ABSTRACT

Objective:To study the clinical effect of different doses of ulinastatin combined with glutamine in treatment of acute severe pancreatitis.Methods:70 patients with acute severe pancreatitis patients admitted in our hospital from Jan. 2017 to Oct. 2019 were enrolled. According to the different dosage of ulinastatin, these patients were divided into 200 000 IU ulinastatin combined with glutamine group (200 000 IU combination group) , 400 000 IU ulinastatin combined with glutamine group (400000 IU combination group) , and 600 000 IU ulinastatin combined with glutamine group (600 000 IU combination group) . The study compared the 1-week mortality rate, abdominal pain relief time, respiratory frequency and heart rate back to normal time, blood amylase, glucose, C-reactive protein, procalcitonin levels, and white blood cell (WBC) count among the different groups.Results:The mortality of 400 000 IU combined group and 600 000 IU combined group were 4.35% and 5.56% respectively, which were significantly lower than 31.58% and 35% of the control group and 200 000 IU combined group ( P< 0.05) . The time of abdominal pain relief, respiratory rate and heart rate returning to normal in the 400 000 IU combined group were (7.21±1.25) d, (8.54±1.83) d and (9.54±2.23) d respectively, and (7.52±1.83) d, (8.13±1.75) d and (9.37±2.31) d in the 600 000 IU combined group, which were significantly lower than (9.12±2.78) d, (9.85± 3.16) d and (10.86±2.68) d in the control group and (8.76±1.96) d, (8.76±1.96) d and (10.62±1.43) d in the 200 000 IU combined group ( P<0.05) , There was no significant difference between the control group and the 200 000 IU combination group ( P>0.05) . Compared with the control group, there were significant differences in blood glucose, CRP and leukocyte count in different doses of ulinastatin+ glutamine treatment group ( P<0.05) ; However, there was no significant difference in blood glucose and CRP levels between different doses of ulinastatin+ glutamine treatment group ( P>0.05) , but compared with 200 000 IU combination group and 400 000 IU combination group, the leukocyte level of 600 000 combination group was significantly higher ( P<0.05) . Conclusion:ulinastatin combined with glutamine can significantly improve the clinical prognosis of patients with severe acute pancreatitis, but the clinical effect varies with the dose of ulinastatin. ABSTRACT

Objective To investigate the effects of D-tryptophan (D-Trp) on the formation of Streptococcus mutans (S. mutans) biofilm and the dispersal of 24 h-old biofilm, and the drug susceptibility of S. mutans against chlorhexidine (CHX) under the role of D-Trp. Methods Optical density assay was used to evaluate the growth curve of S. mutans exposed to 5.0 mmol/L D-Trp for 28 h. The non-treated group was not added with D-Trp. After treatment with 1.0, 2.5 and 5.0 mmol/L D-Trp, crystal violet staining was used to observe the changes of S. mutans biofilm formation in treatment group and non-treatment group. Crystal violet staining and confocal laser scanning microscopy (CLSM) were applied to illustrate the effects of 1.0, 2.5 and 5.0 mmol/L D-Trp on the dispersal of 24 h-old S. mutans biofilm. Resazurin sodium was used to indicate the effect of 5.0 mmol/L D- Trp on the minimum inhibitory concentration (MIC) and the minimum biofilm inhibitory concentration (MBIC) of treatment groups and negative control group. Results The growth curves of planktonic S. mutans within 28 h was consistent in treatment group and the non-treated group, both attained exponential phase after 4 h and reached stationary phase at 22 h. Notably, when compared with non-treated group, the biomass of S. mutans biofilm was increased with time from 0 to 72 h after treatment with 1.0, 2.5 and 5.0 mmol/L D-Trp. And at the same time point, the biomass was significantly less in each subgroup of treatment group than that of non-treated group (P<0.05). Crystal violet staining demonstrated that values of biomass(OD570)were less in treatment groups treated with 1.0, 2.5 and 5.0 mmol/L D-Trp than those of non-treated group (P<0.01). CLSM also showed that bacteria was adhered to the surface of media intreatment groups treated with 1.0, 2.5 and 5.0 mmol/L D-Trp. The values of biomass were lower in treatment groups than those of non-treated group (P<0.01). The MIC against S. mutans was 0.073 mg/L in both experimental group and negative control group. The values of MBIC were 0.293 mg/L and 2.344 mg/L in experimental group and negative control group, respectively. Under the action of 5.0 mmol/L D-Trp, the MBIC of S. mutans was reduced to 1/8. Conclusion Results indicate that D-Trp may inhibit the formation of S. mutans biofilm and promote the dispersal of biofilm already formed. D-Trp may further help CHX exert its bactericidal activity to S. mutans.

ABSTRACT

Objective:To evaluate the effects of humanistic care on medication compliance and social function of schizophrenic patients. Methods:76 patients with schizophrenia were randomly divided into two groups within two days of hospital admission. The observational group was applied humanistic care but the control group with rou-tine care. All the information related to the present study was collected using unified language. At the time of ad-mission, day 1, day 14, day 28 and day 56 after the intervention, the effect of humanistic care was evaluated by medication compliance, Brief Psychiatric Rating Scale ( BPRS) , Personal and Social Performance Scale ( PSP) and Family Concern Index Questionnaire ( APGAR) . Results:The patient′s medication compliance rate in the obser-vational group was higher than those in the control group, especially, at the time of day 56, the patients in the ob-servational group ( full compliance : 66%, partial compliance: 44%) was significantly better than those in the control group ( full compliance 19%, partial compliance 66%, incompliance 15%) . There were no statistical difference in the two groups of the BPRS scores on day 1 and day 14 of admission, however, the scores of the pa-tients in the observational group was significantly lower than those in the control group on day 28 and day 56 . As for PSP, the result was similar to BPRS on day 1 and day 14, while the PSP score of the patients in the observational group was significantly higher than those in the control group on day 28 and day 56. In terms of APGAR, the score of the patients in the observational group was significantly higher than those in the control study except day 1 of ad-mission. Conclusion:Humanistic care has positive effects on medication compliance and social function of schizo-phrenic patients especially after hospital discharge. The implementation of humanistic care would guide patients to maintain the capacity of life, labor and social communication, enhance the patient's sense of control over life, and improve their confidence to overcome the disease. With humanistic care, patients are more willing to cooperate with treatment, and further to reduce the recurrence and degradation of social function.

ABSTRACT

Objective:To evaluate the bond strength of root canal sealer AH Plus and GuttaFlow to root canal walls after final rinse with 4 different final irrigation regimens.Methods:60 teeth with single-canal were prepared chemomechanically using 5.25% sodi-um hypochlorite (NaOCl)as root canal irrigant.The teeth were then randomly distributed into 3 groups(n =20)according to the final irrigation protocol:1 7% EDTA followed by 2% chlorhexidine(CHX)(group 1 ),5.25% NaOCl followed by 1 7% EDTA(group 2), 0.9% normal saline(NS)(group 3).The root canals were then filled with sealers using a lentulo spiral and each group was distribu-ted into 2 subgroups according to the sealers(n =1 0):a.AH Plus;b.GuttaFlow.The roots were transversally sectioned to obtain 2mm thick dentinal slices.Then a push-out bond strength test was carried out and failure mode was observed by stereomicroscope. Results:The bond strength of AH Plus was significantly higher than that of GuttaFlow(P 0.05).When the root canals were filled with GuttaFlow,the push-out strength of group 1 ,2 and 3 was (1 .40 ±0.59)MPa, (1 .26 ±0.62)MPa and (1 .05 ±0.27)MPa respectively(P >0.05).The failure modes of AH Plus and GuttaFlow were both mainly mixed,but the proportion of adhesive failure in the canals filled with GuttaFlow were obviously higher than AH Plus.Conclusion:The bond strength to root canal wall of GuttaFlow is significantly lower than that of AH Plus.1 7% EDTA followed by 2% CHX as fi-nal irrigant can reduce the bond strength of AH Plus but has no marked effect on GuttaFlow.
