ABSTRACT

PURPOSE: The aim of this study was to identify perception, use status and factors influencing use of medical alert identification (ID) in diabetic patients who are on insulin. METHODS: A survey was conducted from December 2013 to April 2014 with 198 people who were asked to participate in the survey. RESULTS: The medical alert ID perception score was 3.54 on a 5-point scale. Most of respondents (82.8%) knew about the medical alert ID, but only 23.2% of respondents wore a medical alert ID and only 43.5% of respondents always wore a medical alert ID. Perception and frequency of possession were positively correlated (r=.41, p<.001). The most influential factor for the possession of a medical alert ID was the perception of the medical alert ID (β=.41, p<.001). The perception of the medical alert ID explained 17% of variance in possession of a medical alert ID. CONCLUSION: Most of diabetic patients who take insulin received education about hypoglycemia and knew about the medical alert ID, but only few of them wore a medical alert ID. Findings from this study indicate that perception is an important variable related to possession of a medical alert ID in this population.

ABSTRACT

BACKGROUND: The purpose of this study was to investigate the association between number of sleep hours and obesity based on body mass index (BMI) after adjusting for socioeconomic position, health behaviors, and clinical factors. METHODS: Data from subjects aged 20-64 years (n=12,688) in the Korea National Health and Nutrition Examination Survey in 2007-2009 was analyzed. Subjects were divided into two groups according to age: 20-44 (young adults) and 45-64 years (middle-aged adults). With respect to sleep time, subjects were classified as short sleepers (<6 hr/day), long sleepers (≥9 hr/day), and normal sleepers (6-8.9 hr/day). Obesity was defined as a BMI ≥25. RESULTS: The proportion of patients who were obese was 27.8%. The distribution of short sleepers, long sleepers, and normal sleepers was 6.4%, 13.7%, and 79.9%, respectively. The adjusted short sleeper and long sleeper odds ratios (OR) for obesity in young adults were significantly different from that in middle-aged adults after adjustment for confounding factors. In women, the adjusted short sleeper OR for obesity was 1.56 (95% CI, 1.02-2.37). CONCLUSION: This community-based random sample of Korean adults showed that women with short sleep duration may be at significant risk of obesity.

ABSTRACT

BACKGROUND AND OBJECTIVES: Left ventricular diastolic dysfunction is known to be a marker of myocardial damage, in particular myocardial fibrosis resulting from hypertension (HT). However, few studies have shown an association between the grade of diastolic dysfunction and blood pressure classification. We investigated the association between diastolic dysfunction and prehypertension (preHT) in apparently healthy adults who underwent routine health examinations. SUBJECTS AND METHODS: The study sample included 4261 Koreans, 45 to 64 years of age with no previous history of HT, diabetes mellitus, malignancy, proven coronary artery disease, or valvular heart disease based on echocardiography, who underwent routine health examinations including echocardiography. The subjects were classified into three groups based on resting blood pressure: prehypertensive, hypertensive, and normotensive. RESULTS: The prevalence of preHT in our study was 42.1%. After adjusting for age, gender, smoking status, alcohol consumption, fasting blood sugar, serum lipid profile, and body mass index, left ventricular diastolic dysfunction grades 1 and 2 were significantly more frequent in subjects with preHT (odds ratio [OR] 1.66 [95% confidence interval {CI} 1.40-1.96] and 1.37 [95% CI 0.95-1.97], respectively). When analyzed according to gender, the increased OR was especially notable in males. CONCLUSION: Left ventricular diastolic dysfunction appears to be significantly associated with preHT in Korean middle-aged males.

ABSTRACT

PURPOSE: The mechanism including changes of proteome within cavernosal tissue after cavernous nerve injury were not evaluated. We performed proteomics and functional analysis to identify proteins of penile corpus cavernosum whose expression was or was not altered by cavernous nerve resection (CNR). MATERIALS AND METHODS: Using 8-week-old male WKY rats, sham and CNR operation under microscope were performed. After 8 weeks, penile tissues of sham and CNR group were harvested. We used 2-DE and MALDI-TOF/TOF (AB 4700) to identify of differently expressed penile proteins. 2-DE gels were stained with silver nitrate and the gels were analyzed with PDQuest. RESULTS: We isolated more than 950 proteins on silver-stained gels of whole protein extracts from normal rat penile corpus cavernous. Of these proteins, 48 prominent proteins were identified using MALDI-TOF/TOF. Protein characterization revealed that the most prominent penile corpus cavernous proteins were those with antioxidant, chaperone, or cytoskeletal structure. Moreover, 11 proteins having levels elevated by CNR were annexin proteins, endoplasmic reticulum protein 29, glutathione s-transferase w-1, and others. In addition, Rho-GDP dissociation inhibitor (RhoGDI), a regulator of Rho proteins, was also increased in CNR rats compared with sham-operated control rats. CONCLUSIONS: The apoptotic signals observed in penile tissues was greatly increased in CNR rats than in sham-operated rats. These results suggest that RhoGDI is one of the proteins regulated by CNR in penile smooth muscle strips, and has a crucial role in the early stage of penile apoptosis.

ABSTRACT

PURPOSE: The purpose of this study was to examine the effects of an inpatient pulmonary rehabilitation program on dyspnea, exercise capacity, and health related quality of life in inpatients with chronic lung disease. METHOD: This quasi experimental study was designed with a nonequivalent control group pre-post test time series. Twenty three patients were assigned to the experimental group and nineteen to the control group. The inpatient pulmonary rehabilitation program was composed of upper and lower extremity exercise, breathing retraining, inspiratory muscle training, education, relaxation and telephone contacts. This program consisted of 4 sessions with inpatients and 4 weeks at home after discharge. The control group was given a home based pulmonary rehabilitation program at the time of discharge. The outcomes were measured by the Borg score, 6MWD and the Chronic Respiratory Disease Questionnaire(CRQ). RESULTS: There was a statistically significant difference in dyspnea between the experimental group and control group, but not among time sequence, or interaction between groups and time sequence. Also significant improvements in exercise capacity and health related quality of life were found only in the experimental group. CONCLUSIONS: An Inpatient pulmonary rehabilitation program may be a useful intervention to reduce dyspnea, and increase exercise capacity and health related quality of life for chronic lung disease patients.
