ABSTRACT

Bowel segments are used either in a tubular or detubular configuration in bladder augmentation and substitutions. The majority of urologists agree that detubularized segments give better clinical results,but detailed studies were not investigated in animals. The present study compared the contractile response of autonomic agonists including carbachol, isoproterenol,alpha, beta-methylene ATP on detubularized and tubularized cystoplastic ileal segments. In addition, histological examinations of cystoplastic ileal segments were performed. Five rats underwent Sham operation as control and eight rats underwent detubularized ileocystoplasty and seven rats underwent tubularized ileocystoplasty. Analysis was done six weeks after surgery. The contractile responses of detubularized cystoplasty ileum to carbachol, alpha, beta-methylene ATP were significantly increased when compared to the responses of tubularized ileum and control ileum. The relaxation responses of detubularized cystoplasty ileum to isoproterenol were significantly higher than control ileum. However, in tubularized ileum, the responses to carbachol, alpha, beta-methylene ATP, isoproterenol were similar for control ileum. The responses of detubularized cystoplasty ileum to carbachol, alpha, beta -methylene ATP were significantly different from control bladder, but detubularized cystoplasty ileum showed responses that resemble control bladder more closely when compared to the responses of tubularized ileum.The histological examination showed urothelialization of cystoplastic ileum with transitional epithelium extending over and covering the junction margin of the cystoplastic ileum. These results suggest detubularized ileocystoplasty induced closer pharmacological changes in the ileal segment towards the bladder than tubularized ileocystoplasty and so we can expect detubularized ileocystoplasty gives better clinical outcome.

ABSTRACT

The free radicals generated during the ischemic/reperfusion period have been suggested as a possible cause for tissue damage in organs. This state was conducted to investigate protective effects of free radical scavengers through changes in the lipid peroxide content of the testis when administered before reperfusion of testicular torsion. These protective agents include superoxide dismutase(SOD), an oxygen radical scavenger and dimethylthiourea(DMTU), a hydroxyl radical scavenger. Forty five Sprague-Dawley rats were divided into 9 groups, each containing 5 rats. Group 1 served to determine basal values of the lipid peroxide content of the testis ; group 2 underwent sham operation as a control ; group 3 was subjected to unilateral testicular torsion lasting 3 hours; group 4, 5 were subjected to the detorsion lasting 1 hour and 2 hours following torsion for 3 hours; group 6, 7, 8, 9 were treated with SOD or DMTU before detorsion following torsion lasting 3 hours. The measurement of thiobarbituric acid reactive products of lipid peroxidation(TBAR) were employed to assess the extent of lipid peroxidation in testicular tissues. TBAR from sham operation control groups did not differ significantly from basal groups (p>0.05). Testicular torsion caused a significant increase in TBAR in the testis (p<0.05) when compared with basal values and detorsion caused a further significant increase when compared with values obtained after torsion. But TBAR after 1 hr detorsion did not change significantly compared with values obtained after 2 hours detorsion. The pretreatment with SOD prevented this further increase but the pretreatment with DMTU didn't. In conclusion, biochemically, reperfusion injury occurs in testis following detorsion after testicular torsion of 720 lasting 3 hours. The pretreatment with SOD, an oxygen radical scavenger, is effective in preventing reperfusion injury.

ABSTRACT

We reviewed 28 patients with renal pelvic tumor during the period from 1974 to 1989 and 17 patients were followed up. The over all survival rate at l, 2, and 3 years was 88%, 63%, and 50%, respectively, by the Kaplan-Meier method. The stage and the grade of the tumor were the most influencing factors on prognosis. Simultaneous tumors of ureter or bladder, non-visualizing kidney on excretory urography, duration of the gross hematuria more than 6 months showed relatively poor prognosis. But development of subsequent bladder tumor did not seem to influence on prognosis.

ABSTRACT

A clinical observation was made with particular attention to radiologic diagnosis, complication and management on 69 patients of renal injury admitted to the Catholic Medical Center during the last 10 years. The results were as follows: 1. Minor renal injury was in 49 cases (71%), major renal injury in 19 cases (27.5%) and pedicle injury in 1 case (1.5%) according to Scott's classification. 2. IVU was performed in 68 cases for initial diagnostic study and 18 cases (31%) needed additional radiologic study for accurate staging. 3. CT was performed in 14 cases and its diagnostic accuracy was 93%. 4. Renal angiography was performed in 6 cases. Angioinfarction was performed in 1 case due to continuous bleeding and the other 5 cases showed localized parenchymal infarction or normal finding. 5. Sonography was performed in 6 cases for initial diagnostic study and in 3 cases for follow up study. 6. In major renal injury patients, the group which decided treatment after CT study revealed 16 7% of complication rate but the group which decided treatment without CT study revealed 38 5% of complication rate. 7. In major renal injury or pedicle injury patients, initial operative treatment group resulted in kidney loss in 70% of cases without complication but initial conservative treatment group resulted in kidney loss in 60% of cases and the delay of operation resulted in 70% of complication rate.From these results, CT is the choice of additional staging technique after initial IVU and initial surgical operation is adequate treatment procedure in major renal injury or pedicle injury patients.

ABSTRACT

Mucinous adenocarcinoma of the renal pelvis is a very rare entity. We report a case of mucinous adenocarcinoma developed in the renal pelvis with brief review of literatures. The patient was a 53 year-old woman who had a history of adenocarcinoma of the cervix at 5 years ago.

ABSTRACT

Two hundred four patients with urinary tuberculosis who underwent surgical intervention were evaluated during the period from 1970 to 1988. Surgical management included nephrectomy in 171 cases, nephrostomy in 35 cases, partial nephrectomy in 4 cases and reconstruction of ureteral stricture and contracted bladder in 57 cases. Of 267 cases, 3 cases of ureteral reimplantation and 6 cases of operation performed with intestinal segment were failed. For the success of operation performed with intestinal segment, creatinine lower than 2.0mg/dL was inevitable. Direct method of ureteral reimplantation were all successful but 1 case of Boari operation and 2 cases of antireflux method were failure. One case of Boari operation and 2 cases of operation performed with intestinal segment were dead. The causes of death were sepsis and acute renal failure.

ABSTRACT

During the last 9 years, 6 patients of renal angiomyolipoma were managed at Catholic University Medical College. All were unilateral and were not associated with tuberous sclerosis. Preoperative diagnosis were hypernephroma (4), angiomyolipoma and renal stone. All patients were treated with nephrectomy. 17 patients were reported in Korean literature previously. In the review of the 26 cases (23 pts.), 16 cases were diagnosed as angiomyolipoma preoperatively and other as hypernephroma(9) and renal stone(1). Almost all cases(22) were managed with nephrectomy and only 4 cases were managed with conservative treatment (arterial embolization or observation).

ABSTRACT

Furosemide acts primarily on the thick ascending limb of the loop of Henle and inhibits the chloride transport in this site, which is the main mechanism of diuretic action of furosemide However, the precise molecular mechanism of diuretic action of furosemide is still unknown. Recent studies have shown that cGMP might be involved in diuretic effect of furosemide. In this study, the effects of furosemide on the renal tissue level of cGMP in vivo and on the renal guanylate cyclase in vitro were investigated. Also, the influence of aspirin on these effects was examined. The results were as follows: 1. The renal tissue level of cGMP was increased after administration of furosemide, but decreased after administration of aspirin. A combined administration of furosemide and aspirin increased the renal tissue level of cGMP, but the degree of elevation was less than those of the furosemide group. 2. The renal guanylate cyclase activity was slightly increased by furosemide, but this increase was not significant. The renal guanylate cyclase activity was significantly increased by arachidonic acid. Furosemide potentiated the effect of arachidonic acid on renal guanylate cyclase activity, which was inhibited by aspirin. These results indicate that effect of furosemide on renal tissue level of cGMP may be indirect effect that furosemide activates guanylate cyclase by means of increasing prostaglandin synthesis.

ABSTRACT

Testosterone therapy in high doses produces male infertility. There are reports that atrophy of the interstitial cells and decrease in testicular size occur in the rat after treatment with testosterone, and the azoospermia which resulted from testosterone therapy could be reversed by simultaneous treatment with HCG. This study was evaluated for 45 days to clarify microscopic changes in the testis. A total of 60 male mice, 30 BALB/C strain, aged 3-4 weeks and with an average body weight of l2 g and 30 BALB/C strain, aged 3 months and with an average body weight of 20 g, were divided into 2 groups; for one group treatment with testosterone propionate only and another group fur treatment with testosterone propionate and HCG. The results of histological structure were as follows: 1. Mature group, treated with testosterone propionate (0.25mg/g B.W.) daily, reduced Leydig cell numbers and showed germinal epithelium atrophy, and inhibited spermatogenesis. 2. Mature group, treated with testosterone propionate (0.25mg/g B.W.) daily and HCG (2.5 I.U./g B.W.) every 3 days, showed normal variation of interstitial cell, germinal epithelium and spermatogenesis. 3. Immature group, treated with testosterone propionate (0.25mg/g B.W.) daily, increased in numbers of Leydig cells slightly and showed tortuous tubules and spermatogenesis. 4. Immature group, treated with testosterone propionate (0.2.mg/g B.W.) daily and HCG (2.5 I.U./g B.W.) every 3 days, showed normal variation of interstitial cell, germinal epithelium and spermatogenesis.

ABSTRACT

It has been postulated that seminal plasma contains a potent immunosuppressive factor(s) which play a critical role h preventing sensitization of females to sperm antigens after insemination. It has been shown that human seminal plasma (HuSePl) inhibited phytohemagglutinin(PHA) and Concanavalin A(ConA) induced blast transformation of normal peripheral blood lymphocytes. To analyze protein fractions of HuSePl and to observe suppression of cell-mediated immune response by each fraction, the authors performed the Sephadex G-100 chromatography and the lymphocyte transformation with parameters of purified PHA(PHA-P) and pokeweed mitogen(PWM) stimulation test. The results were as follows; 1. Sephadex G-100 chromatography of HuSePl yielded five fractions(Fr. 1-5). 2. The molecular weight of each fraction, assessed by sodium dodecyl sulfate polyacrylamide gel electrophoresis(SDS.PAGE), was from 14,000 daltons(D) to more than 85,000 D in Fr. 1, 42,000 D and 33,000 D in Fr. 2, 43,000 D in Fr. 3, 20,000 D in Fr. 4 and 19,000 D in Fr. 5. 3. The mean percentages .of suppression in PHA-P stimulation test by fractions isolated from HuSePl in male (n=I3) were 59.8+10.1% in Fr. 1, 59.5+23.0% in Fr. 2, 84.2+9.4% in Fr. 3, 60.9+/-2l.4% in Fr. 4, 49.4+/-23.2% in Fr. S(F=6.=8, P 0.05). The results revealed that the Fr. 3 has the strongest suppressive effect on lymphocyte transformation activity and these observations support the concept that a local immune response against sperm in the female reproductive tract is actively suppressed by a component in seminal plasma.

ABSTRACT

We report herein a human sparganosis that presented confusing features in aspect of its correct identification, The patient was 35-year-old man who had a past history of eating fried frogs since 3 years prior to his first admission in 198I. Three mature Sparganum mansoni and 69 encapsulated spargana pieces were excised from his left inguinal area, left thigh and scrotum by three consecutive operations in 19 months. The encapsulated spargana were irregular in shape and size and mummified. They were identified histopathologically as degenerated spargana by no sexual organs and many calcareous bodies in the parenchyma. Differentiation of our case from proliferating sparganosis caused by Spargarrum proliferum was discussed.

We report one case in which delayed enhanced computerized tomography (1.5 hr. later) contributed in detecting a dysplastic kidney with a single vaginal ectopic ureter that had been nonfunctioning on excretory urography and had not been detected by other diagnostic methods.

ABSTRACT

This is the report of 75 cases of chronic prostatitis, confirmed by microscopic examination of the 3 glass test of urine and the prostatic secretion and bacterial culture of the prostatic secretion, who visited Department of Urology, St. Mary's Hospital from 1 December 1980 to 30 June 1981. The results were as follows: 1. The result of microscopic examination of the 3 glass test of urine revealed that the first and second glass urine were within normal limit (over 90%), but on the third glass urine, 69 cases (92.0%)showed WBC more than 10/HPF. 2. Microscopic examination of the prostatic secretion showed WBC more than 10/HPF in 69 cases (92.0%)and second glass in 6 cases (8.0%)showed normal findings, but many bacteria were isolated on bacterial culture of the prostatic secretion. The positive findings of prostatic secretion culture were noticed in 63 cases (84.0%). Microorganism was not grown in culture of 12 cases (16.0%), which was shown over WBC 10/HPF on microscopic examination of the prostatic secretion. Therefore, if chronic prostatitis is suspected, both methods should be used in combination because many bacteria could be detected in bacterial culture even when no inflammatory findings were shown in the microscopic examination of the prostatic secretion. In diagnosis of chronic prostatitis, the combined use of the microscopic examination and bacterial culture of the prostatic secretion were more significant than urinalysis alone.

ABSTRACT

A clinical observation was made on 73 patients of renal injury at the Department of Urology, Catholic Medical College, during the period from January, 1972 to December, 1981. The results were as follows: 1. The most favorable age was 20-29 years for 31.5%. The sex ratio, male to female, was 2.6:1. 2. The most common causes of renal injury were traffic accidents for 35.6%, and then kick & blow, fall, sport and introgenic in order. 3. The renal injuries in this series were classified as minor injury (71.2%), major injury (27.4%), pedicle injury (1.4%), according to Scott classification. 4. The IVP was done in 68 patients and abnormal findings were obtained in 31 cases. The renal angiography was done in 8 patients and RGP in 5 patients. 5. The complications were noted in 10 cases.

ABSTRACT

Involvement of the vena cava by direct vascular extension of renal cell carcinoma occurs in approximately 5% of patients undergoing nephrectomy for this neoplasm. While successful, removal of caval neoplastic thrombi has been reported occasionally, it has been general impression that renal cell carcinoma extending into the vena cava carried an extremely poor prognosis. A 45 years-old male patient was admitted with painless gross hematuria and palpable mass on fight upper quadrant. Surgical exploration was done through 10th thoracoabdominal incision. A right nephrectomy and retroperitoneal lymph node dissection were done for renal cell carcinoma. It was evident on palpation that the tumor was in the It was evident on palpation that the tumor was in the inferior vena cava Partial resection of the inferior vena cava and removal of intra vena cava thrombus were performed. This patient has led a normal life for 18 months following surgery without clinical evidence of tumor. And so, we were presented with review of the literature.

ABSTRACT

In order to study the method of management of urethral injury, the clinical observation was made on the 81 cases of urethral injury who were treated at Department of Urology, Catholic Medical College, during the period from Jan. 1970 to Dec. 1980. The results were as follows: 1. In age distribution of the 81 cases of urethral injury, age of 59 cases ranged from 10 to 39 years. The main causes of urethral injury were traffic accident (46. 9%) and industrial accident (28. 4%). 2. The symptoms of urethral injury were urethral bleeding (61 cases, 75. 3%), acute urinary retention (26 cases, 32. 1%), inability to void(12 cases, 14.8%) and others. The associated injuries of the urethral injury were pubic bone fracture (45 cases, 55.6%), bladder rupture (8 cases. 9.9%), renal injury (2 cases. 2.5%) and others. 3. Among 69 cases of urethral injury in whom urethrography were performed, two cases (2.9%) had injury in the penile urethra, 22 (31. 9%) in the bulbar and 45 (66.2%) in the posterior. twenty four (34. 8%) had partial injury and 45 (65. 2%) complete. 4. Initial managements of urethral injury consisted of indwelling catheter (11 cases, 13. 6%), primary realignment with interlocking sound (40 cases, 49.4%), urethral anastomosis (5 cases, 6.2%) and suprapubic cystostomy (25 cases, 30. 9%). 5. Of the 11 cases of retaining catheter, no complication was noticed, while among the 40 cases of primary realignment with interlocking sound, the complications were urethral stricture (6 cases). urethral fistula (2 cases), wound infection (2 cases), incontinence (1 case) and bladder stone (1 case). Of the 5 cases having urethral anastomosis, a case developed urethral fistula. Of the 25 cases having suprapubic cystostomy, 7 (28%) were successfully treated only with cystostomy Two to five months after cystostomy. 18 cases having residual urethral stricture urethroplasty, were performed and the complications were urethral fistula (1 case), impotence (1 case) and incontinence (1 case). In urethral injury, the definitive diagnosis must be made by urethrography and the initial management which must be provided is catheterization without excessive force. If catheterization fails, the suprapubic cystostomy is the best alternative management, the definitive urethroplasty have to be followed 3 to 6 months later which remained urethral stricture.
