ABSTRACT

@#Abstract: Objective To identify the species of Mycobacteroides abscessus complex (MABC) in patients with pulmonary infection from the Second Affiliated Hospital of Hainan Medical University, and to investigate the species types, drug sensitivity and population distribution of MABC in pulmonary infection in Hainan. Methods Respiratory tract specimens were collected from suspected tuberculosis patients who visited the Second Affiliated Hospital of Hainan Medical University from January 2014 to December 2021 and cultured for Mycobacterium isolation. Non-tuberculous mycobacteria (NTM) strains were preliminarily identified by p-nitrobenzoic acid/thiophen-2-carbohydrazide (PNB/TCH) medium and DNA microarray chip, and then MABC and its subspecies were identified by hsp65 and rpoB gene sequencing. In vitro antimicrobial susceptibility test was performed by broth microdilution method. Results A total of 3 025 respiratory specimens from suspected pulmonary tuberculosis patients were collected during the study period. Among the 123 patients with identified MABC isolates, 124 MABC strains were isolated and identified, including 74 strains of Mycobacteroides abscessus subsp. abscessus, 38 strains of Mycobacteroides abscessus subsp. massiliense and 12 strains of Mycobacteroides abscessus subsp. bolletii. Among them, 118 patients had single MABC subspecies infection, one patient had mixed infection with two MABC subspecies, two patients had mixed infection with MABC and other NTM, and two cases had mixed infection with MABC and M.tuberculosis. There were more female patients than male patients with a ratio of 1:0.64, and those aged 50 and above amounted to 76.42% (94/123, 95%CI: 67.93%-83.61%). There was no significant difference in age distribution between male and female patients (Z=-0.944, P=0.347). The drug susceptibility results showed that all MABC strains were sensitive to Tigecycline (TGC), with a resistance rate of 0.81% (1/124) to Amikacin (AK), and resistance rates of 6.45% (8/124), 32.26% (40/124), and 74.19% (92/124) to Cefoxitin (FOX), Linezolid (LZD), and Imipenem (IPM), respectively. For Clarithromycin (CLR), MABC showed induced resistance , and there was a statistically significant difference in the CLR (14D) resistance rates among the three subspecies (χ2=66.335, P<0.001). The resistance rates to Tobramycin (TOB), Doxycycline (DOX), Moxifloxacin (MFX), Ciprofoxacin (CIP), Trimethoprim/Sulfamethoxazole (TMP-SMX), and Amoxicillin/Clavulanic acid (AMC) were high, all >80%. Conclusion  In Hainan Province, pulmonary infections with MABC are mainly caused by Mycobacteroides abscessus subsp. Abscessus, which show high rates of inducible resistance to CLR. Timely and accurate identification of MABC to subspecies and drug susceptibility testing are of significant important for clinical decision-making.

ABSTRACT

Objective: To study the chemical constitutes from the roots of Tripterygium hypoglaucum. Methods: The chemical constituents from 95% EtOH extract of the roots of T. hypoglaucum at room temperature were isolated and prepared by Silica gel column chromatography, Sephadex LH-20 column chromatography, and semi-pre HPLC after dichloromethane extraction, and their structures were elucidated by a variety of spectral and spectroscopic techniques. Results: Six compounds were isolated and identified as 2-(5’,7’-dimethoxy-2’,2’-dimethyl-2H-benzopyran-6’-)-3-formyl-5,6-dimethoxy-benzofuran (1), 3(R)-4’,5’-dihydroxy- 2’,5,7-trimethoxy-6-(3-methyl-but-2-enyl)-isoflavan (2), 3’-geranyl-5,7,2’,5’-tetrahydroxyisoflavone (3), β-sitosterolpalmitate (4), docosanoic acid 2’,3’-dihydroxypropyl ester (5), and β-sitosterol (6). Conclusion: Compound 1 is a new compound, named as hirtellanine C. Compounds 2-5 are isolated from the plants for the first time.

Objective:To describe the cancer incidences and mortalities in Henan cancer registries in 2013.Methods:Registration data were evaluated according to the criteria of quality control of cancer registry,and the qualified data were analyzed based on areas(urban/rural), gender,age and cancer sites.The age-standardized rates were applied according to Segi's population and the fifth Chinese population census in 2000.Results:The total coverage of population from 19 qualified cancer registries data was 16,225,815(15.13%),Among which,8,370, 772(51.59%)were males and 7,855,043(48.41%)were females;and 2,819,817(17.38%)live in urban areas and 13,405,998(82.62%) live in rural areas.The crude incidence rate in Henan was 250.34/105(males:264.35/105,females:235.42/105),whereas the age-standardized incidence rates by Chinese standard population and by world standard population were 208.66/105and 207.25/105,respectively.The cumulative incidence rate(0-74 years old)was 24.31%.The cancer mortality in Henan was 161.05/105(males:185.50/105,females:134.98/105),whereas the age-standardized mortality by Chinese standard population and by world standard population were 131.18/105and 131.52/105,respectively.The cumulative mortality rate(0-74 years old)was 15.05%.Cancer incidence and mortality rates were increasing by age, reaching the peak values at 80 age group and 85 plus age group,respectively.Lung cancer,gastric cancer,esophageal cancer,liver cancer and colorectal cancer were the most common cancers ranked by the incidence rate,and the most common cause of cancer death was lung cancer,followed by gastric cancer,esophageal cancer,liver cancer and colorectal cancer.Incidence rate and mortality rate of breast cancer ranked the first and the fifth among the females,respectively.Conclusion:Cancer incidence and mortality rates were higher in the rural areas and in males than in the urban areas and in females.Lung cancer,digestive system cancers,and female breast cancer are the major cancer types that require attention for the prevention and control in Henan.

ABSTRACT

Tripterygium hypoglaucum (THH) is a folk medicine with important medicinal property, used in the treatment of cataciasis, arthritis, and cancer. The principal chemical constituents of THH are diterpene, triterpene, and sesquiterpene alkaloids, and there are also flavonoid, steroid, tannin, and glucide in it. Diterpene and alkaloids are the major active ingredients. THH has various pharmacological effects such as anti-inflammation, antitumor, immunosuppressive activity, antiviral effect, and so on. It is usually used to treat rheumatiod arthritis, psoriasis, and chronic urticarial. In this paper, we summarize the chemical composition, pharmacological activity, and clinical application of THH, for reference to study and utilize it.
