ABSTRACT

The intra-articular tibial plateau fracture can induce many complications of the knee joint after conservative treatment or operative treatment. Recent experience could decrease the complications with accurate anatomical reduction, stable internal fixation and early knee ROM exercise, but still unsatisfactory results were reported from 20% to 40%. The authors analyzed the 30 cases that could be follow-up from 1 year to 6 years and were treated with operative methods, and the following results were obtained. 1. The most common cause of injury was traffic accident (14 cases-46.4%). 2. The most common fracture type by Hohls classification was type VI (13 cases-43.1%). 3. According to Blokker's criteria, 26 cases (86.7%) among 30 cases had satisfactory results. 4. The one of the most important factor influencing end results was thught to be the anatomical reduction of the articular margin.

ABSTRACT

In case of the large bone defect due to severe varus or valgus deformity in total knee replacement, there are many different methods for reconstruction of the bone defect for insertion of the tibial component after resection of the proximal tibia. Total knee replacement using the autogenous bone graft were performed in 27 knees (18 patients) with tibial bone defect. Authors report the methods and results with an average 23 months follow-up. We used the bone removed from distal femur and calculated the size of the proximal tibia. We used the screws, if necessary, for rigid fixation of the grafted bone. l. According to the type of bone defect, central defect type were 3 knees, peripheral defect type were 13 knees and combined type were 11 knees. 2. In A-P view of x-ray, the size of bone defect of the tibial plateau ranged 5mm to 25mm (average 11.1mm) in height. 3. Screws were used for fixation of grafted bone in 14 knees and the average size of the screws was 27.8mm. 4. Bony union of the grafted bone achieved from 3 months to 12 months (average 5.7months). 5. Average knee ROM was 105 degrees and improved post-operatively 121 degrees, and average flexion contracture was 22.4 degrees and improved post-operatively 7.5 degrees. 6. Post-operative tibiofemoral angle was average 7.5 degrees valgus. 7. Average knee rating score was 54.5 and improved post-operatively 89.2. 8. The comlication of bone graft was partial resorption in 1 case and the loosening or displacement due to screw were abscent.

ABSTRACT

There are many problems in treatment of intertrochanteric fractures due to osteoporosis and unstable pattern of fractures in old ages and they are different from those in young ages, for example, type of fracture, degree of osteoporosis, and general condition, ect. Anthors studied about the prognostic factors of 42 cases among 88 patients with intertrochanteric fractures, who were treated and followed up from Jan. 1982 to Dec. 1987. 1. The patients under 50 years old were mainly males(94.4%) and its chief cause of injury was traffic accident or fall down(72.2%). The cause of injury over 50 years old was slip down on ground(62.9%). 2. The osteoporosis was increased according to increased age of patients and then the unstable type of fracture was increased. In yaung patients, the unstable intertrochanteric fracture was rare despite of high violent injury. 3. The bone union of the follow-up patients was gained at average 13.8 weeks. 4. The sliding degree of lag screw was increased with unstable fracture and the incidence of metal protrusion into joint cavity was increased in the patients of eccentric fixation of lag screw into femoral head. 5. The all cases of complication were occured in osteoporotic patients over 50 years old and their causes were inadequate fixation of unstable fracture. 6. The most important factors in prognosis of intertrochanteric fractures are initial pattern of fractures, degree of osteoporosis and adequacy of reduction state(postop.).
