ABSTRACT

The use of somastostatin in chyle fistula that occurs after neck dissection is a new approach that has been reported in few cases. We report two cases of chyle fistula that occurred after neck dissection, which were successfully managed with somatostatin subcutaneous injection. Somatostatin treatment resulted in an immediate reduction of chyle leakage in both patients, and might have contributed to the reduction of patients' morbidity and duration of hospital stay. However, optimal treatment regimen with somatostatin is unclear because of its limited case and various administration methods. Further studies are required to clarify the usefulness and optimal regimen of somatostatin injection in the treatment of chyle fistula following neck dissection.

ABSTRACT

The management of neonate with long gap atresia without a fistula(typeA) is complex and controversial. Various esophageal reconstruction include use of native esophagus or replacement with colon, stomach and small bowel. A severe premature male, at 28 weeks gestation weighing 1.2kg, was born with type A esophageal atresia in Chonnam University Hospital. Initial treatment consisted of gastrostomy under the local anesthesia and suctioning of proximal pouch, and than underwent delayed esophageal end to end anastomosis. A minimal leakage and mediastinitis ocurred postoperatively, but was treated by adequate drainage and negative suction from the leakage site through the gastrostomy. The patient was discharged in good general condition and normal weight of 5.4kg after 4 months after the surgery.

ABSTRACT

As is well known, myxoma of the heart is very rare(incidence 0.03%), constituting nearly 50% of all primary cardiac tumors. It is important because it can be abolished by surgical removal but usually fatal if unrecognized or untreated. Recently a wider use of echocardiogram as the screening test of valve lesions can be expected to increase the number of myxomas found preoperatively. We have experienced a case of bilateral atrial myxoma which simulated mitral valvular disease clinically and had typical three cardinal manifestations such as obstructive, embolic and constitutional symptoms. The patients was 39 years old woman. The left atrial myxoma was diagnosed preoperatively by echocardiography and cardiac angiography and the remainder during open heart surgery under the E.C.C. Both of them were removed successfuly and confirmed as typical myxoma microscopically. This is the first case report of bilateral atrial myxoma in Korea to our knowledge.

ABSTRACT

A case of sparganosis was presented. The patient was a 43-year-old Korean male who had a history of having eaten raw flesh of snake and frog in Keoje Island during 1950 to 1952 for the purpose of treatment of arthritis by the oriental custom. In the summer, l959, he had slight itching sensation in the left lower scrotal area. A few months later, he noticed the appearance of a peanut-sized mass in that area. However, the mass enlarged gradually to a hen's egg size during two years. On surgical exploration, the mass was found to be a fibro-connective tissue capsule(3 to 4 cm in diameter) in the lower left scrotal area. From the capsule three larvae(l9 to 28 cm by 2 to 6 mm) were freed by dissection without much difficulty. The sources of infection of some formerly reported sparganosis in Korea were discussed. It is most probable that human sparganosis is acquired by ingestion of raw flesh of snake and frog in this country.
