ABSTRACT

Renal transplantation is the optimal treatment for end stage renal disease and it has been improved through the development of operative methods and immunosuppressants. However some patients must receive dialysis or undergo retransplantation after a loss of the primary graft due to rejection or other causes. Recently the frequency of retransplantation has begun to increase gradually. Some articles have reported that retransplantation results do not significantly differ in comparison with initial transplantation results when living related donor kidneys are used. Our study focused on the outcome of 445 first transplantation and 12 retransplantation cases. The sex distribution of retransplanted patients was 11 male and 1 female. The mean age (yrs) for recipients was 32.3 at the first transplantation and 39.1 at the retransplantation. The underlying causes of end stage renal disease were presumed to be chronic glomerulonephritis in all retransplantion patients; the mean duration of graft survival (mo) for first transplantation was 77.92. The causes of previous graft failure were as follows: 10 due to chronic rejection, 1 due to recurrent glomerulonephritis, 1 resulted from a graft rupture due to a motorcar accident. The interval (mo) between graft failure and retransplantation averaged 6.7 and 9 out of 12 patients underwent regrafting within 1 year of their previous graft loss. Recipient-donor relationships in first transplantations were as follows: 9 were living related and 3 were living non-related. Recipient-donor relationships in second transplantations were as follows: 4 were living related and 8 were living non-related. Acute rejection within 1 month of transplantation occurred in 4 primary transplantation patients and 2 retransplantation patients. The incidence of acute rejection within 1 month was as follows: 23% of 445 first renal transplantation patients, 16.7% of 12 second transplantation patients. ABSTRACT

OBJECTIVES: The purpose of this study was to determine the prevalence and related factors of upper limb musculoskeltal symptoms among workers in automobile related jobs. METHODS: Two-hundred and twenty-one workers were volunteered in this 'study consisting of 112 seat installers and 109 auto mechanics. A questionnaire was administered to the workers consisting of questions on demographics, work type and duration, sleep hours, health habits, and standardized descriptions of NJOSH on musculoskeletal symptoms. RESULTS: Complaint rates of neck and upper extremity musculoskeletal symptoms by anatomical site were as follows: shoulder, 52.0%; neck, 47.1%; wrist and hand, 39.4 elbow, 26.2% and; arm, 24.4%. Significantly higher prevalence of shoulder musculoskele tal symptom was found in middle school than high school graduate, married than unmar ned, less than 6 hours sleep than over 6 hours sleep and, seat installer than auto mechanic group. A significantly higher prevalence of arm, elbow, hand and wrist mus loskeletal symptoms was disclosed in the less than 6 hours sleep than over 6 hours sleep group and the seat installer than the auto mechanic group. Multiple logistic regression analysis identified education, sleep hours and, job type as the main affecting factors for shoulder musculoskeletal symptoms. Sleep hours and job type were the main affecting factors for wrist and hand musculoskeletal symptoms. Sleep hours were the main affecting factors for arm and elbow musculoskeletal symptoms. CONCLUSIONS: The prevalence of neck and upper limb musculoskeletal symptoms was high in workers who did routine tasks at confined places like seat install-line work. It is recommended that workers performing simple and routine tasks be provided adequate sleep time since lack of sleep was the main affecting factor for most upper limb mus loskeletal symptoms in automobile factory related jobs workers.

ABSTRACT

Delayed cutaneous hypersensitivity test had been demonstrated to correlate inversely with stage of disease and predict prognosis of tumor. We evaluated the delayed cutaneous hypersensitivity(D.C.H.) with Multitest CMI between the tumor group and normal control group in Department of Urology, Kosin, Medical College Hospital during the period from October 1986 to August 1988. The result were as follows: 1. D.C.H. with Multitest CMI was compared between the normal control and tumor group: mean total score of positive antigens was markedly decreased in the tumor group(11.3mm for tumor, 15.9mm for control p<0.01) and average number of positive reaction was also decreased in the tumor group(2.6 for tumor, 4.2 for control p<0.01). 2. The percentages of energic response were 11.9% for tumor and 0% for control and the incidence of hypoenergy was also increased in the tumor group (34.2% for tumor, 9% for control). 3. The percentages of Warning score were 27.6% for tumor and 6.8% for control. 4. In comparison with each antigens, there were the strongest reaction and the highest reaction with tuberculin in both group (98.3% for control, 82.9% for tumor) while the weakest reaction with tetanus (16.3% for control, 3.9% for tumor). 5. In comparison of D.C.H. between male and female, low cell mediated immunity was observed in female for the average number of positive antigens in control(4.8 for male, 3.7 for female) and for the average store of in tumor (11.7 mm for male, 9.6 mm for female). 6. In D.C.H. between age groups in control, the youth (17-25 years old) showed the strongest reaction (mean total score 19.1 mm) and 26-65 years old group had relatively stable immunity.

ABSTRACT

Renal lymphoma is usually clinically silent and occurs late in the disease, after the diagnosis of generalized lymphoma has been established. Rarely, however, renal disease may be the earliest manifestation. Herein we report a case of malignant lymphoma involving kidney in a 41-year-old male who underwent radical nephrectomy because of a presumptive diagnosis of renal cell carcinoma.

ABSTRACT

We evaluated the urologic complications of extravesical ureteroneocystostomy in 60 renal transplantation performed in the Department of Urology, Kosin Medical College during the period from December 1984 to August 1987. The results were as follows 1. Of 60 cases of recipient 39 cases were male and 21 cases were female. In donor and recipient`s relationship, 53 cases related and 7 cases were unrelated. 2. Our incidence of postoperative urologic complications of extravesical ureteroneocystostomy in 60 renal transplantation was 5 cases(8.3%). 3. The details of complications were as follow : 4 cases(6.7%) were ureteral obstruction due to perirenal hematoma and 1 case(1.6%) was ureteral fistula. 4. Treatment of complications were immediate perirenal hematoma removal in ureteral obstruction(4) and silastic drain in ureteral fistula(1). 5. Results and prognosis in complicated cases were good.

ABSTRACT

The horseshoe kidney results from fusion of the two renal blastema at the time when they lie closeset together, usually at the fifth or sixth weeks of intrauterine life and occurs in 0.25% of the general population. Hydronephrosis, infection, calculus formation and concomitant other malformation are associated in the one third of patients. The duplicated ureter occurs in 6 to 10% congenital associated anomalies. This report concerns a case of rare combination of horseshoe kidney, bilateral duplication of ureter with obstruction of one ureteropelvic junction causing symptomatic hydronephrosis due to renal artery of right low moiety, which was treated with heminephrectomy.

ABSTRACT

The ureteral fistula is the most common urologic complication and it usually occurs within the first 3 weeks after renal transplantation. The dominant cause is damage to the arterial supply of the transplanted ureter by close dissection at time of donor nephrectomy. The signs and symptoms of ureteral fistula can mimmic acute rejection, which is oliguria, anuria, fever, wound tenderness, swelling and decreased renal function. It was well established that open intervention was standardized for such problem. Recently nonoperative percutaneous approaches to the placement of stents and tubes are used. Herein we report a case of ureteral fistula, misdiagnosed as acute rejection after renal transplantation, which was healed with Silastic drain insertion in a 22-year-old male recipient.

ABSTRACT

Malignant lymphoma of testis is rare, comprising 5% of all testicular tumor, but it is the most common type of testicular tumor in patients more than 5 years old. And malignant lymphoma of urinary bladder is extremely rare, while the majority of the affected patients fall within the to 60 age 40 group and is more common in female. Prognosis of the testicular lymphoma is very poor, and the interval free of disease is short and 9O % of patients die of generalized lymphoma within 2 years. Although in contrast to other nonepithelial malignant tumor, primary lymphoma of bladder carries a relatively favorable prognosis but secondary or disseminated bladder lymphoma has poor prognosis. This report concerns a case of simultaneous involvement of both testes and bladder by malignant lymphoma, which was treated with bilateral radical orchiectomy, transurethral resection of bladder and anticancer chemotherapy.
