ABSTRACT

Medicinal leech /Hirudo medicinalis/ secretes 150 over bioactive substances from savilary gland, which are hirudin, hialuronidasa, bdellin, eglin, destabilasa, lipase, cholesterol esterase. Medicinal leech’s therapeutic effect is anticoagulant, antiplatelet, anti-ischemic, hypotensive, anti-inflammatory, anti-atherosclerotic and thrombolytic. In Mongolia, medicinal leech therapy’s research is rare.The study was designed as selective sampling and clinical trial study. From June 1st between September 1st in 2016, there were enrolled 30 patients with diagnosis of arterial hypertension stage I at “Piyavka” hospital in Hentii province. Patients were used by method of Medicinal leech therapeutic of the Russian Federation in 2011. Blood samples were used SD LipidoCare analyser, Stago Compact automatic coagulation analyzer. Fibrinogen, Activated partial thromboplastin time, Thrombin time, Prothrombin time, International normalized ratio and Blood lipids level were checked before and after treatment. A total of 30 patients were enrolled in the study. 16 were females, 14 were males and the mean age was 59.13±11.62. Activated partial thromboplastin time was before 37.97 ±5.62 sec and after 45.94±6.05 sec therapy of leeches. Thrombin time was before 17.59 ±1.66 sec and after 19.53±1.38 sec therapy of leeches. As patients who was increased from normal lipid level in blood before leech therapy, Total cholesterol was before 220.63±14.51 mg/dl, after 204.63±13.64 mg/ dl therapy of leeches. Triglyceride level was before 253.69±71.99 mg/dl, after 184.12±61.73 mg/ dl therapy of leeches. Mean of Activated partial thromboplastin time, Thrombin time were prolonged with statistically significant after treatment (p=0.0001). As patients who was increased from normal lipid level in blood before leech therapy, Total cholesterol (p=0.031), Triglyceride (p=0.0003) were decreased with statistically significant by therapy of leeches.

ABSTRACT

Oriental medicine, are described in detail about the human body association between systemic and musculoskeletal system, the location of the organs and critical points. Critical point or biological active point in the human body has a total of 302, classified about lives and impact is most dangerous, more dangerous and dangerous. These ascertaining theoretical and usage is important. Our aim was to study in limbs of the body composition and perceived status in the critical point.Total 62 most healthy people obtained to the research from this 31 people were male, 31 people were female and 18-28 ages. We used algometry, anthropometry and VAS. Upper and lower limbs of the body composition and perceived status in the critical point compared with body constitution.Statistical analyses were conducted in SPSS (PASW) Version 17.0.The mean age were had male 18.47, female 23.42 obtained to the research. “Wind-bile”, “windphlegm” body constitution people were 22.58%, “phlegm-bile” body constitution people were 8.06% and “wind-bile-phlegm” body constitution people not available. A high significant decrease, high correlation in “phlegm-bile” body constitution people were the most high measured in 9 point, “bilewind” body constitution people were lowest measured in 8 point by anthropometry.Comparing results by anthropometric indication, algometry tools, VAS methods is dependent to the positive or reserve.

ABSTRACT

Today, there are 382 million people living with diabetes. A further 316 million with impaired glucose tolerance are at high risk from the disease – an alarming number that is set to reach 471 million by 2035. One of the most overlooked of all serious complications of diabetes is cardiovascular autonomic neuropathy (CAN) which encompasses damage to the autonomic nerve fibers that innervate the heart and blood vessels, resulting in abnormalities in heart rate control and vascular dynamics. The present report discusses the clinical manifestations (eg, resting tachycardia, orthostatic hypotension exercise intolerance, intra operative cardiovascular liability, silent myocardial infarction (MI), and increased risk of mortality) in the presence of CAN. The reported prevalence of CAN varies greatly depending on the criteria used to identify CAN and the population studied. CAN prevalence ranges from as low as 2.5% of the primary prevention cohort in the Diabetes Control and Complications Trial (DCCT) to as high as 90% of patients with long-standing type 1 diabetes who were potential candidates for a pancreas transplantation. Objective: The aim of this study was to evaluate the Cardiac Autonomic Neuropathy (CAN) among diabetic patients.This study included patients with T1 DM and 20 patients with T2 DM total (97 male, 86 female) diabetic patients. The CAN diagnosed by 6 clinical tests: Resting Heart Rate (RHR), Expiration : Inspiration (E:I) ratio, Heart rate response to standing (30:15 ratio), Orthostatic hypotension (OH) and Sustained Hand Grip (SHG) using Cardiac Autonomic Neuropathy System Analyzer CAN-504. CAN was indicated at least two of five tests are abnormal.Diabetic patients’ mean age was 48.74±12.74, diabetes duration 7.55±5.72, systolic blood pressure 136.25±22.76mm Hg, diastolic blood pressure 84.82±11.90 mmHg, cholesterol 5.04±1.04mmol/l, triglyceride 2.20±1.24mmol/l, LDL2.64±0.85mmol/l, HDL 1.12±0.41mmol/l, non-HDL 3.71±1.06, cholesterol/HDL ratio 4.70±1.29, HBA1c 10.08±2.39%. Result of RHR resting heart rate test was normal 92%, borderline 0.5% and abnormal 7.1%,Expiration:inspiration(E:I) ratio was normal 72.7%,borderline 13.7% and abnormal 14%, Heart rate response to standing (30:15ratio) was normal 47%,borderline 13.714% and abnormal 39.3%, Valsalva was normal 97.8%,borderline 2.2% and abnormal 0%, Orthostatic hypotension (OH) was normal 66%,borderline 29% abnormal 6% and Sustained hand grip(SHG) test was normal 4.9%,borderline 9%, and abnormal 87.8%.Number of abnormal cardiac autonomic neuropathy test results 2 (with cardiac autonomic neuropathy) was in 97(53%) among diabetic patients. Among diabetic patients cardiac autonomic neuropathy (CAN) was 53%.
