ABSTRACT

Retrospective analyses were conducted for the clinical data of two cases with pseudoMeigs' syndrome at our hospital.Both had respiratory symptoms,such as cough and dyspnea.Radiological examinations revealed ascites,pleural effusion and pelvic mass.The definite pathological diagnosis was pelvic malignant tumor.After surgical tumor removal,ascites and pleural effusion disappeared without recurrence.Along with reviewed cases from PubMed in the last decade,a total of 49 cases had pseudoMeigs' syndrome.The age range was 11-73 years.Their clinical manifestations include dyspnea (78%),abdominal distension (69%),cough (14%),abdominal pain (12%),fatigue (10%),weight loss (6%) chest pain (4%),fever (4%),abdominal mass (4%),and oliguria (2%).CA125 was commonly elevated.The primary tumors in pelvic cavity accounted for 78%.And ovarian metastasis from colon cancer was one of the most common in metastatic tumor.

ABSTRACT

Objective To observe the effect of bronchial asthma combined with allergic rhinitis by azelastine hydrochloride nasal spray.Methods Eighty-seven cases of bronchial asthma combined with allergic rhinitis patients were divided into treatment group(45 cases)and control group (42 cases) by random number table method,both groups were given the treatment of bronchial asthma according to the guidelines for regulating,treatment group on the basis was combined with azelastine hydrochloride 1 spray per side,2times per day,for 3 months.Observe two groups of asthma symptoms within 3 months control situation,the number of antibiotics and hospitalization for acute exacerbation.Results Treatment group total effective rate,asthma control test score,number of antibiotics and due to the number of exacerbations hospitalization were superior to control group [93.33%(42/45) vs.76.19%(32/42),(23.80 ± 2.15)scores vs.(22.05 ±3.16) scores,13.3% (6/45) vs.35.7% (15/42),11.1% (5/45) vs.33.3% (14/42)] (P < 0.05 or < 0.01).Conclusion Combined by azelastine hydrochloride nasal spray may make a good control,reduce the use of antibiotics for acute attack and times of hospitalization,worthy of clinical application.

ABSTRACT

Objective To determine the causes and risk factors of antimicrobial treatment failure in patients with community-acquired pneumonia(CAP). Methods Hospitalized adults with CAP from January 2006 to December 2006 were analyzed retrospectively. Treatment failure was defined as appearance of nonresponding pneumonia and progressive pneumonia. Patient's clinical features were analyzed. Results All of 378 patients were involved in this study. Total antimicrobial treatment failure was happened in 50 patients(32 patients with non-responding pneumonia and 18 patients with progressive pneumonia). The causes were infectious (35 patients,70% ), non-infectious (11 patients,22% ) and undetermined (4 patients,8% ).Mortality of antimicrobial treatment failure was 18%(9/50, 8 patients died of infectious cause, 1 patient had no clear cause of death). Stepwise Logistic regression analysis showed that C-reactive protein, multilobar pneumonia,albumin < 30 g/L,renal function lesion,liver function lesion were related with antimicrobial treatment failure. Independent factors of treatment failure were multilobar pneumonia (P= 0.002) ,albumin <30 g/L(P = 0.001 ) and renal function lesion (P = 0.000). Conclusion The major challenge associated with antimicrobial treatment failure in hospitalized patients with CAP is infection, most of which is infection of drug resistant strain.

ABSTRACT

Objective To observe the effects of peritoneal ventilation with pure oxygen in the rabbits with hypoxaemia and hypercapnia induced by mechanical controlled hypoventilation.Methods Sixteen rabbits were invasive|y ventilated after trachea incision.Hypoxaemia and hypercapnia were induced by hypoventilation which was implemented both by degrading ventilation parameters and respiratory depression induced by intravenous infusion of muscle relaxant.Then pure oxygen was insufflated into the peritoneal cavity and arterial blood gases were measured every 30 minutes for two hours.Results The PaO2 was (52.50±3.46)mm Hg at baseline and increased to (76.46±7.79) mm Hg, (79.62±9.53) mm Hg, (78.54±7.18) mm Hg, and (81.1±8.3) mm Hg, respectively at 30,60,90, and 120 minutes after the peritoneal ventilation with pure oxgen (all P < 0.05).Meanwhile PaCO2 was (63.84±9.09)mm Hg at baseline and (59.84±14.22) mm Hg, (59.16±15.5) mm Hg, (60.02±7.07) mm Hg, and (61.38± 6.56)mm Hg, respectively at 30,60,90, and 120 minutes after the peritoneal ventilation with pure oxgen with no significant change (P > 0.05).Conclusion Peritoneal ventilation can obviously improve hypoxaemia induced by mechanical controlled hypoventilation, whereas hypercapnia remains unchanged.

ABSTRACT

Objective To evaluate efficacy and safety of locally-produced pazufloxacin mesilate sodium chloride injection in the treatment of bacterial infections of respiratory and urinary tract.Methods A multi-center double-blind randomized controlled clinical trial was carried out to evaluate efficacy and safety of pazufloxacin mesilate sodium chloride in treatment for acute bacterial infection, as compared to those of levofloxacin hydrochloride and glucose injection as control treatment.A total of 244 patients with acute bacterial infection of respiratory and urinary tract were enrolled in the studies.120 in trial group and 120 in control group, with four withdrawals.Pazufloxacin mesilate and levofloxacin were administered intravenously by drip at a dose of 300 mg and 200 mg, every 12 hours for 7 to 14 days for trial and control groups, respectively.Resuits Overall efficacy of pazufloxacin mesilate was 77.0 percent and 93.5 percent in treatment for acute bacterial infections of respiratory and urinary tract.respectively, and that of levofloxacin was 80.6 percent and 89.6 percent, respectively.Overall bacterial clearance rate WaS 91.5 percent for pazufloxacin mesilate, 89.6 percent for respiratory tract infection and 94.1 percent for urinary tract infection, respectively.and 93.4 percent for levofloxacin, 97.3 percent for respiratory tract infection and 89.7 percent for urinary tract infection, respectively.No significant difference in adverse drug reactions between the two groups(P>0.05)was found, with 4.88 percent and 7.44 percent for trial and control groups, respectively.Conclusions Pazufloxacin mesilate sodium chloride injection produced locally is a safe and effective antibiotic in treatment for acute infections of respiratory and urinary tract.

ABSTRACT

AIM: To observe the effect of phagotrophic ability of macrophage on pseudomonas aeruginosa which was cocultured with low concentration of erythromycin. METHODS: The pseudomonas aeruginosa ATCC 27853 was cultured in LB both with low concentration of erythromycin (Group A) and without erythromycin (Group B) for 18 h separately, then incubated with macrophage from murine abdominal cavity for 2 h. The ratio of phagocytosis of macrophage was determined by microscope detection. RESULTS: The ratio of phagocytosis of group A in which erythromycin was added was higher than the group B, and the pseudopodia of macrophage were increased in the group A under electron microscope. CONCLUSION: Low concentration of erythromycin may enhance the phagocytosis ability of macrophage in vitro by influencing the virulent factors of PA.

ABSTRACT

This article deals with the detection of the left ventricular band by 2-dimensional echocardiography in 102 cases.The nomenclature of the band and its detection rate,diagnosis and differential diagnosis were discussed.The mechanism of the ventricular premature beat and systolic murmur in the cardiac apical region,and their clinical significance were also dis- cussed.This article shows that this band is usually regarded as an anatomic feature.It does not relate to any cardiac diseases.The premature beat and murmur caused by the band belong in benign premature beat or physiologic murmur.
