ABSTRACT

El objetivo del estudio fue evaluar aquellos factores de riesgo en el tumor primario que se asocian a compromiso de ganglios pelvianos al momento de la cirugía, en pacientes en etapas precoces de carcinoma escamoso de cuello uterino. De 76 pacientes con estadio lb/lla sometidas a histerectomia radical entre junio de 1985 y diciembre de 1999, sólo 64 tenían la información necesaria para ser evaluadas adecuadamente. De 64 paciente estudiadas 54,8 por ciento, 35,4 por ciento y 9,6 por ciento, correspondían a estadios lb1, lb2 y lla, respectivamente. Diecinueve (29,7 por ciento) presentaban compromiso ganglionar pelviano. De todas las variables analizadas, etapa (p=0,003), permeaciones vasculares-linfáticas (p=0,003), invasión parametrial (p=0,002), compromiso vascular-linfático del parametrio (p=0,035), y compromiso vaginal por tumor (p=0,029), fueron factores relacionados significativamente con metástasis ganglionares en el análisis univariado. Sin embargo, sólo etapa (p=0,017) y permeaciones vascular-linfáticas en el tumor (p=0,021) fueron predictores independientes de metástasis ganglionares pelvianas cuando los factores encontrados significativamente relacionados en el análisis univariado fueron sometidos a análisis multivariado

ABSTRACT

El presente trabajo resume nuestra experiencia reciente con ligadura de arterias hipogástricas en el manejo de 7 pacientes con hemorragía periparto severa en el período 1999-2001. Se discute la técnica de ligadura de arterias hipogástricas, su aplicabilidad, complicaciones y resultados. En un 85 por ciento de los casos se logró resolver la emergencia mediante la técnica de ligadura bilateral de arterias hipogástricas. Se revisa la literatura con especial énfasis en el manejo conservador de la hemorragía periparto

ABSTRACT

Papillary serous carcinoma with low malignant potential, similar to those described in the ovary, can also originate in the peritoneum. Characteristically they show peritoneal spread without involvement of the ovary and, if present, it corresponds to a superficial implant similar to those seen in the rest of the peritoneal cavity. Histologically they correspond to a low malignant potential tumor; they are slow growing and have good prognosis. They are common in young women, usually they present few symptoms and are frequently discovered during other surgical procedures. The treatment is surgical and it can be conservative in cases of women who want to preserve their fertility, without coadyuvant therapy. We report a 34 years old woman with a primary peritoneal serous carcinoma with low malignant potential and discuss its management

ABSTRACT

Background: Adequate management and treatment of ovarian carcinoma requires a complete surgical staging supported by frozen-section examination. To achieve this goal it is necessary a high level of accuracy. Aim: To evaluate the accuracy of frozen-sections in ovarian carcinoma considering the influence of tumor diameter and weight. Patients and methods: Retrospective study of frozen-sections performed in patients with ovarian tumors who underwent surgery. Frozen- and permanent- sections were divided into three categories (benign, borderline and malignant) and stratified by diameter (<10 cm, 10 to 20 cm, >20 cm) and weight (<700 g, 700 a 1400 g, >1400 g). The diagnostic correlation, sensitivity, specificity, predictive values and accuracy of each frozen-section diagnosis were determined. Results: Eight hundred forty two ovarian tumors that underwent frozen-sections between january 1988 and october 1998 were studied. Final diagnosis was 86,7 percent benign, 2,7 percent low malignant potential (LMP) and 10,6 percent malignant. The diagnosis correlation between frozen- and permanent-sections was 98,2 percent. Misdiagnosis was in epithelial ovarian tumors, particularly in LMP tumors. Sensitivity, specificity, positive- and negative- predictive values and accuracy of the four hundred eighty nine epithelial tumor were 92,6 percent, 99,2 percent, 96,7 percent, 98,2 percent and 97,9 percent, respectively. Diagnostic correlation was higher in epithelial ovarian tumors with diameter <10 cm (98,2 percent v/s 93,8 percent) and weight <700 g (96,9 percent v/s 88,9 percent). Conclusions: Diagnostic correlation with permanent-section examination, sensitivity, specificity and predictive values of frozen-sections are high in ovarian tumors. Accurate diagnosis at frozen sections of epithelial ovarian tumors with diameter >10 cm or weight >700 g (particularly in LMP tumors) is difficult because of the extensive sampling required. Frozen-sections diagnoses are important to determine the type and extent of surgery performed at the initial operation

ABSTRACT

Se presentan 36 pacientes portadoras de tumores de ovario de bajo potencial maligno tratadas entre 1987 y 1998. En 9 pacientes se decidió realizar un manejo conservador por deseo de fertilidad. En 2 de estas pacientes hubo recurrencia de la enfermedad. La sobrevida libre de recurrencia y la sobrevida total a 10 años fueron 90,2 por ciento y 96,3 por ciento respectivamente. Se revisa la literatura y se actualiza los conceptos en el enfrentamiento de esta enfermedad
