracterizar e apontar os novos marcadores prognósticos permanece objeto de muitos estudos clínicos e foi o objetivo do grupo responsável em discutir as diretrizes do TCTH no consenso da Sociedade Brasileira de Transplante de Medula Óssea - SBTMO. Assim, consideramos que o TCTH para a leucemia linfoide crônica (LLC) deve seguir, para sua indicação, os critérios do European Group for Blood and Marrow Transplantation (EBMT) e, quando houver disponibilidade de um doador aparentado, a opção deve ser do TCTH alogênico com regime não mieloablativo. O TCTH alogênico não aparentado e o autólogo devem ser considerados como opção secundária de indisponibilidade de doador, situações especiais e ensaios clínicos.


Patients with chronic lymphocytic leukemia usually have an indolent and prolonged clinical course and need to be differentiated from those who have an aggressive and fatal disease. Younger patients with high-risk criteria may benefit with a more aggressive treatment that includes hematopoietic stem cell transplantation (HSCT). Autologous transplantation, despite of the encouraging results with cases of molecular and/or cytogenetic remission and low mortality rates, does not present a plateau in survival curves and has a high relapse rate. Allogeneic transplantations using myeloablative regimens, have high toxicity and mortality rates, but also demonstrate the graft-versus-leukemia effect that increases the possibility of cure of these individuals. So the option of allogeneic transplants for patients with CLL is directed to conditioning using non-myeloablative regimens, which can also be applied to older patients or those with comorbidities, and maintain a potential graft-versus-leukemia effect. The identification of patients who may benefit from these procedures and the characterization of new prognostic markers remain the subjects of many clinical studies and were the objective of the group responsible for discussing guidelines for CLL of the consensus on HSCT SBTMO. Thus we believe that HSCT for CLL should follow the criteria of the EBMT. When a sibling donor is available the best option is allogeneic HSCT with a myeloablative regimen. The strategy of unrelated allogeneic or autologous HSCT must be considered as a second option when no donor is available, for special situations and clinical trials.

Este artigo busca esclarecer e discutir as relações entre a arte moderna e os imaginários da alteridade - entendendo imaginário não apenas como um repertório de imagens ou os caminhos temáticos possíveis de serem reapropriados pelo artista, mas também a própria capacidade de imaginar, de perceber e recriar as imagens do mundo. Deste modo, mostra-se como um dos pontos de ruptura de toda uma série de artistas ocidentais, que já podem ser classificados como modernos, foi precisamente uma nova postura com relação ao imaginário e à arte de outros povos. O texto aborda mais especificamente as alteridades oriundas dos povos orientais e as alteridades oriundas da África negra, demonstrando como, a partir destes imaginários de alteridade, os modernos artistas ocidentais conseguiram recriar a sua própria arte


This article attempts to clarify and discuss of relations between modern art and the imaginaries of the alterity - understanding imaginary as not only a repertory of images or the thematic ways possible of been reapropriated by the artists, but also the capacity itself of to imagine, of perceive and recreate the images of the world. In this way, the intent is to show how one of the points of rupture of several occidental artists, which can be classified as moderns, was precisely a new posture in relation to the imaginary and the art of the other peoples. The essays aboard more specifically the alterities referent to the oriental peoples and the alterities referents to the black Africa, showing how, working with these alterities imaginaries, the modern occidental artists had success in recreate his own art
