ABSTRACT

ABSTRACT Purpose To develop and validate a chest cavity simulator for teaching video-assited thoracic surgery (VATS). Methods The first phase of the study consisted of developing a chest cavity simulator. A quasi-experimental study was performed in the second phase, and 25 surgeons and residents participated in a three-stage pulmonary suture experiment. The videos were recorded and timed. Generalized linear regression models for repeated measures were used to analyze the outcome change over time. Results The chest cavity simulator consists of a console simulating the left hemithorax. Among the participants, 96% rated the design, visual aspect, positioning ergonomics, and triangulation of the portals as very good or excellent (face validity). There was a decrease in suturing time in step 1 from 435.7 ± 105 to 355.6 ± 76.8 seconds compared to step 3 (p = 0.001). The evaluation of the simulation effectiveness and performance (content validity) was rated as very good or excellent by 96% ofparticipants. The most experienced surgeon showed significant reduction in procedure time (p = 0.021) (construct validity). Conclusions The thoracic cavity simulator is realistic, showing content and construct validity, and can be used in VATS training. The simulation model allowed skill gain in the endoscopic suture.

Fundamento: Procedimentos cardiovasculares minimamente invasivos têm sido progressivamente empregados no tratamento das cardiopatias. Objetivo: Descrever as técnicas e os resultados imediatos dos procedimentos minimamente invasivos ao longo de uma experiência de 5 anos. Métodos: Estudo unicêntrico, descritivo e prospectivo, com abordagem quantitativa, no qual 102 pacientes foram submetidos a procedimentos minimamente invasivos de forma direta e de forma videoassistida. Foram avaliadas variáveis clínicas, operatórias e evolução imediata dos pacientes operados. Resultados: Quatorze pacientes foram submetidos a procedimentos minimamente invasivos diretos e 88 a videoassistidos. Entre os submetidos a procedimentos minimamente invasivos diretos, 13 tinham cardiopatia valvar aórtica. Entre os submetidos a procedimentos minimamente invasivos videoassistidos, 43 tinham cardiopatia valvar mitral, 41 defeito do septo interatrial e quatro tumores. Entre os portadores de cardiopatia valvar mitral, foram realizadas 26 trocas e 17 reconstruções valvares. As médias de tempo de clampeamento aórtico, de extracorpórea e do procedimento foram, respectivamente, 91,6 ± 21,8, 112,7 ± 27,9 e 247,1 ± 20,3 minutos entre os submetidos a procedimentos minimamente invasivos diretos. Já entre os submetidos a procedimentos minimamente invasivos videoassistidos, foram 71,6 ± 29, 99,7 ± 32,6 e 226,1 ± 42,7 minutos, respectivamente. Considerando os tempos de terapia intensiva e de internamento, foram 41,1 ± 14,7 horas e 4,6 ± 2 dias entre os submetidos a procedimentos minimamente invasivos diretos e 36,8 ± 16,3 horas e 4,3 ± 1,9 dias entre os videoassistidos. Background: Minimally invasive cardiovascular procedures have been progressively used in heart surgery. Objective: To describe the techniques and immediate results of minimally invasive procedures in 5 years. Methods: Prospective and descriptive study in which 102 patients were submitted to minimally invasive procedures in direct and video-assisted forms. Clinical and surgical variables were evaluated as well as the in hospital follow-up of the patients. Results: Fourteen patients were operated through the direct form and 88 through the video-assisted form. Between minimally invasive procedures in direct form, 13 had aortic valve disease. Between minimally invasive procedures in video-assisted forms, 43 had mitral valve disease, 41 atrial septal defect and four tumors. In relation to mitral valve disease, we replaced 26 and reconstructed 17 valves. Aortic clamp, extracorporeal and procedure times were, respectively, 91,6 ± 21,8, 112,7 ± 27,9 e 247,1 ± 20,3 minutes in minimally invasive procedures in direct form. Between minimally invasive procedures in video-assisted forms, 71,6 ± 29, 99,7 ± 32,6 e 226,1 ± 42,7 minutes. Considering intensive care and hospitalization times, these were 41,1 ± 14,7 hours and 4,6 ± 2 days in minimally invasive procedures in direct and 36,8 ± 16,3 hours and 4,3 ± 1,9 days in minimally invasive procedures in video-assisted forms procedures. Conclusion: Minimally invasive procedures were used in two forms - direct and video-assisted - with safety in the surgical treatment of video-assisted, atrial septal defect and tumors of the heart. These procedures seem to result in longer surgical variables. However, hospital recuperation was faster, independent of the access or pathology. .

ABSTRACT

O objetivo é comparar o fluxo sanguíneo total (FS) e a reserva de fluxo coronariano (RFC) aos ramos revascularizados pelas artérias torácica interna esquerda (ATIE) e radial (AR) nos enxertos compostos com os enxertos isolados. Foi realizado um ensaio clínico randomizado de 42 pacientes que foram distribuídos em grupo A ou ATIE e AR composta em Y(n=14), grupo B ou ATIE e AR composta modificada(n=14) e grupo C ou ATIE pediculada para DA e AR aorto-coronariana(n=14). Os pacientes foram submetidos a fluxometria no pós-operatório imediato. A RFC foi de 2,1 ± 0,44, 1,96 ± 0,3 e 2,06±0,42 nos grupos A,B e C, (p=0.7208 A, B x C) e o FS, em ml/min, foi 110±30, 145±59 e 136±58, respectivamente (p=0.3232 A,B x C). Concluindo, não houve diferença significativa do FS total e nem da RFC nos grupos estudados / The objective is to compare the total blood flow (Bf) and coronary flow reserve (CFR) to the left coronary branches that were revascularized with left internal thoracic (LITA) and radial artery (RA) in composite and independent arterial grafts. A randomized trial was realized and 42 patients assigned in group A or composite LITA-RA in a Y configuration (n=14), group B or modified composite LITA-RA(n=14) and group C or pedicled LITA to LAD and aorto-coronary RA (n=14). Patients were submitted to postoperative Bf velocity analysis. CFR was 2,1 ± 0,44, 1,96 ± 0,3 e 2,06±0,42 in groups A,B and C (p=0.7208 A, B x C) and Bf, in ml/min, was 110±30, 145±59 and 136±58, respectively (p=0.3232 A, B x C). In conclusion there was no difference in Bf and CFR in the groups studied...

ABSTRACT

OBJECTIVE - To study mitral valve function in the postoperative period after correction of the partial form of atrioventricular septal defect. METHODS - Fifty patients underwent surgical correction of the partial form of atrioventricular septal defect. Their mean age was 11.8 years and 62 percent of the patients were males. Preoperative echocardiography showed moderate and severe mitral insufficiency in 44 percent of the patients. The mitral valve cleft was sutured in 45 (90 percent) patients (group II - GII). Echocardiographies were performed in the early postoperative period, and 6 and 12 months after hospital discharge. RESULTS - The patients who had some type of arrhythmia in the postoperative period had ostium primum atrial septal defect of a larger size (2.74 x 2.08 cm). All 5 patients in group I (GI), who did not undergo closure of the cleft, had a competent mitral valve or mild mitral insufficiency in the preoperative period. One of these patients began to have moderate mitral insufficiency in the postoperative period. On the other hand, in GII, 88.8 percent and 82.2 percent of the patients had competent mitral valve or mild mitral insufficiency in the early and late postoperative periods, respectively. CONCLUSION - The mitral valve cleft was repaired in 90 percent of cases. Echocardiography revealed competent mitral valve or mild mitral insufficiency in 88.8 percent and 82.2 percent of GII patients in the early and late postoperative periods, respectively

ABSTRACT

Este trabalho objetiva realizar um estudo anatomotopográfico do Canal Arterial em fetos natimortos enfatizando o seu comprimento e seu diâmetro de acordo com o tamanho de cada feto. Além de que, descrevemos as distâncias entre este e pontos de reparo importantes para sua abordagem por videotoracoscopia. Estudamos dezesseis fetos natimortos, sendo que oito eram do sexo masculino. O tamanho do feto e os perímetros torácico e cefálico variaram de 42-57, 26-35 e 29-35,5 cm, respectivamente. A média de comprimento e diâmetro foram 11,06 e 5,56 cm. Distâncias entre o Canal e as seguintes estruturas: clavícula-22,13mm, segunda costela-20,75mm, esterno-33,88mm e a.subclávia esquerda-5,30mm. Acreditamos que as medidas apresentadas podem facilitar a abordagem do canal por videotoracoscopia.
