ABSTRACT

It is very important for oral and maxillofacial surgeon to understand the attitude of maxillofacial deformity patients toward their deformity. The patients' expectations from surgery, motivations, and the psychological meanings of maxillofacial deformity are considerd by surgeon. The patient's satisfaction and self-assessment on his or her apperance and malocclusion are influenced by realistic condition more and less. There are some differences in points of view on malocclusion between specialist and lay-persons. Some differences from social and cultural base may exist as well. The study of psychological implications of malocclusion with general public will be a foundation in understanding of malocclusion patients. The authers studied the relation between malocclusion and body image, self-perception of malocclusion, social meanings of malocclusion with Korean men around early twenties The results were as follows: 1. The malocclusion played important role in body image. 2. There was difference in the weight of teeth in body image distortion according to the malocclusion. 3. Dissatisfaction aginst his malocclusion was salient in the groups of mandibular prognathism, anterior both crowding, deep overjet . The gruops of deep overjet, anterior both crowding weresusceptibe to the teasing in adolescence period.

ABSTRACT

Fibrin glue(FG) has been successfully used in a wide range of surgical fields such as skin graft fixation, nerve repair, cartilage reattachment and microvascular anastomoses. The sustained release of antibiotics from FG for a longer period is desirable to avoid bacterial infection after application. Furthermore, this is indispensible for FG so as to be used as a local antibiotic carrier. Ampicillin sodium(AMP-Na) was selected as a model antibiotic and incorporated into FG. In an attempt to achieve sustained release of it, the incorporation of AMP-Na into FG was tried in two different ways. One was incorporation of AMP-Na in a dispersed state with the modification of the preparation method of FG. The other was incorporation of bovine serum albumin microspheres(BSA-MS) containing AMP-Na. The in vitro release of AMP-Na from various FG was examined. The modifications of FG were found not to be appropriate methods to achieve sustained release of AMP-Na. However, the incorporation of crosslinked BSA-MS containing AMP-Na into FG was found to reduce the initial rapid release of AMP-Na and retared the release of AMP-Na ; t0.9 was 33 hours. The prophylactic efficacy of antibiotic-loaded FG against pathogenic bacteria was evaluated by artificially induced abscess model in the rabbit. The abscess model in the rabbit was induced by inoculation of pathogenic bacteria(synchronized culture of Staphylococcus aureus). Three different experimental group of pathogenic bacteria was injected into oral mucosa of the rabbit and hisopathological assessment was examined to evaluated the prophylatic efficacy of antibiotic-loaded FG. In pathogen only group, large pus pocket was formed after only 24 hours. However, the abscess diameter of the combination group(i.e.,pathogen and AMP-Na loaded FG) showed much smaller than of pathogen only group. Histopathological study also showed that there was the least inflammatory cell in the combination group of pathogen and BSA-MS loaded FG among the three experimental group tested. These results indicate that antibiotic-loaded FG has not only the prophylactic efficacy against post-operative infection, but also has the potential to be employed as a local biodegradable antibiotic carrier.

ABSTRACT

The purpose of present study was to observe the radiographic finding and histologic response by Confocal Laser Scanning Microscopy(CLSM) on interface of the bone and titanium implant coated by chitosan. The tissue of rabbit tibiae to the surgical placement titanium implant coated by chitosan was examined at 3, 9 and 24 days postoperatively. The radiographic finding showed that surrounding bone density of implants was not significantly different compare with the bone on 3 and 9 days group. A large amount new bone was formed on 24 days group, the reason was osteconduction activity by chitosan. The CLSM analysis show that the surface coating by chitosan filled the gap between bone and implant on 3 days group and filled by mew born on 9 days group. On 24 days group, the bone and titanium surface was filled by lamella bone. This results indicated that this enhanced the initial stability of implant significantly and chitosan induced osseointegration around implant. CLSM allows the non-destrutive histo-tomography of bone biopsy as well as clinical practice. We conclude that CLSM allowed a good comprehension of the nature of bone-implant contact, avoiding artifacts due to the thickness of the specimen.
