ABSTRACT

At present, most clinical thrombolytic drugs are plasminogen activators, which are highly dependent on the plasminogen level of the patient. Therefore, the efficacy of those drugs is restricted. Unlike the conventional thrombolytic plasminogen activator drugs, fibrinolytic drugs have direct fibrinolytic activity. Thus, fibrinolytic drugs can directly dissolve the thrombus, and its thromlysis efficacy is not restricted by the patients' plasminogen. This is a new type of thrombolytic drug with higher thrombolytic efficiency and safety, and has become one of the research hotspots at present. Although more and more agents that can be used as fibrinolytic drugs have been discovered, only a few of them can successfully be applied in clinical practice. The mainly underlying reason is the risk of bleeding. In this paper, based on the latest research progress of fibrinolytic drugs, the bleeding mechanisms and coping strategies of fibrinolytic drugs were systematically reviewed, five types of bleeding mechanisms of fibrinolytic drugs were summarized, and three types of coping strategies were proposed. We hope our work can provide theoretical basis for the development of safer and more efficient fibrinolytic drugs.

ABSTRACT

AIM: To observe changes in fundus microcirculation of myopic adolescents after wearing orthokeratology by applying optical coherence tomography angiography(OCTA).METHODS: Prospective study. A total of 40 cases(40 eyes)of adolescents with low to moderate myopia who chose orthokeratology to correct visual acuity at our hospital from April 2021 to June 2022 were collected. The uncorrected distant visual acuity and axial length were evaluated at 1, 3 and 6mo before and after wearing orthokeratology, respectively. Furthermore, the changes in superficial vessel density(SVD), deep vessel density(DVD), central retinal thickness(CRT), foveal avascular zone area(FAZ-A), foveal avascular zone perimeter(FAZ-P), retinal nerve fiber layer(RNFL)thickness and radial peripapillary capillaries density(RPCD)were observed by applying OCTA.RESULTS: The uncorrected distant visual acuity was significantly improved at 1, 3 and 6mo after wearing orthokeratology(P<0.001). There was no statistically significant difference in axial length before and after wearing orthokeratology(P>0.05). Moreover, there were significant differences in both SVD of fovea quadrant and DVD of fovea and lower quadrant(P<0.01), but there were no differences in CRT, FAZ-A and FAZ-P, RNFL thickness and RPCD(P>0.05).CONCLUSION: Wearing orthokeratology can significantly improve visual acuity and increase local retinal vessel density in the macula in adolescents with low to moderate myopia.

ABSTRACT

Aortoiliac occlusive disease (AIOD) refers to the stenosis and occlusion of the distal abdominal aorta and(or) bifurcation of the aortoiliac artery,which is mainly caused by atherosclerosis,leading to pelvic and lower limb ischemia.Open surgery has always been the main treatment for complex AIOD.However,in recent years,with the development of endovascular surgery technologies and medical instruments,its treatment concept has been greatly changed.More and more clinical evidence has proved that the long-term efficacy of endovascular therapy is not inferior to that of traditional open surgery,so minimally invasive endovascular therapy has become the preferred treatment for AIOD.

ABSTRACT

Objective:Based on the hyperlipidemia rat model and network pharmacology technology, the mechanism of action of Trichosanthis Fructus-Allii Macrostemonis Bulbus herb pairs against hyperlipidemia was analyzed. Method:The levels of blood lipids and inflammatory factors were measured through prophylactic administration of low, medium and high-dose Trichosanthis Fructus-Allii Macrostemonis Bulbus herb pairs in hyperlipidemia rats. The active ingredients of Trichosanthis Fructus-Allii Macrostemonis Bulbus herb pairs were screened out through Traditional Chinese Medicine System Pharmacology Database and Analysis Platform (TCMSP) and text mining. The targets of active ingredients screened through the Swiss Target Prediction, Similarity ensemble approach (SEA), DrugBank database. The disease targets were collected through Therapeutic Target Database (TTD), Online Mendelian Inheritance in Man (OMIM), DrugBank, DisGeNET database. The targets of active ingredients and disease target were integrated, and screened through topological parameters to gain the main candidate targets of Trichosanthis Fructus-Allii Macrostemonis Bulbus herb pairs against hyperlipidemia. The Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis and the gene ontology (GO) functional enrichment analysis were conducted through ClueGO and Database for Annotation, Visualization and Integrated Discovery (DAVID), respectively. The traditional Chinese medicine-chemical ingredient-target network model, and the target-pathway network model were constructed through Cytoscape, and their crosstalk target and signal pathway were analyzed. Result:Animal experiments showed that the prophylactic administration of Trichosanthis Fructus-Allii Macrostemonis Bulbus herb pairs significantly reduced the levels of total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C) in serum of rats with hyperlipidemia, increased high-density lipoprotein (HDL-C) levels, and inhibited the expressions of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α). According to the findings, 27 active ingredients, such as mandenol, diosmetin and α-spinasterol, might be the main active ingredients of Trichosanthis Fructus-Allii Macrostemonis Bulbus herb pairs, 16 crosstalk targets and 10 signal pathways might be the main therapeutic targets and pathways, main targeting apolipoprotein A1 (APOA1), apolipoprotein A2 (APOA2), apolipoprotein C3 (APOC3), lipoprotein lipase (LPL), low-density lipoprotein receptor (LDLR) and other crosstalk targets affected cholesterol metabolism, bile secretion, peroxisome proliferator activated receptor (PPAR) signaling pathway in regulating the lipid level, targeting tumor necrosis factor (TNF), IL-6, interleukin-1β (IL-1β), mitogen-activated protein kinase 1 (MAPK1), C-C motif chemokine 2 (CCL2) and other crosstalk targets affected tumor necrosis factor (TNF) signaling pathway, Toll-like receptor signaling pathway, interleukin-17 (IL-17) signaling pathway, and hypoxia inducible factor (HIF) signaling pathway in regulating the inflammatory factor level. The DAVID database for GO enrichment analysis showed that the hyperlipidemia was treated mainly through biological processes, such as inflammation, lipid localization, storage and lipid metabolism. Conclusion:These findings can predict the mechanism of action of Trichosanthis Fructus-Allii Macrostemonis Bulbus herb pairs against hyperlipidemia, and provide a theoretical basis for the material basis and clinical application of Trichosanthis Fructus-Allii Macrostemonis Bulbus herb pairs.

ABSTRACT

As is known to all, it is difficult for the quality control of herbs and products of Chinese materia medica (CMM) because of its complex composition and unstable source. Fingerprint technology is an effective process analysis tool, which can be used in the quality control of CMM. Among them, the electrochemical fingerprint method based on oscillatory chemistry is convenient, sensitive, and accurate, which has been used in qualitative and quantitative analysis and process quality control of CMM. In this paper, we reviewed the development history of oscillatory chemistry, the types of oscillatory systems, and the application of oscillatory chemical fingerprint in the quality control of herbs and products of CMM. Besides, the existing problems in data testing and analysis and the possible improvement direction have been introduced. The paper aimed to provide a new method or new idea for the scientific and systematical quality control of CMM.

ABSTRACT

AIM: To explore the efficacy of EX-PRESS combined with amniotic membrane implantation for open angle glaucoma. METHODS:The data of 43 eyes of 43 patients with open angle glaucoma were enrolled from January 2014 to January 2017. The patients were randomly divided into amniotic group and mitomycin (MMC) group. All cases received EX-PRESS combined with amniotic membrane implantation (amniotic group) or with intraoperative application of mitomycin (MMC group). The early postoperative anterior chamber formation, filtering bleb, intraocular pressure ( IOP) and complications were analyzed. RESULTS: The intraocular pressure of the two groups were significantly lower at 1wk,1 and 6mo after operation than those before operation (P<0. 05). There was no significant difference in the best corrected visual acuity in the two groups at 1wk after operation compared with before operation(P>0.05). At 1 and 6mo postoperatively, the rate of functional bleb formation was 91% and 73% in amniotic group, and they were 90% and 86% in MMC group, respectively, the difference between the groups were not statistically significant (all P>0.05). At 1 and 6mo postoperatively,the total success rate was 95%,86% in amniotic group, and they were 95%, 90% in MMC group, respectively, the difference between the groups were not statistically significant (all P>0. 05). At 1wk postoperatively, the incidence of shallow anterior chamber was 9% in amniotic group and 33% in the MMC group, the difference was statistically significant (P <0 05). At 1mo postoperatively, the incidence of bleb leakage was 5% in amniotic group and 29% in MMC group, the difference was statistically significant (P <0 05). At the end of follow-up, there were 4 eyes of thin-wall bleb in MMC group and no cases in amniotic group. CONCLUSION: EX - PRESS combined with amniotic membrane implantation for open angle glaucoma is good on lowering intraocular pressure with less complications than the combination of MMC.

ABSTRACT

OBJECTIVE@#To explore the accuracy of periapical film in evaluating the infrabone root surface area of single-root teeth with different types of resorption of the alveolar bone.@*METHODS@#In this study, the medical records and imaging data of patients in Department of Periodontology of Peking University Hospital of Stomatology from January 2014 to December 2016 were retrospectively analysed. The involved teeth were divided into two groups: horizontal bone loss group and vertical bone loss group. Digital three-dimensional models of teeth were segmented from cone beam computed tomography (CBCT) data, and the percentage of the infrabone root surface area was measured. While the percentage of the residual alveolar bone was measured on the periapical films. The correlation of the two percentages were analyzed, and the differences between the two percentages of each group were compared.@*RESULTS@#Thirty-two patients and 79 single-root teeth were involved totally, in whom there were 25 men and 7 women, aged 26-60 years, and of which there were 41 teeth in the horizontal bone loss group and 38 teeth in the vertical bone loss group. The remaining alveolar bone height percentages were 45.13%-90.39% on the periapical films, and at the same time, the infrabone root surface area percentages were 36.27%-93.03% on CBCT. The residual alveolar bone height percentage of the horizontal bone loss group was not subject to normal distribution, so the residual alveolar bone height percentage and the infrabone root surface area percentage were analyzed by Wilcoxon matched pair rank test (P=0.382), and the result showed no statistically significant difference. The mean value of difference value of the two measurements was 0, and the consistency limit of 95 % was -9.7%-9.8%, between the clinical consistency limit (-15%-15%).The residual alveolar bone height percentage and the infrabone root surface area percentage of the vertical bone loss group were analyzed by paired T test (P< 0.001), and the result showed statistically significant difference. The mean value of difference value of the two measurements was 7.2 %, and the consistency limit of 95% was -13.1%-27.5%, beyond clinical consistency limit (-15%-15%).@*CONCLUSION@#For the teeth with horizontal bone loss, the proximal residual alveo-lar bone height reflected on the periapical films could show the infrabone root surface area relatively accurately. For the teeth with vertical bone loss, the periapical films could not show the degree of periodontal tissue loss accurately, and it would underestimate the extent of bone destruction usually, so CBCT might be taken to show the bone destruction condition of this type of teeth if necessary.

ABSTRACT

Objective: To investigate the dynamic expressions of tenascin-C (TN-C) at different phases of atherosclerosis in ApoE-/-mice. Methods: Our research included in 2 groups: ApoE-/- group, n=50 SPF male mice with ApoE-/- and Control group, n=50 wild male C57BL/6 mice. Atherosclerosis model was established by high fat diet in both groups. The mice were sacrificed at 4, 8, 16, 24 and 32 weeks, blood lipids were examined, pathologic changes of plaque were observed by microscope for quantitative analysis and TN-C expressions in atherosclerosis plaque were measured by immunohistochemistry. Results: Compared with Control group, ApoE-/- group had elevated blood levels of total cholesterol (TC) and low density lipoprotein cholesterol (LDL-C), both P<0.05. In ApoE-/- group, plaque area and the ratio of plaque area/lumen area were increasing upon prolonged modeling time, all P<0.05; TN-C expressions were increasing by progress of atherosclerosis, the highest TN-C expression was found at 32 weeks of modeling (0.49±0.07) which was higher than it was at 8 weeks (0.04±0.02), 16 weeks (0.12±0.03) and 24 weeks (0.21±0.04), all P<0.05. Conclusion: TN-C expression was increasing with plaque progress which might be related to the development of atherosclerosis and plaque instability.

ABSTRACT

Objective@#To investigate the value of circulating miR-152 in the early prediction of postoperative biochemical recurrence of prostate cancer.@*METHODS@#Sixty-six cases of prostate cancer were included in this study, 35 with and 31 without biochemical recurrence within two years postoperatively, and another 31 healthy individuals were enrolled as normal controls. The relative expression levels of circulating miR-152 in the serum of the subjects were detected by qRT-PCR, its value in the early diagnosis of postoperative biochemical recurrence of prostate cancer was assessed by ROC curve analysis, and the correlation of its expression level with the clinicopathological parameters of the patients were analyzed.@*RESULTS@#The expression of circulating miR-152 was significantly lower in the serum of the prostate cancer patients than in the normal controls (t = -5.212, P = 0.001), and so was it in the patients with than in those without postoperative biochemical recurrence (t = -5.727, P = 0.001). The ROC curve for the value of miR-152 in the early prediction of postoperative biochemical recurrence of prostate cancer showed the area under the curve (AUC) to be 0.906 (95% CI: 0.809-0.964), with a sensitivity of 91.4% and a specificity of 80.6%. The expression level of miR-152 was correlated with the Gleason score, clinical stage of prostate cancer, biochemical recurrence, and bone metastasis (P 0.05).@*CONCLUSIONS@#The expression level of circulating miR-152 is significantly reduced in prostate cancer patients with biochemical recurrence after prostatectomy and could be a biomarker in the early prediction of postoperative biochemical recurrence of the malignancy.

According to the characteristics of the etiology and pathogenesis of child cerebral palsy, on the basis of "regulating the mind in treatment of all kinds of diseases" and "regulating the functions of five zang organs with back-shu points", Professor DONG Gui-rong applied the penetrating needling technique on the scalp points and acupuncture at back-shu points of five zang organs in the treatment of child cerebral palsy. The valuable clinical experiences have been summarized as "regulating the mind with scalp needling technique, benefiting the brain and opening the orifice", "regulating five zang organs with back-shu points" and "integration of acupuncture and rehabilitation, and function reconstruction". Two effective cases were introduced.

ABSTRACT

<p><b>OBJECTIVE</b>To study on the method for establishing the Qi stagnation and blood stasis rat model and analyze the affecting factors.</p><p><b>METHOD</b>The orthogonal design was adopted to study the influences of joint stimulations including noise, light, electricity, ice water bath, tail-clamping on model rats. The 'flying spot' method was used to dynamically simulate blood flow velocity in microcirculation. the pressure sensing technology of MOTO was adopted to detect hemorheology-related indicators. And the coagulation method was used to detect blood coagulation-related indicators.</p><p><b>RESULT</b>Compared with the negative control group, all model groups showed significant reduction in the blood flow velocity in mesenteric microcirculation and increase in the whole blood viscosity at high, medium and low shear rate, the plasma viscosity and the fibrinogen content in four blood coagulation indicators.</p><p><b>CONCLUSION</b>Noise, light, electricity, tail-clamping, bondage and icewater-bath make significant impact on model rats.</p>

ABSTRACT

To investigate the electrophysiologic characteristics of f-wave amplitude and to evaluate its role in development and persistence of chronic atrial fibrillation (AF) associated with rheumatic heart disease (RHD). Methods: Epicardial mapping was performed in 44 patients with chronic AF of RHD who underwent heart valve surgery. Ten patients with supraventricular tachycardia served as the control group. Results:The f-wave amplitude of left atrium (LA) and middle and low LA posterior regions were significantly lower than those of the control group. The f-wave amplitudes of the upper, middle and low sections in LA posterior region were significantly lower than those in right atrium (RA) (P<0.05). The f-wave amplitudes were compared before and after electrocardioversion in 14 patients with chronic AF. The mean atrial electrogram amplitude during sinus rhythm was significantly higher than that during AF (P<0.01).The f-wave amplitude in left appendage was higher than that in LA posterior region (the upper,middle and the lower part),P<0.05.The f-wave amplitude in the upper section of LA was significantly higher than that in the middle section. The f-wave amplitude in AF group was not correlated to the diameter or volume of both atria. Conclusion: There are amplitudes differences between the upper, middle and lower LA,suggesting that the middle and lower sections of LA posterior wall may be the region producing anisotropy and reentrant circle.

ABSTRACT

Streptomyces sp. A048 was cultured in a complete medium to the last stage of log phase,the hyphae were washed and collected by centrifugation. Then the hyphae were inoculated in liquid medium for chitinase production using two-step fermentation. Activity of chitinase produced by two-step fermentation was 1.1 times higher than that from one-step fermentation,and ferment cycle was for 54 hours,which was 66 hours shorter than that of one-step fermentation. The hyphae and the powder of chitin were co-immobilizated and cultured in liquid medium for 36 hours,activity of chitinase was 1.8 times higher than that from one-step fermentation,and ferment cycle was 54h shorter than that of one-step fermentation. By adding 0.4% cellulose to two-step fermentation,activity of chitinase was 18.52 U/mL that was 4 times higher than that from the control and 10 times higher than that from one-step fermentation. Two step fermentation with chitin and cellulose may be the optimal fermentation process to produce Chitinase from Streptomyces sp. A048.

ABSTRACT

A.niger M1, the initial strain, was treated by UV and a mutant with 30% higher xylanase activity was obtained. Zymogram for detecting xylanase showed there are three different xylanases in the mutant mature culture, while two xylanases in initial strain. After orthogonal experiment, the optimum fermentation conditions of the mutant were obtained as follows: concentration of the major carbon resource 4 %, ratio between bran and corncob 5:5, concentration of glucose 0.1%, concentration of ammonium oxalate as supplemental nitrogen resource 2.0%, the initial pH of liquid medium 5.0, 100mL/250mL flask.
