ABSTRACT
Background: There are multisystemic consequences secondary to SARS- CoV-2 infection. Aim: To characterize neurological complications in patients admitted due to SARS-CoV-2 infection. Methods: Review of medical records of patients aged over 15 years with COVID-19 evaluated by the neurology team between April and August 2020 at a university hospital. Severity of the infection, referral reasons, neurological diagnoses and laboratory results were registered. The diagnoses were defined by consensus among the members of the hospital neurology group. Cerebrovascular and inflammatory diseases of the central and peripheral nervous system were defined as "probably associated" or "possibly associated" to COVID-19. Results: Ninety-six patients had at least 1 new neu- rological complication. 74% were admitted due to pneumonia and 20% due to a neurological disease. The most common reasons for neurological referral were impaired consciousness (39%), focal neurological deficit (24%), headache (9%) and seizures (5%). The most relevant neurological diagnoses were delirium in 48 patients, stroke in 24, critical illness polyneuropathy and myopathy in 17, seizures in 14, brachial plexopathy in 3, compressive neuropathies in 5, encephalitis in 1, possible vasculitis in 1 and Guillain-Barré syndrome in 1. Stroke and epilepsy were associated with increased length of hospital stay, but without differences in mortality. Conclusions: The spectrum of neurological complications of COVID-19 is wide. There are clinical entities typical of critically ill patients and also diseases associated directly and indirectly with the SARS-CoV2 infection.

Reporte de caso de síndrome de pseudo-Duane asociado a schwannoma de nervio abducens y neurofibromatosis tipo 2 (NF2). Este síndrome es raro, caracterizado por limitación en abducción, retracción ocular, disminución de hendidura palpebral en abducción y restricción del recto medial ipsilateral en test de ducción forzada. La NF2 es también una enfermedad infrecuente caracterizada por tumores de sistema nervioso central y periférico. Los schwannomas de nervio abducens son también poco frecuentes, y se presentan comúnmente con diplopia. Se revisan brevemente estas enfermedades. Es el primer caso reportado de pseudo-Duane secundario a schwannoma de nervio abducens y NF2 según nuestro conocimiento.
A pseudo-Duane syndrome case associated with abducens nerve schwannoma and neurofibromatosis type 2 (NF2) is presented. This syndrome is a rare disease characterised by abduction limitation, ocular retraction, narrowing of the palpebral fissure in abduction, and ipsilateral medial rectus restriction on forced duction test. NF2 is also an uncommon disease which is characterised by peripheral and central nervous system tumours. Abducens nerve schwannomas are also uncommon and presents usually as diplopia. A short review of these diseases is given. This is the first case of pseudo-Duane secondary to abducens nerve schwannoma, to our knowledge.

Resumen El diagnóstico diferencial de las lesiones destructivas de la línea media es amplio y complejo debido a su presentación clínica poco específica, pudiendo abarcar desde cuadros infecciosos, neoplásicos y patologías autoinmunes. Una entidad a considerar son las lesiones destructivas de la línea media inducidas por cocaína (CIMDL), la que se caracteriza por una extensa destrucción de estructuras a nivel medio-facial. Se presenta el caso de un paciente con antecedentes de consumo de cocaína con una gran lesión destructiva endonasal asociada a una perforación palatal, fiebre, cervicalgia y meningismo. Se realizó un estudio nasofibroscópico y exámenes de laboratorio donde destaca un leve aumento de parámetros inflamatorios y serología positiva para anticuerpos anticitoplasma de neutrófilo (ANCA). Los exámenes imagenológicos destacan una trombosis de carótida interna derecha, osteítis del clivus y paquimeningitis retroclival. Se decide ingresar a pabellón para realizar un aseo y tomar biopsias. Al descartar los diagnósticos diferenciales de CIMDL, el paciente inició terapia antibiótica y antifúngica endovenosa evolucionando en forma satisfactoria. La CIMDL es una condición infrecuente cuyo principal diagnóstico diferencial es la granulomatosis con poliangeitis. Dada similitud en las presentaciones clínicas y la gran diferencia en el manejo de ambas patologías la necesidad de un diagnóstico certero es fundamental.
Abstract Differential diagnosis of destructive midline injuries is complex due to its nonspecific clinical presentation, and the broad variety of conditions that share the same signs and symptoms, such as infections, neoplasms and autoimmune diseases. One of the etiologies that should be considered is cocaine-induced destructive midline lesions (CIMDL), cha- racterized by extensive destruction of midface structures. The following case describes a patient with history of cocaine abuse with a large destructive endonasal lesion asso- ciated with a palatal perforation, fever, cervicalgia and meningism. Clinical assessment included a nasofibroscopy and laboratory tests. Results revealed a slight increase in in- flammatory parameters and positive antineutrophil cytoplasmic antibody serology. Ad- ditionally, imaging revealed a thrombosis of the right internal carotid, clivus osteitis, and retroclival pachymeningitis. Surgical debridement was performed, and biopsies of the nasal cavity were taken. Once the other possible diagnoses were ruled out, the patient was started on intravenous antibiotic and antifungal therapies. The patient showed a positive response to treatment and was successfully discharged after two months. CIMDL is a rare condition whose main differential diagnosis is granulomatosis with polyangiitis. Given the similarity in clinical presentations and the important differences in the management of both pathologies, the need for an accurate diagnosis is essential.

La esclerosis múltiple (EM) es la enfermedad inflamatorio-desmielinizante del Sistema nervioso central más prevalente en adultos. La resonancia magnética (RM) juega un rol cada vez más importante en el estudio de esta patología, en especial en su diagnóstico precoz, por lo que la diferenciación imagenológica de variantes frecuentes e infrecuentes de EM con otras patologías de sustancia blanca que comprometen encéfalo y médula espinal es esencial. Mediante una revisión pictórica se ilustrarán características típicas en RM del compromiso por EM y de variantes menos habituales de lesión desmielinizante, y se ilustrarán hallazgos característicos de lesiones relacionadas a vasculopatías inflamatorias y no inflamatorias, encefalomielitis diseminada aguda (ADEM), neuromielitis óptica (NMO) y enfermedades vasculares de la médula espinal que pueden simular EM, con énfasis en el diagnóstico diferencial radiológico.
Multiple sclerosis (MS) is the most prevalent inflammatory-demyelinating disease of the central nervous system in adult population. Magnetic resonance imaging (MRI) has an increasingly important role, especially in early diagnosis, so the imaging differentiation of frequent and infrequent variants of MS with other white matter diseases of brain and spinal cord is essential. Through a pictorial essay we show typical MR features of MS and more infrequent variants of demyelinating lesions and illustrate characteristic imaging findings of inflammatory and non-inflammatory vasculopathies, acute disseminated encephalomyelitis (ADEM), neuromyelitis optica (NMO) and vascular diseases of spinal cord that may simulate MS, with emphasis on imaging differential diagnosis.

La hemorragia subaracnoidea (HSA) no traumática es un subtipo de ictus hemorrágico que representa aproximadamente el 5% de todos los accidentes vasculares encefálicos (AVE). El 85% de los casos de HSA espontánea (no traumática) son secundarios a un aneurisma intracraneano roto, el 10% a hemorragia perimesencefálica no aneurismática y el otro 5% a otras causas. Entre estas se incluyen malformaciones arterio-venosas, fístulas durales, vasculits, trombosis de vena cortical, síndrome de vasoconstricción reversible, angiopatía amiloidea y síndrome de encefalopatía posterior reversible. La aproximación inicial a una HSA no traumática requiere un estudio angiográfico no invasivo con tomografía computada para la toma de decisiones terapéuticas. Si no se detecta un aneurisma sacular intradural que explique el sangrado, las conductas a seguir dependerán del patrón de distribución de la sangre. En esta revisión sugerimos una aproximación basada en 1) revisar el estudio inicial tomando en cuenta los puntos ciegos para la detección de aneurismas, 2) analizar el patrón de distribución de la sangre y 3) analizar los hallazgos en imágenes de acuerdo a las posibles causas según patrón.
Non-traumatic subarachnoid hemorrhage represents approximately 5% of strokes. From these, 85% of nontraumatic subarachnoid hemorrhage are secondary to a ruptured aneurysm, 10% to nonaneurysmal perimesencephalic hemorrhage and the other 5% to other causes. These include but are not limited to arteriovenous malformations, dural fistulae, vasculitis, cortical vein thrombosis, reversible cerebral vasoconstriction syndrome, amyloid angiopathy and posterior reversible encephalopathy syndrome. Initial workup of nontraumatic subarachnoid hemorrhage requires a non-enhanced CT and CT angiography for decision making and management. If there is no aneurysm as a source of hemorrhage, subsequent imaging studies will depend on blood distribution pattern. In this review we suggest an approach: 1) review blind spots for aneurysm detection in the initial CT angiography, 2) analyze blood distribution pattern and 3) evaluate imaging findings and possible causes according to each pattern.

Resumen: El tratamiento de aneurismas intracraneanos rotos y no rotos incluye opciones quirúrgicas (clips vasculares) y endovasculares. Existen varios dispositivos y técnicas de manejo endovascular (coils, stents, diversores de flujo), cuyo conocimiento permite elegir e interpretar adecuadamente las técnicas de imágenes para el seguimiento no invasivo. Mediante una revisión pictográfica del control de aneurismas intracraneanos tratados quirúrgicamente y vía endovascular, se muestran las diferencias en la capacidad de caracterización en el seguimiento con angio TC, angio RM sin contraste (con técnica de "tiempo de vuelo" o "TOF" por sus siglas en inglés), angio RM con gadolinio endovenoso (C+) y angiografía por sustracción digital (ASD). Adicionalmente se entregan recomendaciones para el seguimiento de aneurismas según modalidad y dispositivo de tratamiento.
Abstract: Treatment of intracranial ruptured and unruptured aneurysms can be performed with surgical (vascular clips) or endovascular procedures (coils, stents, flw diverters). There are several devices and techniques for endovascular treatment. Knowledge of this techniques allows physicians to choose and interpret accurately imaging modalities for non-invasive follow-up. Through a pictorial essay of imaging follow-up of treated intracranial aneurysms we show the differences in characterization with computed tomography angiography, magnetic resonance angiography without contrast (time of flght or "TOF" technique), contrast enhanced MR angiography (C+) and digital subtraction angiography (DSA). Additionally we recommend imaging modalities for each treatment device.

The aim of this paper is to provide basic tools for the interpretation of diagnostic accuracy studies, by analyzing the data that the contingency table provides. Concepts are given in the application and meaning of sensitivity, specificity, positive and negative predictive values. Furthermore, by analyzing the pre-test and post-test probabilities, construction and implementation of the likelihood ratio are explained. The diagnostic odds ratio and its limitations are described. Finally, the importance of calculating confidence intervals is explained. Understanding the contingency table and the construction of the parameters that we can calculate from this, is essential for the critical evaluation and development of diagnostic accuracy studies, as well as their application in daily clinical work.
El objetivo del presente artículo es entregar herramientas básicas para la interpretación de los estudios de exactitud diagnóstica, mediante el análisis de los datos que entrega la tabla de contingencia. Se entregan conceptos en la aplicación y significado de sensibilidad, especificidad, valores predictivos positivo y negativo. Además, mediante el análisis de las probabilidades pre-test y post-test, se explica la construcción e implementación de los likelihood ratio o razones de verosimilitud. Se describe el odds ratio diagnóstico y sus limitaciones. Por último, se explica la importancia del cálculo de intervalos de confianza. Entender la tabla de contingencia y la construcción de los parámetros que de esta podemos calcular es fundamental para la valoración crítica y desarrollo de estudios de exactitud diagnóstica, así como para su aplicación en el quehacer clínico diario.

Introduction: The written radiology report is the most relevant component of any radiological examination. Few studies about referring clinicians opinion concerning structure and contents of radiology reports have been conducted. Objective: To investigate which radiology report issues are significant to the clinician. Methodology: One hundred and sixty-four physicians, among surgeons, internists, residents, and general practitioners were surveyed. Results: According to surveyed clinicians the most important element in any radiologic examination is the reporting radiologist (67.7 percent); 93.9 percent of them expect the radiology report to address the referral diagnosis; 95.7 percent expects the report to present a conclusion, whilst 73 percent of referring clinicians consider ultrasonography as the imaging procedure that commonly exhibits more errors. Surgeons, generally understimate radiologists recommendations for complementary studies. Conclusion: An experienced reporting radiologist is considered the most significant element of any imaging procedure. The radiology report should be problem-oriented (answering clinical question), and contain a conclusion, which should begin by answering the clinician s concerns. General practitioners, internists and residents value further studies reccommended by radiologists.
Introducción. El informe es lo más relevante de un examen radiológico. Escasas publicaciones han estudiado la opinión del clínico. Objetivo. Establecer qué aspectos del informe son relevantes para el clínico. Metodología. Se aplicó una encuesta a cirujanos, internistas, residentes y médicos generales. Resultados. Se encuestó a 164 médicos. Al pedir un examen radiológico, lo más relevante (67,7 por ciento) fue el radiólogo informante; 93,9 por ciento considera importante que se haga referencia al diagnóstico de derivación; 95,7 por ciento estima que debe haber conclusión. El 73 por ciento cree que el examen en el que se cometen más errores es la ecotomografía. Los cirujanos son los que menos valoran las recomendaciones de estudios posteriores. Conclusión. El radiólogo que informa es considerado lo más importante en un examen. El informe debe hacer referencia al diagnóstico, tener conclusión, la que debe comenzar respondiendo la inquietud del clínico. Los médicos generales, internistas y residentes valoran que se sugieran estudios complementarios.
