ABSTRACT

Objective:To investigate the pathogenic variations of a case of 3-methylpenteneduric aciduria (MGA) type Ⅰ.Methods:Retrospective analysis gene variations of the case with MGA type Ⅰ and family members in June 2017 at Yancheng Maternal and Child Health Hospital were detected using high-throughput sequencing combined with Sanger sequencing.The pathogenicity of the novel variations was predicted using the bioinformatic method.The impact of the novel splicing variation was examined through laboratory experiments.Results:Tandem mass spectrometry and gas chromatography-mass spectrometry results diagnosed the case as MGA type Ⅰ.The compound hete-rozygous variations c. 373C>T (p.R125W) and c. 942+ 3A>G of the AUH gene were detected in the patient, which were inherited from the mother and the father, respectively.Bioinformatics analysis indicated that the c. 373C>T(p.R125W) of the AUH gene was pathogenic (3 softwares) and the R125 residue was highly conserved.Reverse transcription-PCR and Sanger sequencing analysis showed that the variation c. 942+ 3A>G caused the deletion of AUH gene exon 9, which was failed to be predicted in the 4 types of software.The patient was treated with Levocarnitine and leucine-free milk powder from 45 days after birth.The physical and mental development was normal. Conclusions:Splicing analysis of blood RNA should be considered for variants of uncertain significance in genetic diseases when the clinical diagnosis is clear.This study enriches the variation spectrum of the AUH gene.

ABSTRACT

Tumor has become one of the major diseases threatening human health at present. Conventional therapies for tumor have serious side effects and are prone to drug resistance, while new therapies are expensive and impose a heavy burden on patients. Chinese herbs effective in the treatment of tumor contain a variety of natural active ingredients, and many anti-tumor drugs used in clinical practice are derived from Chinese herbs. Polysaccharides, ubiquitous in Chinese herbs, are easy to extract and have antitumor, antiviral, and antioxidant activities. Studies have demonstrated that the polysaccharides from Chinese herbs have significant anti-tumor effect and are characterized by multi-target and multi-mechanism functioning, which can avoid the development of drug resistance, exhibiting a great research value and the potential to be developed into novel anti-tumor drugs. Their anti-tumor mechanisms mainly include the inhibition of tumor proliferation, promotion of apoptosis, induction of autophagy, influence on cell cycle, inhibition of tumor angiogenesis, modulation of epithelial-mesenchymal transition (EMT) and immunity, promotion of tumor oxidative stress, and regulation of phosphatidylinositol 3-kinase/ protein kinase B (PI3K/Akt), Toll-like receptor 4 (TLR4), mitogen-activated protein kinase/nuclear factor-kappa B (MAPK/NF-κB), adenosine 5'-monophosphate (AMP)-activated protein kinase/mechanistic target of rapamycin (AMPK/mTOR), epidermal growth factor receptor/extracellular signal-regulated kinase (EGFR/ERK), p53, and Wnt/β-catenin signaling pathways.

ABSTRACT

Objective To investigate the effect of different fractionated radiotherapy of hypofractionated radiotherapy (HFRT) and conventional fractionated radiotherapy (CFRT) on peripheral blood lymphocytes in patients with breast cancer. Methods This retrospective analysis enrolled 40 patients with early breast cancer who underwent radiotherapy post breast conserving surgery in Xuzhou Central Hospital from November 2019 to August 2021. The patients were randomly divided into the observation group (HFRT, n = 20) and the control group (CFRT, n = 20). Changes in peripheral blood lymphocyte count (PLC) before and during radiotherapy were compared between the two groups. Results The baseline PLC was comparable between the observation group and the control group (1.53 ± 0.54 vs 1.64 ± 0.56, P > 0.05). In both groups, the PLC declined steadily during radiotherapy, and the incidence of lymphopenia in the observation group was lower than that in the control group (32.5% vs 50.0%, P > 0.05); the PLC nadir was higher in the observation group than in the control group (0.91 ± 0.28 vs 0.55 ± 0.22, P < 0.001). The ratio of the PLC nadir during treatment to baseline was significantly higher in the observation group than in the control group (0.64 ± 0.24 vs 0.38 ± 0.21, P < 0.05). Conclusion Patients with breast cancer receiving HFRT show a lower risk of radiation-induced lymphopenia versus those receiving CFRT.

ABSTRACT

Objective:To observe the effect of Fangji Huangqitang (FJHQT) on collagen induced arthritis (CIA) and synovial angiogenesis in DBA/1 mice. Method:DBA/1 mice were randomly divided into normal group, CIA group and FJHQT group. DBA/1 mice in CIA group and FJHQT group were immunized with bovine type Ⅱ collagen and complete Freund's adjuvant on the first day, and DBA/1 mice were immunized with bovine type Ⅱ collagen and incomplete Freund's adjuvant on the 21<sup>st</sup> day to establish CIA model. On the day of the second immunization, the drug was given by gavage once a day for 28 days. On the 22<sup>nd</sup> day, the arthritis score and other symptoms of CIA mice were observed. On the 49<sup>th</sup> day, Hematoxylin eosin (HE) staining was carried out to observe the angiogenesis in the synovium of CIA mice, the expression of vascular endothelial cell marker platelet endothelial cell adhesion molecule-1 (CD31) and vascular endothelial growth factor (VEGF) in the synovium of CIA mice were detected. Immunofluorescence double staining was used to detect the mature and immature vessels in the synovium of CIA mice. And the microvascular growth of the rat thoracic aortic ring was induced by VEGF (20 μg·L<sup>-1</sup>). The effects of FJHQT (0.25, 0.5, 1 g·L<sup>-1</sup>) at different concentrations were observed under microscope. Result:Compared with the normal group, the inflammation, joints, red and swelling of the inflammatory joints of the CIA group were significantly increased (<italic>P</italic><0.01). The scores of clinical arthritis, the incidence rate, synovial inflammation and angiogenesis were significantly increased (<italic>P</italic><0.01). The density of blood vessels, the positive expression of CD31 and VEGF, the number of immature vessels in synovial membrane were significantly increased (<italic>P</italic><0.01). And compared with the CIA group, the inflammation, joint swelling, and malformation of the FJHQT group were significantly improved, the clinical arthritis score, incidence rate, synovial inflammation and angiogenesis were significantly reduced (<italic>P</italic><0.01). The vascular density, the positive expression of CD31 and VEGF, and the number of immature blood vessels in synovial membrane were significantly increased (<italic>P</italic><0.01). Compared with blank group, VEGF could significantly induce the growth of microvasculature in rat thoracic aortic ring (<italic>P</italic><0.01). Compared with VEGF group, FJHQT(0.25, 0.5, 1 g·L<sup>-1</sup>) could significantly inhibit the formation of microvasculature in rat thoracic aortic ring (<italic>P</italic><0.01). Conclusion:FJHQT can effectively alleviate the clinical symptoms and condition of CIA mice, reduce the clinical arthritis score and incidence rate,and inhibit the synovial angiogenesis of CIA mice joints and VEGF induced microvascular formation in rat thoracic aortic rings.

ABSTRACT

Objective:To compare the dosimetric difference between volumetric modulated arc therapy (VMAT) plans with flattening filter (FF) and flattening filter-free (FFF) for whole-breast radiation therapy after breast conserving surgery for left breast cancer.Methods:Twenty patients with left breast cancer who underwent breast conserving surgery in Xuzhou Central Hospital from August 2017 to August 2018 were selected by random number table method, and the CT data were obtained. Both FF-VMAT plan and FFF-VMAT plan were designed with ECLIPSE 10.0.4 treatment planning system (TPS) of American Varian company. The prescription dose of the planned target volume (PTV) was 50 Gy (2 Gy/time), and the local tumor bed was irradiated with 10 Gy/5 times. The dosimetric parameters of the two plans were compared.Results:The two plans of all patients met the prescription dose requirements. Compared with FF-VMAT plan, FFF-VMAT plan had better conformity (conformal index: 0.87±0.04 vs. 0.77±0.05) and homogeneity (homogeneity index: 7.36±0.88 vs. 10.89±3.00) (both P < 0.01); FFF-VMAT plan had lower average dose of heart [(7.73±1.44) Gy vs. (9.16±4.24) Gy] and contralateral lung [(3.61±0.74) Gy vs. (8.42±0.45) Gy] (both P < 0.01), FFF-VMAT plan had higher average dose of contralateral breast [(3.92±1.08) Gy vs. (2.02±1.42) Gy] ( P < 0.01), and V 2 Gy in the heart [(79.27±1.18)% vs. (58.94±21.25)%] and V 5 Gy in the ipsilateral lung [(44.89±2.17)% vs.(37.86±8.33)%] (all P < 0.01). The monitor unit of the FF-VMAT plan was (973±89) MU, which was lower than that of the FFF-VMAT plan [(1 356±201) MU] ( P < 0.01). Conclusion:FFF-VMAT plan can effectively reduce the cardiac dose after breast conserving surgery for left breast cancer, but it increases the risk of organ damage at low dose area.

ABSTRACT

Objective To explore the optimal radiotherapy method by comparing the dosimetric differences of target and organs at risk of four radiotherapy plans for left sided breast cancerafter breast-conserving surgery. Methods Twenty-three patients with left breast cancer were randomly selected and given PTV 25 fractions, 50 Gy prescription dose.TheHybrid_IMRT, rj_IMRT, VMAT and t_VMAT plans were designed for each patients. Dosimetric differences were compared, including dose volume histograms of target and OARs, target homogeneity indexes (HI), conformal indexes (CI) and the machine MUs. Results Target Dosimetric comparison, HI: t_VMAT plan target has highest HI and had significant difference (P ≤ 0.001); The target CI of VMAT plans were 0.967 ± 0.016, had significant difference compared with the other 3 plans (P < 0.05). The CI of rj_ IMRT were 0.942 ± 0.018 better than that of IMRT and t_VMATs. Dosimetric comparison of OARs, left_lung mean dose (MLD_L): rj_IMRT were (8.76 ± 1.52) Gy which were best of 4 plans, and had statistical significance (P < 0.05). Heart mean dose: rj_IMRT were (4.68 ± 0.87) Gy were better than that of VMAT (P < 0.05). Conclusion All of these four plans could be applied in clinical treatments, while the limitations of treatment equipment, patients’ physical conditions and some other factors should be considered before selecting an appropriate one.

ABSTRACT

Objective To compare the dosimetric difference between the biological function based on equivalent uniform dose (EUD) and the physical function based on dose volume (DV) in the intensity modulated radiotherapy for stage Ⅲ non-small cell lung cancer. Methods Four different radiotherapy plans were designed for 15 stage Ⅲ non-small cell lung cancer patients: Group A, physical function optimization (DV + DV) was used for target area and organs at risk; GroupB, in the target region, biological function optimization conditions were added on the basis of physical function optimization, and physical function optimization of organs at risk (DV-EUD + DV) was added. Group C, biological function optimization (EUD + EUD) was used for target area and organs at risk. Group D, in the target area, physical function optimization conditions were added on the basis of biological function optimization, and biological function optimization of organs at risk (EUD-DV + DV) was added. The differences in dosimetric parameters of the four plans were compared. Results Target area: PTV: D2%, D98%, D50%, D105% and Dmax values of group C (P < 0.05) is the highest while group B and group D were relatively small (P > 0.05); The homogeneity index: the results of the group B and the group D were better than those of the other two groups (P < 0.05). conformity index: The results of the four groups were similar (P>0.05). Organ at risk: lung tissue mean dose (MLD), V5, V10, V20, V30 and heart V30, V40, Dmean dose parameters were similar (P > 0.05). Spinalcord: Group C and group D D1% were better than the other two groups (P < 0.05). There was no statistical difference in the number ofmonitor unit (MU) among the four groups (P > 0.05). Conclusion The optimization method combining physical and biological function optimization in the target area can improve the conformity of the target area on the premise of ensuring the treatment. The Spinalcord load would be significantly reduced when using biological function optimization or the combination of biological function and physical function optimization.

ABSTRACT

Objective:To explore the effect of closed skills physical activity with additional dynamic visual task on dynamic visual acuity and static visual acuity for pupils with myopia at grade four at primary school. Methods:In September, 2019, 37 pupils with myopia at grade four in Daicheng Experimental Primary School, Suzhou City were selected. In light of mechanism of refractive system, the intervention of closed-skilled physical activity with dynamic visual task was developed. They were intervened for 16 weeks. The dynamic visual acuity and static visual acuity were measured before intervention, and eight and 16 weeks after intervention. Results:Both dynamic visual acuity and static visual acuity improved significantly in all the pupils with myopia after 16 weeks of intervention (F > 15.933, P < 0.001), both pupils with mild and moderate myopia (|t| > 3.353, P < 0.05). The static visual acuity was better in pupils with moderate myopic than with mild myopia (t = -2.830, P < 0.01). Conclusion:Closed skills physical activity with dynamic visual task could improve the visual acuity of puplis with myopia at grade four at primary school.

ABSTRACT

Brain metastases are common in oncologic patients. Stereotactic radiosurgery (SRS) is an effective treatment for oligo brain metastases. However, SRS is not suitable for large metastatic lesions. Fractionated stereotactic radiation therapy (FSRT) is a new option for the treatment of brain metastases. It can deliver high doses of radiation to tumor while protecting the normal tissues and organs outside the irradiation field as much as possible and maintaining good local control rate of tumors. This article reviews the dose-response relationship and adverse reactions of FSRT with different fractionated dose schemes, and the progress of FSRT combined with targeted therapy and immunotherapy.

ABSTRACT

Objective: To assess the feasibility of HEAD-US scale in the clinical application of hemophilic arthropathy (HA) and propose an optimized ultrasound scoring system. Methods: From July 2015 to August 2017, 1 035 joints ultrasonographic examinations were performed in 91 patients. Melchiorre, HEAD-US (Hemophilic Early Arthropathy Detection with UltraSound) and HEAD-US-C (HEAD-US in China) scale scores were used respectively to analyze the results. The correlations between three ultrasound scales and Hemophilia Joint Health Scores (HJHS) were evaluated. The sensitivity differences of the above Ultrasonic scoring systems in evaluation of HA were compared. Results: All the 91 patients were male, with median age of 16 (4-55) years old, including 86 cases of hemophilia A and 5 cases hemophilia B. The median (P25, P75) of Melchiorre, HEAD-US and HEAD-US-C scores of 1 035 joints were 2(0,6), 1(0,5) and 2(0,6), respectively, and the correlation coefficients compared with HJHS was 0.747, 0.762 and 0.765 respectively, with statistical significance (P<0.001). The positive rates of Melchiorre, HEAD-US-C and HEAD-US scale score were 63.0% (95%CI 59.7%-65.9%), 59.5% (95%CI 56.5%-62.4%) and 56.6% (95%CI 53.6%-59.6%) respectively, and the difference was statistically significant (P<0.001). Even for 336 cases of asymptomatic joints, the positive rates of Melchiorre, HEAD-US-C and HEAD-US scale score were 25.0% (95%CI 20.6%-29.6%), 17.0% (95%CI 12.6%-21.1%) and 11.9% (95%CI 8.4%-15.7%) respectively, and the difference was statistically significant (P<0.001). There were significant changes (P<0.05) in the ultrasonographic score of HA before and after onset of hemorrhage in 107 joints of 40 patients. The difference in variation amplitude of HEAD-US-C scores and HEAD-US scores before and after joint bleeding was statistically significant (P<0.001). Conclusion: Compared with Melchiorre, there were similar good correlations between HEAD-US, HEAD-US-C and HJHS. HEAD-US ultrasound scoring system is quick, convenient and simple to use. The optimized HEAD-US-C scale score is more sensitive than HEAD-US, especially for patients with HA who have subclinical state, which make up for insufficiency of sensitivity in HEAD-US scoring system.

ABSTRACT

Objective The expressions of inflammatory factors and brain edema after traumatic brain injury (TBI) are the main factors for deterioration of the condition.TBI after drunkenness is even more difficult to be managed than simple TBI.This study was to discuss the effects of drunkenness on the inflammatory factors TNF-o and IL-6 and the aquaporin-4 (AQP-4) protein in rats after TBI.Methods Forty-eight male adult SD rats were randomly divided into a TBI and an ethanol (ETH) pretreatment group.TBI was induced using the Feeney's method after intraperitoneal injection of 3% chloral hydrate at 30 mg/kg (the TBI group) or following gavage of ETH (the ETH group).At 1,3 and 5 days after modeling,modified neurological function scores (mNSS) were obtained,the expressions of TNF-α,IL-6 and AQP-4 protein determined by Western blot,and the levels of TNF-α.IL-6 and AOP-4 mRNA measured by RT-PCR at 6,24 and 72 hours.Results Compared with the TBI group,the ETH group showed significantly decreased mNSS at 1 day (9.00±0.63 vs 7.17±1.72,P<0.05),3 days (7.00±1.10 vs 4.83±1.47,P<0.05) and 5 days after modeling (5.50±1.05 vs 3.83± 0.75,P< 0.05),but remarkably up-regulated expressions of TNF-α (0.068± 0.008 vs 0.257 ± 0.008,P< 0.01),IL-6 (0.102 ±0.013 vs 0.320±0.016,P<0.01) and APQ4 (0.054±0.007 vs 0.212±0.015,P<0.01) at 6 hours,as well as at 24 and 72 hours (P<0.01).Conclusion Drunkenness may increase the expressions of inflammatory factors and brain edema after traumatic brain injury and consequently aggravate secondary brain injury.

ABSTRACT

With the promotion and application in medicine and sanitary fields,the reporting quality of Bland-Altman agreement evaluation is worrying.This study aimed at developing a set of reporting standards for Bland-Altman agreement evaluation as the guidance for proper application to improve the reporting quality.A research group was launched to work on reporting standards for Bland-Altman agreement evaluation,and all the requirements for personnel on various levels were made clear.Early in the study,we carried out articles reviews,regular meetings,expert consultations,literary evaluation,item integration and extraction.Then,we invited a multi-disciplinary panel of experts to improve overall design,evaluate reporting items and form the first draft by brainstorming method.Multiple rounds of expert consultations were also conducted with reference to Delphi method to integrate expert advice and form the basic framework of reporting items.Finally,through the thorough analysis and demonstration,we proposed the reporting items for the agreement evaluation of Bland-Altman method (RiBAM) as the recommended report.A list of recommended items called RiBAM was formed,which consisted of 17 first-level items and 23 secondary-level items.RiBAM is more comprehensive and systematic as an important reference for improving the quality of reports by avoiding the omission of reporting contents and achieving the clarity,integrity and transparency of the report.RiBAM recommended items can be a guide for authors in reporting Bland-Altman agreement evaluation as well as a basic reference for journal editors,peer reviewers and readers.

ABSTRACT

Objective To investigate the epidemiological characteristics and the influencing factors of the unintentional injuries among Wenzhou elementary school children. Methods A total of 3 050 students were selected from the 1st to 6th grades in 6 primary schools in Wenzhou urban area by multi - stage sampling and were investigated during 2013 to 2015. Results 2 859 students completed the investigation. The morbidity of annual average unintentional injuries was 30. 81%(95% CI:29. 12% - 32. 50% )and the occurrence density was 2. 79. Unintentional injuries occurred mainly at home (37. 69% )and school(32. 39% ),and the most common site of injuries were leg(35. 02% )and hand(33. 79% ). For overall incidence,the slip falling injury(35. 64% )was highest,followed by cut or pierced(33. 40% )and crush injury (25. 08% ). Schoolgirl was a protective factor of the injury of crushed,electrical,traffic accident,bashed,burn , exploded,drown,poisoning and bit(OR ﹤ 1),but was a risk factor of multiple injuries(OR = 1. 323). Age was a risk factor of injury of fallen,crushed,bashed,burn,cut or pierced,exploded and bit(OR ﹥ 1. 1),but was a protective factor of poisoning(OR = 0. 671). compared with the students who walked to school alone,the children who were accompanied by their family member and rode alone to school were protective factors of the traffic accident. Conclusion The incidences of the unintentional injuries of elementary school children were increased with the age,and the boy was easier to be hurt. It was suggested for parents,schools and the government to strengthen healthy education for children at the early age.

ABSTRACT

Objective To evaluate the clinical effects and adverse reaction of raltitrexed combined with radiation for esophageal carcinoma in elderly patients.Methods Sixty patients were randomly divided into two groups by the envelope method, 30 patients in experimental group received raltitrexed combined with radiotherapy and 30 patients in control group received radiotherapy only.Patients in both groups received conventional radiotherapy with a total dose of 56-60 Gy/28-30 F.In experimental group, raltitrexed 2.6 mg/m2 was administered concurrently with the radiotherapy on d1 and d22.Two cycles of concurrent chemotherapy were administered during radiotherapy.The short-term effects, survival times and adverse reactions of the two groups were compared.Results The total effective rates of experimental group and control group were 93.3% and 73.3%, respectively, and there was statistically significant difference between the two groups (χ2=4.320, P=0.038).The median survival times of experimental group and control group was 24.0 months and 12.0 months, respectively, and there was statistically significant difference by Log-rank test (χ2=6.048, P=0.014).The major adverse reactions of grade 3-4 in experimental group and control group were radiation-induced esophagitis (10.0% vs.3.3%;χ2=0.268, P=0.605), leukopenia (13.3% vs.10.0%;χ2=0.000, P=1.000), thrombocytopenia (3.3% vs.0;P=1.000), nausea and vomiting (6.7% vs.0;χ2=0.517, P=0.472), and the differences were not statistically significant.Conclusion Raltitrexed combined with radiotherapy can enhance the short-term effect and prolong the survival time for the elderly esophageal carcinoma patients, and the adverse reactions are mild.It is worthy of further clinical study.

Objective To investigate the expression and significance of ADAMDEC1 in glioma.Methods We prospectively observed the expression of ADAMDEC1 in 77 glioma patients,of which 28 cases were grade Ⅳ glioma,26 were grade Ⅲ glioma (high-grade glioma);of which 23 cases were Grade Ⅱ glioma,5 were grade Ⅰ glioma (low-grade glioma).For 10 cases in the control group,brain tissue samples were collected from internal decompression patients with heavy traumatic brain injury.The expression of ADAMDEC1 in glioma tissue was detected immunohistochemistry,RT-PCR,and Western blot.With follow-up survey we explored the relationship between the survival time and the expression of ADAMDEC1 in the patients with high-grade gliomas and low-grade gliomas.Results Compared with the low-grade glioma group and the control group,qPCR,immunohistochemistry and Western blotting all showed that the protein and mRNA levels of ADAMDEC1 in high-grade glioma group were significantly increased,while ADAMDEC1 was expressed little in low-grade glioma group and not expressed in control group (P < 0.05).The survey results showed that the survival time of high-grade glioma patients was significantly shorter than low-grade glioma patients;however,the expression level of ADAMDEC1 in the highgrade glioma tissue was significantly higher than in low-grade glioma tissue (x2=5.031,P<0.05).Conclusion The expression of ADAMDEC1 is closely related to the malignant degree of glioma cells and the prognosis of glioma patients.Therefore,ADAMDEC1 can be used as an index to evaluate the malignance degree of glioma and the prognosis of glioma.

ABSTRACT

Objective To establish a loop-mediated isothermal amplification(LAMP) quantitative method for rapid detection of Japanese encephalitis and dengue fever virus.Methods According to the LAMP principle,design primers for LAMP detection and reaction system,establish LAMP detection method,and to evaluate the linear relationship between initial copy number and the specificity,sensitivity,repeatability and the reaction time(fluorescence signal value of 1×104 corresponding time).Results One sets of LAMP primers could be used to complete the detection work in 0.5 h.The sensitivity of LAMP detection technology was 10 times higher than that of the classical PCR technology,and no cross reaction with other viruses,and the coefficient of variation of the average test was less than 5%.There was a good linear relationship between cycle threshold and template concentration.Conclusion This method has high specificity,sensitivity,simple operation,which is easy to get the results,low equipment requirements and rapid,suitable for primary health institutions and the field inspection agencies for wide applications.

ABSTRACT

Aim to discuss whether the contrast enhanced ultrasound [CEUS] can effectively monitor the efficacy on neoadjuvant chemotherapy of breast cancer or not by analyzing the indicators on chemotherapy CEUS and breast cancer tumor biology, especially tumor microcirculation indicator on animal mode. Human breast cancer cell lines MCF-7 are planted under the skins of nude mice. By simulating clinical neoadjuvant chemotherapy regimen periodically inject CMF [cyclophosphamide, methotrexate and fluorouracil] into the experimental group, and normal saline into the control group. Then detect the data from CEUS and record the parameters: maximum intensity [IMAX], rise time [RT], time to peak [TTP] and mean transit time [mTT]. Execute animal after CEUS, obtain tumor biological indicator and record parameters: micro vessel density [MVD], vascular endothelial growth factor receptors 1/2/3/4 [VEGFR-1/2/3/4] and tumor cells. In the aspect of tumor biological indicator, the experimental group after the first drug delivery: inter- and intra-group comparisons of VEGFR-1/4drop significantly. The experimental group after the second drug delivery: interand intra-group comparisons of MVD, VEGFR-1/3/4drop significantly. In the aspect of parameters on tumor CEUS, the experimental group after the first drug delivery: inter- and intra-group comparisons of IMAX drop significantly. The experimental group after the second drug delivery: inter- and intra-group comparisons of IMAX decrease steeply; while inter-and intra-group comparisons of TTP rise significantly. There are great changes about the intra-group comparisons of the number of tumor cells before and after the experiment. In the process of chemotherapy, it maintains the consistency of the changes of CEUS parameters IMAX and TTP, tumor microcirculation indicators MVD and VEGFR- 1/3/4 and tumor cells. So CEUS has a potential to make an early prediction on the efficacy of neoadjuvant chemotherapy

ABSTRACT

Objective To evaluate the effect of replication protein A (RPA) gene suppression on the radiosensitivity of esophageal cancer cells (TE-1R ) and underlying mechanism. Methods A radioresistant human esophageal cancer cell line TE-1 R was screened out by fractionated irradiation to TE-1 cells,then siRPA1 or siRPa2 was transfected to TE-1R cells.The untransfected (Con) group and nonsense siRNA transfected (NC) group were set as control groups.The survival was measured with colony-forming assay and the cell cycle distribution was measured with flow eytometry.Results Compared with the Con and NC groups,the protein expression of RPA1 and RPA2 decreased significantly 48 h after siRPA1 and siRPA2 transfection.The D0,Dq,and SF2 values reduced from 2.09,1.70,0.85 in NC group to 1.67,0.71,0.44 and 1.82,0.89,0.51 in siRPAl and siRPA2 transfection groups,respectively.Accordingly,the sensitization enhancement ratios of Dq were 2.39 and 1.91,respectively.The G2/M arrest in siRPA1 and siRPA2 transfection groups increased from (18.701 3.14)% of NC group to (26.95 ± 3.96)% and (25.28 ± 2.74) % (t =2.827,2.853,P <0.05),respectively.Conclusions Knocking down of RPA1 or RPA2 genes can enhance the raidosensitivity of human esophageal cancer cells TE-1R,where the inhibition of radiation-induced sublethal damage repair may be involved.Accordingly,RPA may become a new target of radiosensitization in esophageal cancer.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the influence of knock-down of miR-21 expression on the radiosensitivity of radioresistant glioma SHG-44 cells and its mechanism.</p><p><b>METHODS</b>Radioresistant cell line SHG-44(R) cells were established from radiosensitive parental SHG-44 glioma cells. Then the expression levels of miR-21 in SHG-44(R) and SHG-44 were determined by quantitative real-time PCR. The effect of miR-21 on the radiosensitivity was assessed in SHG-44(R) with miR-21 inhibitor to decrease the miR-21 expression.</p><p><b>RESULTS</b>miR-21 was up-regulated 1.49-fold in SHG-44(R) cells relative to the SHG-44 cells. But after transfection with As-miR-21, the miR-21 expression in SHG-44(R) cells was knocked down significantly. As-miR-21 combined with radiation could synergistically enhanced mitotic death and apoptosis of SHG-44(R) cells, with SER of D(0) and D(q) being equal to 1.48 and 1.54, respectively. Expression levels of caspase-3 in the radiation group, AS-miR-21 transfection group and combination group were 2.24 ± 0.14, 2.05 ± 0.19 and 5.72 ± 0.45, respectively, and its expression in the combination group was higher than that in the other two groups.</p><p><b>CONCLUSIONS</b>miR-21 may be involved in the formation of radioresistance of glioma cells and as a potential target for enhancing the response of radioresistant-glioma cells to radiotheapy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the therapeutic effect and toxicity of intensity-modulated radiation therapy (IMRT) or three-dimensional conformal radiotherapy combined with chemotherapy (3-DCRT) with docetaxel and cisplatin in the treatment of locally advanced esophageal carcinoma.</p><p><b>METHODS</b>Sixty patients with locally advanced esophageal carcinoma were randomly assigned in two equal groups to receive IMRT or 3-DCRT, both combined with the chemotherapy with docetaxel and cisplatin. The total dose of radiotherapy was 64 Gy, administered in 30 fractions in 6 weeks.</p><p><b>RESULTS</b>The complete response rate (complete and partial remissions) of IMRT group was 90.0%, significantly higher than the rate of 80.0% in 3-DCRT group (P>0.05). The 1-, 2-, and 3-year survival rates of IMRT group were 86.7%, 70.0%, and 66.7%, as compared to 70.0%, 63.3%, and 63.3% in 3-DCRT group, respectively, showing no significant differences between the two groups (P>0.05). IMRT showed advantages over 3-DCRT in terms of the V20 and V30 parameters of the lung (P<0.05), and the incidences of radiation-induced esophagitis were comparable between the two groups (P>0.05).</p><p><b>CONCLUSION</b>When combined with the chemotherapy with docetaxel and cisplatin, IMRT appears to be a more effective treatment than 3-DCRT for locally advanced esophageal cancer.</p>
