ABSTRACT

OBJECTIVE: To evaluate the use of epinephrine in arthroscopic infusion serum as a measure to improve the quality of surgical viewing during procedures for treating rotator cuff tears. METHODS: This was a prospective randomized double-blind comparative study in which 49 arthroscopic repair procedures on rotator cuff tears were evaluated. Patients presenting ASA I and II surgical risk were included. The patients were placed into two groups: the first with epinephrine (1 mg/L) in the infusion serum and the second with pure physiological solution. A single surgeon was responsible for the procedures, without knowledge of the medication usage. The surgeon rated his quality of viewing during the operation, on an increasing scale from 0 to 10. Interscalene block or suprascapular nerve block was chosen randomly and used in association with general anesthesia. The anesthetist issued final report relating to possible intercurrences. RESULTS: The group with epinephrine received an average score of 9.29 and the group without epinephrine received an average score of 7.16. The difference was statistically significant (p < 0.05). There was no important clinical alteration relating to use of this drug. CONCLUSION: As well as being safe, addition of epinephrine at a concentration of 1 mg/L to the infusion serum was shown to be effective for improving the visual field during arthroscopy to repair rotator cuff injuries.. .


OBJETIVO:Avaliar o uso da epinefrina no soro de infusão artroscópica como medida para melhorar a qualidade da visualização cirúrgica durante procedimentos de tratamento das rupturas do manguito rotador. MÉTODOS: Este é um estudo prospectivo, randomizado, comparativo e duplo cego, em que foram avaliados 49 procedimentos de reparo artroscópico da lesão do manguito rotador. Foram incluídos pacientes com risco cirúrgico ASA I e II. Os pacientes foram colocados em dois grupos, contendo o primeiro grupo epinefrina (1mg/L) no soro de infusão e o segundo grupo solução fisiológica pura. Um único cirurgião foi responsável pelos procedimentos sem conhecimento sobre o uso da medicação. O cirurgião atribuiu uma nota crescente (0-10) referente à sua visualização durante o ato operatório. O bloqueio interescalênico ou o bloqueio no nervo supraescapular foram associados à anestesia geral, escolhidos de forma aleatória. O anestesista emitiu um relatório final referente às possíveis intercorrências. RESULTADOS : O grupo que recebeu epinefrina obteve nota média de 9,29 e o grupo que não recebeu obteve a nota média de 7,16. A diferença foi estatisticamente significativa (p < 0,05). Não houve alteração clínica importante relacionada ao uso do fármaco. CONCLUSÃO : Além de segura, a adição da epinefrina na concentração de 1mg/L ao soro de infusão se mostrou eficaz quanto à melhoria do campo visual durante a artroscopia para reparo das lesões do manguito rotador. .

ABSTRACT

PURPOSE: To investigate abdominal wound healing using specific biomaterials in incisional hernias. METHODS: Incisional hernias were produced in 40 rabbits, after that they were reoperated with or without the use of meshes: PREMILENE® (PPL), ULTRAPRO® (UP), PROCEED® (PCD) or repairing without mesh (TRANSPALB). After 30 days a macroscopic and microscopic study of the part withdrawn from the abdominal wall was performed. RESULTS: Macroscopic: adhesion Area: PPL> UP and PCD (p = 0.031). Vascularization: PPL> UP and PCD (p = 0.001). PPL groups (p = 0.032) and PCD (p <0.001) showed greater meshes shrinkages when compared to UP. Microscopic: neutrophils: PCD> PPL, UP and TRANSPALB (p = 0.010); eosinophils: PPL> UP, and TRANSPALB PCD (p = 0.010); granulation tissue: PPL and PCD> UP and TRANSPALB (p <0.001); macrophages : PPL, UP and PCD> TRANSPALB (p <0.001); lymphocytes: PPL and PCD> UP (p = 0.009) and TRANSPALB (p <0.001); giant cells: PPL, UP and PCD> TRANSPALB (p <0.001); viscera adhered: PPL and UP> PCD and TRANSPALB (p <0.001). CONCLUSION: All types of meshes caused the formation of adhesions. The UP and PCD groups showed lower area and vascularization of the adhesions. The PPL and PCD groups showed higher meshes shrinkage and there was a predominance of acute inflammatory process in the PCD group.

ABSTRACT

PURPOSE: To compare the wound healing of the abdominal wall of rabbits exposed to nicotine and submitted to abdominoplasty using 2-octyl cyanoacrylate or nylon thread for the surgery suture. METHODS: Thirty two rabbits were used. They were divided in subgroups: A1, A2, B1 e B2. Group A received saline 0.9%; group B received nicotine, both groups for 14 days before surgery. We performed an abdominoplasty with a nylon suture into the A1 and B1 subgroups; as for A2 and B2 groups the suture was performed with cyanoacrylate. The euthanasia happened in the 14th post-operative day. After, we evaluated: swollen process, fibroblast proliferation, collagen, neovascularization, and macroscope and microscope epithelization of the scars. RESULTS: We observed the presence of eosinophils in all scars exposed to the cyanoacrylate, and a significant increase of neovascularization in the subgroup B2 comparing to the A2 one (p=0.037). The other variables haven't showed any statistical difference. CONCLUSIONS: Nicotine hasn't influenced the swollen process, the fibroblast proliferation, the presence of collagen, neither the epithelialization. The neovascularization showed cicatricial immaturity when comparing group A2 to group B2. The eosinophils in the scars repaired with glue showed that the substance has acted as an allergen.


OBJETIVO: Comparar a cicatrização da parede abdominal de coelhos expostos à nicotina e submetidos à abdominoplastia utilizando 2-octil cianoacrilato ou nylon na síntese cirúrgica. MÉTODOS: Utilizou-se 32 coelhos. Estes foram distribuídos em subgrupos: A1, A2, B1 e B2. O grupo A recebeu solução de NaCl 0,9%; o B recebeu nicotina, ambos durante 14 dias do pré-operatório. Nos subgrupos A1 e B1 foi realizada abdominoplastia e sutura com "nylon"; enquanto A2 e B2 a síntese ocorreu com cianoacrilato. A eutanásia ocorreu no 14º dia do pós-operatório. Na pesquisa avaliou-se: processo inflamatório, proliferação fibroblástica, colágeno, neovascularização, epitelização macro e microscópica das cicatrizes. RESULTADOS: Observou-se presença de eosinofilia em todas as cicatrizes expostas ao cianoacrilato, e aumento significativo da neovascularização no subgrupo B2 em comparação com o A2 (p=0,037). Demais variáveis não apresentaram diferença estatística. CONCLUSÕES: A nicotina não influenciou o processo inflamatório, a proliferação fibroblástica, a presença de colágeno e a epitelização. A neovascularização indicou imaturidade cicatricial na comparação dos grupos A2 e B2. A eosinofilia nas cicatrizes reparadas com cola indica que a substância atuou como alergeno.
