ABSTRACT

Introducción: La biopsia ganglionar retroperitoneal es un procedimiento frecuentemente requerido en el estudio de neoplasias; resulta deseable optimizar su rendimiento con baja morbilidad. Este artículo describe la utilidad y complicaciones de biopsias ganglionares retroperitoneales por laparoscopia en una institución oncológica de Latinoamérica. Material y Métodos: Cohorte retrospectiva de pacientes con biopsia ganglionar retroperitoneal o mesentérica laparoscópica entre 2011 y 2021 en el Instituto Nacional de Cancerología, en Bogotá, Colombia. Se recogieron datos demográficos, quirúrgicos, complicaciones y mortalidad a 30 días, resultados histopatológicos y su rol en la clínica. Resultados: Se incluyeron 41 pacientes; 73% con diagnóstico de malignidad, principalmente linfoma. La indicación fue mayormente sospecha de recaída, seguida por sospecha de enfermedad hematológica de novo. Siempre se obtuvo tejido adecuado y suficiente para diagnóstico histológico. Requirieron conversión a laparotomía cinco pacientes (12%). No hubo complicaciones Clavien-Dindo III /IV ni mortalidad a 30 días. Se presentó morbilidad grado I o II en 3 casos (7%) y un incidente intraoperatorio grado III. Conclusión: La naturaleza invasiva y el carácter diagnóstico de la biopsia retroperitoneal laparoscópica, constituyen un desafío frecuente en la práctica del cirujano general. La planeación estratégica e individualizada y la técnica quirúrgica depurada son las claves para lograr el máximo rendimiento, con baja morbimortalidad.


Introduction: Retroperitoneal lymph node biopsy is a frequently required procedure in the study of neoplasms; it is desirable to optimize its performance with low morbidity. This paper describes the usefulness and complications of retroperitoneal lymph node biopsies by laparoscopy in a cancer institution in Latin America. Material and Methods: Retrospective cohort of patients with laparoscopic retroperitoneal or mesenteric lymph node biopsy between 2011 and 2021 at the National Cancer Institute, in Bogotá, Colombia. Demographic and surgical data, complications and 30-day mortality, histopathological results and their clinical role were collected. Results: 41 patients were included; 73% diagnosed with malignancy, mainly lymphoma. The indication was mostly suspected relapse, followed by suspected de novo hematologic disease. Adequate and sufficient tissue was always obtained for histological diagnosis. Five patients (12%) required conversion to laparotomy. There were no Clavien-Dindo III/IV complications or 30-day mortality. Grade I or II morbidity occurred in 3 cases (7%) and a grade III intraoperative incident. Conclusion: The invasive nature and diagnostic character of laparoscopic retroperitoneal biopsy constitute a frequent challenge in the practice of the general surgeon. Strategic and individualized planning and a refined surgical technique are the keys to achieving maximum performance, with low morbidity and mortality.

Resumen: Introducción: El tumor de células gigantes (TCG) es un tumor óseo intramedular benigno que surge con frecuencia en los extremos de los huesos largos. Después del fémur distal y la tibia proximal, el radio distal es el tercer sitio más afectado con tumores, particularmente agresivos. Nuestro objetivo es la presentación del caso clínico de una paciente con diagnóstico de TCG de radio distal clasificada en grado III de Campanacci que recibió un tratamiento ajustado a sus posibilidades económicas. Reporte de caso: Paciente femenino de 47 años, sin solvencia económica y sin ningún servicio médico. El tratamiento incluyó resección en bloque, reconstrucción con autoinjerto de peroné distal y artrodesis radiocarpiana con placa de compresión bloqueada. Dieciocho meses después, la paciente presentaba una buena fuerza de prensión (80% respecto al lado sano) y tenía una función motora fina en la mano. La muñeca presentó estabilidad con pronación de 85o, supinación de 80o, flexión-extensión de 0o y una puntuación de 6.7 en el cuestionario de evaluación de resultados funcionales DASH. Su evolución radiológica a cinco años después de su cirugía continuó sin datos de recidiva local y afectación pulmonar. Conclusión: El resultado en esta paciente, junto con los datos publicados, indican que la técnica de resección tumoral en bloque, más el autoinjerto de peroné distal y la artrodesis con placa de compresión bloqueada proporcionan un resultado óptimo de funcionalidad para el tumor radial distal grado III a bajo costo.


Abstract: Introduction: Giant cell tumor (GCT) is a benign intramedullary bone tumor that frequently arises at the ends of long bones. After the distal femur and proximal tibia, the distal radius is the third most affected site with particularly aggressive tumors. Our objective is the presentation of the clinical case of a patient diagnosed with distal radius GCT classified in grade III of Campanacci who received a treatment adjusted to her economic possibilities. Case report: A 47-year-old female, without economic solvency and with some medical service. Treatment included block resection, reconstruction with distal fibula autograft, and radiocarpal fusion with blocked compression plate. Eighteen months later, the patient had good grip strength (80% on the healthy side) and had fine motor function in the hand. The wrist presented stability with pronation of 85o, supination of 80o, flexion-extension of 0o and a score of 6.7 in the DASH functional outcomes assessment questionnaire. His radiological evaluation five years after his surgery continued with no evidence of local recurrence and pulmonary involvement. Conclusion: The result in this patient, together with the published data, indicate that the block tumor resection technique, plus distal fibula autograft and arthrodesis with blocked compression plate provide an optimal result of functionality for the grade III distal radial tumor at low cost.

Background@#Access to nutritious food among the nutritionally at risk preschool children remains a challenge even in urban households in the Philippines. @*Objectives@#This study determined if the daily consumption of rice-soy meals for 120 days improved the anthropometric status of preschool children enrolled in selected child development centers in low-income barangay in Taguig City. Specifically, it determined if the proportion of underweight preschool children decreased at endline and if there was an increase in the weight of preschool children at endline. @*Methodology@#The study used the before and after study design. The Dietary Supplementation Program was implemented by the LGU’s day care teachers. An NGO supplied the rice-soy meal packs. The children were fed with different variants of rice-soy meal pack containing 23 essential micronutrients. Anthropometric measurements were taken before and after 120 days. Data were subjected to Mc Nemar’s test to determine changes in the children’s nutritional status. @*Results@#A total of 538 children were enrolled in the DSP. Results of Mc Nemar's test showed that there was a significant improvement in the nutritional status (weight-for-age) of underweight preschool children (p=0.0000001) while the paired t-test showed that the increase in the weight of preschool children after the 120 day-dietary supplementation program was extremely statistically significant (p<0.001). @*Conclusion@#The daily feeding improved the weight of the undernourished children but overweight children maintained their overweight status at endline. The DSP Guidelines should include feeding mechanism for overweight children. The DSP complementary components should also be assessed in improving household food security.

The Brazilian Cerrado biome consists of a great variety of endemic species with several bioactive compounds, and Anadenanthera peregrina (L.) Speg is a promising species. In this study, we aimed to perform phytochemical characterization and evaluate the antioxidant and antibacterial activities against Staphylococcus aureus and Escherichia coli of the hydroethanolic extract of A. peregrina stem bark. The barks were collected in the Botanical Garden of Goiânia, Brazil. The hydroethanolic extract was obtained by percolation and subjected to physicochemical screening, total phenolic content estimation, high-performance liquid chromatography (HPLC) fingerprinting, and antioxidant (IC50 values were calculated for the 2,2-diphenyl-1-picrylhydrazyl assay - DPPH) and antibacterial activity determination. The pH of the extract was 5.21 and density was 0.956 g/cm3. The phytochemical screening indicated the presence of cardiac glycosides, organic acids, reducing sugars, hemolytic saponins, phenols, coumarins, condensed tannins, flavonoids, catechins, depsides, and depsidones derived from benzoquinones. The extract showed intense hemolytic activity. The total phenolic content was 6.40 g GAE 100 g-¹. The HPLC fingerprinting analysis revealed the presence of gallic acid, catechin, and epicatechin. We confirmed the antioxidant activity of the extract. Furthermore, the extract did not inhibit the growth of E. coli colonies at any volume tested, but there were halos around S. aureus colonies at all three volumes tested. These results contribute to a better understanding of the chemical composition of A. peregrina stem bark and further support the medicinal applications of this species.

Resumen La carcinomatosis meníngea es una entidad poco frecuente, que puede formar parte de la historia natural de muchos procesos neoplásicos. Se presenta habitualmente con síntomas poco específicos, como cefalea, cambios en la conducta o alteraciones motoras y sensitivas. A continuación, presentamos el caso de una paciente con carcinomatosis meníngea por melanoma metastásico y su evolución clínica.

About 3000 tons of beans are not used in human food due to hardening. Several studies on bean-derived bioactive peptides have shown potential to treat some diseases, including those relying on oxidative dysfunctions. We assessed the effects of peptides extracted from hardened bean Phaseolus vulgaris (PV) on reactive oxygen species (ROS) and nitric oxide (NO) production, cytotoxic and cytoprotective effects in endothelial cells, and oxidonitrergic-dependent vasodilating effects. Extract was composed by peptide fraction <3 kDa (PV3) from hardened common bean residue. PV3 sequences were obtained and analyzed with bioinformatics. Human umbilical vein endothelial cells were treated with 10, 20, 30, and 250 µg/mL PV3. Oxidative stress was provoked by 3% H2O2. Cytotoxicity and cytoprotective effects were evaluated by MTT assay, whereas, ROS and NO were quantified using DHE and DAF-FM fluorescent probes by confocal microscopy. NO- and endothelium-dependent vasodilating effects of PV3 were assessed in isolated aortic rings. We found 35 peptides with an average mass of 1.14 kDa. There were no cell deaths with 10 and 20 μg/mL PV3. PV3 at 30 μg/mL increased cell viability, while cytotoxicity was observed only with 250 μg/mL PV3. PV3 at 10 μg/mL was able to protect cells from oxidative stress. PV3 also increased NO release without causing cell death. It also reduced relative ROS production induced by H2O2. PV3 vasodilating effects relied on endothelium-dependent NO release. PV3 obtained from low-commercial-value bean displays little cytotoxicity and exerts antioxidant effects, whereas it increases endothelial NO release.

ABSTRACT

Abstract In January 2018, a young male of Didelphis aurita (Wied-Neuwied, 1826), the Brazilian common opossum, was admitted for veterinary evaluation at the Recovery Clinic of Wild Animals of Estácio de Sá University, CRAS, in Rio de Janeiro, Brazil, due to the presence of a partially duplicated right hind limb. After x-ray evaluation, polymelia associated with syndactyly was diagnosed. The images revealed the following alterations on the right limb: single femur with increased diameter, two tibiae, two fibulae and absence of hallux. The observed malformations were restricted to the hind right limb, which remains functional. This is the first report of such anomalies in an individual of Didelphis aurita.


Resumo Em janeiro de 2018, um macho jovem de Didelphis aurita (Wied-Neuwied, 1826), o gambá-de-orelha-preta, foi recebido para atendimento veterinário na Clínica de Recuperação de Animais Silvestres (CRAS) da Universidade Estácio de Sá, no Rio de Janeiro, por ser portador de membro posterior direito parcialmente duplicado. Após exame radiográfico, constatou-se a ocorrência de polimelia associada a sindactilia. As imagens radiográficas revelaram as seguintes alterações no membro posterior direito: um fêmur com diâmetro aumentado, duas tíbias, duas fíbulas e ausência de hálux. As malformações observadas restringiram-se apenas ao membro posterior direito, o qual mantém sua funcionalidade. Essas alterações nunca antes foram descritas na espécie Didelphis aurita.

ABSTRACT

Abstract Platydoryctes Barbalho and Penteado-Dias 2000 is a small genus of Neotropical parasitoid wasps (Hymenoptera: Braconidae: Doryctinae) with four described species to date. Two new species of the genus are described from Brazil, and a key to species is presented in this paper.


Resumo Platydoryctes Barbalho and Penteado-Dias 2000 é um pequeno gênero Neotropical de vespas parasitoides (Hymenoptera: Braconidae: Doryctinae), com quatro espécies descritas até o presente. Duas novas espécies do gênero são descritas para o Brasil e uma chave para espécies é apresentada neste trabalho.

O presente estudo utilizou embriões de Danio rerio expostos aos elutriatos dos sedimentos estuarinos do rio Capibaribe, dos períodos chuvoso e seco, e analisou os efeitos letais, teratogênicos, bem como a frequência cardíaca. Os testes de toxicidade com os embriões seguiram as diretrizes da OECD 236. Mediante os resultados obtidos, a frequência cardíaca e a teratogenicidade foram os efeitos mais observados nos animais quando submetidos às amostras. Entre os efeitos teratogênicos, o retardo geral no desenvolvimento dos embriões foi o mais frequente durante as análises. Tais efeitos tóxicos se modificaram entre os pontos e entre os períodos de coleta. Essa variação de toxicidade pode estar relacionada à diversidade de atividades realizadas no entorno desse estuário, a influência do regime de chuvas, marés e correntes, indicando que a análise dos efeitos subletais e da teratogenicidade em embriões de D. rerio constitui bom parâmetro para avaliações de toxicidade de amostras ambientais.(AU)


The present study used Danio rerio embryos exposed to the elutriates of the estuarine sediments of the Rio Capibaribe, from the rainy and dry periods, where the lethal effects, teratogenic and heart rate were analyzed. Embryotoxicity tests followed the guidelines of OECD 236. Based on the results obtained, heart rate and teratogenicity demonstrated higher sensitivity to the samples. Among the teratogenic effects, the general delay in embryo development was the most frequent effect during the analyzes. These toxic effects changed between the points and between the collection periods. This variation of toxicity may be related to the diversity of activities carried out around this estuary, the influence of rainfall, tides, and currents, indicating the analysis of sublethal effects and teratogenicity in the D. rerio embryos are useful parameters for toxic evaluation of environmental samples.(AU)

ABSTRACT

Early mobilization is beneficial for critically ill patients because it reduces muscle weakness acquired in intensive care units. The objective of this study was to assess the effect of functional electrical stimulation (FES) and passive cycle ergometry (PCE) on the nitrous stress and inflammatory cytometry in critically ill patients. This was a controlled, randomized, open clinical trial carried out in a 16-bed intensive care unit. The patients were randomized into four groups: Control group (n=10), did not undergo any therapeutic intervention during the study; PCE group (n=9), lower-limb PCE for 30 cycles/min for 20 min; FES group (n=9), electrical stimulation of quadriceps muscle for 20 min; and FES with PCE group (n=7), patients underwent PCE and FES, with their order determined randomly. The serum levels of nitric oxide, tumor necrosis factor alpha, interferon gamma, and interleukins 6 and 10 were analyzed before and after the intervention. There were no differences in clinical or demographic characteristics between the groups. The results revealed reduced nitric oxide concentrations one hour after using PCE (P<0.001) and FES (P<0.05), thereby indicating that these therapies may reduce cellular nitrosative stress when applied separately. Tumor necrosis factor alpha levels were reduced after the PCE intervention (P=0.049). PCE and FES reduced nitric oxide levels, demonstrating beneficial effects on the reduction of nitrosative stress. PCE was the only treatment that reduced the tumor necrosis factor alpha concentration.

ABSTRACT

The term inflammaging is now widely used to designate the inflammatory process of natural aging. During this process, cytokine balance is altered, presumably due to the loss of homeostasis, thus contributing to a greater predisposition to disease and exacerbation of chronic diseases. The aim of the study was to analyze the relationship between pro-inflammatory markers and age in the natural aging process of healthy individuals. One hundred and ten subjects were divided into 5 groups according to age (22 subjects/group). Interleukin-6 (IL-6) and tumor necrosis factor α (TNF-α) were quantified using the ELISA method. High-sensitivity C-reactive protein (hsCRP) was analyzed by turbidimetry according to laboratory procedures. The main findings of this study were: a positive correlation between hsCRP and IL-6 as a function of age (110 subjects); women showed stronger correlations; the 51-60 age group had the highest values for hsCRP and IL-6; women presented higher values for hsCRP in the 51-60 age group and higher values for IL-6 in the 61-70 age group; and men showed higher values in the 51-60 age group for hsCRP and IL-6. In conclusion, the natural aging process increased IL-6 and hsCRP levels, which is consistent with the inflammaging theory; however, women presented stronger correlations compared to men (IL-6 and hsCRP) and the 51-60 age range seems to be a key point for these increases. These findings are important because they indicate that early preventive measures may minimize the increase in these inflammatory markers in natural human aging.

ABSTRACT

Objetivo. Se estudió la diversidad genética de las poblaciones naturales de parásitos de Plasmodium vivax mediante el estudio de la región variable del gen Pvmsp-3α. Materiales y métodos. Se realizó PCR-RFLP en 29 muestras de pacientes con malaria por P. vivax con complicaciones (PC) y 32 pacientes con malaria por P. vivax sin complicaciones (PNC), todos los pacientes provenientes de la región Bajo-Córdoba-Uraba, zona altamente endémica para malaria. Fueron colectadas muestras de sangre total para la extracción de DNA utilizando la tecnica saponina/chelex-100. Se usó la tecnica de RFLP usando las enzimas de restricción HhAI y AluI. Diez patrones de restricción fácilmente distinguibles, se detectaron después que los productos de la PCR fueron digeridos con la enzima Alu I y 9 con la enzima Hha I. Resultados. El gen Pvmsp-3α exhibió gran polimorfismo y los resultados sugieren que este gen puede ser utilizado en Colombia como un marcador molecular epidemiológico para el genotipado de P. vivax, ademas el patron PH1con la enzima AluI mostró mayor frecencia en los pacientes complicados. Conclusiones. El gen Pvmsp3α puede ser un marcador genetico de variabilidad en las cepas circulantes de P. vivax y el PH1 sugiere ser un marcador para predecir una complicación en pacientes con malaria vivax.


Objective. The genetic diversity of the natural populations of Plasmodium vivax parasites was studied by studying the variable region ofthe Pvmsp-3α gene. Materials and methods. Total blood samples were collected for DNA extraction using the saponin /chelex-100 technique. The RFLP technique was used using there striction en zymes HhA Iand AluI. Teneasily distinguis hablere striction patterns were detected after the PCR products were digested with the enzyme Alu I and 9 with the enzyme Hha I. Results. The Pvmsp-3α gene exhibited large polymorphismand the results suggest that this gene can be used in Colombia asa moleculare pidemiological marker for the genotypin go fP. vivax. Further more, the PH1 pattern with the AluIen zyme showed higher frequency in the complicated patients. Conclusions. The Pvmsp3 gene may be agenetic marker of variability in the circulating strain so fP. vivax and the PH1 suggest samarker for predicting a complication in patients with vivax malaria.

RESUMEN El dolor lumbopélvico es una condición muy frecuente, afectando un 50 a 70% de las mujeres durante el embarazo. En la mayoría de los casos ha habido una recuperación al mes o 3 meses postparto. Sin embargo, estudios reportan que la mejoría es incompleta, pudiendo persistir o aumentar años posterior al parto. Esta condición puede causar limitaciones en la vida diaria, provocando algún grado de discapacidad que muchas veces no es abordado con la importancia que se merece. Objetivo Determinar la prevalencia de dolor lumbopélvico y su relación con el grado de discapacidad durante el tercer trimestre de embarazo y 3 meses postparto. Material y método Estudio observacional y cohorte prospectivo; la muestra son 81 mujeres estudiadas en su tercer trimestre de embarazo y 3 meses postparto atendidas durante 2014-2015 en el Departamento de Obstetricia & Ginecología, Clínica Las Condes. Se aplicó un cuestionario y el Índice de discapacidad Oswestry en ambas etapas y autoadministrada. El análisis de datos se realizó con STATA10.0. Resultados El 81,48% de las embarazadas presentaron dolor lumbopélvico, siendo más prevalente y con una asociación significativa en el tercer trimestre (72,84%, p<0,001). El 25,93% de las mujeres lo mantiene después del parto. Existe una asociación significativa entre dolor lumbopélvico y grado de discapacidad (p<0,001), específicamente con actividades de la vida diaria, actividad sexual y actividades domésticas. Conclusiones Existe una alta prevalencia de dolor lumbopélvico en el embarazo, y un porcentaje no menor lo mantiene postparto, y en ambos casos está asociado a un grado de discapacidad.


ABSTRACT Lumbopelvic pain is a very frequent condition that affects between 50 to 70% of pregnant women. In many cases have been a recovery over the month or three months postpartum. However, some studies showed an incomplete improvement, so it could persist or increase through years after childbirth. This condition can cause daily life limitations, it is provoking some disability grade, that many times it didn't understand with the importance that deserve. Aim To identify the lumbopelvic pain prevalence and the relation with disability grade during the third trimester of pregnancy and three months postpartum. Material and Methods Observational and prospective cohort study; the sample was 81 women studied in her third trimester of pregnancy and three months postpartum during 2014-2015 in the "Departamento de Obstetricia & Ginecologia de Clínica Las Condes". It applied a questionnaire and the Oswestry Disability Index in both stages and self-administered. Data analysis was using STATA 10.0 software. Results 81, 48% of pregnant showed lumbopelvic pain, there were being more prevalent and significantly associated with third trimester (72, 84%, p<0,001). 25, 93% of women maintained this condition postpartum. There is a significant association between lumbopelvic pain and disability grade (p<0,001), specifically with daily life, sexual and domestic activities. Conclusions There is a high prevalence of lumbopelvic pain during pregnancy, and not less percentage maintained this condition postpartum, that associated with a high disability grade.

ABSTRACT

Aims: The purpose of the present study was to evaluate the flexural strength of specimens made of nickel-chromium(Ni-Cr) and cobalt-chromium (Co-Cr) alloys and joined by tungsten inert gas (TIG) welding and conventional brazing. Ni­Cr and Co­Cr base metal specimens (n = 40, each) were cast and welded by TIG or brazing. The specimens were divided into six groups (2 base metals, four welded specimens). Ceramic systems were applied to the central part of all the specimens. A three-point bending test with a velocity of 0.5 mm/m was performed on the specimens up to the point of the first ceramic bond failure by measuring the flexural strength. Data were analyzed using two-way ANOVA and Bonferroni's tests. Conventional welding showed the lowest flexural strength results for both alloys, while the TIG weld and the control group presented with varying bond strengths for the alloys studied. We concluded that TIG welding was superior to the conventional welding method for both Ni­Cr and Co­Cr alloys with regard to the flexural strength of the ceramic (AU)

Resumen: Las redes neuronales artificiales (RNA) son un método computacional extensamente utilizado para resolver problemas complejos y realizar predicciones en sistemas de relación no lineal. En este trabajo se utilizaron RNA para predecir la respuesta fisiológica obtenida al adicionar una concentración específica de digoxina a corazones de Tivela stultorum, un organismo modelo para probar fármacos cardíacos que se pretenden utilizar en humanos. Las entradas de la RNA fueron el peso, volumen, largo y ancho del corazón, la concentración de digoxina, el volumen utilizado para la dilución de digoxina, el máximo y mínimo de contracción, tiempo de llenado, y frecuencia cardíaca antes de adicionar la digoxina, las salidas fueron el máximo y mínimo de contracción, tiempo de llenado y frecuencia cardíaca esperados después de agregar digoxina al corazón. Las RNA se entrenaron, validaron y probaron con los resultados de experimentos in vivo. Para elegir la red óptima se utilizó el valor más pequeño del error medio cuadrado. Se obtuvo una correlación alta entre los valores predichos y calculados, excepto en el caso del tiempo de llenado. Se lograron obtener predicciones acertadas de la cardioactividad de la almeja T. stultorum cuando se les agrega una concentración específica de digoxina haciendo uso de RNA; esto con el fin de utilizarse como una herramienta para facilitar las pruebas en el laboratorio de los efectos de la digoxina.


Abstract: Artificial neural networks (ANN) are a computational method that has been widely used to solve complex problems and carry out predictions on nonlinear systems. Multilayer perceptron artificial neural networks were used to predict the physiological response that would be obtained by adding a specific concentration of digoxin to Tivela stultorum hearts, this organism is a model for testing cardiac drugs that pretends to be used in humans. The MLP-ANN inputs were weight, volume, length, and width of the heart, digoxin concentration and volume used for diluting digoxin, and maximum contraction, minimum contraction, filling time, and heart rate before adding digoxin, and the outputs were the maximum contraction, minimum contraction, filling time, and heart rate that would be obtained after adding digoxin to the heart. ANNs were trained, validated, and tested with the results obtained from the in vivo experiments. To choose the optimal network, the smallest square mean error value was used. Perceptrons obtained a high performance and correlation between predicted and calculated values, except in the case of the filling time output. Accurate predictions of the T. stultorum clams cardioactivity were obtained when a specific concentration of digoxin was added using ANNs with one hidden layer; this could be useful as a tool to facilitate laboratory experiments to test digoxin effects.
