ABSTRACT

Objective:Rapid assessment of the outcome after percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS) is an important clinical issue. In this study, an electrocardiogram (ECG) analysis method based on dynamic learning was proposed.Methods:A total of 203 patients with ACS after successful PCI were enrolled for prospective analysis at the Emergency Department of Qilu Hospital of Shandong University from April 2019 to December 2020. All patients were divided into group without ≥70% postoperative stenosis ( n=72) and group with ≥ 70% postoperative stenosis ( n=131) according to the presence of 70% or more stenosis after PCI. The clinical data of ACS patients were collected and analyzed by χ2 test, t-test, or Mann-Whitney test. ECGs were recorded before and 2 h after PCI, and were dynamically analyzed to generate cardiodynamicsgram (CDG) using dynamic learning. In the group without ≥ 70% postoperative stenosis, the model and CDG index for evaluating myocardial ischemia were obtained by training support vector machine (SVM) using 10 times 10-fold cross-validation. Results:There was no significant difference in clinical data between the two groups. The prediction accuracy and sensitivity of the support vector machine model for myocardial ischemia in group without≥70% postoperative stenosis were 73.61%, and 84.72% respectively. CDG transformed from disorderly to regular after PCI, and CDG index decreased significantly ( P<0.001): 90.28% (65) patients in group without≥70% postoperative stenosis, and 79.39% (104) patients in group with≥70% postoperative stenosis had lower CDG indexes than before PCI. Conclusions:In this study, CDG obtained by dynamic learning can intuitively and effectively evaluate the changes of myocardial ischemia before and after PCI, which is helpful to assist clinicians to formulate the next treatment plan.

ABSTRACT

Objective@#Testicular germ cell tumors (TGCT) are the most common cancer among men aged 15 to 39 years. Previous studies have considered factors related to TGCT survival rate and race/ethnicity, but histological type of the diagnosed cancer has not yet been thoroughly assessed.@*Methods@#The data came from 42,854 eligible patients from 1992 to 2015 in the Surveillance Epidemiology and End Results 18. Frequencies and column percent by seminoma and nonseminoma subtypes were determined for each covariates. We used Cox proportional hazard regression to assess the impact of multiple factors on post-diagnostic mortality of TGCT.@*Results@#Black males were diagnosed at a later stage, more commonly with local or distant metastases. The incidence of TGCT in black non-seminoma tumors increased most significantly. The difference in survival rates between different ethnic and histological subtypes, overall survival (OS) in patients with non-seminoma was significantly worse than in patients with seminoma. The most important quantitative predictor of death was the stage at the time of diagnosis, and older diagnostic age is also important factor affecting mortality.@*Conclusion@#Histological type of testicular germ cell tumor is an important factor in determining the prognosis of testicular cancer in males of different ethnic groups.

ABSTRACT

Based on 2D speckle tracking imaging (2D-STI), myocardium layer-specific strain can be used for quantitative assessment of myocardial function, but is load dependent. Added after-load (blood pressure) measurement to myocardium layer-specific strain, pressure strain loops (PSL) technique can effectively restrict load dependence and quantitatively analyze the work of whole and segmental myocardium. The research progresses of PSL in evaluation of myocardial work were reviewed in this article.

ABSTRACT

Objective:To investigate the expression of microRNA125a in infiltrating macrophages during myocardial ischemia reperfusion, and to analyze the effect of microRNA125a on the polarization of infiltrating macrophages during myocardial ischemia reperfusion.Methods:The rat model of myocardial ischemia-reperfusion was established. Myocardial injury tissues were taken, infiltrated macrophages were isolated, and RNA was extracted. qRT-PCR was used to detect M1 and M2 markers in macrophages and evaluate the types of infiltrated macrophages. Subsequently the expression level of microRNA125a in infiltrating macrophages was detected. Finally, the effect of microRNA125a on macrophage polarization was further elucidated.Results:Macrophages in myocardium of ischemia reperfusion showed high expression of M1 markers (TNF-alpha and IL-8), presenting M1 macrophages. At the same time, microRNA125a was also highly expressed in this macrophage. Knockdown or overexpression of microRNA125a in macrophages in vitro can affect the macrophages polarization.Conclusions:Macrophages in myocardial ischemia reperfusion are M1 type, and microRNA125a is highly expressed in this macrophage and promotes its migration to M1 type.

ABSTRACT

Objective To observe the effect of early bundle therapy on prognosis of patients with sepsis/septic shock and analyze the risk factors for death.Methods A retrospective cohort study was conducted to select patients with sepsis/septic shock at the Second Soochow University Hospital betweenJanuary 1,2016,and December 31,2016.Data pertaining to demographic variables,compliance rate of bundle therapy,and incidence of organ failure were collected.Patients were categorized into the nonsurvivor or survivor groups based on 28-day mortality.Logistic regression analysis was used to identify risk factors for 28-day mortality.Results Totally 118 sepsis/septic shock patients were included in the analysis;28-day mortality was 32.2%.Compared to the survivor group,patients in the non-survivor group were more likely to have chronic heart dysfunction and cerebrovascular disease,lower lactate clearance,lower 6-h compliance rate of bundle therapy and higher incidence of failure of one or >2 organs.Age,leukocyte,blood urea nitrogen,creatinine,brain natriuretic peptide,sequential organ failure score and acute physiological and chronic health scores Ⅱ on admission,and lactate after bundle therapy were higher than that of the survivor group.Logistical regression analysis showed that age ≥ 75 years [odds ratio (OR)1.012],6-h lactate clearance <30% (OR=1.122),chronic heart failure (OR=1.741),failure of >2 organs (OR=1.769),and 6-h compliance rate of bundle therapy (OR=1.958) were independent risk factors for 28-day mortality.Conclusions Patients with sepsis/septic shock need early diagnosis and resuscitation to improve the compliance rate of bundle therapy and reduce the mortality.

ABSTRACT

AIM To study the chemical constituents from the fruit bodies of Daedalea dickinsii (Berk.) Yasuda.METHODS The methanol extract from the fruit bodies of D.dickinsii was isolated and purified by silica,Sephadex LH-20 and HPLC,then the structures of obtained compounds were identified by physicochemical properties and spectral data.RESULTS Six compounds were isolated and identified as (22E,24R)-ergosta-5,7,22-trien-3β-ol (1),(22E,24R)-5α,8α-epoxyergosta-6,22-dien-3β-ol (2),stellasterol (3),6-methoxycer-evisterol (4),16-acetylpolyporenic acid C (5),and 3 α-carboxyacetoxy-24-methylene-23-oxolanost-8,24 (31)-dien-26-oic acid (6).CONCLUSION Compounds 1-4,6 are isolated from this fungus for the first time.

ABSTRACT

Objective To investigate the effect of heat-sensitive point moxibustion on high-normal blood pressure in persons with yin or yang deficiency.Methods Eighty-six persons with high-normal blood pressure were randomized to a treatment group (42 cases, 20 with yin deficiency and 22 with yang deficiency) and a control group (44 cases, 19 with yin deficiency and 25 with yang deficiency). The control group received health education and diet guidance and the treatment group, heat-sensitive point moxibustion in addition. Both groups were followed up twice a week with blood pressure measured. The clinical therapeutic effects, hypertension diagnosis rates and neutral constitution transformation percentages were compared between the two groups of persons with different constitutions after one month of treatment.Results The total efficacy rate was 90.0% in persons with yin deficiency and 100% in persons with yang deficiency of the treatment group and correspondingly 78.9% and 92.0% of the control group; there was a statistically significant difference between the two groups (P<0.05). Post-treatment hypertension diagnosis rate 15.0% in persons with yin deficiency and 9.1% in persons with yang deficiency of the treatment group and correspondingly 36.8% and 16.0% of the control group; there was a statistically significant difference between the two groups (P<0.05). Post-treatment neutral constitution transformation percentage was 23.3% in persons with yin or yang deficiency of the treatment group and 13.6% in those of the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusions Heat-sensitive point moxibustion can reduce high-normal blood pressure and hypertension incidence and effectively rectify biased constitution in persons with yin or yang deficiency.

AIM: To compare the safety and efficacy of conjunctival autograft transplant ( CAT ) and limbal - conjunctival autograft transplant (LCAT) for primary pterygium. METHODS: In this prospective, randomized and controlled clinical study, 120 patients ( 120 eyes ) diagnosed with primary pterygium were collected in NO. 474 Hospital of Chinese PLA from January 2014 to January 2015. The 120 patients enrolled in the study in turn, and divided into two group by the odd and even number. The odd number underwent CAT and the even number underwent LCAT. RESULTS:One hundred and seven patients (107 eyes) completed the follow-up of 1a, including 54 patients (54 eyes) from the CAT group and 53 patients (53 eyes) from the LCAT group. Four patients (4 eyes, recurrence rate 7. 4%) in the CAT group and 2 patients ( 2 eyes, recurrence rate 3. 8%) in the LCAT group developed recurrence. However, there were no significant in recurrence rate between CAT group and LCAT group (P =0. 678). CONCLUSION: CAT and LCAT might be both effective treatment for primary pterygium for reducing the recurrence rate of pterygium. However, LCAT is the optimal surgical method for primary pterygium.

ABSTRACT

AlM: To demonstrate the effect of comprehensive treatments for meibomian gland dysfunctional dry eye. METHODS:Totally, 86 cases (172 eyes) in our hospital, who suffered from meibomian gland dysfunctional dry eye, were measured by BUT, CFS, the machine of infrared meibomian gland function and testing meibomian gland after extrusion. The scores of meibomian gland function missing and secretions were observed, which as a diagnosis, and assessed the effectiveness of treatment, after took comprehensive treatments in different stages ( comprehensive treatments including cleaning, hot covering, massaging, TobraDex, Pranoprofen and Lubricant, etc. ) . RESULTS:All patients were treated and observed for 3wk, in which 46 cases (92 eyes) of cured, 29 cases (58 eyes) effective, 11 cases (22 eyes) ineffective, the total effective rate was 87. 2%. After 6wk, 68 cases (136 eyes) of cured, 13 cases (26 eyes) effective, 5 cases (10 eyes) ineffective, the total effective rate was 94. 2%, not appeared adverse ocular and systemic reactions in the process of treatment. CONCLUSlON: Taken comprehensive treatments for meibomian gland dysfunctional dry eye could effectively shorten the course of the disease, quickly remove meibomian gland tube obstruction, restore the meibomian gland secretion and stable the lipid layer, and could reduce the discomfort of patients with dry eye positively and effectively.

ABSTRACT

To expand the evidence base to inform future public policy aimed at accident prevention, we investigated the impact of different categories of injury on average life expectancy in China. We used data from the National Death Cause Registration Information System and National Maternity and Children Health Surveillance databases, as well as 2010 population data from the National Bureau of Statistics. We then calculated the average life expectancy of the Chinese population, in addition to life expectancy after eliminating injury-related mortality. The average life expectancy of the Chinese population in 2010 was 74.93 years. After eliminating deaths due to injuries, the fourth leading cause of mortality in China, average life expectancy increased by 1.36 years. When this was broken down by population sub-groups, these gains were 1.76 and 0.79 years in men and women, 0.94 and 1.56 years in urban and rural residents, and 1.11, 1.30, and 1.67 years for residents in the Eastern, Central and Western regions respectively. After eliminating all categories of injury, the average life expectancy of the Chinese population was found to increase by 1.36 years. This figure was higher for males and residents of rural areas and Western China.

ABSTRACT

To develop an automated coding software related to the underlying causes of death,based on the National Registration Information System on deaths,which could improve the quality of coding on the underlying causes of death in the conventional death surveillance system.Following the coding roles of the underlying death cause of ICD-IO and the design on principles of software of underlying death cause automated coding from some other countries,as well as in accordance with the coding strategy table from the USA,we implemented the automatization of the underlying death cause coding.Based on national registration information system on cause of death,an automated coding software of underlying death cause was developed with the coding correction rate closed to 85%.The automated coding software of underlying death cause could code the death cases of underlying death cause with high rate of correction,similar to that of the same kind softwares developed in other countries.

ABSTRACT

Objective To identify the sex-related DNA copy number alterations (CNA) in hepatocellular carcinoma (HCC). Methods High-resolution array comparative genomic hybridization Carray- CGH) was used to examine 17 female and 46 male HCCs. Two-tailed Fisher's exact test or χ2 test was used to compare the differences in CNA between females and males. Results The overall frequencies and patterns of CNA in female and male cases were similar. However, female HCC tumors presented more copy number gains compared to male on lq21. 3-q22(76. 5% vs 37. 0%, P = 0. 009), llqll(35. 3% vs 0. 0%, P = 0. 000 2) and 19ql3. 31-ql3. 32(23. 5% vs 0. 0%, P = 0. 004), and more loss on 16pll. 2(35. 3% vs 6. 5%, P = 0. 009). Relative to females, male cases had more copy number loss on llqll(63. 0% vs 17. 6%, P = 0. 002). Further analyses showed that llqll gain was correlated with 19ql3. 31-ql3. 32 gain(P = 0. 042) and 16pll. 2 loss(P = 0. 033), while lq21. 3-q22 gain was correlated with 19ql3. 31-ql3. 32 gain(P = 0. 046). Conclusion Our findings suggest that CNA may play an important role in sex-related difference in HCC development.

ABSTRACT

<p><b>OBJECTIVE</b>To assess the in-stent lumen visibility and image quality of coronary stents by dual-source computed tomography (DSCT) coronary angiography, and the diagnostic accuracy of DSCT in the detection of coronary in-stent restenosis.</p><p><b>METHODS</b>DSCT was performed at 147 stents in 78 patients at an interval of (21.8?22.2) months after coronary stent implantation. Axial multi-planar reconstruction of the stents and curved-planar reconstruction through the median of the stents were evaluated for stent image quality on a 5-point scale, and the stent lumen diameters were detected. Thirty out of these 78 patients underwent conventional coronary angiography within one month after CT angiography. The patency of 60 stents were independently evaluated by two blinded readers.</p><p><b>RESULTS</b>Image quality was good to excellent on average score (1.6?0.6) . Stent image quality score was correlated with stent diameter, stent location, and heart rate. All stents were assessable in lumen visibility with an average visible lumen diameter percentage of (72.2?12.2) %. Visible lumen diameter percentage was correlated with stent diameter and stent location. For the stents with calcified plaques, the visible lumen diameter percentage at the calcified site was significantly lower than that at the non-calcified site (P<0.001) . Compared with the conventional coronary angiography, 12 out of 14 in-stent stenoses were correctly detected. The sensitivity, specificity, positive predictive value, and negative predictive value for the detection of in-stent stenosis was 85.7%, 95.7%, 85.7%, and 95.7%, respectively. For stents whose diameter >0.275cm, the sensitivity, specificity, positive predictive value, and negative predictive value were all 100%. The agreement between CT findings and coronary angiography results was 93.3%, and it was correlated with stent diameter and heart rate.</p><p><b>CONCLUSIONS</b>Using a DSCT, coronary stent lumen is partially visible and the image quality is high. Stent diameter and location can influence the stent lumen visibility and image quality. DSCT has a high diagnostic accuracy for the detection of in-stent restenosis and may be a valuable modality for the follow-up of coronary artery stent patency."</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the feasibility of a dual-energy computed tomographic angiography (DECTA) protocol using test-bolus injection with reduction of contrast material (CM) dose in second generation dual-source CT system.</p><p><b>METHODS</b>Totally 57 consecutive patients underwent CT angiography scan covering the cervical and cerebral arteries. CT was performed with second generation dual-source CT system. The time to peak (T) using a test-bolus injection was calculated. The patients were divided into three groups (A, B, and C) with different CM doses (40, 45, and 50 ml) and different delay time points [ (T+1) , (T+1) , and (T+2) s] . All the patients were followed by a 48 ml saline flush. Arterial enhancements were quantified by measuring attenuation values of the aortic arch, bifurcation of common carotid artery, contralateral internal jugular vein of the CM injection, superior vein cava, proximal middle cerebral artery, basilar artery, and straight sinus on source images. Visualizations of intracranial artery and ipsilateral venous effect of the CM injection were rated on a four-point grading scale on CTA images for qualitative assessment.</p><p><b>RESULTS</b>Although the attenuation of internal jugular vein and straight sinus were significantly lower in group A than in groups B and C (P<0.05) , the attenuation of aortic arch, superior vein cava, common carotid artery, middle cerebral artery, and basilar artery vessels showed no significant differences among these three groups. The scores of the visualizations of intracranial artery and ipsilateral venous effect of the CM injection were also not significantly different among these three groups.</p><p><b>CONCLUSION</b>Based on the delay time calculated by a test-bolus injection, a reduced-dose contrast material may provide an equal degree of arterial attenuation and a lower attenuation of vein for dual-energy CTA covering the craniocervical region in second generation dual-source CT system.</p>

ABSTRACT

Objective To evaluate the effect of curcumin on diabetic neuropathic pain (DNP) in rats. Methods Forty-eight male SD rats weighing 200-230 g were randomly divided into 6 groups ( n = 8 each) : group Ⅰ normal control (group C); group Ⅱ DNP (group D) ; group Ⅲ DNP+ DMSO (solvent of curcumin) (group DD) and group Ⅳ , Ⅴ , Ⅵ DNP + curcumin 50, 100 or 200 mg/kg ( group DC50, 100, 200 ). Diabetes was induced with intraperitoneal (IP) streptozocin 75 mg/kg in group Ⅱ -Ⅵ. Curcumin 50, 100 and 200 mg/kg were given IP once a day for 14 consecutive days starting from 14 d after streptozocin in group Ⅳ , Ⅴ and Ⅵ respectively. Venous blood samples were taken before and 72 h after IP streptosocin for determination of blood glucose level. Successful induction of diabetes was defined as blood glucose > 14.6 mmol/L. Mechanical paw withdrawal threshold to yon Frey stimuli (MWT) and paw withdrawal latency to thermal nociceptive stimulus (TWL) were measured 2 d before and 14 d after IP streptesocin and on the 1, 3, 7, 14 d of curcumin administration. Animals with pain threshold measured at 14 d after IP streptozocin decreasing by less than 15% of the baseline were excluded from the study. The animals were sacrificed after the last pain threshold measurement and the lumbar segment of the spinal cord and p65 was significantly higher in group D than in group C ( P < 0.05). Curcumin administration significantly reduced thermal and mechanical hyperalgesia induced by diabetes and down-regulated the expression of p-JNK and horn and DRG.

ABSTRACT

The purpose of this study is to characterize the stress relax properties of PVA-H prosthetic nucleus via the four-parameter linear viscoelastic model and to analyze the influence of swelling ratio and initial PVA content upon the properties of dissipating compressive stress. The four-parameter linear viscoelastic model can simulate the viscoelastic property of prosthetic nucleus well (corr > 0.99) and be more effective than the three parameter linear viscoelastic model. From the parameters of this model we have obtained the following results: the prosthetic nucleus of higher water content can dissipate compressive stress more quickly than that of lower water content can do, but the quantity of stress relax can not be influenced by water content; the higher the initial PVA content,the slower and smaller the dissipation of compressive stress;the relax time of prosthetic nucleus is similar to that of human nucleus.

<p><b>OBJECTIVE</b>To investigate the chemical constituents of Boletus vioaceo-fuscus.</p><p><b>METHOD</b>The compounds were isolated with column chromatography. The structures were determined by spectroscopic techniques.</p><p><b>RESULT</b>Six compounds were isolated from the fruiting bodies of Boletus vioaceo-fiuscus. They were identified as ergosta-5, 7, 22-triene-3beta-ol (1), dihydrofuran-2, 5-dione (2), (22E, 24R)-5alpha, 6alpha-epoxyergosta-8, 22-diene-3beta, 7alpha-diol (3), (22E, 24R)-5alpha, 6alpha-epoxyergosta-8 (14), 22-diene-3beta, 7alphadiol (4), cerebroside B (5) and adenosine (6), respectively.</p><p><b>CONCLUSION</b>All the Compounds were obtained from the fruiting bodies of Boletus vioaceo-fiscus for the first time.</p>

Methods 33 early stage breast cancer patients were confirmed by surgical operation and pathology and diagnosed by mammography at the same time to analyse the relationship between the signs of X-ray and early stage breast cancer. Results 16 of 33 patients X-ary signs included mass with spiculate border and lobulate margins,followed by 8 cases of calcification like needle point,5 cases duct enlarge and 14 cases indirect signs. Conclusion Clinical touch examination cambined with mammography is very important to diagnose early breast cancer,especially mammography is necessary to the cases which were found nothing by clinical touching.
