ABSTRACT

Jaw motion tracking, which is introduced in recent case reports, is a method which records the patient’s individualized pathway of the mandibular movements along with facebow transfer, and reproduces the information in the virtual space of computer-aided-design/computer-aided-manufacturing (CAD-CAM) software.In this present case, a collapse of the occlusal plane was observed, due the loss of posterior teeth for a long period. Full-mouth rehabilitation with an increase in the occlusal vertical dimension was planned. First, the patient’s mandibular movements were recorded on the newly established jaw relation by jaw tracking, and this information was assembled with the patient’s intraoral data to create a virtual patient. Implant planning and diagnostic wax-up was done on the virtual patient, leading the fabrication of the provisional prosthesis. On the newly established jaw relation with an increase in the occlusal vertical dimension, canine guidance of the provisional prosthesis was checked. Finally, the provisional prosthesis was carried out to the definitive prosthesis. Using the advantages of the technologies in the digital dentistry, the patient was satisfied with the function and the esthetics after the treatment.

ABSTRACT

Background@#Although depression and motion sickness are prevalent in military personnel and seafarers, the association between depression and seasickness has been not yet elucidated. We aimed to evaluate the relationship of depression with initial susceptibility and adaptation to seasickness amongst military seafarers. @*Methods@#This retrospective cohort enrolled Navy seafarers who started seafaring between 2017 and 2019. Three groups were established according to the Beck Depression Inventory (BDI) score: no depression (BDI score of 0), minimal depression (BDI score 1–9), and mild-tomoderate depression (BDI score 10–29). The occurrence of seasickness requiring treatment was observed as the prescription of medication for the first 30 distant seafaring days.Considering adjustment period, the two different outcomes were defined. The susceptibility to seasickness was evaluated via at least one day suffered from seasickness requiring treatment during the early period (the first 5 seafaring days), and adaptation ability to seasickness was defined by more than 10% of the ratio, calculated days suffered from seasickness requiring treatment/days of seafaring during the late period (the 6–30th seafaring days). Binary logistic regression was further evaluated to estimate the odds of BDI groups and BDI score adjusted for age and workplace whether outside visual perception was possible. @*Results@#Among the 185 recruits, 179 participants (97%) sailed for more than 5 days were included in the study. Of the participants, 36% was susceptible to seasickness in the early and 17% was poorly adapted to seasickness in the late period. Multivariable model revealed that mild-to-moderate depression had elevated risk of poor adaptation (odds ratio [OR], 4.63; 95% confidence interval [CI], 1.31–16.98) whereas the results were not statistically significant for susceptibility to seasickness in the early period BDI score was independently associated with increased odds of poor adaptation (OR, 1.10; 95% CI, 1.04–1.18). @*Conclusion@#The present study suggests that depression is associated with poor adaptation to seasickness in Navy seafarers. Depression screening tool might be helpful for providing preventable strategies for population at risk.

ABSTRACT

Traumatic subclavian artery injury is a rare complication of clavicular fracture. An arterial injury can present as a pseudoaneurysm, which can cause a hypercoagulable state, thrombus formation, deep vein thrombosis, and distal embolism. We report a case of subclavian artery pseudoaneurysm that presented as embolic stroke in a 81-year-old man at 2015 1 month after clavicular fracture.

ABSTRACT

BACKGROUND: Cigarette smoking is an important modifiable cardiovascular risk factor, which is known to acutely increase arterial stiffness. But the chronic effects of smoking on arterial stiffness are still controvertial. We studied the effects of chronic cigarette smoking on arterial stiffness in Korean subjects. METHODS: Our subjects included those >18 years of age (n=2685, 991 females) who had undergone a pulse wave velocity from July 2004 to June 2008. They were classified into two groups, smokers (n=641) and non-smokers (n=2044). In addition to the pulse wave velocity (PWV), we reviewed the highly sensitive C-reactive protein levels in a random manner when it was available. RESULTS: Males were predominant in the smokers group. Smokers had significantly higher heart-femoral PWVs than non-smokers (1083.6+/-264.1 cm/sec vs. 1041.3+/-257.3 cm/sec; p<0.001). There was no linear relationship between smoking duration and PWV (r= -0.225, p=0.506) even after adjusting for age, sex, and body mass index. Furthermore, there was no difference in the ankle-brachial index between the two groups. On the multivariate regression analysis, age, male gender, hypertension, and diabetes were independent predictors of increased arterial stiffness. CONCLUSION: In this study, chronic cigarette smoking increased central arterial stiffness in Korean subjects.

ABSTRACT

PURPOSE: The purpose of this study is to estimate muscle defect by ultrasonography in the patients with secondary deformities of the lip. We investigated the association between the muscle defect in the repaired cleft lip and the philtral appearance not only at resting state but also maximal puckering. METHODS: From December 2006 to November 2007, 52 children were evaluated after primary or secondary cheiloplasty. Digital photographs were taken both from the front and both three quarter views in repose and at maximal pucker. Video clips were also taken in repose and at maximal pucker. A panel of four, scored the philtral ridge and dimple seen on these photographs and videos by using two visual analog scales. Eminence of the philtral ridge was scored by a 5 point grading scale, from "conspicuous groove" to "normal philtral ridge" and the philtral dimple was scored by 3 point grading scale, from "no dimple" to "prominent dimple". Ultrasound images of the upper lip were made using a linear array transducer at the resting position of the lip and evaluated by a single radiologist. RESULTS: The philtral ridge eminence scored 2.79+/-0.54 and 1.40+/-0.53 at resting and maximal pucker, correlating with "flat" and "conspicous groove". The philtral dimpling scored 1.44+/-0.53 and 2.27+/-0.66 at resting and maximal pucker, correlating with "no dimple" and "slight dimple". Ultrasound imaging showed the average muscle dehiscence to be 3.78+/-2.14mm at resting position. Correlation between the muscle defect in ultrasound imaging and philtral ridge eminence at rest was statistically significant (p<0.050), but was not significant(p=0.756) at maximal pucker using Spearman's rank correlation. Correlation between the muscle defect in ultrasound imaging and philtral dimpling was not statistically significant both at rest (p=0.920) and at maximal pucker(p=0.815) using Spearman's rank correlation. CONCLUSION: Quantitative assessment of the muscle defect using ultrasonography correlates with the static philtral appearance, but does not correlate with the dynamic appearance. Also, the size of the muscle defect does not show any correlation with the philtral dimpling. Our findings reveal that ultrasound imaging partially reflect static appearance of philtrum but cannot reflect dynamic appearance and suggest the need for further research to evaluate dynamic appearance.

ABSTRACT

Hyperthyroidism can be a cause of congestive heart failure. Left heart failure is occasionally complicated in patients with hyperthyroidism. However, predominant right heart failure without LV systolic dysfunction rarely develops in patients with hyperthyroidism. We present herein a case of reversible severe tricuspid regurgitation with predominant right heart failure associated with thyrotoxicosis.

ABSTRACT

Rhinophyma is characterized by a disfiguring soft tissue hypertrophy of the nose. Severe cosmetic deformity and impairment of breathing may coexist, making the surgical treatment necessary. We present a case of giant rhinophyma, who had difficulty in nasal breathing and took preventive aspirin for CVA. We treated this patient with scalpel tangential excision and skin graft. A giant rhinophyma involved two thirds of nasal dorsum, nasal tip, and both alae. The patient withdrew aspirin 7 days prior to surgery, and rhinophyma was excised with scalpel. The defect was covered with full-thickness skin graft from right supraclavicular area. Five days after surgery, the patient resumed to take aspirin. A skin graft was taken completely and a pathologist confirmed an excised specimen to be a rhinophyma. Within 1 month postoperatively, the patient showed more natural nasal contour and improvement of nasal respiration. Six months after the operation, the nose represented a good appearance without recurrence of rhinophyma. We present a case of giant rhinophyma which had bleeding tendency because of preventive aspirin. We treated this case with scalpel excision and full-thickness skin graft, instead of secondary intention wound healing.

ABSTRACT

PURPOSE: Reconstruction of microtia using costal cartilage graft is commonly used technique nowadays. The chest wall depression at the donor site after the graft, however, has been noticed in many articles. Prevention or correction technique for the depression at the donor site also has been reported and we also have been concerned about the problem. This article is a case report about a new technique preventing chest wall depression after costal cartilage graft. METHODS: We selected total 15microtia patients who visited our clinic, from December 2005 to July 2007. They were 10 male and 5 female patients and the mean age was 11.9 years. The average follow up period was 9.2months(2 to 15months). We used 6, 7, and 8th costal cartilage for microtia reconstruction. And then we turned over pivot of cartilage resection margin, after bihalving costal cartilage involving about 5-6cm of 6th and 7th rib bone. After microtia reconstruction, chest donor sites were evaluated by physical examination and radiography. RESULTS: Postoperative depression at the donor site was much less when the costal cartilage turnover technique was performed. Postoperative physical examination and three dimensional reconstruction CT showed that the rest part of rib bone was turned over and it supported the soft tissue defect during respiration. CONCLUSION: We expect that the turnover rib bone will not be absorbed after graft, as well as offering mechanical support, compared to the other reports.

ABSTRACT

PURPOSE: We investigate the possibility of vessel preservation with glycerol and evaluate the potential of preserved gastroepiploic artery as a tool for the microsurgical practice. METHODS: In 5 early gastric cancer patients, IRB(No. C-0603-040-170), we gained gastroepiploic artery specimens(5 segments, about 10cm) after gastrectomy. Each segment was rinsed in a normal saline and subsequently placed in a bottle, containing 50mL anhydrous glycerol (87%). The bottles were refrigerated at 4degrees C, the longest preservation time being 5 months. RESULTS: At first glance, no fragmentation was detected and the surface of vessel seemed mild sclerotic. In histological examination, vascular structures remained intact though preservation with glycerol led to a mild atrophy of the smooth muscle in the tunica media. Especially, we found out the elastic fibers and endothelial lining of the intima were preserved until 5 months. Adequate status for using microsurgical practice was also maintained and leakage test was performed successfully with gentian violet ink. CONCLUSION: Based on the results obtained in this study, the gastroepiploic artery preservation with glycerol as a training model for microvascular anastomosis is a technically very simple procedure and useful for the novice microsurgeon.

ABSTRACT

PURPOSE: Deformities after bilateral cleft lip repair usually involve the philtrum and the central upper lip, which presents as wide philtrum, wide Cupid's bow, whistling deformity and prominent prolabium. The object of this study is to introduce a simple method to correct such deformities of bilateral cleft lip, that is the redo-operation following the modified Millard's method. METHODS: We present 2 patients with secondary deformities of the philtrum and central upper lip after bilateral cleft lip repair. A simple design was used to excise the excessive skin and mucosa of the central upper lip, and the philtrum and Cupid's bow were restored to the standard average width of Korean population. RESULTS: The wide central upper lip and philtrum were successfully restored to the appropriate width(10-12mm) and shape. Also the border of the red vermilion was aligned properly resulting in a good contour of the upper lip. Both patients were satisfied with the results. CONCLUSION: In patients with wide philtral deformities, our simple method can be utilized for a satisfying result.

ABSTRACT

PURPOSE: The spreading, orientation, and chemotaxis with the gradient of a chemoattractant of smooth muscle cells (SMCs) were studied on the micro-grooved substrata by the light, fluorescence and scanning electron microscopy. METHOD: Vertical-walled grooves were produced in silicon wafers by the micromachining technique. All grooves were 4~20micrometer deep and 10~80 micrometer wide. SMCs were cultured on each microgroove and examined under stereo-microscope. RESULT: Cell clusters were markedly oriented by all the grooved substrata examined. Time-lapse images acquired from CCD (Charge Coupled Device) showed that the grooves directed the migration of SMCs. There was no prominent difference in the migration speed of SMCs according to the grooves. All the cytoskeletal fibers were reorganized in the same direction with grooves. Especially the alignments of microtubule and intermediate filaments were distinguished in the SMCs on the micro grooves. CONCLUSION: These results could be applied to the analysis of vascular restenosis and the development of artificial blood vessels.

BACKGROUND: The fragile X syndrome is the most common cause of inherited mental retardation, is almost always caused by abnormal CGG trinucleotide amplication within the FMR1(fragile X mental retardation) gene located in Xq27.3 METHODS: DNA samples were obtained from the amniotic fluids of known carrier mother and 35 mothers without risk factors of the fragile X syndrome. Polymerase chain reaction(PCR) and Southern blot analysis were performed to evaluate the number of CGG repeats in the FMR1 gene. RESULTS: The DNA samples from the carrier mother gave a large fragment over 300 repeats by PCR. All 35 control samples showed fragments sized under 35 repeats. CONCLUSIONS: Prenatal diagnosis of the fragile X syndrome could be done with mid-trimester amniotic fluid using PCR and Southern blot method.

ABSTRACT

PURPOSE: Endothelial cells (ECs) are exposed to continuous shear stress from their birth and also respond to the hemodynamic environmental changes which may be localizing factor in vascular diseases, such as atherosclerosis. The hemodynamic shear stress is implicated in the pathogenesis of atherosclerosis, thrombosis, and also restenosis. The objective of this study is to investigate the morphological and molecular biological changes of vein ECs under complicated flow flield which could occur in the anastomosis site of the autogenous vein bypass graft. METHOD: We developed a laminar flow chamber for the normal vessel and a sudden expansion flow chamber to simulate the recirculation or the stagnation zone of vascular graft. RESULT: Normal flow shear stress transformed ECs from random oriented polygonal, cobblestone shape to elongated shape aligned along the flow direction. However the stagnation and flow separation zone could not show the morphologic change of ECs and could be the region of low shear stress prone for intimal hyperplasia and atherosclerosis initiation. CONCLUSION: It also represents that the ECs can sense the magnitude and the direction of the flow shear stress and change their phenotype through the remodeling of the actin microfilaments.

ABSTRACT

Anhidrotic ectodermal dysplasia is inherited as an X linked recessive trait. This disor der is characterized by hypotrichosis, hypodontia and hypohidrosis. The diagnosis is often delayed until the first or second year of life, after repeated episodes of potentially damaging high fever. In the newborn period, the diagnosis is more difficult, but early diagnosis is of importance in ensuring that the appropriate enivironment and medical measures be taken to avoid uncontrolled hyperthermia. We have experienced a case of anhidrotic ectoclermal dysplasia in an8-day-old male patient who showed charecteristic features including hypotrichosis, peeling or scaling of the skin, recurrent fever and a characteristic face. A skin biopsy from the right palm revealed no sweat gland strutures. A brief rview with related literature is also presented.
