ABSTRACT

Objective:To investigate the feasibility of deep learning radiomics model in the prediction of neoadjuvant chemotherapy (NAC) response in breast cancer based on ultrasound images at an early stage.Methods:Between January 2018 and June 2021, 218 patients with breast cancer who underwent NAC were enrolled in the retrospective study. All patients received a full cycle of NAC before surgery and underwent standard ultrasound examination before NAC and after the second cycles of NAC. Of all the patients, 166 patients came from institution 1 (the First Affiliated Hospital of Nanjing Medical University) were allocated into a primary cohort.Based on the architecture of Resnet 50 convolutional neural, a deep learning prediction model was built.Further validation was performed in an external testing cohort ( n=52) from institution 2 (General Hospital of Eastern Theater Command, PLA). The clinical model was constructed using independent clinical variables. To evaluate the predictive performance, areas under the curve (AUCs) of these models and two radiologists were compared by using the DeLong method. Results:The Resnet 50 model predicted the response of NAC with accuracy. The deep learning model, achieving an AUC of 0.923 (95% CI=0.884-0.962) in the primary cohort and an AUC of 0.896 (95% CI=0.807-0.980) in the test cohort, outperformed the clinical model and also performed better than two radiologists′ prediction (all P<0.05). Furthermore, the two radiologists achieved a better predictive efficacy (AUC 0.832 and 0.808 for radiologists 1 and 2, respectively) when assisted by the DL model (all P<0.01). Conclusions:The deep learning radiomics model is able to predict therapy response in the early-stage of NAC for breast cancer patients, which could guide clinicians and provide benefit for timely treatment strategy adjustment.

ABSTRACT

【Objective】 To explore the clinical effect of PRP on refractory ulcer of diabetes foot on the basis of routine treatment. 【Methods】 Sixty-four patients who suffered from diabetes foot and treated in our hospital from January to December 2020 were divided into the routine treatment group (44 cases) vs PRP plus routine treatment group (20 cases, using liquid or gel PRP for diversified treatment) according to a simple random sampling method. The general conditions of the two groups were evaluated to compare the wound surface, wound healing rate, treatment time, wound healing speed rate, adverse reactions and healing conditions after the treatment. 【Results】 The wound surface[0.05(0.00, 0.70)vs 0.35(0.00, 4.54)], wound healing rate[0.99(0.84, 1.00)vs 0.80(0.26, 1.00)] and wound healing speed rate[0.16(0.04, 0.27)vs 0.06(0.01, 0.18)] in PRP group were significantly higher than those in routine treatment group (P0.05), so was the adverse reactions to treatments[0(0/20)vs 2.27(1/44)](P>0.05). The response rate[100(20/20)vs 61.36(27/44)] of PRP group was significantly better than that of routine group, and the difference was statistically significant (P<0.05). 【Conclusion】 The therapeutic effect of PRP group was significantly superior to that of routine treatment group.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the effects differences for peptic ulcer between acupuncture based on the theory of "the compatibility of the five meridians" inand conventional western medication.</p><p><b>METHODS</b>Sixty patients with peptic ulcer of liver-stomach disharmony type (LSDT) and weakness of spleen and stomach type (WSST) were assigned into an observation group and a control group by block randomization according to syndrome differentiation.Finally,28 cases (17 with LSDT and 11 with WSST) in the observation group,29 cases (18 with LSDT and 11 with WSST) in the control group were included.In the observation group,patients with LSDT were treated with acupuncture at Zhongwan (CV 12),Taichong (LR 3),Xingjian (LR 2),Qimen (LR 14),Zusanli (ST 36),Gongsun (SP 4),Shaofu (HT 8),Jingqu (LU 8),Neiguan (PC 6); those with WSST,at Zhongwan (CV 12),Dadu (SP 2),Taibai (SP 3),Yinlingquan (SP 9),Zusanli (ST 36),Shaofu (HT 8), Taichong (LR 3),Yingu (KI 10),Taixi (KI 3),Taiyuan (LU 9) according to the theory of "the compatibility of the five meridians" in.Reinforcing and reducing were according to syndrome differentiation.The treatment was given once a day with needle retained for 30 min,5 days a week,2 days at interval.In the control group,the conventional triple drugs (omeprazole,amoxicillin and clarithromycin) were prescribed orally for Hp positive patients,and omeprazole for Hp negative patients.All the patients were treated for 4 weeks.The clinical syndrome score,ulcer healing under gastroscope,anti-Hp infection and Hp negative conversion ratio rate were observed in the two groups before and after treatment as well as 1 month after treatment.The total effects were evaluated.</p><p><b>RESULTS</b>The syndrome scores after treatment and at 1 month ofter treatment decreased in the two groups (all<0.05),while without significant difference between the two groups (both>0.05).The scores within the group between the two syndromes showed no significance in the two groups (both>0.05).The cure rates under gastroscope,anti-Hp infection rates and the total rates had no statistically significant difference between the two groups (all>0.05).The Hp negative conversion ratio was 22.2%(4/18) in the observation group,which was worse than 52.6%(10/19) in the control group (<0.05).</p><p><b>CONCLUSIONS</b>Acupuncture based on the theory of "the compatibility of the five meridians" infor peptic ulcer can improve clinical symptoms,ulcer coalescence and anti-Hp infection,which is similar to conventional western medication.While the Hp negative conversion ratio is less than that of conventional western medicine.</p>

ABSTRACT

“Acupoint characteristics” has a significant influence on the study of acupoints. The description pattern of “property of Chinese medicine - function - indication” of analogy TCM put forward the description pattern of “acupoint characteristics-function-indication axis”. The study was carried out from the following three aspects:sorting, forming and improving the theories on acupoint characteristics; determining the functions according to present theories and indications of acupoints, methodizing the indications of acupoints; through functions, gradually simplifying and supplementing. Therefore, the above would make great sense to further research and systematic study and memorizing of acupoints and promoting clinical thinking about acupuncture, with a purpose to better guide clinical application.

ABSTRACT

<p><b>OBJECTIVE</b>To study the effects of a Gold Belt (GB, a traditional Chinese herbal medicine) combined with methyl-prednisolone (MP) on the motor function and brain-derived neurotrophic factor (BDNF) expression in rats with contusive spinal cord injury (SCI).</p><p><b>METHODS</b>Thirty adult female SD rats were randomly divided into 5 equal groups, namely the sham-operated group, SCI group, SCI with MP treatment group (MP group, with intramuscular injection of 50 mg/kg MP within 8 hours after SCI and then dosage reduced 10 mg/kg daily), SCI with GB treatment group (GB group, with intragastric gavage of GB 50 mg/kg once daily for 7 days), and combined GB and MP treatment group. The Basso, Beattie and Bresnahan (BBB) locomotor scale was used to evaluate the hindlimb motor function of the rats on days 1, 3, 7, 14, 21 and 28 after the injury. After the last evaluation the rats were sacrificed for immunohistochemistry to observe the localization of BDNF in the ventral and dorsal horn of spinal cord.</p><p><b>RESULTS</b>BDNF were distributed mainly in neurons in the spinal cord grey matter ventral horn and dorsal horn of the rats. The number of BDNF-positive neurons and BBB scores in the combined treatment group were significantly higher than those in the other 4 groups (P<0.05).</p><p><b>CONCLUSION</b>GB combined with MP produces better therapeutic effects for treating SCI than GB or MP used alone, and such effects are probably related with enhanced BDNF expression in the spinal cord.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the alveolar bone thickness and root length changes of anterior teeth with cone-beam computed tomography (CBCT).</p><p><b>METHODS</b>CBCT scans were taken for 12 skeletal Class III patients who accepted the improved corticotomy (IC) procedures during pre-surgical orthodontics. The CBCT data in T1 (the maxillary dental arch was aligned and leveled) and T2 (extraction space closure) were superimposed and the alveolar bone thickness at root apex level and root length measurements were done.</p><p><b>RESULTS</b>From T1 to T2, the buccal alveolar bone thickness for the upper lateral incisors increased from (1.89±0.83) to (2.47±1.02) mm (P<0.05), and for central incisors and for canines from (2.32±0.71) to (2.68±1.48) mm and from (2.28±1.08) to (2.41±1.40) mm, respectively. According to Sharpe Grading System, the root resorption grade for 69 teeth of 72 was located in Grade 1, two teeth in Grade 2, one tooth in Grade 3.</p><p><b>CONCLUSIONS</b>The improved corticotomy had the potential to increase the buccal alveolar bone thickness and the root resorption in most teeth was in Grade 1 according to Sharpe grading system.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of the fibronectin extra domain A on the aggressiveness of salivary adenoid cystic carcinoma (SACC) cells, via the clustered regularly interspaced short palindromic repeats (CRISPR)/ associated proteins (Cas) system.</p><p><b>METHODS</b>One sgRNA was designed to target the upstream of the genome sequences of extra domain A(EDA) exon and the downstream. Then the sgRNA was linked into plasmid PX-330 and transfected into SACC-83 cells. PCR and DNA sequence were used to testify the knockout cells, and the monoclones of EDA absent SACC cells were selected (A+C-2, A+C-6, B+C-10). CCK-8 cell proliferation and invasion was then tested in control group and the experimental group.</p><p><b>RESULTS</b>The sgRNA was successfully linked into PX-330 plasmid. Part of adenoid cystic carcinoma cells' SACC-83 genomic EDA exon was knocked out, and the knockdown efficiency was above 70%, but the total amount of fibronectin did not change significantly. Three monoclones of EDA absent SACC- 83 cells were successfully selected with diminished migration and proliferation.</p><p><b>CONCLUSIONS</b>The CRISPR/Cas9 system was a simplified system with relatively high knockout efficiency and EDA knockout could inhibiting SACC cell's mobility and invasiveness.</p>

ABSTRACT

Objective To investigate the influence of different hemostatic way on ovarian function in laparoscopic excision of ovarian cysts. Methods 87 cases with unilateral ovarian cysts by laparoscopic excision were chosen from Jun. 2012 to Jun. 2013 in our hospital, and they were divided into the electrocoagulation group (44 cases) and the suture group (43 cases) according to hemostatic way. The electro-coagulation group were given bipolar coagulation while the suture group were given endoscopic hemostasis. The ovarian hormone levels and blood parameters before and after surgery were compared between the two groups. Results There was no statistically significant differenc in parameters of E2,FSH,LH,P before surgery between the two groups (P>0. 05), and there was no statistically significant differenc before and after surgery in the same group (P>0. 05). Compared with the suture group, there were statistically significant difference in terms of pa-rameters of E2,FSH in the electrocoagulation group(P0. 05). It was of no statistically significant difference in ovary sectional area between the two groups after surgery (P>0. 05), but it was of statistically significance in PSV (P<0. 05). Conclusion Compared with electrocoagulation, the laparoscopic excision of ovarian cysts with endoscopic hemostasis had less damage to ovarian, and it could protect the ovarian function.

ABSTRACT

Objective To explore the application value of ultrasound-guided fine needle aspiration biopsy(FNAB) in diagnosing thyroid nodules of TI-RADS 4-6 categories.Methods A retrospective analysis was made on the pathologic data of 494 patients (totally 501 nodules) who underwent TI-RADS ultrasonographic stratification and ultrasound-guided FNAB.168 thyroid nodules of TI-RADS 4-6 categories were selected from 163 patients and classified into three groups:group T,TI-RADS 4A categories;group Ⅱ,TI-RADS 4B category; group Ⅲ,TI-RADS 5-6 category(the pathologically confirmed cases in category 6 had been removed).The ultimate pathologic findings were taken as golden standards to calculate the accuracy,sensitivity,specificity,positive predictive value of ultrasound-guided FNAB in the three groups.ROC curve was drawn to evaluate the diagnostic value of FNAB.Results The accuracy,sensitivity,specificity,positive predictive value of the ultrasound-guided FNAB in the three groups were listed as follows:group Ⅰ 74.4%,73.3%,75.0%,64.7%;group Ⅱ 83.9%,87.5%,71.4%,91.3%;group Ⅲ 89.3%,91.7 %,75.0%,95.7 % respectively.The positive predictive values of the ultrasound-guided FNAB for group Ⅰ and group Ⅱ was higher than that of conventional TI-RADS classification criteria,so the difference was statistically significant; while the positive predictive values for group Ⅲ was close to the conventional ultrasound TI-RADS classification criteria,the difference was not statistically significant.Conclusions The combination application of TI-RADS classification criteria and FNAB can improve the accuracy of ultrasound diagnosis in group Ⅰ and group Ⅱ,and reduce the puncture rate of group Ⅲ.

ABSTRACT

Objective: To demonstrate that expression of the CCRSDelta32 protein in PBMCs able to down-regulate surface expression of the HIV-1 coreceptor CCR5 and CXCR4.Methods:CCR5Delta32 gene was amplified from human peripheral blood mononuclear cells(PBMCs)genomic DNA by using PCR, and then cloned into lentiviral vector pLenti6/V5-D-TOPO.Recombinant lentiviral particles were produced by packaging using 293T cells.Human PBMCs were transfected with the constructed recombinant lentiviral particles and the expression of CCR5Delta32 was detected by Western blot.The level of CCR5 and CXCR4 expression on transfected PBMCs was detected by FACS analysis.Results: The recombinant lentiviral vector pLenti-CCR5Delta32 was constructed successfully, and the target protein was expressed in PBMCs.FACS analysis showed that CCR5Delta32 protein expressed in PBMCs was able to down-regulate cell surface expression of CCR5 and CXCR4.Conclusion: This study is expected to be used for the gene therapy on AIDS, which deserves further study.

ABSTRACT

Objective To study the effect of blood transfusion on earthquake injuries with crush syndrome in WenChuan earthquake.Methods The amount of red blood cell suspension given to the earthquake victims were studied. The transfusion effect, Hb concentrations, and capillary hemorrhage of tissues of the injuries were also observed.Results Two victims, who underwent amputation surgery and cerebral hemorrhage operation, received a relatively small amount of transfusion, and showed a good transfusion effect and prognosis. While the other 6 victims, who underwent amputation surgery, surgical debridement, or continuous renal replacement therapy, received a large amount of blood, and showed a bad transfusion effect and prognosis.Conclusion The amount of blood transfusion in patients with crush syndrome was correlated negatively with the outcome.

Objective To study the relationship between micro-vessel density(MVD), proliferating cell nuclear antigen(PCNA) expression and clinicopathological characteristics in cervix squamous cell carcinoma. Methods PCNA and CD 34 expression in 32 cervix squamous cell carcinomas were examined by SP immunohistochemical staining. MVD and proliferation index (PI) were calculated. Results PI and MVD were significantly correlated with the differentiatial degree and FIGO stage of cervix squamous cell carcinomas, and in the low differential cancers with high FIGO stage were obviously higher than those in the high differential cancers with low FIGO stage. In addition, PI and MVD in the cancers with lymph node metastasis were significantly higher than those in the cancers without one (P
