ABSTRACT

Background: cardiovascular diseases are the most common cause of deaths in Iran and other developing countries. The risk factors for cardiovascular diseases are divided into two categories; the variable risk factors and the non-variable risk factors. Many recent studies evaluated the relationship between higher eosinophilia and allergy levels with the incidence, progress and severity of cardiovascular diseases, but the exact correlation between these two still remains unknown. The current study was designed to assess the relationship between allergic responses and eosinophilia amongst patients with cardiovascular diseases in Ilam province, in comparison with healthy individuals


Materials and Methods: in this case-control study, we enrolled 59 cardiovascular patients and 55 healthy individuals without any history of allergy and parasitic infections. A questionnaire including questions about demographic data, family history of heart disease, history of diabetes, hyperlipidemia, physical activity, smoking, stress, dietary fat consumption, salt intake, allergies to certain substances, history of parasitic disease and history of hypertension was completed. The blood was taken from each participant and CBC and IgE titer were measured


Results: there was a significant relationship for the variables such as the family history of cardiovascular disease [P<0.001], diabetes [P<0.003], hyperlipidemia [P<0.0001], high blood pressure [P<0.0001] and physical activity [P<0.0001] between the case and the control groups. The mean IgE titer in case group was 95.3+/-71 and 62.44+/-49 in control group. The mean eosinophilia level in peripheral blood was 3.95+/-1.057 in case and 1.53+/-0.57 in control group. The difference between the IgE and eosinophilia levels in the case and the control groups was statistically significant [P<0.0001]


Conclusion: based on our results, it can be concluded the increase in levels of IgE and eosinophilia can be considered by cardiologists as a reliable diagnostic tool for predicting cardiovascular diseases

ABSTRACT

Background: multiple sclerosis is considered as an autoimmune disease of the central nervous system that is the main cause of disability in young adults around the world. The purpose of this study was to determine changes in antibodies in the prognosis of multiple sclerosis, and the use of antibody against aquaporin 4 for the diagnosis of multiple sclerosis


Materials and Methods: in this case - control study, 21 patients with a definite diagnosis of multiple sclerosis and 21 healthy subjects were selected as the study population. Blood and urine samples were collected, and nephelometry technique was used to assess the presence or absence of IgG, IgM and IgA in serum and urine samples. ELISA method for measuring of antibodies against aquaporin 4 was used


Results: there was no major difference in the mean of the total IgM in the case and control groups, but the mean IgA and IgG levels in the control group were evidently higher than in the case group. It was releaved that IgA, RBC and Hb mean differences between the two groups are statistically significant. Parallel with an increase in IgG, the probability of disease exacerbation was increased by 0.22, whereas with increasing ages, the probability of disease exacerbation was 15.0. There was also a positive and significant relationship between the average level of antibodies, IgG and IgM with the degree of illness However, the relationship between the mean serum IgA level and the degree of illness was inverse. It also became clear that antibodies against AQP-4 in serum and urine of patients with different degrees of illness showed no significant difference. The difference between the mean of antibodies against AQP in the serum of patients with mild and moderate MS was 54.1, but in mild and severe MS it was 53.3


Conclusion: the findings of this research suggest that serum antibody levels are directly related to the disease levels and can be used as a prognostic factor. Accordingly, it appears that the use of antibodies against aquaporin-4 in serum and urine for the diagnosis of this disease can be considered as a reliable approach

ABSTRACT

Immune responses play critical roles in the leishmaniasis eradication. IL-10 is a key regulator of immune responses, and the polymorphisms within its promoter region are associated with alteration in its expression. Therefore, this study was designed to examine the correlation between polymorphism at the -1082 position of the IL-10 gene and visceral leishmaniasis [VL]. The IL-10 -1082 polymorphism and anti-Leishmania antibody titration were examined in 110 patients with clinical presentation of VL and seropositive for the Leishmania [group 1], 74 seropositive patients but without clinical presentation [group 2] and 113 healthy controls [group 3] using the PCR-RFLP and immunofluorescence techniques, respectively. The polymorphism at IL-10 -1082 [A/G] position was significantly associated with VL and A/G genotype was significantly higher in VL patients when compared to the groups 2 and 3 [P< 0.001]. However, the results demonstrated that the A and G alleles were not associated with VL [P= 0.263]. Previous investigations have shown that the polymorphism at the -1082 position of the IL-10 gene can influence its expression and also it has been proved that IL-10 level was increased during VL. Our results suggest that the A/G genotype may be considered as a risk factor for VL

ABSTRACT

Radiography is the most widely available imaging modality. Precise evaluations of wrist x-ray can help diagnosis and evaluate the prognosis of many wrist disorders. We measured length, angles and indices in 150 posteroanterior and lateral wrist x-rays to determine normal dimensions and variations according to age and sex. All x-rays were made with standard exposure, with the wrist and forearm in a neutral position. The average carpal height ratio was 0.52 +/- 0.03 with the Youm method and 1.5 +/- 0.09 with the Nattrass method. Mean ulnar variance was +0.99 +/- 1.6 mm and mean radial inclination was 25 +/- 4 degrees. The average radial tilt was 10 +/- 5.1 degrees. Mean scapholunate angle was 50 +/- 8.4 degrees [normal range 40 -60]. Carpal height, third metacarpal and capitate length were smaller in women than in men. There was a significant positive relationship between all dimensions. Our data base may be used to follow-up in conditions such as carpal instability, osteoarthritis and osteonecrosis, as well as for clinical research

ABSTRACT

The varieties of treatment methods for the scaphoid nonunion have always been a discussable issue for orthopaedic surgeons. The current study, considered to be review for treatment results of the patients who sustained the scaphoid nonunion with or without avascular necrosis and compared these two groups clinically and radiologically. The clinical and radiologic files of the patients who sustained the scaphoid nonunion and underwent surgical treatment between 2004 and 2009 in our hospital, were collected and in the final follow-up, 37 patients and 38 nonunions underwent physical exam, grip power test, DASH questionnaire and radiography. Avascular necrosis was detected in 22 cases and 16 cases had no avascular necrosis changes. Following open reduction and bone grafing, union was seen in 36 cases, and nonunion in 2 cases which one was in the avascular necrosis group and another in without avascular necrosis group. The comparison of the residual pain, range of motion, DASH score and grip power in two groups, did not show a meaningful difference [p>0.05]. Although multiple studies indicated that in the presence of the avascular necrosis in the proximal fragment of the scaphiod, possibility of union in the conventional graft methods was lower than vascular pedicle grafts, but this study demonstrated that the conventional treatment method yet was associated with a considerable successs and a reliable treatment method

ABSTRACT

All perilunate fracture-dislocations combine bone avulsions, ligament disruption and fractures in different forms. The most frequent pattern is the dorsal transscaphoid perilunate fracture-dislocation. The reported risk of missed initial diagnosis in perilunate dislocations is as high as 20%. Concerning pathomechanics of the perilunate fracture-dislocation, it should be noted that most dorsal injuries usually result from a fall on the outstretched hand or motor vehicle accidents although other mechanisms are also mentioned. We describe here a case of dorsal trans-scaphoid trans-capitate perilunate fracture-dislocation together with posterior olecranon fracture-dislocation of the left upper extremity. To the best of the authors' knowledge, such a case with this concomitant elbow fracture-dislocation has not been previously reported in the literature
