ABSTRACT

The Functional endoscopic sinus surgery through transnasal approach is a common modality of treatment for disorders of the nasal cavity, paranasal air sinuses as well as cranial cavity. The olfactory fossa (OF) is located along the superior aspect of cribriform plate which varies in shape and depth. This variable measurement of the depth of OF is mostly responsible for greater risk of intracranial infiltration during endoscopic procedures in and around the nasal cavity. The morphology of frontal and ethmoid sinus (ES) vary from simple to complex. This cadaveric study is planned to improve the ability of the otolaryngologist, radiologist to understand the possible morphological variations and plan steps of less invasive “precision surgery” to have a safe and complication free procedures. A total of 37 human head regions were included in the study. For classification of OF, Modified Kero’s classification was used. The size, shape and cells of frontal and ES were noted.We found, type II (60.8%) OF was more common followed by type I (29.7%) than type III (9.5%). The shape of frontal sinus was comma shaped (55.4%) followed by oval (18.9%) than irregular (16.2%). Most common two cells type of ES was seen in 50.0% of both anterior and posterior ES. Out of 74 ES, 8.1% of Onodi cells and 14.9% of agger nasi cells were seen.

ABSTRACT

@#A 31-year-old Indian female with a history of near-total thyroidectomy 2.5-years prior presented with recurrent neck swelling. Magnetic resonance imaging (MRI) of the neck revealed an infiltrating mass involving the thyroid bed. Biopsy from the mass and review of slides from the previous thyroidectomy revealed a spindle cell tumour with interspersed areas of fibrosis and infiltrative edges entrapping thyroid follicles. Beta-catenin immunopositivity and CTNNB1 mutation confirmed the diagnosis of fibromatosis. The case is being reported for its rarity and the discussion of its differential diagnoses.

ABSTRACT

@#A 35-year-old female presented with abdominal pain, fever, projectile vomiting, and a diffuse tender epigastric mass. She was diagnosed to have acute persistent pancreatitis with a pancreatic pseudocyst. Elevated serum calcium levels provided an etiologic link between hypercalcemia and pancreatitis. On examination, a nodule was found in the left side of her neck which was later diagnosed as a giant left inferior parathyroid adenoma. This report highlights the critical analysis of history, examination, and investigations to reach an ultimate diagnosis. Pseudocyst drainage and parathyroidectomy resolved her symptoms.

ABSTRACT

Background@#Organochlorine pesticides (OCPs) exposure may induce an endocrine disruption which may lead to the risk of developing diabetes through alteration and disturbance of glucose metabolism, insulin resistance, and destruction of β-cells. The present study determines the recent trend of OCPs residue in blood samples and their association with the known risk factors responsible for developing the risk of diabetes among the North Indian population. @*Methods@#Blood sample of 300 patients (100 each of normal glucose tolerance [NGT], prediabetes and newly detected diabetes mellitus [DM]) between the age group of 30 to 70 years were collected. OCPs residue in whole blood samples was analyzed by using gas chromatography equipped with a 63Ni selective electron capture detector. @*Results@#Significantly higher levels of β-hexachlorocyclohexane (HCH), dieldrin, and p,p’-dichloro-diphenyl-dichloroethylene (DDE) were found in the prediabetes and newly detected DM groups as compared to NGT group. Insulin resistance showed to be significantly positive correlation with β-HCH and dieldrin. Also, fasting and postprandial glucose levels were significantly positively correlated with levels of β-HCH, dieldrin, and p,p’-DDE. Further, when OCPs level was adjusted for age and body mass index (BMI), it was found that β-HCH, dieldrin, and p,p’-DDE levels in blood increases the risk of diabetes by 2.70, 2.83, and 2.55 times respectively. Moreover, when we adjust OCPs level based on BMI categories (BMI 25 kg/m2); β-HCH and p,p’-DDE showed a significant risk of developing newly detected DM with BMI >25 and ≥23 and ≤25 kg/m2. @*Conclusion@#The OCPs level present in the environment may be responsible for biological, metabolic, and endocrine disruptions within the human body which may increase the risk of developing newly detected DM. Hence, OCPs exposure can play a crucial role in the etiology of diabetes.

ABSTRACT

Gangetic plains are known to have a very high incidence of cholelithiasis, the eiopathogenesis of which cannot be ascertained. Aim: to conduct a study to find a correlation, if any between the incidence of gallstone disease and the serum Iron and serum Calcium levels of patients and compare it with controls. Methods: 50 patients and 50 controls were taken and serum Iron and serum Calcium of both were evaluated. The distribution of age, sex and gender were taken as factors. Results and conclusion: The number of female patients was higher than the males but no strong association could be proven in the study. Among the symptoms dyspepsia had the highest incidence but could not be taken as a reliable marker for the diagnosis of cholelithiasis clinically. Hence, ultrasonography remains a very important tool for the diagnosis of the diasease .The role of serum Iron and serum Calcium in the etiopathogenesis could not be established. There was however a strong correlation of previous surgery in the formation of new gallstones .The family history was a strong factor for the formation of new gallstone disease however, the association was not established in this study. Gallstone disease had stronger association with anaemia than with serum Serum Iron. Calcium levels were comparable between the cases and the controls.

Introduction: Study was conducted to determine the conditionsfor insertion of LMA following induction of anaesthesia with(Propofol, 2 mg/kg) and (Thiopentone, 5 mg/kg, preceded by10%Lignocaine spray to the posterior oropharyngeal wall) byobserving six different parameters (gagging, laryngospasm,body movements, coughing, ease of insertion, jaw relaxation)and assessing all six parameters together with Lund andStovner gradingsystem. The hemodynamic parameters wererecorded during induction of anaesthesia and following LMAinsertion.Material and Methods: Patients were allocated randomlyinto two groups, (40 patients per group), Group A- Propofoland Group B- Thiopentone, preceded with 10%Lignocainespray.Results: In Group A significant decrease in HR (p<0.05),systolic BP (p<0.01), diastolic BP (p<0.01) and mean BP(p<0.01) was observed as compared to Group B. The overallassessment for the ease of insertion of LMA was 100%inGroup A and 97.5%in Group B which was statistically notsignificant (p=0.314). No significant difference was observedin gagging (p=0.152), laryngospasm (p=0.314), bodymovements (p=0.221), coughing (p=0.221), jaw relaxation(p=0.314) and the ease of insertion (p=0.314).Discussion: 10% Lignocaine spray suppresses upper airwayreflexes in Thiopentone group and facilitates LMA insertionwithout any adverse response.Conclusion: Thiopentone preceded with 10%Lignocainespray to the posterior oropharynx (Group B) provides almostequal conditions for LMA insertion as compared to Propofol(Group A) with better hemodynamic stability

ABSTRACT

The major drawback of Simvastatin to formulate a dosage form is its poor aqueous solubility. In this study a solvent and carrier less technique called Melt sonocrystallization was utilized to form tiny crystals of Simvastatin with enhanced solubility in distilled water and Phosphate buffer pH 7.4, with an insignificant (p > 0.05) change in partition behaviour. The Melt sonocrystallized Simvastatin (MSCSIM) was characterized by FT-IR, DSC and SEM. The flow property was characterized and compared with pure Simvastatin. The MSCSIM was incorporated in directly compressed matrix tablets formed with varying proportion of HPMC K100 LV and starch. The formed tablets were evaluated for pharmacotechnical parameters along with drug release study. After melt sonocrystallization the chemical integrity of drug was unchanged and showed a sound flow property. The pharmacotechnical features of the tablets were noted to be within pharmacopoeial limit. The formulation F5 displayed a comparatively more controlled release of drug (96.45%) over 12 hours. The major release mechanism was found to be anomalous diffusion, with n value of 0.58 from the Korsmeyer-Peppas model, suggesting the drug release driven by swelling of matrix and diffusion. This technique may prove to be beneficial for poorly water soluble drugs and the melt sonocrystallized drugs can lend themselves for developing controlled release formulations.

ABSTRACT

Protein–protein interaction (PPI) site prediction aids to ascertain the interface residues that participate in interaction processes. Fuzzy support vector machine (F-SVM) is proposed as an effective method to solve this problem, and we have shown that the performance of the classical SVM can be enhanced with the help of an interaction-affinity based fuzzy membership function. The performances of both SVM and F-SVM on the PPI databases of the Homo sapiens and E. coli organisms are evaluated and estimated the statistical significance of the developed method over classical SVM and other fuzzy membership-based SVM methods available in the literature. Our membership function uses the residue-level interaction affinity scores for each pair of positive and negative sequence fragments. The average AUC scores in the 10-fold cross-validation experiments are measured as 79.94% and 80.48% for the Homo sapiens and E. coli organisms respectively. On the independent test datasets, AUC scores are obtained as 76.59% and 80.17% respectively for the two organisms. In almost all cases, the developed F-SVM method improves the performances obtained by the corresponding classical SVM and the other classifiers, available in the literature.
