ABSTRACT
Na região estética da maxila, os pacientes frequentemente precisam de extração, sendo o tratamento da zona edêntula realizado com implantes imediatos quando possível. É aceitável, em alguns casos, a abordagem de implantes em estágio único com a colocação de parafuso cicatrizador, desde que, a transmissão de cargas mastigatórias seja evitada ao implante, e também, haja um controle rígido do biofilme bacteriano durante a osseointegração. O paciente apresentando falhas no tratamento endodôntico foi submetida à instalação imediata de implantes em área estética de maxila com customização de parafuso cicatrizador. A colocação de cicatrizador personalizado logo após a instalação dos implantes deve ser inerte ao tecido perimplantar e objetiva, principalmente, manter a arquitetura gengival até a realização da prótese definitiva, quando não é possível a carga imediata, como ocorreu neste caso. A personalização do parafuso cicatrizador deve ainda obedecer a uma excelente lisura superficial, a fim de evitar a retenção de restos alimentares/ biofilme bacteriano, fatores que prejudicariam a cicatrização periodontal. Autilização do parafuso cicatrizador personalizado constitui-se como boa alternativa para manutenção da arquitetura gengival quando não é possível proceder à carga imediata(AU)
In the aesthetics maxilla area, patients often require extraction and the treatment of the edentulous area with immediate implants is performed when possible. It's acceptable in some cases the installing of implants in a single stage by placing healing screw, since the transmission masticatory loads for the implant are avoided, and exist also a strict control of bacterial biofilm during osseointegration. Patient presenting failures in endodontic treatment was submitted to immediate implant placement in aesthetic area of maxilla with customization healing screw. The placement of custom screw healing after implant placement should be inert to the peri-implant tissue and objective mainly maintaining gingival architecture until the completion of final prosthesis, when immediate loading is not possible, as occurred in this case. The customization of the healing screw must still obey an excellent surface smoothness, in order to prevent the retention of food debris / bacterial biofilms, factors that would harm the periodontal healing. The use of customizing healing screw constitutes a good alternative to the gingival architecture maintenance when it is not possible to immediate loading(AU)
