ABSTRACT Background: The inflammatory response plays a significant role in the outcome of coronavirus disease (COVID-19). Methods: We investigated plasma cytokine and chemokine concentrations in non-infected (NI), asymptomatic severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2)-infected blood donors (AS), and patients with severe COVID-19 (SC). Results: The SC group showed significantly higher levels of interleukin 6 (IL-6), IL-10, and CCL5 than the AS and NI groups. The SC and AS groups had considerably greater CXCL9 and CXCL10 concentrations than the NI group. Only NI and infected people showed separate clusters in the principal component analysis. Conclusions: SC, as well as AS was characterized by an inflammatory profile.

Resumen Introducción y objetivos: Los programas de rehabilitación cardiaca (RHC) son considerados como los más eficaces entre las intervenciones de prevención secundaria. El cual su función es mejorar la sobrevida como así también, la calidad de vida de estos enfermos. El objetivo fundamental de este trabajo es analizar el impacto de un Programa de Rehabilitación Cardiaca en pacientes portadores de cardiopatía isquémica con respecto a parámetros bioquímicos, antropométricos y funcionales. Pacientes y métodos: Fue un estudio observacional retrospectivo, unicéntrico, con seguimiento de 3 años. Se incluyo un total de 228 pacientes. Con criterios de inclusión: >18 años, con EAC y al menos una comorbilidad de riesgo cardiovascular que completaran el PRC. Se excluyó enfermedad valvular sin cardiopatía isquémica preexistente, infarto agudo al miocardio reciente, ángor inestable, obstrucción del tracto de salida del ventrículo izquierdo, y los que no cumplieron el PRC. Resultados: De los 228 pacientes que se incluyeron el PRC del HSVP el 70,7% eran hombres y el 29,3% eran mujeres con diagnóstico de cardiopatía isquémica. El promedio de edad era de 60.2 +- 11.4 años en total entre hombres y mujeres. El peso de los pacientes masculinos previo al ingreso del PRC fue de 77,7 kg +-13,2 kg, y las mujeres 69,7 kg +- 13,1 kg. Al final del fue de 75,5 kg +-13,1 kg y el de las mujeres era de 68,3 +- 13,1 kg. En la caminata de 6 minutos el promedio de mujeres al inicio fue de 390,0 mts y el de los hombres de 386,6, y la segunda vez posterior a la rehabilitación en hombres fue de 595,8 +- 107,2 y el de las mujeres fue de 549+-102,4. Los niveles de PCR de los hombres al inicio del programa fue de 1,2 +-2,4 mg/dL y el de las mujeres fue de 1,5 +- 1,9 mg/dL al finalizar los hombres tuvieron un promedio de 1,8+-3,0 Conclusiones: Los PRC dependen de la participación de profesionales de la salud que trabajen en equipo para alcanzar resultados finales, los cuales están basados no solo en el ejercicio sino también en el cambio de estilo de vida del paciente, por lo tanto, necesita de servicios asociados como fisioterapia, nutrición, psicología.


Abstract Effect of the Cardiac Rehabilitation Program of the Hospital San Vicente Paúl on biochemical, anthropometric and functional parameters in patients with ischemic heart disease from January 1, 2014 to December 31, 2015 Introduction and objectives: Cardiac Rehabilitation Programs, are considered the most effective programs among secondary prevention interventions. The function is to improve survival as well as the quality of life of these patients. The main objective of this work is to analyze the impact of Cardiac Rehabilitation Program in patients with ischemic heart disease with the respect biochemical, anthropometric and functional parameters. Patients and method: A observational, retrospective single-center, study with a 3-year-follow up. A total of 228 patients were included, witch 70.7% were men with an average of 60.2+-11.4 years. The inclusion criteria were: > 18 years with CAD and at least one cardiovascular risk comorbidity and completed the Cardiac Rehabilitation Program. Valvular disease without pre-existing ischemic heart disease, recent acute myocardial infarction, unstable angina, left ventricular outflow tract obstruction, and those who did not went to the Program. Results: The 228 patients who were included in the HSVP CRP, 70.7% were men and 29.3% were women with a diagnosis of ischemic heart disease. The average age was 60.2 + - 11.4 years in total between men and women. The weight of male patients prior to admission to the CRP was 77.7 kg + -13.2 kg, and women 69.7 kg + -13.1 kg. At the end of the program, the weight of the men was 75.5 kg + -13.1 kg and that of the women was 68.3 + - 13.1 kg. The total waist circumference at the start of the program was 100.1 ± 11.4 cm. In women the average was 98.4 + - 12.7cm, that of men was 101.1 + - 10.8 cm. At the end of the program, the total average of men and women was 96.7 + - 11.0, the average of women at the end of the program was 96.2 + - 12.6 cm and of men was 96. 9 + - 10. In the 6-minute walk, the average of women at the beginning was 390.0 meters and that of men was 386.6, and the second time after rehabilitation in men was 595.8 + - 107.2 and the of women it was 549 + -102.4. In men, the previous total cholesterol was 154.8 + -39.7 mg / dL and that of women was 162.0 + -40.2 mg / dL and at the end of the program the value of men was 161 .6 + -46.0 mg / dL and 170.8 + -41.8 mg / dL for women. The CRP levels of the men at the beginning of the program was 1.2 + -2.4 mg / dL and that of the women was 1.5 + - 1.9 mg / dL at the end of the program, the men had an average of 1.8 + -3.0. Conclusion: The Cardiac Rehabilitation Program depends on the partipation of health professionals care who work as a team to achieve final results, witch are based not only on exercise but also on the change in the patient's lifestyle, therefore, they need associated services such as physiotherapy, nutrition, psychology.

ABSTRACT The aging of the population leads to an increase in the prevalence of dementia and mild cognitive impairment (MCI). Alzheimer's disease (AD) is the most common cause of dementia. Recent studies highlight the early non-amnestic deficits in AD and MCI. The European Union report shows the importance of thoroughly assessing cognitive aspects that have been poorly evaluated, such as processing speed (PS), which could represent early indicators of cognitive decline. Objective: To analyze the diagnostic accuracy of PS measures in older adults with MCI, AD, and those who are cognitively-healthy. Methods: A cross-sectional study was conducted by performing an extensive neuropsychological assessment in three samples: 26 control participants, 22 individuals with MCI, and 21 individuals with AD. Analysis of variance (ANOVA) was employed to test the relationship between dependent variables and the clinical group. Post hoc tests (Bonferroni test) were used when a significant ANOVA result was found. Finally, the Receiver Operating Characteristic (ROC) curve for PS measures was performed in older adults with MCI and AD compared with cognitively-healthy older adults. Results: The results showed that deficits in PS measures can be early indicators of cognitive decline in cases of MCI, even when executive functions (EFs) and functionality are preserved. Conversely, AD versus MCI presented differences in PS, EFs, and functionality. Conclusions: The ROC analyses showed that PS measures had discriminative capacities to differentiate individuals with MCI, AD, and cognitively-healthy older adults.


RESUMO O envelhecimento da população leva ao aumento da prevalência de demência e comprometimento cognitivo leve (CCL). A doença de Alzheimer (DA) é a causa mais comum de demência. Estudos recentes destacam os déficits precoces não amnésicos em DA e CCL. O relatório da União Europeia mostra a necessidade de avaliar em maior profundidade aspectos cognitivos que atualmente são negligenciados, como a velocidade de processamento (VP), e que podem representar indicadores precoces de declínio cognitivo. Objetivo: O objetivo deste estudo foi analisar a acurácia diagnóstica de medidas de VP em idosos com CCL, DA e participantes controles. Métodos: um estudo transversal foi desenvolvido, no qual realizou-se uma extensa avaliação neuropsicológica em 3 amostras: 26 participantes controles, 22 casos de CCL e 21 DA. A relação entre as variáveis dependentes e o grupo clínico foi testada com uma análise de variância (ANOVA). Se uma ANOVA significativa fosse encontrada, testes post hoc foram utilizados. Por fim, a curva ROC para medidas de VP foi realizada em CCL e DA em comparação com indivíduos controles. Resultados: os resultados mostraram que déficits nas medidas de VP podem ser indicadores precoces do declínio cognitivo nos casos de CCL, mesmo quando as funções executivas (FE) e a funcionalidade estão preservadas. Por outro lado, DA versus CCL mostrou diferenças em VP, EF e funcionalidade. Conclusões: As análises ROC mostraram que as medidas de PS tinham capacidades discriminativas para diferenciar CCL, DA e participantes controles.

ABSTRACT

Resumen El virus SARS-CoV-2 se ha extendido a nivel mundial, rápidamente ha sobrecargado los sistemas de salud. Esta emergencia ha implicado cambios en la atención usual del infarto agudo miocardio con elevación del ST (IAMCEST) puesto que la actividad habitual de las salas de hemodinamia y las vías de traslado de los pacientes se ha visto afectada. La afectación del personal de salud también es una preocupación relevante por lo que presentamos un documento de Consenso de la Asociación Costarricense de Cardiología que pretende generar una guía de trabajo al personal que atiende esta patología y garantizar la atención adecuada del IAMCEST durante la pandemia en Costa Rica.


Abstract The current COVID-19 has spread worldwide, the outbreak is altering the usual activity of the catheterization laboratorios and the usual treatment pathways of patients with chronic diseases or emergencies, such as Acute Coronary Syndrome could be disrupted. The involvement of health personnel is a relevant concern, so we created a consensus document of the Costa Rican Association of Cardiology that aims to generate a decision-making workflow to treat this pathology and guarantee adequate and continuous care for ST elevation myocardial infarction during the COVID-19 outbreak.

Abstract Bartonella is a genus of emerging zoonotic bacteria that are mainly associated with mammalian erythrocytes and endothelial cells. Bats are natural reservoirs for a variety of important pathogens that impact human and animal health. Recent reports have highlighted the role of bats and bat flies in the maintenance of Bartonella. Here, we showed that none of the 29 bat DNA blood samples obtained from five bat species in São Luís Island, state of Maranhão, northeastern Brazil, were positive for Bartonella in qPCR assays targeting nuoG. On the other hand, three out of 15 DNA samples (20%) from flies in the family Streblidae were positive for Bartonella. The BLASTn results showed that the gltA and rpoB sequences shared identities ranging from 97.2% to 100%, with Bartonella sequences amplified from bats or bat flies from Costa Rica and Brazil. These findings were supported by phylogenetic analyses based on Bayesian inferences. The present study showed that Bartonella genotypes are present in bat flies, thus shedding some light on the distribution of bat fly-related Bartonella genotypes in South America.


Resumo Bartonella é um gênero de bactérias zoonóticas emergentes associadas principalmente a eritrócitos e células endoteliais de mamíferos. Morcegos são reservatórios naturais para uma variedade de patógenos importantes que afetam a saúde humana e animal. Além disso, estudos recentes destacaram o papel dos morcegos e de moscas associadas a morcegos na manutenção de Bartonella. No presente estudo, nenhuma das 29 amostras de DNA obtidas a partir do sangue de cinco espécies de morcegos amostrados na ilha de São Luís, estado do Maranhão, Nordeste do Brasil, foi positiva para Bartonella nos ensaios de qPCR direcionados ao gene nuoG. Por outro lado, três das 15 (20%) amostras de DNA de moscas da família Streblidae foram positivas para Bartonella. Os resultados do BLASTn mostraram que as sequências dos genes gltA e rpoB compartilharam identidade, variando de 97,2% a 100%, com as sequências de Bartonella amplificadas em morcegos ou moscas amostrados na Costa Rica ou Brasil. Tais resultados corroboraram as análises filogenéticas realizadas por Inferência Bayesiana. O presente estudo mostrou a ocorrência de Bartonella em moscas de morcegos, auxiliando a esclarecer a distribuição dos genótipos de Bartonella relacionadas a moscas Streblidae na América do Sul.

ABSTRACT

Abstract Cochliomyia hominivorax (Coquerel, 1858), the New World screwworm, causes primary myiasis in wild and domestic animals in tropical and subtropical regions of Brazil. Although this species is considered to occur throughout the country, organized information about its recorded distribution has not been available until now. This article aimed to provide a comprehensive review of the historical and current data published on both immature (myiasis) and adult stages of C. hominivorax in Brazil. A total of 174 articles were found; of these, 141 articles reported myiasis cases in cattle (146 records), humans (68 records), and other mammalian hosts (40 records), and captures of adult flies were reported in 33 articles. C. hominivorax is widespread in Brazil, having been recorded in 208 municipalities in all major biomes of the country.


Resumo Cochliomyia hominivorax (Coquerel, 1858), a mosca-da-bicheira, causa miíase primária em animais silvestres e domésticos em regiões tropicais e subtropicais do Brasil. Embora esta espécie seja considerada de ocorrência em todo o país, informações organizadas sobre sua distribuição com base em registros não estavam disponíveis até o momento. Este artigo teve como objetivo fornecer uma revisão abrangente dos dados históricos e atuais publicados sobre o registro de estágios imaturos (miíase) e adultos de C. hominivorax, no Brasil. De 174 artigos encontrados, 141 relataram casos de miíase em bovinos (146 relatos), humanos (68 relatos) e outros mamíferos hospedeiros (40 relatos) e capturas de adultos de C. hominivorax foram registradas em 33 artigos. C. hominivorax encontra-se amplamente distribuída no Brasil, tendo sido registrada em 208 municípios brasileiros em todos os principais biomas do país.

Resumen La enfermedad periodontal y la enfermedad cardiovascular son dos de las enfermedades más prevalentes en la población mundial. Son ambas enfermedades de alto impacto en la salud pública. La enfermedad periodontal se define como una inflamación crónica de los tejidos que dan soporte a los dientes causada por bacterias bacilos gram negativos y espiro quetas que se generan en biofilms alrededor de los dientes. La prevalencia de la periodontitis severa va desde el 1% en pacientes entre 20 y 29 años hasta de 39% en pacientes mayores de 65 años. La presencia de microorganismos genera una respuesta inflamatoria local y sistémica. La producción de reactantes de fase aguda y mediadores proinflamatorios aumenta el riesgo de inflamación en las placas ateroescleróticas haciéndolas propensas a ruptura. Existe además lo po sibilidad de la entrada de bacterias al torrente sanguíneo, dichas bacteremias se han relacionado con la posibilidad de endocarditis infecciosa. Estudios epidemiológicos han demostrado un aumento del riesgo de enfermedad cardiovascular en pacientes con enfermedad periodontal activa, independientemente de otros factores de riesgo que comparten ambas patologías. El riesgo relativo varía entre diferentes autores y trabajos realizados pero está cercano al 19% como lo demos tró Wei-Dong en su metanalisis. El efecto de la periodontitis se ha evidenciado en otros aspectos del espectro clínico de la enfermedad cardiovascular como lo son la alteración de la actividad insulínica, glicogénesis hepática y alteración de la microflora intestinal que lleva a procesos de inflamación sistémica y cambios metabólicos. Por estas razones es aconsejable realizar una revisión exhaustiva de la condición dental de los pacientes en riesgo de enfermedad cardiovascular. En aquellos con datos de enfermedad activa se aconseja la extracción y reparación de piezas dentales. El uso de clorhexidina es aconsejable como parte del tratamiento de la enfermedad así como la prevención de la misma en pacientes de riesgo o que vayan a ser sometidos a procedimientos quirúrgicos. En casos especiales se aconseja profilaxis antibiótica para prevenir la endocarditis infecciosa.


Abstract Relationship between Periodontal disease and cardiovascular disease. The need for a management protocol Periodontal disease and cardiovascular disease are two of the most prevalent diseases in the world population. Both diseases have high impact on public health. Periodontal disease is defined as a chronic inflammation of the tissues that support the teeth caused by gram-negative bacilli bacteria and spirochetes that are generated in biofilms around the teeth. The prevalence of severe periodontitis ranges from 1% in patients between 20 and 29 years old to 39% in patients older than 65 years. The presence of microorganisms generates a local and systemic inflammatory response. The production of acute phase reactants and proinflammatory mediators increases the risk of inflammation in atherosclerotic plaques making them prone to rupture. There is also the possibility of entrance of bacteria into the bloodstream and bacteremias have been linked to the possibility of infective endocarditis. Epidemiological studies have shown an increased risk of cardiovascular disease in patients with active periodontal disease, independently of other risk factors that share both pathologies. The relative risk varies between different authors and works performed but is close to 19% as demonstrated by Wei-Dong in his meta-analysis. The effect of periodontitis has been evidenced in other aspects of the clinical spectrum of cardiovascular disease such as the alteration of insulin activity, hepatic glycogenesis and alteration of the intestinal microflora that leads to processes of systemic inflammation and metabolic changes. For these reasons it is advisable to carry out a comprehensive review of the dental condition of patients at risk for cardiovascular disease. In those with active disease data suggest it is appropriate to remove and repair dental pieces. The use of chlorhexidine is advisable as part of the treatment of the periodontal disease as well as the prevention in patients at risk or who will undergo surgical procedures. In special cases antibiotic prophylaxis is recommended to prevent infective endocarditis.

ABSTRACT Objective: to identify the defining characteristics of Ineffective airway clearance with better predictive power using classification trees. Method: the predictive power of the defining characteristics of Ineffective airway clearance was evaluated based on classification trees generated from the data of 249 children with acute respiratory infection. Results: Ineffective cough and adventitious breath sounds were identified as the main defining characteristics when screening for Ineffective airway clearance in accordance with trees based on three different computational algorithms. Conclusion: Ineffective coughing and adventitious breath sounds had better predictive capacity for Ineffective airway clearance in the sample.


RESUMO Objetivo: identificar as características definidoras de Desobstrução ineficaz de vias aéreas com melhor poder preditivo usando árvores de classificação Método: o poder preditivo das características definidoras da Desobstrução ineficaz de vias aéreas foi avaliado com base em árvores de classificação geradas a partir dos dados de 249 crianças com infecção respiratória aguda. Resultados: tosse ineficaz e ruídos adventícios respiratórios foram identificados como as principais características definidoras na detecção de Desobstrução ineficaz de vias aéreas de acordo com as árvores, com base em três diferentes algoritmos computacionais. Conclusão: Tosse ineficaz e ruídos adventícios respiratórios tiveram melhor capacidade preditiva para Desobstrução ineficaz de vias aéreas na amostra.


RESUMEN Objetivo: identificar las características definitorias de la Limpieza ineficaz de las vías aéreas con un mejor poder predictivo utilizando árboles de clasificación. Método: el poder predictivo de las características definitorias de Limpieza ineficaz de las vías aéreas se evaluó basado en los árboles de clasificación generados a partir de los datos de 249 niños con infección respiratoria aguda. Resultados: La tos ineficaz y los sonidos respiratorios anormales se identificaron como las principales características definitorias cuando se realizaba una Limpieza ineficaz de las vías aéreas de acuerdo con los árboles en función de tres algoritmos computacionales diferentes. Conclusión: La tos ineficaz y los sonidos respiratorios anormales tienen una mejor capacidad predictiva para la Limpieza ineficaz de las vías aéreas en la muestra.

ABSTRACT

OBJETIVO: Avaliar a coexistência da talassemia alfa (a-Tal) e sua interferência no curso clínico dos pacientes com Doença Falciforme no Hemocentro Regional de Montes Claros-MG. Metodologia: Estudo transversal analítico, com amostra aleatorizada, na qual foram incluídos pacientes triados pelo Programa Estadual de Triagem Neonatal de Minas Gerais e encaminhadas ao Hemocentro Regional de Montes Claros, com perfil eletroforético compatível com anemia falciforme, nascidos no período entre 26/01/2000 e 13/05/2014. Os dados clínicos dos pacientes foram coletados nos prontuários médicos do Ambulatório do Hemocentro Regional de Montes Claros. A genotipagem de a-Tal foi realizada por PCR multiplex (alelos: -a3.7; -a4.2; --SEA; --FIL; --MED; -(a) 20.5 e --THAI) no Serviço de Pesquisa Serviço de Pesquisa da Fundação Hemominas. Os dados foram analisados em teste estatísticos qui-quadrado em Software SPSS versão 16.0. Resultados: Foram estudados 50 pacientes, sendo 25 (50%) do sexo masculino e 25 (50%) do sexo feminino. A idade dos pacientes variou de 9 meses a 15 anos de idade. A prevalência da a-Tal foi de 30%. Não houve associação estatística significativa entre a presença de a-Tal e infecção, internação, crises álgicas, sequestro esplênico, esplenectomia, transfusão sanguínea e Acidente Vascular Cerebral (AVC). No entanto, a frequência de crises álgicas, esplenectomia e AVC foi menor nos pacientes que apresentavam coexistência da a-Tal. Conclusões: A prevalência de a-Tal em indivíduos com anemia falciforme no nosso estudo foi 30%. Algumas manifestações graves da AF ocorreram de forma menos frequente nos pacientes com a interação da a-Tal/anemia falciforme. (AU)


Objective: To evaluate the coexistence of alpha thalassemia (a-Tal) and its interference in the clinical course of patients with sickle cell disease at Hemocentro Regional de Montes Claros-MG. Methodology: This is a cross-sectional, analytical study with a randomized sample carried out with patients screened by the State Neonatal Screening Program of Minas Gerais and referred to the Hemocentro Regional de Montes Claros with an electrophoretic profile compatible with sickle cell anemia, born between 01.26.2000 and 05.13.2014. The clinical data of the patients were collected in the medical records of the Outpatient Clinic of the Hemocentro Regional de Montes Claros. The a-Tal genotyping was performed by multiplex PCR (alleles: -a3.7; -a4.2; --SEA; --FIL; --MED; - (a) 20.5 and --THAI) in the Research Service Fundação Hemominas. The data were analyzed in chi-square statistical test in SPSS Software version 16.0. Results: Fifty patients were studied, 25 (50%) were male, and 25 (50%) were female. Patients´ ages ranged from 9 months to 15 years old. The prevalence of a-Tal was 30%. There was no significant statistical association between the presence of a-Tal and infection, hospitalization, painful crises, splenic sequestration, splenectomy, blood transfusion and cerebrovascular accident (CVA). However, the frequency of painful seizures, splenectomy and CVA was lower in patients with a-Tal coexistence. Conclusions: The prevalence of a-Tal in individuals with sickle cell anemia in our study was 30%. Some severe manifestations of SCA occurred less frequently in patients with a-Tal/sickle cell anemia interaction. (AU)

ABSTRACT

Objetivou-se avaliar a carga parasitária da cama de frango de uma granja avícola localizada no município de Paço do Lumiar - MA. Foram coletadas 50 amostras de cama de frango de 10 galpões. Essa cama já havia sido utilizada seis vezes nos lotes anteriores. Para as análises parasitológicas foram utilizados os métodos de Sedimentação Espontânea, Método de Sheather, Esporulação para diagnóstico de oocistos de coccídios. Foram identificados ovos de nematódeos dos gêneros Strongyloides (72%), Heterakis (18%) e Ascaridia (10%). As larvas as do gênero Strongyloides destacaram-se como as mais prevalentes, com frequência de 97% de casos, seguido geohelmintos (3%). Encontrou-se 1200 oocistos de Eimeria, no estágio de esporulação. Foram encontrados também ácaros e cascudinhos (Alphitobius diaperinus). Conclui-se que a alta frequência de parasitos presentes na cama de frango pode estar relacionada à falha de manejo.


The objective of this study was to evaluate the parasitic load of chicken litter from a poultry farm located in the municipality of Paço do Lumiar - MA. Fifty samples of chicken bed of 10 sheds were collected. This bed had already been used six times in the previous batches. For the parasitological analyzes the methods of Spontaneous Sedimentation, Sheather Method, Sporulation for the diagnosis of coccidia oocysts were used. Eggs of nematodes of the genera Strongyloides (72%), Heterakis (18%) and Ascaridia (10%) were identified. The larvae of the genus Strongyloides were the most prevalent, with a frequency of 97% of cases, followed by geohelminths (3%). There were 1200 Eimeria oocysts, in sporulation stage. Mites and kites were also found (Alphitobius diaperinus). It is concluded that the high frequency of parasites present in chicken litter may be related to management failure.

ABSTRACT

O estudo teve como objetivo verificar as características definidoras com melhor poder de predição para auxiliar na classificação de Padrão respiratório ineficaz, utilizando árvores de classificação, em crianças com infecção respiratória aguda. Estudo transversal, realizado em dois hospitais pediátricos juntamente a 249 crianças com infecção respiratória aguda. Para a coleta, foi utilizado um instrumento específico desenvolvido para o estudo. Empregaram-se três algoritmos de indução para a geração das árvores, CHi-square Automatic Interaction Detection, Classification and Regression Treese Quick, Unbiased, Efficient Statistical Tree. Construíram-se três árvores para auxiliar na identificação de Padrão Respiratório ineficaz. As árvores de classificação geradas apresentam probabilidades condicionais à ocorrência do diagnóstico associada a dispneia e alterações na profundidade respiratória. Padrão respiratório ineficaz esteve presente 65,5% da amostra. Assim, a probabilidade da ocorrência do referido diagnóstico nas crianças com infecção respiratória aguda foi de 100% com a presença de dispneia e de alterações na profundidade respiratória.


The objective of the study was to verify defining characteristics with greater predictive power to aid in the classification of ineffective breathing pattern using classification trees in children with acute respiratory infections. A cross-sectional study was carried out in two pediatric hospitals with 249 children with acute respiratory infection. For data collection, a specific instrument developed for the study was used. Three induction algorithms were used to generate the trees: Chi-square Automatic Interaction Detection, Classification and Regression Trees, and Quick, Unbiased, Efficient Statistical Tree. Three trees were constructed to aid in the identification of ineffective breathing pattern. The classification trees generated present probabilities conditionalto the occurrence of the diagnosis associated with dyspnea and changes in respiratory depth. Ineffective breathing pattern was present in 65.5% of the sample. Thus, the probability of occurrence of this diagnosis in children with acute respiratory infection was 100% with the presence of dyspnea and changes in respiratory depth.

ABSTRACT

Resumen Objetivo: Determinar las características clínicas y el manejo de los pacientes con insuficiencia cardíaca (IC) en Costa Rica. Métodos: El RENAIC CR es un registro observacional y prospectivo en curso que actualmente incluye pacientes con IC atendidos en Costa Rica. Resultados: Se incluyeron 695 pacientes (68,3% ≥63,5 años de edad; 57,7% de sexo masculino). La causa más frecuente de IC era cardiopatía isquémica (58,0%). La mayoría de los pacientes tenía clase funcional II (33,1%) o III (35,2%) de acuer do con la NYHA. En todos los pacientes con IC se realizó un ecocardiograma, aunque en la mitad de estos pacientes no se determinaron los niveles de péptidos natriuréticos. Muchos pacientes no recibían tratamiento para la IC basado en la evidencia. Conclusión: Este registro puede ser valioso para desarrollar estrategias que mejoren el manejo de los pacientes con IC en Costa Rica y en países similares.


Abstract National registry of heart failure in Costa Rica. The RENAIC CR study Objective: To determine the clinical features and management of patients with heart failure (HF) in Costa Rica. Methods: The RENAIC CR is an ongoing, observational and prospective registry that is currently including HF patients attended at Costa Rica. Results: 695 patients (68.3% ≥63.5 years; 57.7% male) were included. The most common cause of HF was ischemic heart disease (58.0%). Most patients were on NYHA functional class II (33.1%) or III (35.2%). In all HF patients an echocardiogram was performed, but in half of these patients natriuretic peptides were not determined. Many patients were not taking evidence-based HF therapies. Conclusion: This registry may be helpful for developing strategies to improve the management of patients with HF in Costa Rica and similar countries.

ABSTRACT

Este trabalho objetivou realizar um estudo retrospectivo sobre os aspectos epidemiológicos da Leishmaniose Visceral Canina (LVC) e Humana (LVH) no estado do Maranhão no período de 2009-2012. A amostra constituiu-se dos dados de fichas de registros provenientes de animais do Centro de Controle de Zoonose (CCZ) de São Luís-MA e dos registros notificados na Secretaria de Estado da Saúde (SES) no período de 2009 a 2012. Foram registrados um total de 2.259 atendimentos a caninos com suspeita de Leishmaniose no CCZ, a maior frequência de registros foi de machos 1.198 (53,03%). Desses atendimentos, 564 (24,96%) foram soropositivos para Leishmania e a maior frequência ocorreu no período seco 326 (57,80%). Dos casos de LVH, foram notificados 2.755, destes 1.577 (57,24%) foram confirmados e 59 (3,74%) vieram ao óbito. Dos casos notificados para a LVH no estado do Maranhão, a maioria era do sexo masculino 1.656 (60,12%). A faixa etária onde houve maior ocorrência de casos notificados estava compreendida entre 1-4 anos de idade com 282 (37,95%) em 2011. Verificou-se que a LVH estava presente em 129 municípios do estado do Maranhão, sendo que os de maior frequência foram Imperatriz com 126 (4,57%). Dessa forma, pode-se concluir que ocorreu elevado número de casos de Leishmaniose Visceral Canina e Humana no estado do Maranhão no período pesquisado.

ResumenEl cor triatriatum sinester es una anormalidad cardiaca congénita poco frecuente, que constituye del 0,1 al 0,4% de todas las malformaciones cardiacas. Se caracteriza por una división anómala de la aurícula izquierda por una membrana fibromuscular, la cual genera dos cavidades que se comunican por uno o más orificios con distinto grado de obstrucción.Se asocia con otras anomalías cardiacas, principalmente con comunicación interauricular. Se puede presentar en la infancia, cuando suele ser fatal, o más tardíamente en el adulto, como insuficiencia cardiaca, o incluso cursar de forma asintomática, dependiendo del tamaño de los orificios y del gradiente de presión entre las cavidades. En este artículo se presenta el caso de un masculino de 30 años, sin patologías crónicas, quien consulta por disnea de grandes esfuerzos en los últimos 6 meses, a quien se le diagnostica por medio del ecocardiograma transtorácico y transesofágico un cor triatriatum sinister.


AbstractCor Triatriatum Sinister is a rare congenital heart defect, representing 0.1 to 0.4% of all cardiac malformations. This condition is characterized for an abnormal division of the left atrium by a fibromuscular membrane that generates two cavities, which are connected by one or more orifices with different degrees of obstruction. This condition is associated with other cardiac abnormalities mainly related to atrial septal defects. Its presentation in pediatric patients is often lethal, in adults it may appear as heart failure symptoms or as an asymptomatic finding in echocardiography depending on the pressure gradient between cavities. We present a case report of a 30 years old male without chronic conditions, who complained of dyspnea with strenuous physical activity occurring in the last 6 months, who was diagnosed with a cor triatriatum sinister by the use of transthoracic and transesophageal echocardiogram.

In order to know the situation of ovine tuberculosis in Maranhão, Brazil, we evaluated 343 sheep in the southwest region of the state, of both sexes (247 females and 96 males), aged over six months, through the comparative cervical test (TCC), with tuberculin inoculation in the right cervical region. Cranially, avian purified protein derivate (PPD) was injected; and caudally, beef PPD was injected, in individual doses of 0.1 mL. All animals were negative for tuberculosis; however, 65 sheep had higher reactions to bovine tuberculin than to the avian one. To investigate the possibility of low awareness or anergy in animals with advanced tuberculosis, one cachectic sheep was necropsied, aged four years, with negative results for TCC, but with reaction to avian tuberculin and enlarged hardened lymph nodes. The results indicated C. pseudotuberculosis, Actinomyces pyogenes, Staphylococcus aureus and Mycobacterium sp infections. Culture and isolation resulted in Pseudomonas aeruginosa and Bacillus sp. No bacterial structures compatible with acid-fast bacteria were found. The results confirmed the absence of animals reactive to M. bovis, assuming no tuberculosis in sheep in the study area.

RESUMO Objetivo: analisar as características definidoras do diagnóstico de enfermagem "desobstrução ineficaz de vias aéreas" em crianças com infecção respiratória aguda. Método: estudo transversal, descritivo, desenvolvido em dois hospitais especializados no atendimento a crianças. Realizou-se avaliação respiratória detalhada da criança para a identificação das características definidoras do diagnóstico em estudo. Resultados: foram avaliadas 249 crianças, sendo 55,8% do sexo masculino e com mediana de idade de 13,76 meses. Desobstrução ineficaz de vias aéreas foi identificado em 222 crianças (89,2%). As seguintes características definidoras apresentaram associação estatisticamente significante: dispneia, expectoração, ortopneia, ruídos adventícios respiratórios, sons respiratórios diminuídos e tosse ineficaz. Sons respiratórios diminuídos, tosse ineficaz e ruídos adventícios respiratórios compuseram o modelo de regressão logística. Conclusão: as características sons respiratórios diminuídos, tosse ineficaz e ruídos adventícios respiratórios apresentam melhor capacidade de predição para o diagnóstico "desobstrução ineficaz de vias aéreas" em crianças com infecção respiratória aguda.

Método: estudio transversal, descriptivo, desarrollado en dos hospitales especializados en la atención al nino. Se realizó evaluación respiratória detallada del nino para la identificación de las características definidoras del diagnóstico en estudio. Resultados: fueron evaluados 249 ninos, siendo 55,8% del sexo masculino y con mediana de edad de 13,76 meses. Desobstrucción ineficaz de vías aéreas fue identificada en 222 ninos (89,2%). Las siguientes características definidoras presentaron asociación estadísticamente significante: disnea, expectoración, ortopnea, ruidos adventicios respiratorios, sonidos respiratorios disminuidos y tos ineficaz. Sonidos respiratorios disminuidos, tos ineficaz y ruidos adventicios respiratorios compusieron el modelo de regresión logística. Conclusión: las características sonidos respiratorios disminuidos, tos ineficaz y ruidos adventicios respiratorios presentan mejor capacidad de predicción para el diagnóstico "desobstrucción ineficaz de vías aéreas" en ninos con infección respiratoria aguda.


ABSTRACT Objective: to analyze the defining characteristics of the nursing diagnosis "ineffective airway clearance" in children with acute respiratory infection. Method: cross-sectional descriptive study, developed in two hospitals specialized in care for children. It was held a detailed respiratory evaluation of the child to identify the defining characteristics of the diagnosis under study. Results: a total of 249 children were evaluated, 55.8% were male and the median age was 13.76 months. Ineffective airway clearance was identified in 222 children (89.2%). The following defining characteristics presented statistically significant associations: dyspnea, expectoration, orthopnea, respiratory adventitious sounds, decreased breath sounds and ineffective cough. Decreased breath sounds, ineffective cough and respiratory adventitious sounds composed the logistic regression model. Conclusion: the characteristics decreased breath sounds, ineffective cough and respiratory adventitious sounds have better predictive capacity for the diagnosis "ineffective airway clearance" in children with acute respiratory infection.
