ABSTRACT

Objective:To observe the clinical efficacy as well as anti-inflammatory and anti-fibrosis mechanism of Yishen Tongluo decoction in the treatment of chronic glomerulonephritis (CGN) with spleen and kidney Qi deficiency and blood stasis syndrome. Method:According to the random number table method, one hundred and twenty patients were divided into control group (60 cases) and observation group (60 cases). The two groups were given oral valsartan capsules, 160 mg/time, 1 time/day, and dipyridamole tablets orally, 50 mg/time, 3 times/day. Patients in control group additionally took Wubi Shanyao pills orally, 9 g/time, 2 times/day, while patients in observation group additionally took Yishen Tongluo decoction orally, 1 dose/day. Both groups were treated continuously for four months. Before and after treatment, 24 h urine total protein (24 h UTP), creatinine (SCr), urea nitrogen (BUN), serum cystatin C (CysC), tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>), interleukin-6 (IL-6), IL-17, interferon-<italic>γ</italic> (IFN-<italic>γ</italic>), transforming growth factor-<italic>β</italic><sub>1</sub> (TGF-<italic>β</italic><sub>1</sub>), connective tissue growth factor (CTGF), matrix metalloproteinase-9 (MMP-9), tissue inhibitor of metalloproteinases (TMP-1) and hypoxia inducible factor-1<italic>α </italic>(HIF-1<italic>α</italic>) levels were detected in both groups. The scores of spleen and kidney Qi deficiency and blood stasis syndrome were graded. Urine protein, and urine red blood cells (urinary RBC) were monitored. Result:The 24 h UTP, SCr, BUN and CysC levels of the observation group were lower than those of the control group (<italic>P</italic><0.01). The score of spleen and kidney Qi deficiency and blood stasis syndrome in the observation group was lower than that in the control group (<italic>P</italic><0.01). The levels of TNF-<italic>α</italic>, IL-6, IL-17 and IFN-<italic>γ </italic>in the observation group were lower than those in the control group (<italic>P</italic><0.01). The levels of TGF-<italic>β</italic><sub>1</sub>, CTGF, TMP-1 and HIF-1<italic>α</italic> in the observation group were lower than those in the control group (<italic>P</italic><0.01). While MMP-9 level was higher than that in control group (<italic>P</italic><0.01). The clinical effective rate was 88.33% (53/60) in the observation group, higher than 73.33% (44/60) in the control group (<inline-formula><alternatives><mml:math xmlns:mml="http://www.w3.org/1998/Math/MathML" id="M2"><mml:msup><mml:mrow><mml:mi>χ</mml:mi></mml:mrow><mml:mrow><mml:mn mathvariant="normal">2</mml:mn></mml:mrow></mml:msup></mml:math><graphic specific-use="big" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/155A9E27-C0C9-44b4-96FF-AF947372054E-M002.jpg"><?fx-imagestate width="3.30200005" height="3.64066648"?></graphic><graphic specific-use="small" xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="alternativeImage/155A9E27-C0C9-44b4-96FF-AF947372054E-M002c.jpg"><?fx-imagestate width="3.30200005" height="3.64066648"?></graphic></alternatives></inline-formula>=4.356, <italic>P</italic><0.05). Conclusion:On the basis of conventional Western medicine treatment, Yishen Tongluo decoction in the treatment of CGN patients with spleen and kidney Qi deficiency and blood stasis syndrome can reduce proteinuria, and improve traditional Chinese medicine (TCM) syndromes, with anti-inflammatory and anti-kidney fibrosis effects. Thereby, it plays a role in protecting renal function and delaying the malignant progression of renal function, with high clinical efficacy and value of clinical use.

ABSTRACT

AIM To prepare nanostructured lipid carriers for volatile oils from Artemisiae argyi Folium.METHODS Heated melting-ultrasonic dispersion method was applied to preparing lipid carriers.Taking solid/liquid lipid ratio,amounts of lipid,emulsifier and volatile oils as influencing factors,and average paticle size as an evaluation index,the formulation was optimized by orthogonal test.With cineole,camphor and borneol as indices,GC-MS was adopted in the content determination of volatile oils.RESULTS The optimal formulation was determined to be 5 ∶ 5 for solid/liquid lipid ratio,1%,3% and 0.5% for amounts of lipid,emulsifier and volatile oils,respectively.The obtained clear and transparent lipid carriers demonstrated the average particle size of (72.33 ±1.93) nm,PDI of 0.273 ± 0.004 5,and Zeta potential of (-30.59 ± 1.42) mV,whose in vitro release rate was lower than that of raw medicine within 120 h,along with a higher stability under 4 ℃ than that under 25 ℃.The entrapment efficiencies of cineole,camphor and borneol were 87.49%,86.45% and 92.12% with the drug loadings of 8.25%,2.00% and 3.38%,respectively.CONCLUSION It is suggested that nanostructured lipid carriers for volatile oils from Artemisiae argyi Folium should be stored under 4 ℃ with the features of sustainedrelease and stable physicochemical properties.

ABSTRACT

<p><b>BACKGROUND</b>It is well recognized that meteorological factors have important infuences on the onset and development of many kinds of diseases. The present study was undertaken to investigate the effects of the meteorological elements on admission rates of cerebral infarction patients with hypertensive nephropathy at Changchun city, Jilin Province, northeast China.</p><p><b>METHODS</b>A total of 763 medical records of inpatients from nine hospitals at Changchun city, during a period from April 6 to April 17 in 2010, were reviewed. These patients were admitted to hospitals due to the occurrence of cerebral infarction. The hypertensive nephropathy was evidenced with certain diagnosis of essential hypertension and hypertension-related kidney injuries. The cerebral infarction was diagnosed according to the World Health Organization (Stroke) standard. All the meteorological data were from practical monitoring records in Jilin Province Meteorological Observatory. The relationships between the epidemiological prevalence of cerebral infarction and meteorological variables were analyzed using the time series models of statistics.</p><p><b>RESULTS</b>Compared with admission rates before the violent change in meteorological status (April 6 to April 17, 2010), the number of admission patients suffering from cerebral infarction remarkably peaked on April 12. Such an increase was highly correlated with heavy precipitation, elevation of daily average relative humidity, and reduction of average daily air temperature. With the betterment of the meteorological conditions on April 17, the admission rates of cerebral infarction patients dropped to the same level as the dates before snowing (April 6 to April 11).</p><p><b>CONCLUSIONS</b>The meteorological changes are highly associated with the occurrence of cerebral infarction in patients with hypertensive renal injury in northeast China. This study also suggested that an intensive medical interference for those patients with hypertension-induced organ injuries is very necessary in preventing the occurrence of cerebral infarction with hypertensive nephropathy when there is a violent change in meteorological condition.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate whether ultrasonic microbubble destruction (US/MB) could enhance the therapeutic effects of hepatocyte growth factor (HGF) gene transfer for acute myocardial infarction (MI).</p><p><b>METHODS</b>MI was induced by left anterior descending artery ligation in male SD rats. Two to 4 hours thereafter, MI rats were randomly treated with tail vein infusing pc-DNA3.1-HGF plasmid mixed with microbubbles (US/MB-HGF group, n = 18); tail vein infusing pc-DNA3.1-HGF plasmid mixed with saline (US-HGF group, n = 18); tail vein infusing empty plasmid mixed with microbubbles (US/MB-P group, n = 18). All rats were exposed to ultrasound treatment thereafter till contrast imaging disappeared in cardiac region. Rats were sacrificed at 24 hours, 7 days or 14 days, respectively (n = 6 each) and myocardial protein expression of bcl-2 and HGF as well as microvascular density (MVD) were determined.</p><p><b>RESULTS</b>The myocardial protein expressions of bcl-2 and HGF in US/MB-HGF group were significantly higher than those in US-HGF and US/MB-P groups at 7 days post MI (all P < 0.01) and MVD was significantly higher in US/MB-HGF group (367.6 +/- 17.6) than that in US-HGF (268.9 +/- 0.8) and US/MB-P (186.8 +/- 11.8) groups (all P < 0.05) at 14 days post MI.</p><p><b>CONCLUSIONS</b>Ultrasound-mediated microbubble destruction could enhance systemic HGF administration induced myocardial angiogenesis and reduce systemic HGF administration induced myocardial apoptosis in rats with acute MI.</p>
