ABSTRACT

Objective To analyze the correlation between K-ras gene mutation and clinicopathological characteristics and prognosis of colorectal cancer patients with different primary sites. Methods The clinical and pathological records of 69 patients who were pathologically confirmed as colorectal cancer and tested K-ras gene status at Wuxi People's Hospital Affiliated to Nanjing Medical University between May 2007 and August 2017 were adopted. The correlation between clinicopathological characteristics and prognosis of colorectal cancer patients with different primary sites and K-ras gene mutation status were retrospectively analyzed. And the patients were visited to adopt the prognosis data and perform the Kaplan-Meier survival analysis. Results The K-ras mutation rate was 50.7%(35/69),including 40.0%(12/30)in left-side colon cancer,73.3%(11/15)in right-side colon cancer and 50.0%(12/24)in rectal cancer. The mutation rate of K-ras gene in patients ≥ 60 years old [61.5%(24/39)vs.36.7%(11/30),χ2= 4.197,P = 0.041] or serum CA19-9 raising up abnormally was conspicuously high [65.6%(19/29)vs.37.5%(15/40),χ 2= 5.486,P =0.019].Other clinicopathological characteristics, such as gender, lesion location, histological classification,TNM stage, serum CEA expression, clinical features had no correlation with K-ras gene mutation status(all P> 0.05).And no relationship was found between prognosis and overall survival of colorectal cancer patients with different primary sites and K-ras gene mutation status(χ 2= 0.001,P = 0.997; χ 2= 0.583,P =0.445).In general, the 5-year survival rate of left-side colon cancer patients was highest(76.9%),followed by rectal cancer(69.7%),and right-side colon cancer was lowest(31.3%).The primary site of colorectal cancer was related to the overall survival of patients(χ2=11.004,P =0.004).Conclusions K-ras gene mutation in colorectal cancer is closely related to age and serum CA19-9 levels of the patients. The prognosis of left-side colon cancer is best, rectal cancer second, and right-side colon cancer poorest.Whether K-ras gene mutation is the prognostic factor of colorectal cancer is not clear.Testing the K-ras gene status and serum tumor index expression, distinguish the primary site and age group will provide the theory basis and promote the clinical targeted therapy and improve the survival of colorectal cancer patients.

ABSTRACT

Objective:To study the preparation of total leaves saponins of panax notoginseng liposomal gels and evaluate the quali-ty. Methods:The total leaves saponins of panax notoginseng liposomes were prepared by the film-dispersion method, and the formula was optimized by the orthogonal experimental design. A high-speed centrifugation method was applied to determine the encapsulation efficiency used as the evaluation index for the optimization of the preparation process. Then the gels were prepared and an HPLC meth-od was used to determine the content of ginsenoside Rb3 . The stability was preliminarily studied as well. Results:The preparation was semi-solid, and viscous gels with low skin irritation and good stability. The quality was accordance with the relevant provisions in Chi-nese Pharmacopoeia (2010 edition). Conclusion: The gels are with the properties of simple preparation process, reliable detection methods and stable and controllable quality.

ABSTRACT

Cystic liver metastasis, which lacks specific clinical manifestations and standardized treatment, rarely occurs after radical gastrectomy. This study presented two gastric cancer cases with cystic liver metastasis treated in Wuxi People's Hospital from September 2008 to November 2012. The two female patients were 66 and 65 years old, respectively. Poorly differentiated ulcerated adenocarcinoma classified as stage III B [Tumor, Node, and Metastasis Classification (T4aN2M0)] and clinical manifestations of abdominal distension and ascites were detected in both patients. Cyst fluids were subjected to cytological analysis. Results confirmed the presence of liver metastasis of gastric cancer. The two patients benefited from local intracavitary chemotherapy or systemic intravenous chemotherapy.

ABSTRACT

Objective: To apply the development of ascites ultrafiltration treatment instrument and ascites ultrafiltration technology for clinical observation research. Methods:By intraperitoneal on peritoneal ascites reinfusion of various causes of refractory ascites were treated. Results: A total of 647 cases of patients with refractory ascites, with the average for 2.8±1.5 times, treatment time 2.4±1.2 hours/times; filtering out ascites in 8543±2427 ml/times, lost an average of 8.4±2.4 kg; abdominal circumference decreased from 108.7 before treatment to 87.4±15.3 cm; 24 hour urine volume increased significantly, from 257.8±235.6 ml to 725.8±436.9 ml (P<0.01). The incidence of adverse reaction was 3.3%(63/1892 cases). Conclusion:Abdominal peritoneal ascites reinfusion method is simple, wide indications, adverse reactions, is a kind of method for the treatment of intractable ascites effectively.

ABSTRACT

Objective To explore the safety and effectiveness of transurethral resection of the prostate (TURP) for large benign prostatic hyperplasia. Methods A total of 56 cases of patients with large benign prostatic hyperplasia were analyzed retrospectively. Results The weight of prostate tissue resected with TURP was 60.2 g (48-122 g).The resection rate was 60%,operation time 113.4 min (70-180 min). The lower urinary tract symptoms (LUTS) after TURP was improved significantly. The international prostate symptom scores (IPSS) of 38 cases were decreased from (25.6±1.5) before operation to (6.0± 2.0) after operation. The maximum urine flow rate were increased from (5.0±1.9) ml/s to (18.0±4.5) ml/s.The mean residual urine volume of 22 cases were reduced from 95 ml (40-250 ml) to 10 ml (0-60 ml). Conclusions Skilled TURP technology for large benign prostatic hyperplasia is a safe treatment with good effect and low complications rates.

ABSTRACT

Efficacy of chemotherapy for advanced gastric cancer is unsatisfactory. Molecular targeted therapies have emerged as a novel approach to the treatment of advanced gastric cancer in recent years. These therapeutic strategies include targeting EGFR signal transduction pathway,anti-angiogenesis therapy,multi-target molecular targeted drugs. Targeted drugs such as trastuzumab, cetuximab, bevacizumab, apatinib, sorafenib sunitinib, lapatinib and everolimus which are targeting HER2,EGFR,VEGF and mTOR pathway, are applied in the comprehensive treatment of advanced gastric cancer, and their therapeutic effects are encouraging.

ABSTRACT

BACKGROUNDTo study the significance of the expression of HCV core protein in PBMC of patients with chronic hepatitis C and to evaluate the relationship between HCV core protein expression and clinical states.

METHODSIdentification of HCV protein antigen (Ag) in PBMC of 66 hepatitis C patients by immunohistochemical method and clinical status of the patients with HCV protein positive expression were investigated. In 27 out off 66 patients the HCV RNA and HCV Ag in PBMC were detected with RT-PCR and immunohistochemical method.

RESULTSThe HCV Ag (core+NS3) was identified in PBMC in 51 out of 66 patients (77.27%). It was also demonstrated that HCV core protein in nucleus showed strong expression and NS3 protein in cytoplasm showed weak expression. The expression of core protein in nucleus of PBMC were 35.29% in advanced chronic hepatitis patients, which was significantly higher than that from moderate cases (5.88%).

CONCLUSIONSThis study suggested that the expression of PMBC-HCV core protein may be related to the clinical state of the patients. The nuclear expression of HCV core protein in PBMC of patients with hepatitis C may be related to the persistence and activity of the chronic hepatitis C virus and play an important role in the pathogenesis of cirrhrosis and hepatocellular carcinoma.
