ABSTRACT

This study was conducted to investigate the association of the occurrence of pediatric disease with environmental, seasonal and atmospheric factors. The data were collected at 5 pediatric clinics in Seoul and the Department of Pediatrics of Yongin Severance Hospital from May 1986 to April 1987. The results were as follows: 1. Vacation periods had a great influence upon the occurrence of pediatric diseases. 2. The majority of pediatric diseases occurred mainly in spring and autumn, not in summer and winter. 3. The higher the average relative humidity was, the less diseases occurred; and the higher the maximum change of daily temperature, the more diseases occurred. 4. In summer, the pattern of diseases varied along with the environmental factors(eg., toilet).

ABSTRACT

Chyle is the digestive product absorbed from intestinal lymphatics which the component have high concentration of triglyceride and protein. The various pathological circumstances which give rise to intrathoracic effusion of chyle are well documented and so the etiology is clearly defined in most instances. However, the occurrence of chylothorax in early infancy, in the absence of other demonstrable diseases and without apparent birth trauma, suggests the existence of congenital malformation of the thoracic ducts. We lately experienced a 20 day old new born who was proved to have congenital chylothorax by clinical history and laboratory findings. So we report the clinical findings and laboratory findings with the review of the literatures on congenital chylothorax.

On admission, physical examination revealed whitish mucosal discoloration on the oral cavity. 3 days after admission, anuric state had been appeared, and general condition has been aggrevated with generalized edema, uremai and hyperkalenia inspite of conservative therapy. On the 5th hospital day, peritoneal dialysis was performed for next 4 days. 3 days after peritoneal dialysis, urine amount was increased up to 800cc/day ard all symptoms were subsided, renal function test and serum electrolyte were normalized. Medical manegement included peritoneal dialysis and adequate antidote in acute renal failure due to mercury poisoning was effective and then the patient was recovered.

ABSTRACT

Three cases of hisiocytic medullary reticulosis occurring in children aged 6 years, 9 years and 14 years, are described. In all children the diagnosis was based on anemia, granulocytopenia, thrombocytopenia and marked erythrophagocytosis by bone marrow and lymph node atypical histiocytes. They all showed immediate remission with combined chemotherapy of vinblastine and prednisolone, but Case 1 eventually died at 6 months after onset of this rapidly progressive, fatal illness and Case 2, 3 are alive 14 months after onset of their illness.
