ABSTRACT

Introduction: Traumatic Brain Injury [TBI] is one of the most important matters of public health in which people affected often lose their functional abilities and have dependent life and in result these patients a caregiver for care and support. Thus families are at front line of challenges and great stresses


Objective: The aim of this study was to determine needs of caregivers of patients with head trauma after discharge and level of its fulfillment in patients referring to one of educational- therapeutic center in Rasht city


Methods: This is a cross-sectional descriptive-analytical study conducted on 51 family members of patients with head injury who were selected by census method from available samples in 2013. In this study two questionnaires of demographics and Standard Family Need Questionnaire [FNQ] were used for data collection and analyzed by descriptive statistics and Chi square, independent T Test, ANOVA and Exact Fischer tests


Results: The most important need was related to subscale of health information and the least related to getting involved with caring. There was a significant relationship between level of caregivers' needs and their occupational status [P<0.046], patients' age range [P<0.023] and patients' clinical condition [P<042]. The maximum rate [3.49 +/- 0.56] of need fulfillment was related to subscales of community support and health information and the minimum [2.965 +/- 0.78] was related to instrumental support. There was a significant relationship between meeting the needs in all domains and health insurance [P<0.044]


Conclusion: The most important family need was related to subscales of health information that were often met. Apprehension of needs and difficulties of caregivers lead to increase of awareness of nurses and other custodians of healthcare about the necessity of helping caregivers of patients with Traumatic Brain Injury

Introduction: marital satisfactions are the key to strength the family and heighten the physical, mental health and economic development which could effect on life and job satisfaction. Professional nurses are under various stresses during their shift work to provide excellent quality of life for patients. They have responsibility for their own personal life too. Objective: this study aimed to determine the role of shift work and predictor variables on marital satisfaction of employed nurses through educational - medical centers


Methods: in this cross - sectional study, 500 marital employed nurses from 8 training medical centers in Rasht were selected using stratified random sampling. Information was collected using ENRICH marital satisfaction questionnaire for demographic data, social and shift work. For data analysis spss16, descriptive and inferential statistical tests [linear regression, Pearson's test, Regression Logistic Ordinal Model] were applied


Results: results showed that there is a significant relationship between marital satisfaction and shift work [P=0/003], history job [P=0/0001], personal income [P=0/002], the place of service [P=0/0001], employment status [P=0/0001], age [P=0/002]. The findings showed each child can increase 1.65 times chance of lower satisfaction. Psychiatric ward was only ward related to marital satisfaction [p<0.0001]. But there was not significant relationship between marital satisfaction job degree, sex and educational level


Conclusion: according to this findings, shift work, and some occupational factors [history job, income, place of service, employment status] are related to marital satisfaction for nurses. Therefore, applying the appropriate ways and moderating factors to improve the marital and career state of nurses are essential

ABSTRACT

Introduction: Anxiety and depression are the most common psychological outcomes in patients undergoing coronary artery bypass surgery [CABG]. Determining the predictors of anxiety and reduction of cardiac events resulting from the outcome of psychological symptoms depression can be useful in ability of patients and therefore, this study was conducted to determine predictors of hospital anxiety and depression in patients undergoing coronary artery bypass surgery


Methods: This study is a cross-sectional study in which 170 patients undergoing coronary artery bypass surgery hospitalized in cardiac hospitals of Guilan University of Medical Sciences were selected by Convenience sampling method. Tools for data collection included demographic and social, Standardized questionnaire, Hospital Anxiety and Depression Scale and the stressors questionnaire. The data were analyzed by SPSS software, version 17, and descriptive and inferential statistic tests were used


Results: The findings showed 33/75% and 17/83%of patients had anxiety and depression at discharge. Based on logistic regression test predictors of hospital anxiety at "discharge pain or discomfort following surgery" sex and education level and depression at discharge "Lack of support from family and friends "age and sex were determined


Conclusion: Results of this study showed an occurrence of anxiety and depression in patients with coronary artery bypass surgery. Recommendations for further assessment of the mental status of the patients at discharge and considering predictors for planning appropriate interventions is recommended

ABSTRACT

Critical thinking is an index of cognitive ability. The nurses' cognitive ability, specifically their ability for information processing and decision making, is the key component of nursing practice. The aim of this study was to determine the relationship between critical thinking and clinical decision making in nursing students of Guilan University of Medical Sciences. Fifty senior nursing students from Guilan University of Medical Sciences participated in this descriptive -correlational study by census. Data was collected using California Critical Thinking Questionnaire and clinical decision making cards by Q-Sort and analyzed by Chi- Square and Fischer Exact tests using SPSS- Pc [v.14]. Findings did not show a significant relationship between critical thinking and clinical decision making. Chi square test showed a significant relationship between critical thinking and educational average score [p=0.001] and between decision making and history of exposing to patients undergoing Billrut 2 surgery [p=0.008]. Conducting the same research with a larger sample using multiple tools is recommended. The findings could be used as a guide to nursing educational planning

ABSTRACT

Introduction: Immobility refers to limitation of physical activity. If immobility continues for a long term can cause serious complications such as skin injuries that form pressure sore. A study conducted in USA reported the morbidity rate due to pressure sore in patients was 19% and an another study showed this rate in patients in orthopedic ward in Tehran was 25%


Objective: Since promoting the quality of care can prevent many immobility complications, therefore this descriptive study was performed. The purpose of this study was to determine quality of care in prevention of pressure sore


Materials and Methods: The tool of data collection had three sections including: predicting Braden Scale, demographic data form, check list for determining quality of care in prevention of pressure sore and quality of structures


Results: The results of study showed that in the majority of cases [87.1%] the performed care process for prevention of pressure sore was almost satisfactory. Findings also showed that quality of structure in this educational-therapeutic center, in the majority of cases [72.9%] was undesirable. Other findings indicated a statistically significant relationship between performed quality of care process and quality of structure [P<0.004, r=0.341]


Conclusion: Quality of structure in the majority of cases was undesirable, this can affect on quality of care in prevention of pressure and it seems the managers' knowledge can arise quality of care in that cent
