ABSTRACT

The concentrations of Sodium and Potassium were measured by flame photometer in the human for milk obtained at clostral(1st 5days postpartum), early transitional (2nd 5days postpartum), late transitional (11th to 29th day postpartum) and mature milk period (1to 15 months postportum) from 92 healthy nursing mothers who delivered at term. The results were summerized as follows: 1) The concentrations of Na and K at different stages of lactation showed the highest value in colostrum, tended to decrease therafter and maintained the lowest and nearly constant value in mature milk(.01 2) Average Na and K concentrations(Mean+/-S.D., mEq/L) at different stages of lactation were colostrum : 20.8+/-4.47, 15.9+/-3.17, early transitional milk : 15.3+/-4.73, 14.2+/-3.03, late transitional milk : 11.7+/-4.28, 12.8+/-3.40, and mature milk : 8.1+/-3.59, 11.2+/-2.88 respectively. 3) The content of Na and K among primi-and multiparae showed the highest value in colostrum, also tended to decrease therafter, and maintained the lowest value in mature milk. No statistical significance was found between the concentrations of Na and K at different stages of lactation between primi-and multiparae(p>.05). 4) Definite inverse relationship could be established between Na and K concentrations and days of lactation(Na:r=-0.6, p<.001, K:r=0.4, p<.001). 5) The Mean Na and K concentration in late transitional milk showed the most consistent percentage decrease that in colostrum.

ABSTRACT

A clinical study was done on 199 cases of Acute Glomerulonehritis, who had been admitted to the department of Pediatrics, Chonnam National University Hospital during the five years period from January 1976 to December 1980. The following results were obtained. 1) Males were affected more frequently than females by a 2:1 ratio, and the highest incidence was noted in children between 4 and 6 years of age, with a seasonal peak in Autumn and Winter. 2) The 57.8% of the total patients had history of preceding infections. The upper respiratory tract infection was most common, which was 51.3% of the total patients, and the skin infection was identified in 4.0% of the total patients. 3) The most common chief complaint was edema. Blood pressure of greater than 90mmHg in diastolic pressure was noted in approximately half of the patients. 4) Chest X-ray revealed cardiomegaly in21.9%, pulmonary edema in 12.8%, and pleural effusion in 9.1%. 5) The percentage of positive beta-hemolytic streptococcal culture was 12.3%, and Anti-Streptolysin O titer of greater than 333 Todd units was noted in 64.5%. 6) The mean value of serum C3 concentrationin the Acute Glomerulonephritis Group as a whole was lower than that of the Control Group and the difference was statistically significant(p<.001). 7) Gross hematuria and edema disappeared in most of the patients within 2 weeks after the onset of the symptoms, and the elevated blood pressure returned to normal within 2 weeks after admission. 8) Microscopic hematuria and proteinuria disappeared in most of the patients within 3 months after admission.

ABSTRACT

Protein, and essential nutrient for the growth of children, is the chief nitrogenous substant in the serum of humans. For the purpose of establishment of the normal data for evaluation of nitrogen balance, we examined the serum levels of urea nitrogen, creatinine, uric acid and serum amylase activity as well as that of protein in 560 normal school children. The results are as follows; 1. The serum level of protein is 6.1+/-0.75mg%, albumin; 3.9+/-0.57gm%, globulin ; 2.2 +/- 0.77gm%, urea nitrogen ; 14+/-3.8mg%, creatinine; 0.5+/-0.20mg%, uric acid; 3.9+/-0.91mg%, and the serum amylase activity is 127+/-41.4% units/dl. And the reference rnage of the above data is 4.6~7.6gm%, 3.3~4.4mg%, 1.5~3.0gm%, 7~22 mg%, 0.3~0.7mg%, 2.1~5.7mg% and 45~210 units/dl, respectively. 2. There is evidence of age relationship in the serum level of urea nitrogen which tends to increase with age. 3. We observed sex differences in the serum levels of total protein and albumin examined and those of urea nitrogen and creatinine examined, female was predominant in the former and male in the latter.

ABSTRACT

We experienced a case of cryptococcosis in a 13-year-old female who had been admitted to our hospital because of intermittent high fever and generalized lymphadenopathy accompanied by multiple nodular eruptions with erosions on the face, scalp, neck and back. The patient was treated under the impression of tuberculous lymphadenitis without any improvement. A surgical biopsy of the lymph node and the subcutaneous nodule, which was performed 14 days later, revealed chronic granulomatous tissue and Langhan's type giant cells which contained numerous cryptococci. A brief review of the literature was done.

ABSTRACT

Forty-Five cases of Shigellosis were treated with Ampicillin, TMP/SMX and Rifampin from April 1980. to November 1980. Of the 18 strains of shigellae, in-vitro sensitivity test was performed against twelve antimicrobial agents. The percentage of resistant strains was 77.8% in Ampicillin and 100% in TMP/SMX. Inhibition zone diameter by Rifampin disc was 8~10mm in all cases and clinical improvement with treatment was noted in nearly all cases, therefore we regarded inhibition zone diameter above 8mm sensitive to Rifampin. In clinical evaluation, the percentage of effectiveness by antibiotics was as follows; Ampicillin-60%, TMP/SMX-70% and Rifampin-93.3%. Rifampin appears to be the best, available drug bacteriologically and clinically for the treatment of Shilgellosis.

ABSTRACT

To evaluate the hepatotoxicity of RMP in tuberculous children during treatment, liver function tests were performed regularly in 40 children who visited or admitted in Chonnam University Medical School Hospital and Mok-po Children's Tuberculosis Hospital from January 1979 to August 1980. The subjects were divided into three groups according to the administered regimens which were INH+EMB or PAS, INH+RMP(10??15mg/kg) and INH+RMP(16??20)mg/kg respectively. The mean observation period wart 16 weeks and the obtained results were compared between each group which were summerized as follows; 1) Significant increase of serum transaminase activity was noted in the patients who received RMP. 2) Increase of serum transaminase activity developed within 4 weeks after the administration of RMP and the mean value was highest in the second week. 3) The incidence of increased serum transaminase activity was slightly higher in the female subjects. 4) No relationship could be found between the increase of serum transaminase activity and the dosage of RMP. 5) Serum alkaline phosphatase showed no significant difference among three groups. 6) Serum bilirubin concentration was not increased in all subjects.

ABSTRACT

Pneumomediastinum, pneumothorax and subcutaneous emphysema are uncommon in pediatric practice, but they may be rarely ocured in association with respiratory distress and excessive ressusciation in the neonatal period, and as a complication of bronchial asthma, measles in childhood. And also, there was a report of a few cases that developed to pneumomediastinum and subcutaneous emphysema caused by vomiting in patient of diabetic coma. It is rare so far in this country that above events are complicated by miliary tuberculosis, but they may lead the patient to fatal and lifethreatening conditions, even though we perform a considerable medical treatment for that. We presnted 3 cases of 6 month 20 days old female infant, and each 10 year and 11 year old boys, developed to pneumomediastinu, pneumothorax and subcutaneous emphysema, and suddenly expired during hospital treatment of miliary tuberculosis. And, we reviewed the literatures, too.

ABSTRACT

To study the effect of anticancer drugs on the RESFunction, cyclophosphamide and 6-mercaptopurine were intramuscularly injected and evaluated the reticuloendotherial phagocytic activity by the carbon clearence method in rats. Experimental groups were subdivided by the dosage, into the small and large dosage groups, and by the duration of treatment, into the 1, 3, 5 days and 8days (3 days cessation after 5 days treatment) groups. The results obtained were as follow. 1. Cyclophosphamide-treated groups showed the splenic atrophy which was in proportion to duration of treatment. RES phagocytic activities seemed to be slightly increased in large groups, but to be within normal ranges in ranges in small dosage groups, which were normalized in the 8 days groups. 2. 6-Mercaptopurine-treated groups didn't show the hepatic and splenic alterations significantly. RES phagocytic activities were variably changed as in triphagic curve fashion in both small and large dosage subgroups of 1, 3, 5 days treated groups, which were normalized in the 8 days groups.

ABSTRACT

A Clinical observation on 272 cases of epidemic encephalitis admitted to the Department of Pediatrics, Chonnam University Hospital with 5 year duration from 1971 to 1975 was carried out and the serum protein fractions were studied paperelectrophoretically on 28 cases in 1975 The following results are obtained. : 1) The peak age group was from 6 to 10 years old, and the male to female ratio was 1.7:1 2) The complaints on admission were fever(100%), neck stiffness(78.3%), Kernig's sign(71.7%), vomiting(64.0%), unconsciousness(59.6%), and headache(57.4%), in order of frequency. 3) Laboratory findings were as follows ; In the most cases(79.4%), peripheral leukocytosis with increased polymorphocyte was noted. And the findings of C.S.F. showed; Pleocytosis (98.5%), increased protein content(90.1%), normal or slightly increased sugar content(97.8%) and within normal limits of chloride level. 4) Mortality rate was 20.2% 5) The total protein and beta-globulin level showed no change and decreased albumin and A/G ratio. The gamma-globulin level showed increased initially and thereafter decreased to normlal limits gradually.
