ABSTRACT

Objective:To observe the morphological characteristics of L 4, 5 facet joints in patients with degenerative lumbar spondylolisthesis (DLS) of different spinopelvic types based on Roussouly classification. Methods:We retrospectively analyzed 142 patients with DLS who visited the department of orthopaedics in the Affiliated Hospital of Southwest Medical University from August 2018 to May 2022. There were 33 males aged 65.0±10.7 years and 109 females aged 61.8±9.6 years. The following morphological parameters of the L 4, 5 facet joint were measured on the CT images: facet joint angle (FJA), pedicle facet angle (PFA), facet joint tropism (FT) and facet joint osteoarthritis (OA) degree; the sacral slope (SS), lumbar lordosis (LL) and percentage of L 4 slip distance (SDP) were measured on preoperative standing neutral lumbar radiographs. According to the Roussouly classification, the patients were divided into four groups (type I, type II, type III, and type IV). The differences of morphological parameters of the facet joints and SDP were compared among the four groups, and the correlation between the FJA and PFA was analyzed. Results:There were 142 patients, including 28 type I, 50 type II, 43 type III, and 21 type IV according to the Roussouly classification. The SDPs of type I, type II, type III, and type IV were 19.1%±3.4%, 18.6%±3.9%, 21.7%±3.9%, 25.0%±2.4%, respectively. Except for types I and II, there were statistically significant differences in pairwise comparison among all other types ( P<0.05). The FJAs in type I and type II (31.4°±6.3°, 35.2°±6.8°) were larger than those in type III (28.4°±5.6°) and type IV (23.4°±4.5°), and the FJA in type III was larger than that in type IV. Conversely, the FJA in type I was smaller than that in type II. These differences were statistically significant ( P<0.05). The PFAs in type I and type II (113.9°±4.9°, 111.3°±5.6°) were smaller than those in type III (116.3°±4.4°) and type IV (121.8°±3.5°), and the PFA in type III was smaller than that in type IV, while, the PFA in type I was larger than that in type II. These differences were statistically significant ( P<0.05). The degree of OA in both type I and type II was lower than that in type III and type IV, with statistically significant differences ( P<0.05). However, there were no statistically significant differences in the degree of OA between type I and type II and between type III and type IV ( P> 0.05). Additionally, there were no statistically significant differences ( F=0.40, P=0.752) in the FT values among type I, type II, type III, and type IV (5.8°±2.3°, 5.6°±2.4°, 6.1°±1.8°, 5.9°±1.9°). Pearson correlation analysis showed that FJA was negatively correlated with PFA ( r=-0.68, P<0.001). Conclusion:In the slip segment of DLS, the facet joint morphology was part of the joint configuration in different spinopelvic types, not just the result of joint remodeling after DLS. Morphological characteristics of the facet joints and DLS interacted with each other.

ABSTRACT

COVID-19 has been prevalent for three years. The virulence of SARS-CoV-2 is weaken as it mutates continuously. However, elderly patients, especially those with underlying diseases, are still at high risk of developing severe infections. With the continuous study of the molecular structure and pathogenic mechanism of SARS-CoV-2, antiviral drugs for COVID-19 have been successively marketed, and these anti-SARS-CoV-2 drugs can effectively reduce the severe rate and mortality of elderly patients. This article reviews the mechanism, clinical medication regimens, drug interactions and adverse reactions of five small molecule antiviral drugs currently approved for marketing in China, so as to provide advice for the clinical rational use of anti-SARS-CoV-2 in the elderly.

ABSTRACT

Bibliometric and scientific knowledge graph methods were used to analyze the research status and hot spots of acupuncture-moxibustion in treatment of myofascial pain syndrome (MPS) and explore its development trend. The articles of both Chinese and English versions relevant to MPS treated by acupuncture-moxibustion were searched in CNKI, VIP, Wanfang, SinoMed and WOS from the database inception to March 20, 2023. Using Excel2016, CiteSpace6.2.R2 and VOSviewer1.6.18, the visual analysis was conducted by means of the cooperative network, keyword co-occurrence, keyword timeline, keyword emergence, etc. From Chinese databases and WOS database, 910 Chinese articles and 300 English articles were included, respectively. The annual publication volume showed an overall rising trend. Literature output of English articles was concentrated in Spain, China, and the United States, of which, there was less cross-regional cooperation. In the keyword analysis, regarding acupuncture-moxibustion therapy, Chinese articles focused on "acupuncture", "electroacupuncture" and "acupotomy"; while, "dry needling" and "injection" were dominated for English one. Clinical study was the current hot spot in Chinese databases, in comparison, the randomized controlled double-blind clinical trial was predominant in WOS. Both Chinese and English articles were limited in the report of mechanism research. The cooperation among research teams should be strengthened to conduct comparative research, dose-effect research and effect mechanism research with different methods of acupuncture-moxibustion involved so that the evidences can be provided for deeper exploration.

ABSTRACT

A user-friendly teaching software for visual analysis of acupoint compatibility laws has been developed based on the principles of partial order mathematics. This software is designed to provide auxiliary teaching of structured organization and visualization of law knowledge of compatibility data of acupuncture and moxibustion prescriptions from ancient texts, textbooks, and clinical case records. The software is installed as a plugin in the Microsoft Office Excel, allowing the generation of visually appealing graphs and associated rules that align with the cognitive patterns of teachers and students majoring in acupuncture and moxibustion. Its aim is to facilitate the discovery and analysis of underlying patterns and structured knowledge embedded in acupoint compatibility data, thus contributing to the enhancement of teaching effectiveness in acupoint compatibility.

ABSTRACT

Hepatocellular carcinoma (HCC) is a common malignant tumor with poor prognosis and high mortality. In this study, we demonstrated a novel vaccine targeting HCC and tumor neovascular endothelial cells by fusing recombinant MHCC97H cells expressing porcine α-1,3-galactose epitopes (αGal) and endorphin extracellular domains (END) with dendritic cells (DCs) from healthy volunteers. END+/Gal+-MHCC97H/DC fusion cells induced cytotoxic T lymphocytes (CTLs) and secretion of interferon-gamma (IFN-γ). CTLs targeted cells expressing αGal and END and tumor angiogenesis. The fused cell vaccine can effectively inhibit tumor growth and prolong the survival time of human hepatoma mice, indicating the high clinical potential of this new cell based vaccine.

ABSTRACT

OBJECTIVE@#To establish a quantitative evaluation parameter system for basic acupuncture manipulations based on 3D motion tracking technology, and to further analyze the characteristics of acupuncture manipulation.@*METHODS@#The motion tracking system Simi Motion Ver.8.5 was used to collect parameters of acupuncture manipulations, so as to establish a quantitative evaluation system mainly in spatial control parameters, time control parameters and stability parameters. Based on the evaluation parameter system, data of acupuncture manipulations (even reinforcing-reducing manipulation using lifting-thrusting technique and twirling technique) were collected from 19 teachers majored in acupuncture-moxibustion, of which the characteristics were analyzed and were compared with the results tested by traditional ATP-Ⅱ acupuncture manipulation parameter determination apparatus (ATP-Ⅱ).@*RESULTS@#The evaluation parameter system could quantitatively analyze the basic acupuncture manipulations in 3 aspects, i.e. space, time and stability. Acupuncture manipulations were performed by shifting the needle body in a certain range along X, Y and Z axes. The amplitude and speed of thumb movement were greater than those of index finger. The time-history of lifting was similar to thrusting, while the time-history of twirling to left was similar to twirling to right in even reinforcing-reducing manipulation. The coordinate-time curve of finger movement of the evaluation system had consistent wave pattern compared with the voltage-time curve generated by ATP-Ⅱ.@*CONCLUSION@#The quantitative evaluation parameter system based on 3D motion tracking technology can objectively evaluate basic acupuncture manipulations, and show the characteristics of different manipulations and the movement details of finger joints.

ABSTRACT

Objective:To investigate the expression changes of serum C-reactive protein (CRP) and tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) in patients with acute viral infection and the value of clinical diagnosis and treatment.Methods:83 patients with acute viral infection in Hunan Provincial Brain Hospital from March 2018 to June 2019 were selected as the observation group, 81 patients with acute bacterial infection in the same period were selected as the control group A, and 81 healthy people as the control group B for prospective study. On the day of admission or physical examination, peripheral venous blood was drawn; the serum CRP and TRAIL levels were detected. The serum CRP and TRAIL levels of the three groups were compared, and the serum levels of CRP and TRAIL in different treatment groups were analyzed, and the application value of serum CRP and TRAIL in the diagnosis of acute viral infection was analyzed.Results:The serum CRP level of the observation group was lower than that of the control group A, which was higher than that of the control group B ( P<0.05); the serum TRAIL level of the observation group was higher than that of the control group A and B ( P<0.05); the area under the receiver operating characteristic (ROC) curve of the combined diagnosis of serum CRP and TRAIL was 0.921, which was higher than that of the two alone; When the cut-off value of CRP and TRAIL were 5.664 mg/L and 146.82 pg/ml, the combined diagnostic sensitivity was 95.00%, and the specificity was 80.00%; after treatment, the serum CRP and TRAIL levels of the observation group were significantly lower than those before treatment ( P<0.05), and the serum CRP and TRAIL levels of the effective group were lower than those of the ineffective group ( P<0.05). Conclusions:Serum CRP and TRAIL levels have certain value in the identification and efficacy evaluation of patients with acute viral infection. Combined detection can significantly improve the diagnostic value and provide a reliable reference for clinical treatment and prognosis evaluation.

ABSTRACT

ObjectiveThere is still controversy about which internal fixation method should be used in oblique lateral interbody fusion (OLIF). This paper aims to compare the biomechanical stability of OLIF with different internal fixation methods.MethodsA 31-year-old healthy male volunteer was selected to have a 64-slice spiral CT scan of his lumbar spine. Mimics 19.0, Geomagic Studio 2013, SolidWorks 2017 and other software were used to build a three-dimensional model of L3-L5, and OLIF surgery was simulated to build OLIF finite element models with five different fixation methods: pedicle screw (PS), lateral single rod screw (LSRS), lateral double rod screw (LDRS), lateral single rod screw+ipsilateral translaminar facet screw (LSRS+ITLFS), lateral single rod screw+contralateral translaminar facet screw (LSRS+CTLFS). After validating the validity of the model, the motion modes of spinal flexion, extension, lateral bending and rotation were simulated, and the fixed segment activity and stress distribution characteristics of each model were compared.ResultsIn terms of fixed segment activity, PS had the best fixation effect, and its range of motion (ROM) was the smallest in all 6 modes. The ROM of the vertebral body was maximized when the LSRS was fixed in all directions. LSRS+ITLFS, LSRS+CTLFS and PS had the similar ROM. In terms of maximum stress of cage, PS had the minimum one except in the left bending. LSRS+ITLFS had little stress in all directions except in flexion; LSRS+CTLFS had little stress in all directions except in extension. In terms of the maximum stress in internal fixation, PS had the least one in all directions; LSRS+CTLFS followed, and the maximum stress appeared in extension and right bending (123.05MPA and 91.74MPA, respectively).ConclusionIn OLIF surgery, PS has the best biomechanical effect. LSRS+CTLFS has the similar effect and its clinical operation is simple with relatively small surgical injury, thus providing a reference for clinical choice.

ABSTRACT

The aim of this study was to determine the effects of extremely low frequency electromagnetic field (ELF-EMF) on energy metabolism and oxidative stress in Caenorhabditis elegans (C. elegans). Worms in three adult stages (young adult stage, egg-laying stage and peak egg-laying stage) were investigated under 50 Hz, 3 mT ELF-EMF exposure. ATP levels, ATP synthase activity in vivo, reactive oxygen species (ROS) content, and changes of total antioxidant capacity (TAC) were detected, and worms' oxidative stress responses were also evaluated under ELF-EMF exposure. The results showed that ATP levels were significantly increased under this ELF-EMF exposure, and mitochondrial ATP synthase activity was upregulated simultaneously. In young adult stage, worms' ROS level was significantly elevated, together with upregulated TAC but with a decreased ROS-TAC score indicated by principal component analysis. ROS level and TAC of worms had no significant changes in egg-laying and peak egg-laying stages. Based on these results, we concluded that ELF-EMF can enhance worm energy metabolism and elicit oxidative stress, mainly manifesting as ATP and ROS level elevation together with ATP synthase upregulation and ROS-TAC score decrease in young adult C. elegans.

OBJECTIVE@#To realize real-time measurement and recording of temperature parameters inside acupoints during acupuncture treatment.@*METHODS@#The fine wire thermocouple was embedded in hollow needle body as acupoint detection probe. According to the structure of acupoint temperature signal processing circuit, the Agilent 34420A nanovolt/micro-eurometer was used to design the technology method of acupoint temperature data acquisition, and an acupoint temperature collection platform was established. In addition, a verification experiment was conducted to verify the feasibility and stability of the system.@*RESULTS@#The contrast experiment with mercury thermometer measurement showed that the temperature measurement curve of the designed acupoint temperature data collection platform was highly consistent with that of the standard thermometer, which verified the stability and reliability of the designed temperature measurement platform. The experiment of animal in-vivo measurement showed that the acupoint temperature probe had good feasibility for in-vivo acupoint temperature measurement.@*CONCLUSION@#Acupoint temperature probe and acupoint temperature data collection technology can measure the temperature inside acupoints, and ensured the accuracy and veracity of the results. As an experimental method of acupoints temperature parameter collection, it can quantitatively record the temperature changes during the acupuncture treatment.

ABSTRACT

A teaching training and assessment system for warming acupuncture is developed. This system is suitable for the technique operation of multi-acupoint warming acupuncture, and could evaluate the training effects of warming acupuncture at each acupoint. The hardware structure of this system is mainly composed of man-machine interface, control unit and training unit. The software program includes reset module, system clock module, needle depth signal control module, vibration control module and communication module. The teaching training and assessment system of warming acupuncture adopted touch-screen human-machine interface, which is simple to operate. The basic training unit, through multi-angle and multi-dimension adjustment of universal base, could realize the simulation training of warming acupuncture at different acupoints and angles of human body. Each training unit is relatively independent, and relevant parameters are set separately; the relevant parameters information is displayed in real time. The parameters of training unit includes needling time, vibration frequency and vibration time. In conclusion, the teaching training and assessment system for warming acupuncture could be used in the teaching and assessment of warming acupuncture. According to the criteria including operation time and effect, the students' technique ability of warming acupuncture could be evaluated objectively, which is helpful to set teaching objectives and technical operation requirements in warming acupuncture teaching.

ABSTRACT

Objective To explore the application of nerve ultrasound score (DCEC score) in the diagnosis of diabetic peripheral neuropathy. Methods Seventy-three patients with type 2 diabetes were divided into subclinical diabetic peripheral neuropathy group (n=16) and diabetic peripheral neuropathy group (n=58). DCEC score was obtained from all participants. The cut-off value of DCEC score was defined by the receiver operating characteristic (ROC) curve, and its sensitivity, specificity, accuracy, positive likelihood ratio and negative likelihood ratio were analyzed. Results When the DCEC score was ≥14.5, 50 cases of diabetic peripheral neuropathy were diagnosed. The sensitivity was 81.0% , the specificity was 80.0% , the accuracy was 80.8% , the positive likelihood ratio was 4.05, and the negative likelihood ratio was 0.24. Conclusion DCEC score can effectively diagnose diabetic peripheral neuropathy which can be used as a new method to diagnose diabetic peripheral neuropathy.

OBJECTIVE@#This study aimed to analyze the treatment for mandibular coracoid fractures retrospectively.@*METHODS@#A retrospective study on 37 patients with mandibular coracoid fractures treated at Department of Traumatic and Plastic Surgery, West China Hospital of Stomatology, Sichuan University from January 2010 to December 2015 was conducted. Eleven patients were treated conservatively, and 26 patients underwent surgical restoration and internal fixation. Mouth opening and pain degree were used as indicators to analyze treatment results.@*RESULTS@#The 37 cases of coracoid fractures accounted for 3.18% of the total mandibular fractures. The average age of patients was 38.05 years. Satisfactory results were obtained in both treatments. A considerable change in the degree of mouth opening before and after 6 months was found in the two groups. The pain degree before treatment and 1 day after operation, 1 day and 4 weeks after operation, and 4 weeks and 6 months after operation indicated that the two groups did not significantly differ. However, substantial changes between the two groups were found before treatment and 6 months after operation.@*CONCLUSIONS@#Conservative treatment is recommended for patients with linear, temporalis muscle-located, and non-displaced coracoid fractures. Surgical treatment is recommended for patients with large fractures and those with accompanying zygomatic arch and mandible fractures.

ABSTRACT

<p><b>OBJECTIVE</b>To measure imaging data of occipitocervical angle in healthy people at different ages by X-ray.</p><p><b>METHODS</b>One hundred and fifty asymptomatic volunteers(75 males and 75 females) aged from 20 and 70 years old with an average of(44.83±13.88) years old were divided into 5 groups according to different ages(20 to 29 years old, 30 to 39 years old, 40 to 49 years old, 50 to 59 years old and 60 to 70 years old), and 30 people in each group. All people underwent lateral radiography of neck on neutral position. McGregor line (the lowest connection line between upper margin of hard palate and occipital scales of plating), occipitocervical angle(OCA) formed by edge of C2 vertebral body, takami's occipitocervical angle formed between connection line of posterior marginal of C2 vertebral body and parallel lines of hard palate were collected. The data were performed statistical analysis according to gender and different ages, and analyzed correlation between OCA and TOCA.</p><p><b>RESULTS</b>OCA of 75 males at different ages were (14.71±3.09)° and(14.22±4.27)° in 75 females. TOCA of 75 males at different ages were (90.50±4.63)° and (90.57±6.67) ° in 75 females. There were no statistical difference in OCA and TOCA in people at different ages(<0.05). There were no significant meaning among 5 groups at different ages in OCA and TOCA(>0.05). The relation analysis results showed positive correlation between OCA and TOCA(=0.454, <0.01).</p><p><b>CONCLUSIONS</b>The paper provided normal values for occipitocervical angle in population of southwestern China between 20 and 70 years old, and results may be useful for posterior occipitocervical fixation and fusion.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To establish osteoblast-osteoclast cell co-culture system in a Transwell chamber, and detect cell viability of osteoblasts and osteoclasts in system.</p><p><b>METHODS</b>Osteoblast MC3T3-E1 and mouse monocytes RAW264.7 were cultivated in vitro. RANKL-induced mouse RAW264.7 monocytes differentiated into mature osteoclasts, osteoblast-osteoclast cell co-culture system was established in Transwell chamber. Cell activity of osteoblasts and osteoclasts were detected by CCK-8 experimenting, Alizarin Red staining, TRAP staining. The expression of OPG, ALP, RANKL, TGF-b1 gene and RANKL protein in osteoblast MC3T3-E1 were detected by PCR, Western-Blot methods. Also, the expression of RANK, NF-κB in gene and protein level in osteoclast were measured through the same method respectively.</p><p><b>RESULTS</b>The co-culture system of Mouse MC3T3-E1 cells and RAW264.7 cell were established in Transwell chamber. Co-culture system affected cell division activities of osteoblasts and osteoclasts. Differentiation of osteoblasts were increased, while differentiation of osteoclast division were slight decreased under microscope observation. OPG (0.65±0.08) and ALP (0.16±0.01) gene expression of co-culture system were less than single culture OPG(1.00±0.08) and ALP (1.01±0.16); TGF-b1(4.42±0.21) and RANKL(4.12±1.04) of osteoblasts in co-culture system were higher than TGF-b1(1.00±0.10) and RANKL(1.00±0.09) under single culture. However, gene expression of RANK(0.63±0.06) and NF-κB(0.64±0.08) in co-culture system were decreased than RANK(1.00±0.08) and NF-κB(1.00±0.09), in single culture, and had significant differences. Similarly, protein expression of OPG(0.43±0.05) and NF-κB(0.59±0.05) of co-culture system were less than OPG(0.84±0.06) and NF-κB(1.13±0.03) of single culture. While RANKL protein expression (0.54±0.03)of co-culture system was more than single culture RANKL(0.31±0.03), and had statistically differences, which was in agreement of the trend of gene expression change.</p><p><b>CONCLUSIONS</b>Co-culture system of mouse MC3T3-E1 cells and RAW264.7 cell was viable in Transwell chamber, and the activity of osteoblasts is higher than osteoclasts in co-culture system.</p>

ABSTRACT

BACKGROUND: Bioabsorbable biomaterials are of crucial importance in tissue engineering applications, and various factors affect their degradation. OBJECTIVE: To compare the degradation characteristics of concentrated growth factor (CGF) clot and CGF membrane in simulated body fluid (SBF) and simulated saliva fluid (SSF). METHODS: Fifteen volunteers were selected, and human blood samples were collected for the preparation of CGF clot or CGF membrane. All specimens from each subject were averagely divided into four groups: group A, CGF clot in SBF; group B, CGF clot in SSF; group C, CGF membrane in SBF; group D, CGF membrane in SSF. The specimens were subjected to the immersion test. The average daily rate of degradation of each group was calculated after the samples were thoroughly degraded, and weight loss ratio per unit time was also determined. RESULTS AND CONCLUSION: (1) The mean degradation time in groups A-D were (14.0±0.7), (9.7±0.9), (9.9±1.2) and (7.2±0.7) days, respectively. (2) By comparing CGF membrane with CGF clot in the same simulated fluid, the average daily degradation rate of CGF clot (groups A, B) was statistically significantly lower than counterparts of CGF membrane (groups C, D) (P < 0.05). By comparison between SBF and SSF, the average daily degradation rate in the SBF (groups A, B) was significantly lower than counterparts in the SSF (groups C, D) (P <0.05). Overall, the degradation rate of CGF membrane is higher than that of CGF clot under the same degradation environment; for CGF membrane or CGF clot, the degradation rate in SSF is higher than that in SBF.

ABSTRACT

Artificial intelligence technology has made breakthroughs in the field of clinical medicine, including diagnosis, imaging, and disease classification. Electronic medical record contains a large number of clinical data such as disease description, diagnosis, examination and treatment. With the participation of medical experts and information scientists, the studies of data mining of electronic medical record using artificial intelligence technology have greatly increased. Although now the method has some limitations, it is more rapid, economic and convenient compared with the traditional method, and is expected to promote the development of human health. In this paper, we reviewed the current status of data mining of electronic medical record using artificial intelligence technology, regarding related technologies, specific examples, and limitations.

ABSTRACT

Objective To introduce a self-developed left atrial appendage occluder,LACBES,and to explore the clinical feasibility of using it for the occlusion of left atrial appendage (LAA).Methods Eight healthy canines were used in this experimental study.The LAA of each canine was occluded with LACBES occluder through trans-femoral vein approach.After the procedure of occlusion,the compression ratio of the occluder was calculated,the residual shunt was assessed by left atrial angiography.The left atrium pressure was monitored before and after the procedure,and the immediate effect of LAA occlusion on the left atrium pressure was statistically analyzed.Results Implantation of LACBES occluder was successfully accomplished in all the eight canines.The compression ratio of the occluders ranged from 10% to 15%.Small amount of postoperative residual shunt was detected in one canine.After occlusion two canines died of procedure-related complications,including shifting of occluder and formation of hematoma at puncturing site.No device-associated death occurred.After occlusion,the left atrial systolic pressure increased instantly,which went up from preoperative (25.4±2.8) mmHg to postoperative (27.5±3.4) mmHg (P<0.05),but it returned to the baseline of (25.4±2.8) mmHg within 15 minutes.Conclusion For the occlusion of LAA,the use of LACBES occluder carries higher instant success rate and lower residual shunt rate with less device-associated complications,although the left atrial systolic pressure has a transient rising immediately after the occlusion.Therefore,it is expected that LACBES will be able to be applied in clinical practice.
