ABSTRACT

Objective:To explore the effect of correcting atrophic and loose breast in women with golden section theory and minimally invasive suspension and silicone prosthesis through areola incision. Methods:A total of 126 female patients with postpartum breast atrophy and ptosis were enrolled in Nanjing Kangmei and Changsha Aiste Medical Beauty Hospital from March 2012 to March 2019. The 126 cases, aged from 25 to 46 years, had a history of 2 to 16 years. The glandular suspension was performed after the implantation of the prosthesis through the incision above the areola according to the golden section. Results:A total of 252 breasts were followed up for 6 to 84 months. No severe complications such as scar, infection or skin necrosis occurred. Among them, 69 cases were very satisfied, 45 cases were satisfied, and 12 cases were basically satisfied. The satisfactory rate was 90.4%.Conclusions:This method is effective with small wound and less complications for micromastia patients with mild to moderate mastoptosis, which is worthy of clinical application.

ABSTRACT

Objective:To observe the effects antiarrhythmic peptide 10 (AAP10) aon acute ventricular arrhythmia and the phosphorylation state of ischemic myocardium connexin. Methods:Acute total ischemia and partial ischemia models were established by ceasing perfusion and ligating the left anterior descending coronary artery in SD rats. The effects of AAP10 (1 mg/L) on the incidence rate of ischemia-induced ventricular arrhythmia were observed. The ischemic myocardium was sampled to detect total-Cx43 and NP-Cx43 by immunofluorescent staining and western blotting. the total-Cx43 expression was detected through image analysis system by semi-quantitative analysis. Results: AAP10 could significantly decrease the incidence of ischemia-induced ventricular tachycardia and ventricular fibrillation. During ischemic stage, total ischemia (TI) and AAP10 total ischemia (ATI) groups were compared with partial ischemia (PI) and AAP10 partial ischemia (API) groups. The rates of incidence for arrhythmia in the ATI and API groups (10% and 0%) were lower than those in the TI and PI groups (60% and 45%). The difference between the two groups was statistically significant (P=0.019, P=0.020). The semi-quantitative analysis results of the ischemic myocardium showed that the total-Cx43 protein expression distribution areas for TI, ATI, PI and API groups were significantly decreased compared with the control group. On the other hand, the NP-Cx43 distribution areas of TI, ATI, PI and API groups were significantly increased compared with the control group (P>0.05). AAP10 could increase the total-Cx43 expression in the ischemic area and decrease the NP-Cx43 expression. Western blot results were consistent with the results of immunofluorescence staining.Conclusions:AAP10 can significantly decrease the rate of incidence of acute ischemia-induced ventricular tachycardia and ventricular fibrillation. Acute ischemic ventricular arrhythmias may have a relationship with the decreased phosphorylation of Cx43 induced by ischemia. AAP10 may stimulate the phosphorylation of Cx43 by increasing the total-Cx43 expression and decreasing the NP-Cx43 expression in the ischemic area, so as to decrease ventricular arrhythmia.

ABSTRACT

Objective To observe the effect of alpha lipoic acid on quality of life in patients with type 2 diabetic peripheral neuropathy. Methods 76 cases diagnosed with type 2 diabetic peripheral neuropathy,in accordance with the random number table, were divided into the control group (37cases)and treatment group (39cases).All the patients received diabetic diet, exercise guidance and blood glucose control. The control group was treated with methy-cobal 500μg muscle injection once a day for 2 weeks. The treatment group was treated besides above treatment with intravenous drip alpha lipoic acid 600mg once a day was added for 2 weeks. Then, the changes of motor nerve conduc-tion velocity, sensory nerve conduction velocity and quality of life (QoL)score (somatic symptoms, cognitive func-tion,health happy feeling, social participation, emotional state, work performance, life satisfaction and total score)of the two groups after treatment were observed. Results After treatment, the motor nerve conduction velocity of the control group were as follows:median nerve (40.7 ±4.5)cm/s,common peroneal nerves (41.3 ±4.9)cm/s,The sensory nerve conduction velocity of the control group were as follows:median nerve (38.6 ±4.3)cm/s,common per-oneal nerves (38.3 ±4.5)cm/s.After treatment, the motor nerve conduction velocity of the treatment group were as follows:Median nerve (45.4 ±5.7)cm/s,common peroneal nerves (44.9 ±6.4)cm/s,The sensory nerve conduction velocity of the treatment group were as follows:Median nerve (45.0 ±2.0)cm/s,common peroneal nerves (43.6 ± 3.2)cm/s.Both the two groups′motor nerve conduction velocity and sensory nerve conduction velocity were significantly increased after treatment (P <0.05).Compared with control group, the motor nerve conduction velocity and sensory nerve conduction velocity in the treatment group were significantly improved after treatment, which had statistically significance,(t =2.63,2.51,2.85,2.79,all P <0.05).After treatment, the somatic symptoms, cognitive function, health happy feeling, job performance, social participation, emotional state, life satisfaction and the total score of control group were (52.4 ±9.6)points,(27.0 ±7.8)points,(35.7 ±10.3)points,(19.6 ±7.3)points,(17.4 ± 3.1)points,(16.5 ±3.9)points,(185.4 ±40.7)points,respectively.After treatment, the somatic symptoms,cogni-tive function, health happy feeling, job performance, social participation, emotional state, life satisfaction and total score of treatment group were (41.9 ±7.4)points,(24.1 ±8.6)points,(28.3 ±9.2)points,(14.5 ±5.5)points, (12.6 ±5.6)points,(11.9 ±4.7)points,(135.0 ±38.7)points,respectively.The quality of life score of the treat-ment group was obviously lower than the control group, which had statistically significance,(t =5.14,2.54,2.96, 2.87,2.69,3.05,6.25,all P <0.05).Conclusion Alpha lipoic acid can improve the nerve conduction function of patients with type 2 diabetes peripheral neuropathy, and improve the quality of life.

ABSTRACT

Objective To investigate the clinical characteristics, treatment effect,long-term follow up results, guanosine triphosphate (GTP) cyrclohydrolase Ⅰ (GCH Ⅰ)gene and tyrosine hydroxylase(TH) gene mutations in patients with dopa-responsive dystonia (DRD).Methods The clinical features of 3 families with 4 affected members were analyzed and all of 4 patients were screened for mutations of the GCH Ⅰ gene and TH gene with DNA sequences. Results Four patients were females, average age at onset was (15.3 ± 5.6) years (range:from 9 to 20 years). The initial symptoms were a gait disorder, stiffness or tremor of the lower limbs in all patients presented with diurnal fluctuation. As the increase of disease duration, bilateral hand tremor was found in three patients, systemic torsion was found in one patient and torticollis was found in one patient. All patients' symptoms were in complete remission after administration of low dose of levodopa.Four patients were followed up for 0.5 to 10.0 years, and all were still responsive to the levodopa treatment and effective dosage was decreased as the increase of the disease duration. No longterm side effects of levodopa had occurred after long-term treatment. One patient was found to have c.607G >A(p,Gly203Arg) heterogenetic mutation in GCH I gene.Molecular analysis revealed a compound heterozygous mutation in the TH gene (p.Y447Ter and p.V468M) in one patient. No point mutations in both genes were found in other patients. Conclusions DRD patients have dramatic and sustained response to levodopa and no long-term side effects of levodopa after long-term treatment. The detection of GCH Ⅰ and TH gene mutations is helpful in early diagnosis but the negative results could not exclude the diagnosis of DRD.
