ABSTRACT

OBJECTIVE To evaluate the impact of mastoid obliteration withsingle-pedicle muscle flap covered by bone pate on vestibular stimulation. METHODS A retrospective study was performed on 59 patients who were treated for chronic otitis media with or without cholesteatoma by two techniques: canal wall down tympano-mastoidectomy(CWD) and subsequent mastoid obliteration(MO). The postoperative vestibular functions of all the patients in both groups were assessed by vestibular function tests and questionnaires. Finally, the data of examination a nd symptoms were a nalyzed. RESULTS After a minimum follow up period of 12 months, the rate of ear dry was 84%(22/26) for MO group and 55%(18/33) for CWD group(χ2=4.72, P <0.05). The dry ear time were 5.46±1.39 weeks for MO group and 8.67±2.3 weeks for CWD group(t =6.2529, P <0.05). When compared latent period of Caloric testing in the MO group (10.3±2.57)s and CWD group (12.7±3.33)s, significant differencewas found(t =3.1639, P <0.05). The postoperative caloric vestibular tests revealed an average nystagmus count of 52.96±20.82 beats per minute in the MO group and 69.94±18.98 beats in the CWD group(t =3.2688, P <0.05). By analyzing the questionnaire, 30%(10/33) of the patients who received CWD treatment reported vertigo by caloric stimuli such as wind, water compared with MO group(0)(χ2=7.45, P <0.05). The rate of suction cleaning induced vertigo was 48%(16/33) in CWD group and 23%(6/26) in MO group(χ2=3.17, P =0.075). CONCLUSION Our technique of mastoid obliteration with single-pedicle muscle flap covered by bone pate results in small cavities with complete epithelialization of all surfaces. Furthermore, obliteration of mastoid cavities provides protection to the labyrinthine organ and reduces postoperative vertigo to caloric stimulation.

ABSTRACT

The effect of paper publication time on knowledge discovery in biomedical literature was studied by analyzing the role of time factor in indirect link mining and tested using the PubMed-covered data set, which showed that recently published papers played a better role than early published papers in knowledge discovery, indicating that the time factor plays a certain role in knowledge discovery in biomedical literature and can thus improve the efficiency of knowledge discovery in biomedical literature.

ABSTRACT

Objective To observe the infiltration situation of Langerhans cells(LC)in laryngeal squamous cell carcinoma tissue and to investigate its clinical significance. Methods Seventy-two cases of laryngeal squamous cell carcinoma were selected and the tumor tissue in all cases was operatively resected. The immunohistochemical SP method was adopted to detect the infiltration situation of LC in laryngeal squamous cell carcinoma tissue. Then the relationship between the LC infiltration degree with tumor stage, relapse, metastasis and prognosis was analyzed. Results Among 72 cases of laryngeal squamous cell carcinoma,27 cases(37.5%)were LC infiltration degree of +,37 cases(37.5%)were ++ and 18 cases(25.0%)were +++.The LC infiltration degree in laryngeal carcinoma tissue was correlated with the cervical lymph node metastasis, recurrence, disease free survival,3-year survival and 5-year survival(P<0.05),and had no correlation with the age, classification,TNM stage and postoperative radiotherapy(P>0.05).Conclusion LC infiltration can be seen in the tumor tissue of the patients with laryngeal squamous cell carcinoma. The LC infiltration degree is higher, indicating that the prognosis would be better.

ABSTRACT

Objective To explore the clinical effect of application of mechanical insufflation-exsufflation (MI-E) for airway mucus clearance in patients with artificial airway. Methods 72 patients were divided into the observation group (36 cases) and the control group (36 cases)according to admission order by random number table. All subjects were given with individual treatment and nursing, while the control group was given with the application of rock sputum ejection machine twice per day,the observation group was given with application of both cough assist and rock sputum ejection machine for sputum excretion. The clinical effect of airway mucus clearance after 5 days' treatment was compared between two groups. Results Compared with the control group, oxygenation index after 5 days' treatment in the observation group increased [(48.16±9.57) kPa vs.(37.99±14.67) kPa,t=-3.48,P<0.01],clinical pulmonary infection score [(3.39±0.77) scores vs.(5.33±1.47) scores] and procalcitonin level [0.10 (0.07,0.27) μg/L vs.0.43 (0.12,0.79) μg/L] were lower (t=-7.02,Z=-3.28,P<0.01),the mean sputum amount was bigger [(3.64±0.43) ml vs.(2.31 ±0.44) ml] (t=-4.84,P<0.01),and the mean interval of suction was longer [(97.81 ±7.28) min vs.(82.00±8.88) min](t=-3.08,P<0.05).Conclusions MI-E can clear the airway secretion effectively so as to improve oxygenation and the inflammatory markers. And MI-E is a simple, noninvasive and safe technique.

ABSTRACT

A biomedical entity association evolution network was constructed by mining the implicit associations in PubMed-covered literature, which can help scientific researchers to form new scientific hypotheses, to analyze the topological features of associated network, to study the scientific literature-enriched knowledge structure, associa-tions, development rules, to introduce new visual angles and methods for literature-based knowledge discovery, and to improve the knowledge discovery efficiency.
