ABSTRACT

AlM: To examine the retinal nerve fiber layer ( RNFL ) changes in early primary open angle glaucoma ( POAG ) by comparing the superior half of the optic disc with the inferior disc half.METHODS: lt was a clinical observational study. The study included 30 patients ( 39 eyes ) with POAG and 20 normal subjects ( 40 eyes ) . Visual field was tested by Zeiss Humphrey750 and the optic disc topographic parameters and RNFL thickness were measured by Zeiss Cirru HD-OCT. Collecting information included visual field mean defect ( MD ) , glaucoma hemifield test ( GHT ) , intraocular pressure ( lOP ) , C/D ratio and RNFL thickness. Data were analyzed by statistic software SPSS18. 0. The differences between two groups were assessed using t test.RESULTS: The differences of superior RNFL minus inferior RNFL thichness on the corresponding clock-hour locations were caculated in two groups. There was a significant difference in superonasal- inferonasal RNFL thickness between two groups (t=2. 526, P=0. 014), and there were no significant differences in the others ( all P>0. 05).CONCLUSlON: The asymmetry of optic disc hemifield RNFL is found in early primary open angle glaucoma patients. The superonasal RNFL ( 1 o’clock for right eye, and 11 o’clock for left eye ) is more fragile to loss than inferonasal RNFL (5 o’clock for right eye, and 7 o’clock for left eye) .

ABSTRACT

? AlM: To estimate the clinical significance of the microculture of humor and vitreous and vancomycin intraocular injection in treatment of suppurative endophthalmitis associated with intraocular foreign bodies. ?METHODS: Totally 65 patients with penetrating eye trauma and retained intraocular foreign bodies in emergency operation and intraocular injection from January 2012 to September 2014 were regarded as the study group, another 62 patients with penetrating eye trauma and retained intraocular foreign bodies in emergency operation without intraocular injection before August 2011 were regarded as the control group. Aqueous humor and vitreous humor were taken from each patient of the study group and the control group for bacteria and fungus cultivation. The study group was treated with 1mg vancomycin intraocular injection after operation, while the control group was not. ?RESULTS: The incidence of endophthalmitis in the control group was 16% ( 10 cases ) , while in the study group was 3% ( 2 cases ) , with significant difference between two groups (x2 =6. 32, P0. 05). The positive rate of vitreous humor germiculture in study group was 14% (9 cases), and the incidence of endophthalmitis was 3%. The positive rate of vitreous humor germiculture in control group was 11% (7 cases) and the incidence of endophthalmitis was 16%, with significant differences between two groups (P<0. 05).?CONCLUSlON: lntraocular foreign bodies treated with emergency operation and vancomycin intraocular injections can decrease the incidence of suppurative endophthalmitis and have a good vision prognosis for the second stage of operation.

ABSTRACT

Background Selective laser trabeculoplasty (SLT) is a safe and effective therapy to the treatment of primary open angle glaucoma (POAG),and the clinical application is approved by American FDA.However,the relevant researching outcomes from several single-center study are in dispute owing to non-uniform approach in China.A muhicenter clinical trial is need to verify the effectiveness of SLT for Chinese POAG.Objective This study was to investigate the safety and efficacy of SLT in Chinese POAG patients.Methods A prospective,multicenter and self-controlled clinical trail was designed.Sixty-two eyes of 62 POAG patients with the age of ≥ 18 years were enrolled.Frequency SLT was performed around 360° angle using Lumenis Selecta DuetTM or Lumenis Selecta Ⅱ laser instrument,with the 100 non-overlapping spots,3 ns preset time,400 μm size of laser spot and 0.6 mJ energy.The operated eyes were followed-up for 6 months.The curative effects were checked including intraocular pressure (IOP),IOP-lowing value,number and percentage of IOP-lowing 20% and 30%.The therapyrelated complications were recorded including the number of conjunctival congestion and cloudy of the anterior chamber.Results The preoperative IOP was (25.7±2.6) mmHg in the 62 eyes.The IOP was significantly lowed 1 day,2 weeks,4 weeks,3 months and 6 months after SLT in comparison with preoperative IOP (all at P<0.001) with the lowest value of (16.5±4.5)mmHg in postoperative day 1.IOP was stable from 4 weeks to 6 months after operation.The mean lowing-value of IOP was 5.3-9.2 mmHg from 1 day through 6 months after SLT.The percentage of eyes 20% drop in lOP was 83.9%,and that of 30% drop was 58.1% in postoperative day 1.In 6 months after SLT,percentages of eyes 20% and 30% drop in IOP were 56.5% and 27.4%,respectively.Conclusions SLT is a safe and effective method of lowing IOP for POAG eyes in Chinese population.

ABSTRACT

To assess the postoperative outcomes of piggyback implantation using Acrysof Toric intraocular lens ( lOL ) in high myopia combined with corneal astigmatism. ●METHODS: Sixty patients who had phacoemulsification with lOL implantation due to high myopia, cataract and corneal astigmatism from January 2010 to June 2013 were randomly divided into observation group (piggyback Toric lOL implantation, both an Acrysoft lQ Toric lOL and a minus foldable acrylic three piece lOL were implanted in the capsular bag, n= 30) and control group (foldable lOL implantation, n = 30). Postoperative follow - up went on 6mo. lnformation collected included uncorrected visual acuity ( UCVA), lOL position, residual astigmatism and complications. ● RESULTS: The UCVA increased from 3. 52 ± 0. 03 preoperatively to 4. 78±0. 01 at 6mo postoperatively in the observation group, from 3. 51±0. 03 preoperatively to 4. 30± 0. 13 at 6mo postoperatively in the control group. The observation group's postoperative UCVA was better than that of the control group. There was statistically significant difference ( t = 3. 612, P ●CONCLUSlON: Piggyback implantation using Toric lOL can help to solve the problem of no matching Toric lOL power for the high myopia combined with corneal astigmatism at the current stage. lt improves the UCVA and reduces the astigmatism after the cataract surgery.

ABSTRACT

AIM: To observe the clinical effect of phacoemulsification and posterior chamber intraocular lens ( IOL) implantation in the treatment of unilateral high myopia combined with age-related cataract. METHODS: A retrospective clinical study was carried out in a single ophthalmology center. All patients diagnosed of unilateral high myopia with age- related cataract were performed phacoemulsification and posterior chamber IOL implantation. The axial length of operation eye was ≥27mm, and the axial length of unoperated fellow eye was≤25mm. Visual acuity and complications were followed up for 1, 7d, 1, 3mo postoperative. RESULTS:Thirty-three patients with average age 68. 8±9. 26 (41-87) y were enrolled in this study, 7 cases were men, 26 cases were women. The mean axial length of operation eyes was 29. 44±1. 80(27. 1-32. 71) mm and the mean axial length of unoperated fellow eyes was 23. 39±0.92 ( 21. 72 - 24. 90 ) mm. The average axial length difference of binocular eyes was 6. 06±1. 97(2. 78-9. 92) mm. The best corrected visual acuity ( BCVA ) increased from 0. 02±0. 02(light perception~0. 12) preoperatively to 0. 20±0. 20 ( 0. 02 - 0. 8 ) at 3mo postoperatively. There was statistically significant difference (t= -5. 278, P0. 3. Among the 13 patients with binocular axial length difference of 5. 01-7. 00mm, 46.2% of BCVA were >0. 3,46.2% achieved 0. 05-0. 25. Another 10 patients of binocular axial length difference between 7. 01-9. 99mm, 90% of BCVA were CONCLUSION: Phacoemulsification and posterior chamber IOL implantation can effectively improve the postoperative visual acuity and it is safe for unilateral high myopic patients combined with age-related cataract. The recovery of visual acuity is related with binocular axial length difference.

ABSTRACT

Objective To evaluate the infection status and genotype distribution of high risk human papillomavirus (HPV) in Chongqing women with different healthy status of cervix .Methods A retrospective analysis including 3 118 cases from June 2013 to March 2014 were performed ,945 of which were carried out multiplex real time PCR ,and data of cervix healthy status and HPV positive cases were subjected to chi‐square test .Results The general positive percentage of high risk HPV was 27 .49% ,16 .35%in healthy check‐up group ,and 24 .42% in cervicitis group and 39 .87% in cervical neoplasia lesion ,there was a significant difference of HPV positive ratio between these three groups(P< 0 .05) ;and HPV positive ratio was higher in older women without significant difference .In Chongqing region ,the three most common HPV types were HPV52 (21 .97% ) ,HPV16 (20 .00% ) and HPV58 (14 .75% ) .However ,the three most common HPV types in various groups are different :HPV52 (26 .20% ) ,HPV58 (14 .3% ) and HPV51 (10 .70% ) in health check‐up group ; HPV52 (21 .10% ) ,HPV58 (20 .00% ) and HPV16 (18 .90% ) in cervicitis group while HPV16 (29 .00% ) ,HPV52 (19 .80% ) and HPV58 (11 .50% ) in cervical neoplasia lesion group .The multiple infec‐tion rate of HPV was 5 .86% ,as the cervix status gets worse ,HPV concurrent infection increases ,and concurrent HPV types vary :mostly with HPV 52 in healthy check‐up group while with HPV 16 in cervical neoplasia lesion group .Conclusion The prevalence and genotype distribution of HPV in Chongqing women with different healthy status of cervix are significant difference.

ABSTRACT

<p><b>OBJECTIVE</b>To study the clinical and laboratory characteristics of adult Japanese encephalitis (JE) patients in a JE outbreak in Yuncheng, Shanxi Province in 2006.</p><p><b>METHOD</b>All the clinical data from the Second People's Hospital in Yuncheng city were analyzed, part of patients' sera and cerebrospinal fluid were tested by serology and RT-PCR.</p><p><b>RESULTS</b>The majority of patients were middle-aged and elderly, 77.8% (35/45) of the total cases were more than 40 years old. Severe and fulminating type cases accounted for 60.0% (27/45). Most patients had underlying diseases. IgM antibody to JE virus (JEV) in serum was positive in each of the 45 patients analyzed and 4-fold or greater rise in sera neutralization antibody titer were found in convalescent serum. JEV nucleic acid was positive in part of cerebrospinal fluid specimens.</p><p><b>CONCLUSION</b>Viral encephalitis emerged in Yuncheng city, Shanxi Province was Japanese encephalitis B, and most of the cases belonged to elderly group.</p>

ABSTRACT

Objective To analyze the epidemiology and clinical characteristics of epidemic encephalitis B in Yuneheng area of Shanxi Province in 2006.Methods Seventy-nine cases diagnosed with epidemic encephalitis B were enrolled in this study and correlated data,epidemic and clinical characteristics,laboratory examinations and treatment outcome were analyzed.Results Sixty-two of 79 patients(78.5%)were middle-aged or old people,all cases(100%)occurred in July,August and September,69 cases(87.3%)were peasants.All patients(100%)had fever,73(92.4%)had conscious disturbance,27(34.2%)had respiratory failure.Encephalitis B specific IgM antibody was examined and 40 cases(85.1%)were positive.Twenty cases(25.3%)had complications.When they were discharged,37 cases(46.8%)recovered completely,14 cases(17.7%)died,12 were improved and 16 were voluntarily discharged,7 cases(8.9%)left more or less neurological deficits. Conclusion The epidemiology and clinical characteristics are important basis to diagnose epidemic encephalitis B.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the relationship between right ventricular (RV) mechanical delay and RV dysfunction in patients with pulmonary hypertension (PH).</p><p><b>METHODS</b>A total of 51 patients with PH were divided into mild, moderate and high PH groups and 30 healthy volunteers served as control group. RV mechanical delay (defined as the difference in time to peak systolic annular velocity between the RV free wall and the ventricular septum) and RV fractional area change were obtained on apical four chamber view with Quantitive Tissue Velocity Imaging (QTVI).</p><p><b>RESULTS</b>RV mechanical delay increases in proportion to pulmonary pressure. RV fractional area change was similar between control and mild PH patients while significantly reduced in moderate and high PH patients compared to controls. RV mechanical delay significantly correlated to RV fractional area change in all PH groups (r = -0.79; r = -0.66; r = -0.80, all P < 0.05).</p><p><b>CONCLUSION</b>RV mechanical delay measured by QTVI can be used to evaluate RV dysfunction in patients with pulmonary hypertension.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the surgical indications, approaches and the clinical results of the total spondylectomy and instrumentation reconstruction in the treatment of cervical spinal tumor.</p><p><b>METHODS</b>From October 1998 to October 2003, 39 patients with lower cervical bone tumors, including 34 cases with primary tumor and 5 cases with metastatic tumor, were admitted and operated on with total spondylectomy. The patients underwent anteroposterior total spondylectomy including anterior cervical plating, titanium mesh reconstruction and posterior instrumentation based on the location of tumor lesions in the lower cervical spine. One vertebral level total spondylectomy was performed in 29 cases, two level in 7 cases and three level in 3 cases.</p><p><b>RESULTS</b>The postoperative follow-up ranged from 6 months to 4 years. A majority of patients achieved good results postoperatively. Nineteen cases had complete relief of neurological status. One patient died of multiple metastases and systemic failure 24 months later. One case with malignant neurilemmoma developed local recurrence one year postoperatively.</p><p><b>CONCLUSION</b>Anteroposterior total spondylectomy and reconstruction can reduce local recurrence, improve neurological function and increase operation therapeutic effect. Meanwhile, the technique of cervical total spondylectomy carries relatively greater risks and should be more attention to the operation indication.</p>

Objective: To synthesize the collagen-GAG template and to evaluate its feasibility to be used as the MSCs vehicle for meniscal tissue engineering. Methods: The collagen-GAG template was synthesized from rat tail type Ⅰ collagen and GAG using Yannas method. Then the post-stimulated MSCs by bFGF and TGF-β1 were added in. The MSCs-enriched collagen sponges were cultured in vitro, two weeks later the histological and ultrastructure detection was performed. Results: The histological and ultrastructure of the collagen-GAG template remained intact after 2 weeks' culture, and the MSCs in it remained viable. Conclusion: The collagen-GAG template synthesized in this experiment is suitable for the meniscal tissue engineering reconstruction as the vehicle for MSCs seed cells.
